Immunization against urinary tract infections.
Over the last few years there has been increasing emphasis placed on the importance of the role of bacterial adhesion, both in the initial stage and the clinical manifestations of urinary infection. It has been clearly demonstrated that all urinary infections are accompanied by an immunity response usually related in degree to the severity of the infectious lesion. Moreover, various in vitro and in vivo studies have demonstrated that bacterial adhesion to uro-epithelial cells can be inhibited immunologically. A study was conducted to analyze and compare results of different immunization procedures, in an animal model (female rat), on in vivo inhibition of adhesion to bladder epithelium of various bacterial strains. The bacteria selected were E. coli (075) and E. coli (06) with respectively a mannose-resistant (MR) hemagglutinin (HA) and a Klebsiella pneumoniae. The three immunization procedures were: intravaginal instillation, oral doses completed by intraperitoneal injection and subcutaneous administration. The instilled or injected antigen was made up of a formalized bacterial suspension emulsified in Freund's incomplete adjuvant: only the orally administered antigen was composed of live bacteria. Quantitative in vivo test of bacterial adhesion was by radioactive labelling of bacteria instilled into the bladder. Of the three immunization routes, only vaginal instillation significantly inhibited bacterial adhesion to bladder mucosa whatever the HA type involved. Furthermore, although E. coli (075), which possesses an HAMR, adhered more than E. coli (06) to the bladder wall, a marked and comparable inhibition of its adhesion was noted after immunization by the homologous (E. coli (075)) as by the E. coli (06) strain.(ABSTRACT TRUNCATED AT 250 WORDS)

Precancerous and cancerous breast lesions during lifetime and at autopsy. A study of 83 women.
Among 83 consecutive, unselected female autopsy cases, 6 had been treated for invasive breast carcinoma (IBC) during lifetime. At autopsy, of these six women, two had IBC in the contralateral breast, and another two had in situ carcinoma. Among the 77 women without previous clinical breast cancer, one case of IBC and 14 cases of in situ carcinoma were found at autopsy. Thus, the total occurrence of primary malignant breast lesion was as high as 25.4%. Assuming that all IBCs evolve from in situ forms and that in situ carcinomas do not undergo total regression, our results seem to indicate that about one third of all in situ carcinomas develop into IBC.

Occupational accidents and alcohol.
Alcoholic intoxication is commonly considered to be an important cause of occupational accidents. Over a period of one year the alcohol concentration in the breath and/or blood was studied in 5439 patients, who, following an occupational accident, attended the emergency departments of a large Danish provincial city. It is concluded that only a few, apparently less than 3%, were inebriated in the legal sense of the term (BAC above 0.8%) at the time of the accident.
The accuracy of clinical examination of injury of the knee joint.
This study is an analysis of the clinical examination of 118 consecutive patients with severe, acute injury of the knee joint, that is, with intra-articular effusion or with clinical suspicion of ligamentous instability. Both the clinical examination and the examination under anaesthesia for ligamentous instability involve a large number of falsely positive and falsely negative diagnoses. The greatest uncertainty is attached to the examination of the anterior cruciate ligament and menisci. The diagnostic specificity and the diagnostic sensitivity of the clinical examination of the anterior cruciate ligament are 0.75 and 0.62 respectively. The corresponding values for examinations under anaesthesia are 0.86 and 0.76 respectively. Two-thirds of the patients without intra-articular effusion, whose clinical examination gives rise to suspicion of ligamentous instability, however, have subclinical intra-articular haemorrhage and have as many cruciate and combined lesions as the whole series, viz. respectively 63 and 47 per cent. On the basis of this we conclude that arthroscopy should be carried out in all patients with a traumatic haemarthrosis or indication of ligamentous instability.
Enhanced immune response in the urinary tract of the rat following vaginal immunization.
The immune response in the rat urinary tract was assessed by an enzyme-linked immunosorbent assay for immunoglobulins following vaginal immunization and subsequent intravesical challenge with live E. coli 06. Vaginally immunized rats showed a strong and rapid bladder immune response following intravesical challenge, in contrast to nonimmunized control rats. The immune response was localized to the urinary tract in that antibody to E. coli 06 was not detected in serum, saliva and vaginal washes from immunized rats.
