Dissociation of biochemical and hypotensive effects of debrisoquine in hypertensive patients.

The 24 h urinary excretion of adrenaline, noradrenaline, metadrenaline, normetadrenaline and vanillylmandelic acid, plasma renin activity and plasma and urinary debrisoquine were measured before and during chronic treatment with oral debrisoquine in 14 in-patients with essential hypertension. There was a significant fall (mean +/- SD) in the 24 h urinary excretion of vanillylmandelic acid (15.3 +/- 2.8 to 6.7 +/- 1.9 micronmol) noradrenaline (199.0 +/- 105.8 to 125.2 +/- 43.3 nmol) and plasma renin activity (0.71 +/- 0.47 to 0.40 +/- 0.20 pmol Angio I ml-1 h-1) while the urinary normetadrenaline/noradrenaline ratio increased (10.4 +/- 6.1 to 17.1 +/- 5.1). No significant change was seen in the output of adrenaline or of O-methylated metabolites. Debrisoquine produces extensive noncompetitive inhibition of platelet monoamine oxidase in vivo at low therapeutic plasma concentrations. These changes support the view that treatment with debrisoquine produces intraneuronal inhibition of monoamine oxidase and post-ganglionic blockage. There was a significant correlation between the change in standing diastolic blood pressure and the daily dose (rs = -0.52), pre-dose plasma concentration (rs = -0.85) and mean daily urinary recovery (rs = -0.80), of debrisoquine. The full extent of the biochemical changes were seen at low dose and low plasma concentration and were not directly correlated with the fall in standing or supine blood pressure.

What the patients say: A study of reactions to an intensive care unit.

One hundred consecutive patients were interviewed between the 3rd and 7th days, inclusive, of their discharge from an intensive care unit to a general ward. The patients' recall of events related to their admission to the ICU was generally poor, and 41% of them felt that they had been confused at some time during their stay though much of this seems to have passed unnoticed by the nursing staff. Lack of sleep was a problem to about a quarter of the patients and 75% of these thought that the inability to lie comfortably was a factor preventing sleep. Other factors included pain, anxiety and noise. The patients seemed satisfied whilst in the ICU, and less than half said they were pleased to return to a general ward.

Genetic analysis of 23 RhL-A histocompatibility antigens of the rhesus monkey.

The lymphocytotoxicity of 116 rhesus monkey alloantisera was evaluated in 92 unrelated rhesus monkeys and in 33 pedigreed rhesus families. The study was conducted using a standard complement-dependent microcytotoxocity assay. A computer-assisted chi2 analysis of the reactivity of these sera in unrelated monkeys generated 23 groups of highly correlated antisera. The two-locus model of the mammalian major histocompatibility complex was assumed for the monkey, and genetic criteria for RhL-A antigens were determined before study. Seventeen groups of antisera which had met these predetermined criteria in a previous study using a different random population of monkeys were confirmed in the present analysis. The remaining six groups also met these predetermined criteria, although the genetic data for two were incomplete. Of the 23 antigens defined, 12 appeared to be products of the A locus and 11 of the B locus. Nineteen were similar or identical to antigens previously described by us or by Balner and coworkers in The Netherlands. Two groups have not been previously described. A frequency analysis indicated that these 23 antigens represented approximately 75% of the total expression of the RhL-A-A and RhL-A-B loci.

Byssinosis and chronic respiratory disease in U.S. cotton gins.

A pilot study of cotton gin workers was undertaken in the ginning industry so as to determine if a definitive study could be undertaken and to make some first order estimates of byssinosis and chronic respiratory disease and to compare the findings to those reported in the African and European gins. Baseline medical data were collected on 203 gin workers and 260 controls, consisting of a medical symptom questionnaire, a chest x-ray and the forced expiratory spirogram pulmonary test. Results revealed a prevalence of functional reactors equal to that found in the textile industry and other gin studies (44%) as determined by spirometric testing, without any association to smoking history. The major reaction group within the ginning population was those working in the baling press area. These same workers demonstrated evidence of chronic depressions in their spirometry data. No excess of chronic respiratory disease as determined by the questionnaire was detected within the gin worker population.

Reducing the pill swill: an audit of clinical pharmacy.

A drug surveillance programme involving pharmacists in a general medical ward has been shown to reduce the number of drugs prescribed to patients and also to reduce the number of dispensing errors. The evidence presented suggests that continuing involvement of pharmacists in patient care at the ward level is required to maintain the improved prescribing habits.

Schistosomiasis haematobium.

In our patient, schistosomiasis haematobium was not diagnosed and was treated as a urinary tract infection by several physicians. Physicians are urged to become increasingly aware of the need to recognize strange and exotic diseases normally not present in this country.

Circulating thymic-hormone activity in congenital immunodeficiency.

Circulating thymic-hormone activity was assayed by measuring Thy 1-2 antigen induction on null lymphocytes from athymic mice incubated with human plasma or serum. Plasma from 19 normal children aged under 10 had inductive activity equivalent to 10-6-16-2 ng thymopoitin/ml. Plasma from 15 infants were severe combined immuno-deficiency, 2 of whom had appreciable immunoglobulin synthesis, and from 2 infants with DiGeorge syndrome had little or no inductive activity. Successful reconstitution with thymus or bone-marrow grafts and with red-cell infusions (if adenosine-deaminase deficiency is present) was followed by a rise in circulating thymic-hormone activity.

Medical student performance skills in otolaryngology.

A clinical rating scale was developed to assess clinical skills of third-year medical students completing a two-week required clerkship in otolaryngology. Thirty-seven students in six consecutive small groups were rated with the instrument by themselves, their student peers, and the departmental staff, and the ratings compared. The instrument was usable and aceeptance was good. Agreement among the differenet rating bodies was poor, but single observations by staff or peers were suitably reliable when multiple raters were used. The "halo effect" was particularly evident in staff ratings.
