[Lithium exchange in from epithelium: determination of unidirectional flux using the stable isotopes 6Li and 7Li in an ionic analyzer].

The use of the stable isotopes 6 and 7 of lithium, and detection with the ionic analyser, allow the measurement of unidirectional transepithelial Li-fluxes, between "external" and "internal" solutions which both contain 80% Na and 20% Li. Efflux was found to be bigger than influx. An important lag-phase on the curve, reveals the interference of a Li-compartment, with a high capacity, in the epithelium itself.

[Combined surgery of the cardiac valves and the coronary arties (author's transl)].

The authors report on an experiment with 17 cases, in which valvular substitution and bypass were combined. This type of operation carries a greater operative risk than that of separate operations, but a complete surgical correction can be obtained. The mortality rate diminishes with the improvement of the surgical team's experience.

[Cardiac surgery in Jehovah's witnesses. Apropos of 47 cases].

47 cardiac defects in Jehova's witnesses were operated on without using any blood during the operation. In 9 cases the patients were under 15 years of age. 7 cases were of congenital heart defects in which the operation could be carried out with the heart still beating or by a closed heart technique: 4 of these were adults and 3 were children. In 40 cases, extracorporeal circulation was required: 19 valve defects, 8 coronary areterial cases, 10 congenital cardiac lesions, 2 valve defects associated with coronary artery disease, and 1 aneurysm of the thoracic aorta. Of these 40 patients, 4 died. The details and limits of this total haemodilution are analysed, as are the causes of failure and complications. This technique does not worsen the postoperative prognosis appreciably, but limits the scope of the surgery, and cannot be applied to a child of less than 10 kg.

[Coronary circulation in abnormal left coronary artery arizing from the pulmonary artery trunk, before and after correction].

Two patients, both of them young girls, aged 14 and 15, had this condition and were studied by haemodynamic methods preoperatively and during the seventh postoperative month. In both cases, the retrograde perfusion of the left coronary vascular tree was shown up well, demonstrating the "coronary steal" and a left-right shunt of 2 volumes. In both cases also, the postoperative coronary arteriogram showed a return of the left coronary circulation to normal, disappearance of the large anastomatic vessels, and above all a return to normal dimensions of the right coronary artery, previously extremely tortuous and dilated. The 30 or so cases of total correction published in the world literature have been reviewed in the light of these two new cases.

[Resistance and facilitation gradient in synchronized cardiac stimulation in the dog].

Electrical stimulation synchronized on spontaneous or paced cardiac rhythms demonstrates, in dogs : 1) phenomena of intermittent capture (I.C.), and intermittent loss (I.L.) of control, at all moments in the idastole ; 2) constant lowering of Excitability Threshold (E.T.) and Pacing Threshold (P.T.). These experiments confirm the existence of Resistance Gradients (including I.C. and I.L. phenomena) and Facilitation Gradients (measuring E.T. and P.T. lowerings). These gradients -- that we previously described in human pacing -- are parameters of the cardiac excitability.

[Granulopenia of infectious origin].

Among the various infectious and parasitic disease liable to produce granulopenia, the authors selected certain diseases which represented their personal experience. Classically, leukopenia may occur during typhoid fever, but was not present in all of the series of 114 adults they observed with this disease. It only occurs after antibiotic treatment (Thiamphenicol in the majority of cases). During acute brucellosis (188 cases studied) granulopenia was constant. It occurs early and is lasting. It reappears during septicemic relapse. Leukopenia is corrected during treatment by tetracycline antibiotics. Neutropenia during disease is frequent but usually labile. This characteristic explains why there was no granulopenia in 90 cases of mumps and 64 cases of chickenpox. Finally, during Kala Azar, 8 cases confirmed certain already well known data: the considerable reduction in granulocytes but also anemia and thrombocytopenia.

[Pulmonary edemas of infectious origin].

Diffuse pulmonary edema, capable of arising in the absence of hemodynamic disorders is rare in infectious disease. They take on two different clinical appearances: a) acute edema of the lung with the syndrome of asphysia, b) a subacute dyspneic pneumonia with hypoxemia and hypo or normocapnia. These initial disorders can be followed by progressive respiratory failure secondary to the development of diffuse interstitial lesions with fibrosis and intra-alveolar hyaline deposits. The bronchiolo-alveolar lesions which induce a fibrin rich exudate are directly caused by the patogenic agent: myxovirus, essentially influenzae, and more rarely adeno or herpes virus. The role of bacteria and of certain parasites is more debateable.
