Insulin receptor substrate-1 enhances growth hormone-induced proliferation.

GH exerts a variety of metabolic and growth-promoting effects. GH induces activation of the GH receptor (GHR)-associated cytoplasmic tyrosine kinase, JAK2, resulting in tyrosine phosphorylation of the GHR and activation of STAT (signal transducer and activator of transcription), Ras-mitogen-activated protein kinase, and phosphoinositol 3-kinase signaling pathways, among others. GH-stimulated tyrosine phosphorylation of insulin receptor substrate (IRS) proteins has been demonstrated in vitro and in vivo. IRS-1 is a multiply phosphorylated cytoplasmic docking protein involved in metabolic and proliferative signaling by insulin, IL-4, and other cytokines, but the physiological role of IRS-1 in GH signaling is unknown. In this study, as noted by others, we detected in murine 3T3-F442A pre-adipocytes GH-dependent tyrosine phosphorylation of IRS-1 and specific GH-induced coimmunoprecipitation with JAK2 of a tyrosine phosphoprotein consistent with IRS-1. We further examined this interaction by in vitro affinity precipitation experiments with glutathione-S-transferase fusion proteins incorporating regions of rat IRS-1 and, as a source of JAK2, extracts of 3T3-F442A cells. Fusion proteins containing amino-terminal regions of IRS-1 that include the pleckstrin homology, phosphotyrosine-binding, and Shc and IRS-1 NPXY-binding domains, but not those containing other IRS-1 regions or glutathione-S-transferase alone, bound JAK2 from cell extracts. Tyrosine-phosphorylated JAK2 resulting from GH stimulation was included in the amino-terminal IRS-1 fusion precipitates; however, neither tyrosine phosphorylation of JAK2 nor treatment of cells with GH before extraction was necessary for the specific JAK2-IRS-1 interaction to be detected. In contrast, in this assay, specific insulin receptor association with the IRS-1 phosphotyrosine-binding, and Shc and IRS-1 NPXY-binding domains was insulin and phosphotyrosine dependent, as previously shown. To test for significance of IRS-1 with regard to GH signaling, IRS- and GHR-deficient 32D cells were stably reconstituted with the rabbit (r) GHR, either alone (32D-rGHR) or with IRS-1 (32D-rGHR-IRS-1). As assayed by three independent methods, GH induced proliferation in 32D-rGHR cells, even in the absence of transfected IRS-1. Notably, however, GH-induced proliferation was markedly enhanced in cells expressing IRS-1. Similarly, GH-induced mitogen-activated protein kinase activation was significantly augmented in IRS-1-expressing cells relative to that in cells harboring no IRS-1. These results indicate that IRS-1 enhances GH-induced proliferative signaling.

Short-term oxandrolone administration stimulates net muscle protein synthesis in young men.

Short term administration of testosterone stimulates net protein synthesis in healthy men. We investigated whether oxandrolone [Oxandrin (OX)], a synthetic analog of testosterone, would improve net muscle protein synthesis and transport of amino acids across the leg. Six healthy men [22+/-1 (+/-SE) yr] were studied in the postabsorptive state before and after 5 days of oral OX (15 mg/day). Muscle protein synthesis and breakdown were determined by a three-compartment model using stable isotopic data obtained from femoral arterio-venous sampling and muscle biopsy. The precursor-product method was used to determine muscle protein fractional synthetic rates. Fractional breakdown rates were also directly calculated. Total messenger ribonucleic acid (mRNA) concentrations of skeletal muscle insulin-like growth factor I and androgen receptor (AR) were determined using RT-PCR. Model-derived muscle protein synthesis increased from 53.5+/-3 to 68.3+/-5 (mean+/-SE) nmol/min.100 mL/leg (P < 0.05), whereas protein breakdown was unchanged. Inward transport of amino acids remained unchanged with OX, whereas outward transport decreased (P < 0.05). The fractional synthetic rate increased 44% (P < 0.05) after OX administration, with no change in fractional breakdown rate. Therefore, the net balance between synthesis and breakdown became more positive with both methodologies (P < 0.05) and was not different from zero. Further, RT-PCR showed that OX administration significantly increased mRNA concentrations of skeletal muscle AR without changing insulin-like growth factor I mRNA concentrations. We conclude that short term OX administration stimulated an increase in skeletal muscle protein synthesis and improved intracellular reutilization of amino acids. The mechanism for this stimulation may be related to an OX-induced increase in AR expression in skeletal muscle.

Adoptively transferable tolerance induced by CD45RB monoclonal antibody.

The phenomenon of rejection remains the most serious problem in transplantation. The ultimate goal in transplant immunology is to develop therapeutic strategies that lead to tolerance. It has been shown that two injections of a monoclonal antibody to CD45RB leads to indefinite acceptance of renal allografts in mice. Moreover, the CD45RB monoclonal antibody reverses acute rejection and still induces tolerance. The purpose of this study was to assess mechanisms that could underlie this therapeutic benefit. It was shown that splenic lymphocytes from tolerant animals augmented proliferation in allogeneic mixed lymphocyte reactions against donor alloantigens, and the serum of tolerant mice contained donor-specific antibodies, mainly of the IgG1 isotype, suggesting the presence of TH2 cytokines. Tolerance could not be broken by interleukin-2 infusion, but tolerance could be adoptively transferred by transfusion of tolerant mouse CD4+ splenic lymphocytes into naive allografted animals. These data suggest that an active immunoregulatory mechanism is partly responsible for the therapeutic effect. CD45RB-directed therapy may find clinical application in organ transplantation in human patients.

Recombinant human platelet-activating factor acetylhydrolase reduces the frequency of diabetes in the diabetes-prone BB rat.

Platelet-activating factor (PAF) has been implicated in the development of type 1 diabetes. Our previous studies have suggested that PAF inhibitors reduce insulitis and the frequency of diabetes in BB rats. In this study, serum PAF levels were reduced to address the hypothesis that PAF is important for the development of insulitis. From the age of 35 days on, DP-BB rats were treated with human recombinant PAF acetylhydrolase (rPAF-AH), which efficiently inactivates PAF. Our data indicate that intraperitoneal injections of rPAF-AH reduce the incidence of diabetes in the DP-BB rat. Daily intraperitoneal injections of 6.0 mg/kg body wt rPAF-AH reduced the frequency of diabetes in saline-injected rats from 90% (27/30) to 57% (17/30) (P = 0.004). As found by morphometric analysis on pancreatic islets, DP-BB rats protected from diabetes had less severe degrees of insulitis in a dose-dependent manner. DP-BB rats protected by rPAF-AH also had a higher percentage of insulin-positive cells in pancreas sections compared with those from diabetic animals. We therefore speculated that the beta-cells were protected from insulitis by rPAF-AH.

Hypoxia down-regulates placenta growth factor, whereas fetal growth restriction up-regulates placenta growth factor expression: molecular evidence for "placental hyperoxia" in intrauterine growth restriction.

Early placental development occurs in an environment of relative hypoxia. Hypoxia promotes angiogenesis and up-regulates vascular endothelial growth factor (VEGF) expression while it down-regulates placenta growth factor (PIGF) that possess 53% homology with VEGF. Morphological studies show poor placental vascular development and an increase in the mitotic index of cytotrophoblasts in intrauterine growth restriction (IUGR). We hypothesized that the reported relatively high oxygen level in the intervillous space in contact with IUGR placental villi will limit angiogenesis by changes in VEGF and PIGF expression and function. Western immunoblot analysis demonstrates a diametric expression of PIGF and VEGF proteins throughout pregnancy with PIGF levels increasing and VEGF levels decreasing, consistent with placental oxygenation. In IUGR placentae, the ratio of PIGF/GAPDH mRNA was increased by 2.3-fold (p < 0.03) and PIGF protein levels were also increased, (p < 0.05) as compared with gestationally-matched normal placentae. PIGF mRNA and protein were localized to the trophoblast bilayer and villous mesenchyme of the human placenta throughout gestation. In vitro studies demonstrated that increasing oxygen tension (hyperoxia) up-regulated PIGF protein in term placental villous explants, whereas hypoxic culture of a term trophoblast choriocarcinoma cell line (BeWo) down-regulated PIGF mRNA and protein and VEGFR-1 (Flt-1) autophosphorylation. The addition of PIGF-1 to a spontaneously transformed first trimester cytotrophoblast cell line stimulated DNA synthesis while PIGF-2 had little effect. VEGF and PIGF exert their biological actions by means of a common receptor VEGFR-1. In the first trimester trophoblast cells, PIGF-1 increased the association of phosphorylated extracellular signal-related kinase (ERK) with VEGFR-1 immunoprecipitates while both PIGF-1 and PIGF-2 also potentiated endogenous VEGF mediated association of phosphorylated extracellular related kinase (ERK) with VEGFR-2 (KDR). More importantly, the addition of PIGF-1 had little effect while PIGF-2 inhibited cell growth in cultured endothelial cells derived from human umbilical vein. Nitric oxide (NO) is reported to promote angiogenesis and PIGF-2 inhibited the basal release of NO from the first trimester trophoblast. The tissue expression and functional studies support the hypothesis of "placental hyperoxia" in early-onset IUGR because hypoxia down-regulates trophoblast PIGF levels, PIGF expression is increased in IUGR, and PIGF-2 inhibits endothelial cell growth. Taken together, these changes provide a cellular explanation for the observed poor angiogenesis in the pathogenesis of IUGR and show that the two PIGF isoforms may modulate trophoblast and endothelial cell function differently, possibly through potentiation of VEGF mediated activation of VEGF-2.

Prodrug approaches to the improved delivery of peptide drugs.

Undesirable pharmaceutical and biopharmaceutical properties, which include low water solubility, poor stability, and low permeability through biological membrane barriers, often hinder the clinical development of biologically active peptides. Finding solutions to these problems is a contemporary issue in developing clinically the vast number of biologically active peptides as drugs. In recent years, significant progress has been made in developing prodrug approaches for the improvement of the water solubility, stability, and membrane permeability of peptides. For improving water solubility, the focus has been on the bioreversible introduction of ionizable functional groups to peptides, which helps to increase the polarity and thus water solubility of the peptide drugs. For improving stability, efforts have focused on stabilizing peptides against exopeptidase-mediated hydrolysis by bioreversibly masking the terminal carboxyl and/or amino groups. For improving permeability through biological barriers, recent efforts have focused on both improving the lipophilicity of a peptide in order to facilitate its passive permeation through biological membranes and conjugation of a peptide to a carrier which allows for the active transport of the peptide-carrier conjugate. Many of the prodrug systems developed recently have the potential to be used clinically for the delivery of peptide drugs to the desired site of action.

[A preliminary study on the genetic susceptibility of asthma in a Chinese population].

OBJECTIVE: To study the asthma genetic susceptibility by way of ccandidate region so as to accumulate data on the related loci in Chinese population and determine whether genetic susceptibility to asthma is linked to the chromosome region. METHODS: One hundred ninety-two samples from 32 families were collected from Zhaoan county in Fujian province. Two Rsa I polymorphic sites within uncoded region of the high-affinity IgE receptor beta chain gene (Fc(epsilon) RI-beta) located on chromosome 11q13 were detected by PCR/Rsa I restriction endonuclease digestion. The polymorphic markers D5S436 and D5S393 within chromosome 5q31-33 were amplified by PCR incorporated with radioactive isotope. The asthma family samples were compared with unrelated samples randomly selected from general population. RESULTS: Genotypes containing A allele within intron 2 polymorphic site of Fc(epsilon) RI-beta gene were associated with asthma (P<0.05,OR = 2.039). Significant difference in the level of total serum IgE was noted among three genotypes of intron 2 polymorphic site of Fc(epsilon) RI-beta gene (P<0.05). The association between A allele and elevated total serum IgE was also significant (P<0.05, RR = 1.361). No relationship between exon 7 polymorphic site of Fc epsilonRI-beta gene and asthma was shown. By sib-pair analysis, significant evidence for linkage to asthma was observed with D5S436 (P<0.05) but not with D5S393. No significant linkage to total serum IgE was identified with both of these markers. CONCLUSION: The above-accumulated data suggested that both Fc epsilonRI-beta gene and chromosome 5q31-33 could be very attractive candidate gene and region for further studies in Chinese population, and to confirm these preliminary results, more markers within these regions merit testing in additional population samples.

[A preliminary study of developmental genetics of polymorphic serum esterase in domestic fowls].

Polymorphism of serum esterase was examined in 3 Super-Y 2000 broiler breeding populations differing in developmental stages (i.e. day 120, 180 and 300) using vertical polyacrylamide gel electrophoresis. The results indicated that both Es-1 and Es-2 loci exhibited polymorphism. Two to three polymorphic enzyme bands were detected at zone Es-1, and one band at zone Es-2 after electrophoresis. It was first discovered in this work that obvious developmental difference in products of Es-1 alleles existed with female birds, polymorphic bands of which disappeared after onset of lay. An individual tracing experiment conducted in a Super-Y 380 commercial layer population and an extensive experiment carried out in 2 ISA B380 parent CD stocks supported the previous finding. It could thus be inferred that phenotype "O" (no band at Es-1) discovered in hens was not genetically controlled by the recessive allele Es-1(0), which itself did not exist, but might be an evidence of gene regulation at laying period. For female birds, expression of Es-1 alleles was active before sexual maturity, and depressed after onset of lay, to meet their physiological need of high level of blood esterase for laying. It could also be inferred that this developmental difference in serum esterase polymorphism might be a common phenomenon in all avian species. If this hypothesis were true, the synthesis of esterase inside avian bodies could be a desirable model for investigating expression, and regulation of expression, of genes in avian populations.

[Experimental study of ta chengchi tang decoction for relieving lung injury during acute necrotizing pancreatitis].

The objectives of the study were to investigate the changes of leukocyte adhesiveness and tumor necrosis factor (TNF) in the early stage of acute necrotizing pancreatitis (ANP), to go further into the relation of those changes to lung injury of ANP, and to evaluate the prohibitive effect of ta chengchi tang Decoction on leukocyte adhesion and TNF secretion. 14 canines were randomly divided into 3 groups. The treatment group (n = 5): experimental ANP model treated with ta chengchi tang Decoction; the saline control group (n = 5): experimental ANP model treated with normal saline; and the sham operative group (n = 4). The adhesion of leukocytes to vascular endothelial cells and TNF activities in serum and in bronchi-alveolar lavage fluid (BALF) were observed and measured, and the degrees of lung injury were assessed. The results showed that the adhesiveness of leukocytes was markedly increased and the activities of TNF were elevated in peripheral blood and BALF during ANP; that the leukocyte adhesiveness in the treatment group was weaker than that in the saline group (P < 0.05) and the serum and BALF TNF activities were also significantly lower than those in the saline group (P < 0.01); and that the degrees of lung injury in the treatment group were significantly milder than those in the saline group. These indicate that the increased leukocyte adhesiveness and hypersecretion of TNF take part in the pathogenesis of lung injury in ANP, and ta chengchi tang Decoction is demonstrated an efficacious medicine for alleviating the degree of lung injury mediated by both the leukocyte adhesion and TNF in ANP.

[A preliminary study on hepatitis E virus antibody IgG and IgM for the diagnosis of acute hepatitis E].

OBJECTIVE: To evaluate the significance of hepatitis E virus antibody (anti-HEV) IgG and IgM for the diagnosis of acute hepatitis E. METHODS: Acute phase sera from a total of 143 patients with sporadic hepatitis E in 7 cities of China were determined for anti-HEV IgM and IgG by using an enzyme-linked immunosorbent assay (EIA). 359 serial sera of 56 patients with hepatitis E and 68 serial sera of 4 rhesus macaques experimentally infected with hepatitis E virus (HEV) were also detected for anti-HEV IgG and IgM. RESULTS: In the 143 patients the positive rate of anti-HEV IgG was 100.0%, which was significantly higher than that of anti-HEV IgM (73.4%, 105/143). 97.2% of anti-HEV IgG positive patients had a titer over 1:40. The positive rate of anti-HEV IgM increased with the titer of anti-HEV IgG in sera. It was 0% (0/4), 44.4% (8/18) and 80.2% (97/121), respectively in patients with the anti-HEV IgG titer of 1:20, 1:40 and >or=1:80 (P < 0.001). All the anti-HEV IgM positive patients were also anti-HEV IgG positive. No patients were found to be anti-HEV IgM positive alone. Anti-HEV IgG was detected as early as 2 days after onset of the disease, with a cumulative positive seroconversion rate of 100% by 1 month. The negative seroconversion rate of anti-HEV IgG increased with course of the disease and 43.3% of the patients lost their anti-HEV IgG by 6 months after illness. Though anti-HEV IgM also seroconverted at the same time as anti-HEV IgG, its cumulative positive seroconversion rate was only 71.4% and declined rapidly. Up to 37.5% of anti-HEV IgM positive patients became negative by 1 month after onset. Similar antibody responses were observed in 4 rhesus macaques experimentally infected with HEV. CONCLUSION: It is suggested that as a result of the poor sensitivity of currently available anti-HEV IgM EIA kits, anti-HEV IgG will be a more reliable marker for the diagnosis of acute hepatitis E as compared with anti-HEV IgM.

[Protective effects of bactericidal/permeability-increasing protein on the vital organ function after endotoxic shock in rats].

OBJECTIVE: To investigate the protective effects of BPI on the vital organ function after endotoxic shock in rats, and to provide experimental evidence for further clinical studies. METHODS: 12.5 mg/kg of E. coli 026:B6 LPS were bolus injected via i.a. to reproduce endotoxic shock. Immediately after injection of LPS, 5 mg/kg of BPI or equal volume of physiological saline were bolus injected via i.a. RESULTS: The survival time of shocked animals was prolonged in the BPI group, 24 h survival rate was higher than that in the saline group. The values of mean arterial pressure, left intraventricular systolic pressure, isovolemic ventricular pressure, and dp/dtmax in the BPI group were markedly higher than those in the saline group. The plasma glutamic-pyruvic transaminase and urea nitrogen levels were obviously lower than those in the saline group. The plasma endotoxin and TNF levels in the BPI group were significantly lower than those in the saline group. CONCLUSIONS: BPI has good protective effects on cardiac, hepatic, and renal function in endotoxic shock in rats, indicating that BPI might be useful in clinic treatment of sepsis/septic shock.

[Clinical protocols for the treatment of severe acute pancreatitis].

OBJECTIVE: To set up principles for the treatment of severe acute pancreatitis (SAP). METHODS: The therapeutic principles and results of SAP in three periods were analyzed. Period I (1980 - 1990) was characterized by early surgical intervention supplemented with integrated traditional and western medical therapy, period II (1991 - 1993) by integrated traditional and western medical therapy supplemented with surgery for early complications, and period III (1994 - 1997) by integrated traditional and western medical therapy supplemented with surgery for local infectious complications only. RESULTS: Because of the improvement of intensive care, the operation rates kept declining in the three periods. They were 77.59%, 54.55% and 29.55% with a mortality rate of 40.52%, 17.17% and 11.36%, respectively. CONCLUSIONS: Integrated traditional and western medicine has been effective in most SAP patients, and surgery is indicated only for late complications such as necrotizing infection. Early surgery should be restricted.

[Nutritional impact of glycyl-glutamine dipeptide on the autotransplanted small intestine in pig].

OBJECTIVE: To observe the impact of glycyl-glutamine (Gly-Gln) dipeptide on the autotransplanted small bowel. METHODS: Ten unbred pigs underwent small bowel autotransplantation and were divided randomly into two groups: GTPN and STPN. In the GTPN group (n = 5), the pigs received TPN supplemented with Gly-Gln for 28 days, whereas in the STPN group (n = 5), the pigs received isonitrogenous and isocaloric standard TPN for 28 days. The content of plasma glutamine (Gln), the concentrations of mucosal Gln, protein, villous DNA and RNA, villous height, mucosal thickness, crypt depth, villous surface area, D-xylose absorption test, and intestinal permeability to (99m)Tc-DTPA were measured on the operative day or on the 1st and 28th day after operation. RESULTS: All the indexes except the intestinal permeability (IP) to (99m)Tc-DTPA were significantly higher in the GTPN group than in the STPN group (P < 0.05) on the 28th day after operation, but the IP to (99m)Tc-DTPA was significantly lower in the GTPN group than in the STPN group (P < 0.05). CONCLUSIONS: Gly-Gln has special nutritional effect in improving the metabolism, structure and function of autotransplanted small intestine in pig.

[Progress in the protective medicine against [correction of aganist] rocket propellents].

To review the progress in the major assignment, the organization and implementation of protection against liquid rocket propellent. The safety detection methods of the rocket [correction of rocked] propellent in the launching field were also discussed. Three steps of the sanitation and protection of the liquid propellent, the toxicity and the toxicology of hydrazine on central nervous system, blood circulatory system, assimilation system, respiratory system, immune system, liver, kidney, eye, skin and its hereditary toxicology were described. In addition, the clinical types of poisoning, the current principle and the common ways of the prevention and treatment of hydrazine and nitrogen oxides poisoning were summarized.

[Applied anatomic study on intraorbital and intraethmoidal parts of ethmoidal artery].

OBJECTIVE: In order to provide anatomic data for operation of ethmoidal sinus and optic canal decompression. METHOD: Using microanatomic method, the intraorbital and intraethmoidal parts of ethmoidal arteries were observed and measured. RESULT: The length of intraorbital segment of anterior ethmoidal artery was 5.16 +/- 1.24 mm, and its diameter was 0.56 +/- 0.17 mm; the length of intraorbital segment of posterior ethmoidal artery was 9.08 +/- 2.29 mm, with diameter 0.37 +/- 0.14 mm. The ethmoidal artery, nerve and vein were surrounded by fascial sheath which could extend to ethmoidal sinus. Three variants of ethmoidal arteries could be found: It passed through bony canal; It went between the superior wall of ethmoidal cell and ethmoidal mucosa; One part went through bony canal, but the other did not. CONCLUSION: It was important to find ethmoidal artery in operation of ethmoidal sinus and optic canal decompression.

[A computer-aided analysis system for hypertensive retinal image and its clinical application].

With the use of computer image processing technology and microcirculational network analysis technology, we have developed a new system to analyze the outlook of retinal vessles quantitatively. A multimedia structure has been designed for patient's case history file, which includes the basic information of patients, the operating information during sampling and processing patient's retinal image and the result. The system includes image sampling, image displaying, image processing, parameters measuring and analyzing, reports printing and atlas management. Clinical application indicates that the system is useful to hypertension diagnosis.

[A new viscoelastic model for granulocytes].

In this paper, a new viscoelastic model for granulocytes was introduced. The stress was placed on the theory and its experimental results of three-parameter viscous model with micropipette aspirating technique. The small deformation of single rat granulocyte into the micropipette was analyzed by the model. Within the small deformation, the fit of the experiment data using the three-parameter viscous model was more satisfactory than that using the standard solid model, and the former suggested that cytoplasm of granulocyte was treated as a liquid rather than a solid.
