We applied monoclonal antibody typing and restriction endonuclease analysis of plasmid DNA to study 28 clinical and 35 environmental (potable water) isolates of Legionella pneumophila serogroup 1 from three hospitals in Iowa between 1981 and 1986. Monoclonal antibody typing employed a panel of seven antibodies and delineated eight different subtypes. Plasmids were present in 57% of the isolates including 12 of 28 (43%) clinical and 25 of 35 (69%) potable water isolates. The plasmids ranged in size from 28 to 98 kilobase pairs and comprised eight distinct subtypes by restriction endonuclease analysis with Eco RI. Combination of monoclonal antibody and restriction endonuclease subtyping (composite subtyping) revealed 19 different composite subtypes of Legionella pneumophila serogroup 1. The most common composite subtype, 09:04, comprised 29% (18 of 63) of the isolates and was only found in clinical and potable water samples from a single pavilion in hospital A during an outbreak of Legionella pneumophila serogroup 1 pneumonia. Aside from this cluster the diversity of composite subtypes of Legionella pneumophila serogroup 1 observed in clinical and potable water sources over the 5-year period was striking. The combination of monoclonal antibody and restriction endonuclease typing resulted in improved strain delineation and a more useful use of epidemiologic markers for Legionella pneumophila serogroup 1.
Studies on the prevalence of tinea pedis, a frequently encountered dermatophytic infection, have been conducted mostly in swimmers although people who regularly practise other types of physical activities may also have a high rate of clinical or subclinical infection. This investigation was undertaken to establish the rate of infection in marathon runners, and to determine the incidence of occult athlete's foot disease in this population. Among samples obtained from 405 individuals, 22% were positive. The rate of infection was highest in the older age groups. The prevalence of infection was 24.2% in men and 6.1% in women. Trichophyton rubrum and Trichophyton mentagrophytes were the two species of dermatophytes most commonly isolated on culture. Occult athlete's foot disease represented 48% of cases with a positive culture. Finally, routine sampling of both feet was confirmed necessary to adequately establish the rate of infection: 26.9% of cases with a positive culture would have been missed by unilateral sampling. Other epidemiological factors were not clearly linked to the prevalence of disease in marathon runners: weight; presence of pet animals; practice of other sports; race and country of origin. In conclusion, we establish that marathon runners represent a population at risk for the occurrence of both clinical and subclinical tinea pedis infection.
The cardiac complications of radiotherapy of the thorax (e.g. for Hodgkin's disease and carcinoma of the breast) are various: 1) Pleural effusions, occurring a few months to two years after radiotherapy, spontaneously curable but may give signs of tamponnade or lead to constriction. 2) Constrictive pericarditis often several years after irradiation, with a poor prognosis in spite of attempts at pericardectomy which is indicated at the stage of hemodynamic constriction. 3) Myocardial involvement with advanced fibrosis and with various clinical presentations. The failure of pericardectomy and mitral incompetence were evidence of myocarditis in the case reported here.

189 patients treated from 1970 to 1979 were reviewed. The isolated prognostic value on 10-year survival of age, histology, sex, tumor size and metastases was confirmed. Using Cox model, histology, age and sex appeared to be sufficient to settle a prognosis, while histology was the most determining prognostic factor. From these 3 variables, a prognosis score was proposed. Patients were distributed among 3 prognostic groups, the 10 years survivals of which were respectively 93%, 57% and 28% (p less than 0,001).
