Estimating speech threshold from the PI-PB function.

Speech thresholds were determined from the performance intensity function for phonetically balanced (PB) words in 2,117 patients with varying types and degrees of hearing loss. Comparison of this threshold for PB words (PBT) with both pure-tone sensitivity (PTA) and the conventional threshold for spondee words (ST) shows that PBT not only provides a cross-check on pure-tone level, but also conveys additional information about audiometric contour.

Recovery of crossed acoustic reflexes in brain stem auditory disorder.

Crossed acoustic reflexes were serially monitored in a patient receiving radiotherapy for an intra-axial brain stem tumor. Analysis revealed a pattern of systematic recovery as the brain stem lesion responded to therapy. Crossed reflexes were observed at more and more test frequencies, until responses were present on both ears for all test signals. Pure-tone sensitivity and speech intelligibility remained within the normal range throughout the observation period.

A simplified tone decay test.

This report describes a simplified suprathreshold tone decay test procedure (STAT). The sustained test signal is presented for 60 seconds at 110 dB sound pressure level. The patient is asked to respond as long as he hears the sound. If the patient responds for the full 60-second test period, the result is negative. If, however, he fails to respond for the full 60 seconds, the result is considered positive for eighth-nerve site. Clinical experience with the new test procedure suggests that it minimizes false alarms without sacrificing accuracy of identification of eighth-nerve site.

Temporal bone findings in cryptococcal meningitis.

A 19-year-old woman with cryptococcal meningitis died as a result of her disease, in spite of both intravenous and intraventricular administration of amphotericin B. Gross and microscopic examinations revealed multiple cysts containing Cryptococcus neoformans within the central nervous system. Studies of the temporal bones revealed both neural and end organ destruction with the presence of Cryptococcus when stained by Gomori methenamine silver nitrate method. We present the results of audiometric and vestibular examinations. The case history depicts a progressive neurological involvement by Cryptococcus.

Open-ended tympanometric screening: A new concept.

We introduce the concept of open-ended, successive-day, tympanometric screening. In impedance screening of children for middle ear disorders, there is a serious risk of over-referral if decisions are made on the basis of a single tympanogram. Serial tympanometric studies of Australian aboriginal children show that, whereas the type B tympanogram is usually stable, type A and type C patterns are prone to vary from day to day, inviting inappropriate referral decisions unless the classifications are confirmed by tests on successive days.
