The role of oxidative stress in chemical carcinogenesis.

Oxidative stress results when the balance between the production of reactive oxygen species (ROS) overrides the antioxidant capability of the target cell; oxidative damage from the interaction of reactive oxygen with critical cellular macromolecules may occur. ROS may interact with and modify cellular protein, lipid, and DNA, which results in altered target cell function. The accumulation of oxidative damage has been implicated in both acute and chronic cell injury including possible participation in the formation of cancer. Acute oxidative injury may produce selective cell death and a compensatory increase in cell proliferation. This stimulus may result in the formation of newly initiated preneoplastic cells and/or enhance the selective clonal expansion of latent initiated preneoplastic cells. Similarly, sublethal acute oxidative injury may produce unrepaired DNA damage and result in the formation of new mutations and, potentially, new initiated cells. In contrast, sustained chronic oxidative injury may lead to a nonlethal modification of normal cellular growth control mechanisms. Cellular oxidative stress can modify intercellular communication, protein kinase activity, membrane structure and function, and gene expression, and result in modulation of cell growth. We examined the role of oxidative stress as a possible mechanism by which nongenotoxic carcinogens may function. In studies with the selective mouse liver carcinogen dieldrin, a species-specific and dose-dependent decrease in liver antioxidant concentrations with a concomitant increase in ROS formation and oxidative damage was seen. This increase in oxidative stress correlated with an increase in hepatocyte DNA synthesis. Antioxidant supplementation prevented the dieldrin-induced cellular changes. Our findings suggest that the effect of nongenotoxic carcinogens (if they function through oxidative mechanisms) may be amplified in rodents but not in primates because of rodents' greater sensitivity to ROS. These results and findings reported by others support a potential role for oxidative-induced injury in the cancer process specifically during the promotion stage.

Evaluation of universal preenrichment broth for the recovery of foodborne pathogens from milk and cheese.

The use of universal preenrichment broth for the recovery of verotoxigenic Escherichia coli, Salmonella spp., and Listeria monocytogenes from milk and cheese was examined. Universal preenrichment broth supported the growth of low inoculum levels (10 cfu/ml) of these organisms in pure cultures and in mixed cultures containing higher levels of other pathogens or bacterial flora from raw milk. This medium also supported the recovery and growth of heat-injured Salmonella spp., L. monocytogenes, and verotoxigenic E. coli at inoculum levels of 10(2) cfu/ml to yield cell levels of 10(8) cfu/ml in pure cultures and at least 10(5) cfu/ml in the presence of high levels of known competitive pathogens or microflora of cheese samples after 24 h of incubation. Universal preenrichment broth performed better than Listeria enrichment broth in supporting the recovery and growth of heat-injured L. monocytogenes and equally as well as buffered peptone water or trypticase soy broth in supporting the growth of uninjured L. monocytogenes, Salmonella spp., and verotoxigenic E. coli. Coenrichment of these pathogens in universal preenrichment broth reduced the quantity of milk or cheese samples that were required for analysis and also reduced the cost and labor involved in preparing and processing separate preenrichment media.

A field trial of detection and treatment of nerve function impairment in leprosy--report from national POD pilot project.

As part of a collaborative project between the Ministry of Health of China (MOH) and The Leprosy Mission International (TLMI) on leprosy rehabilitation and prevention of disability (POD), a total of 1407 patients was monitored for possible nerve function impairment (NFI) through standardized clinical nerve function assessment between May 1995 and February 1998. Of these, 191 patients were found to have NFI and were put on a fixed regimen of prednisolone. In this study, 36.7% of NFI occurred before diagnosis of leprosy, 35.6% developed during MDT and 25.7% after their release from MDT. Overall, 7.5% (105 out of 1407) of all patients, or 55.9% of patients with NFI, suffered from silent neuropathy. Of the affected nerves, 62.6% had silent neuropathy. Sensory impairment responded to prednisolone satisfactorily, giving a recovery rate of 73.8%, 76.5% and 81.0% in ulnar, median and posterior tibial nerve, respectively. Sensibility in patients even with a NFI duration longer than 6 months made significant improvement (p < 0.05). Motor function improvement was less satisfactory, especially in ulnar and c. popliteal nerve. The possible reasons are analysed. Our findings with regard to sensibility changes confirm that once it becomes clinically detectable, NFI is no longer at the 'early' stage. More sensitive tests are necessary to detect real 'early' sensory impairment in the field. Our study also indicates that with well-trained field staff and proper equipment for nerve function assessment, early detection and treatment of NFI can be practical and effective.

Evaluation and characterization of congenital hypothyroidism in rdw dwarf rats.

The rdw rat is a new strain of dwarf mutant that has decreased blood thyroxine (T4) and growth hormone (GH) concentrations and testicular enlargement during development and aging. To confirm whether this strain can be used as a new hypothyroid model, the experiments reported here were carried out, using adult rdw rats, rdw rats treated with thyroxine, and clinically normal (N) Wistar-Imamichi rats. Clinical parameters of deficient thyroid function in rdw rats were chosen for evaluation and characterization. Body weight, hemoglobin (Hb) concentration, hematocrit (Hct), glucose (GLU), and systolic blood pressure were significantly lower, and serum values for aspartate transaminase (AST), total cholesterol (TC), total protein (TP), and blood urea nitrogen (BUN) were higher in rdw than in N rats. Serum concentrations of total T4 and free triiodothyronine (FT3) were significantly lower, and serum thyroid-stimulation hormone (TSH) concentration was markedly higher in rdw than in N rats. Serum GH concentration was significantly lower in rdw than in N rats. Results of histologic examination indicated that the thyroid gland of rdw rats was markedly atrophied, compared with that of N rats. Results of clinical examination of organs and hematologic and biochemical values in rdw rats corresponded to those of the hypothyroid state in humans. Most organ weights (heart, kidney, spleen, and adrenal gland), hematologic and biochemical values (Hb, Hct TC, TP, BUN), blood pressure, and serum hormone (TSH and GH) values underwent substantial restoration (partial or complete) toward normal in response to replacement therapy. In conclusion, the rdw rat is a useful model of congenital hypothyroidism.

Development of serial bio-shock tubes and their application.

OBJECTIVE: To design and produce serial shock tubes and further examine their application to experimental studies on blast injury. METHODS: Bio-medical engineering technique was used for the design and development of the serial shock tubes. One thousand four hundred and fifty nine animals (757 rats, 105 guinea pigs, 335 rabbits, 240 dogs and 22 sheep) were then used to test the wounding effects of the shock tubes. RESULTS: Three types of bio-shock tubes, that is, large-, medium- and small-scale shock tubes were made in our laboratory. The large-scale shock tube is 39 meters long; the inner diameter of the test section is 1 meter; and the maximum overpressure in the driving section is 10.3 MPa. A negative pressure could be formed by means of the reflected rarefactive wave produced by the end plate. The medium-scale shock tube is 34.5 meters long; the maximum overpressure in the driving section is 22 MPa; the test section is designed to be a knockdown, showing 5 basic types with inner diameter of 77 to 600 millimeters, which could be used for researches on overpressure, explosive decompression, underwater explosion, and so on. The small-scale shock tube is 0.5 meter long with the maximum endured overpressure of 68.6 MPa. Results from animal experiments showed that this set of shock tubes could induce various degrees of systemic or local blast injury in large or small animals. CONCLUSIONS: This set of bio-shock tubes can approximately simulate typical explosive wave produced by nuclear or charge explosion, and inflict various degrees of blast injury characterized by stability and reproducibility. Therefore, they can meet the needs of blast research on large and small animals.

Distribution of endotoxins in tissues and circulation and its effects following hemorrhagic shock.

OBJECTIVE: To systemically investigate 1) distribution of endogenous endotoxin (ET) in tissues and circulation; 2) its relationship with shock duration and organ damage; and 3) its possible mechanism after hemorrhagic shock. METHODS: To further elucidate the intrinsic relationship between endogenous endotoxin translocation and hemorrhagic shock, the present study systematically investigated the distribution of endogenous ET into the liver, lungs, kidneys and circulation, and the relationship between ET levels and the corresponding organ dysfunction with limulus amebocyte lysate (LAL) chromogenic assay following hemorrhagic shock in rats. RESULTS: It was found that ET levels in hepatic homogenate markedly increased (P = 0.09) 1.5 hours following shock compared with that in the sham group. After resuscitation, ET levels in hepatic, pulmonary and renal tissues were all significantly elevated. The levels kept increasing with the prolonged experimental time, and reached as high as 3.88 +/- 0.95 EU (endotoxin unit)/g in the livers, 2.53 +/- 1.46 EU/g in the lungs and 2.51 +/- 0.89 EU/g in the kidneys 12 hours after shock. ET levels in plasma reached a peak of 1.13 +/- 0.42 EU/ml at 1 hour following resuscitation, then rapidly decreased to the sham levels 3 hours following resuscitation. There was a close relationship between endotoxin translocation and shock duration. Correlation analysis further indicated that the changes in glutamic-pyruvic transaminase (GPT), blood urea nitrogen (BUN) in plasma and angiotensin I-converting exzyme (ACE) in pulmonary homogenate were significantly and positively correlated with the ET levels in the liver, kidneys and lungs after hemorrhagic shock. CONCLUSIONS: Hemorrhagic shock can induce obvious endogenous ET translocation, which is closely related to the shock duration. Although only transient endotoxemia occurs after hemorrhagic shock, ET can massively accumulate in tissues (liver, lungs and kidneys), and may play an important role in the development of shock.

Employment of trauma and injury severity score and a severity characterization of trauma in the outcome evaluation of trauma care and their research advances.

OBJECTIVE: To review the application of trauma and injury severity score (TRISS) and a severity characterization of trauma (ASCOT) in the outcome evaluation of trauma care and their research advances. DATA SOURCES: Both Chinese- and English-language literature searched by using MEDLINE/CD-ROM (1985-1996) and Index of Chinese-Language Literature (1985-1996). STUDY SELECTION: Over fifty papers and reviews published over the past ten years were selected. RESULTS AND CONCLUSION: TRISS can be employed for different purposes, that is, preliminary outcome-based evaluation (PRE) and definitive outcome-based evaluation (DEF). TRISS is a method which is now the most extensively used for the outcome evaluation of trauma. Even so, it still has some shortcomings, e.g., trauma can not be given the weights that should be given, and the section of age is too simple. ASCOT is also a physiologic and anatomic combined method for the evaluation of injury severity and outcome. To some extent, this method obviates the shortcomings of TRISS in the calculation of probability of survival (Ps) with injury severity score (ISS). Therefore, ASCOT is considered to be superior to TRISS in the evaluation of Ps. However, TRISS is still now more extensively used than ASCOT just because ASCOT was recently developed.

A clinical study on bing gan ling oral liquid for treatment of hepatitis C.

Based on the principles of cooling the blood to expel the pathogenic toxins and regulating the function of liver and activating the spleen in traditional Chinese medicine, 60 cases of chronic hepatitis C were treated with Bing Gan Ling (BGL), an oral liquid, which is composed mainly of Shuiniujiao (Cornu Bubali), Huzhang (Rhizoma Polygoni Cuspidati), Chishao (Radix Paeoniae Rubra), and Huangqi (Radix Astragali), etc. The total effective rate was 86.7%, which is considerably better than that of the control group (P < 0.01). The animal experiments and pathological studies suggest that the above-mentioned prescription has marked action of protecting the liver and lowering the activity of enzymes; and that it can ameliorate the acute liver injury induced by D-galactosamine.

[Effects of matrine on telomerase activity and cell cycle in K562 cell].

OBJECTIVE: In order to search for new tumor differentiation inducer, the effects of matrine of Chinese herbal origin on differentiation of K562 cells and its mechanism were studied. METHODS: Telomerase activity was determined by PCR-ELISA in untreated or treated K562 cells, cell cycle phases were analyzed by fluorescence-activated cell sorter analysis. RESULTS: Telomerase activity of K562 cells was significantly inhibited when treated with matrine, and cell cycle of the treated K562 cells was changed with marked decrease in S phase. CONCLUSION: Matrine can induce differentiation of K562 cells, the mechanism of which might be related to the inhibition of telomerase activity and cell cycle arrest.

[Compartmental distribution of hepatitis C virus quasispecies in mononuclear cells and liver].

OBJECTIVE: To explore the role of the infection of monouclear cells by hepatitis C virus(HCV) in the chronicity of HCV infection. METHODS: Direct sequencing of the amplified hypervariable region (HVR) gene of HCV was used to distinguish main HCV quasispecies infecting non-hepatocytes from those infecting hepatocytes and circulating virions. The main HCV quasispecies in peripheral blood mononuclear cells(PBMC), liver-associated mononuclear cells (LAMC), serum and liver samples from 6 patients with chronic hepatitis C and from 6 patients with HCV-related cirrhosis were analysed and compared. RESULTS: The main HCV qusispecies in the livers were different from those in PBMC in 3 transplanted cirrhotic patients and 5 patients with chronic hepatitis C. Different substitutions existed between PBMC and LAMC in 3 of 6 patients analysed. Mutations were observed within the first 50 amino acids of E2 gene, which induced amino acid change in 6 of 9 patients analysed. CONCLUSION: In chronically infected patients, the detection of HCV RNA in mononuclear cells is not due to the adsorption of circulating virions but a significant infection. The compartmental distribution of HCV quasispecies in hepatocytes and mononuclear cells could be involved in the chronicity of HCV infection.

[Effects of hypertonic solution on the membrane potential of isolated outer hair cell in the guinea pig cochlear].

To learn effects of hypertonic solution on the membrane potential and cell length of isolated outer hair cell in the guinea pig cochlear, we determined the membrane potential and cell length during hypertonic activation with ACAS 570 and the membrane potential sensitive probe DiBAC4(3). The results showed that the hypertonic activation depolarized the outer hair cell and made it elongate. We think that the outer hair cell has activity, the depolarization and elongation of the outer hair cell caused by hypertonic solution are cyto-electrophysiological basis of hearing loss and recruitment in Meniere's disease.

[Induction of apoptosis and differentiation in human leukemia cell line HL-60 by synthetic retinoid Ro13-7410].

OBJECTIVE: To investigate the effect of the synthetic retinoid Ro13-7410 on HL-60 cells. METHODS: Trypan blue exclusion was used to determine viability. Apoptosis was assessed by cell morphology and PCD-assay-kit. Differentiation was assessed by morphology and NBT reduction. RESULTS: Incubation of the HL-60 cells with 10(-6)-10(-8) mol/L Ro14-7410 resulted in the suppression of cell growth, the induction of cell apoptosis and differentiation. CONCLUSION: Ro13-7410 suppressed HL-60 cell growth mainly via induction of apoptosis.

[Diagnosis and surgical treatment of tuberculosis of mesentric lymph nodes].

OBJECTIVE: To evaluate the diagnosis, operative indication and therapeutic result of tuberculosis of mesentric lymph nodes. METHOD: Fifty-nine cases with tuberculosis of mesentric lymph nodes surgically treated at two hospitals from July 1976 to July 1997 were retrospectively analyzed. RESULT: Only 14 cases were correctly diagnosed before operation. Operative treatment for the tuberculosis of mesentric lymph nodes represented a safe, effective treatment with few complications and no operative death. CONCLUSION: Because patients with tuberculosis of mesentric lymph nodes show no characteristic manifestations, laboratory methods (e.g. polymerase chain reaction), peritoneoscopy, CT scanning and B mode ultrosound techniques should also be considered for diagnosis of the disease. The acute or chronic ileus, hemorrhage of digestive tract, intestinal perforation or fistula, intra-abdominal large tuberculous abscess and abdominal sinus caused by tuberculosis of mesentric lymph nodes should be surgically treated. The exploratory laparotomy also should be performed on the patients with abdominal mass or those whose diagnosis of neoplasm could not be ruled out. The postoperative antituberculosis chemotherapy should be continued to decrease the occurrence of complications.

[The investigation of children's Helicobacter pylori infection by 13C-urea breath test in Yangzhong area of Jiangsu province].

OBJECTIVE: Yangzhong area in Jiangsu province is one of the places in China with high gastric cancer incidence (73.19/1000,00). In order to investigate the relations between gastric cancer and helicobacter pylori infection rate. METHODS: 13C-urea breath test was conducted for 119 children aged from 4 to 12 years old. RESULTS AND CONCLUSIONS: Total H. pylori infection rate was 73.11%, and 81.08% for boys, 60.00% for girls.

[An experimental observation of repair after transection of cauda equina fibers in cat].

OBJECTIVE: To explore the regeneration characteristics after cauda equina transection. METHODS: Fibrin glue was chosen to repair transected left L6,7 roots in cat, and right roots in situ of the transected ends were served as the control. The characteristics of regeneration were observed by immunohistocytochemistry and morphometric analysis. RESULTS: Regeneration was found not only in site of anastomosis, but also in corresponding spinal segments and DRGs. Sensory and motor evoked potentials (SEP and MEP) performed before and after transection and before taking specimen revealed the course of functional recovery on severed and regenerated nerve roots. The regeneration of moter roots was better than that of sensory roots, and they were mainly blocked at the site of central-peripheral transition zone. CONCLUSIONS: The method of adhesion by fibrin glue can replace traditional suture, and it is superior to suture.

[Phacoemulsification and foldable intraocular lens implantation through a clear corneal incision].

OBJECTIVE: To evaluate the therapeutic effects of phacoemulsification with foldable intraocular lens implantation through a clear corneal incision. METHODS: The surgery was performed through a 3.2 mm clear corneal incision on 100 eyes of 90 patients with senile, complicated, congenital and traumatic cataracts. RESULTS: Postoperatively, the visual acuities with spherical correction or without correction were 0.5 or better in 93.0% of eyes at one day, and 1.0 or better in 56.0%, 65.1% and 67.5% at one week, one month and three months respectively. The mean postoperative astigmatism was 0.93 +/- 0.58 D at one week and 0.20 D more than the mean preoperative one (P < 0.001). There was no statistical difference between the mean preoperative astigmatism and postoperative astigmatism at one and three months which were respectively 0.78 +/- 0.60 D and 0.73 +/- 0.59 D. CONCLUSION: The phacoemulsification with foldable intraocular lens implantation through a clear corneal incision can reduce early postoperative astigmatism, shorten the period of visual rehabilitation and simplify operative procedures.
