[Insulin resistance in the pathogenesis of polycystic ovarian disease (PCOD)].

In polycystic ovarian disease there is a strong association between hyperinsulinemia and hyperandrogenism but not with obesity alone. The magnitude of peripheral insulin resistance is similar to that seen in non-insulin-dependent diabetes mellitus. Mild hyperinsulinemia in PCOD patients is not impair the carbohydrate metabolism. The elimination of the cause of hyperandrogenism by bilateral oophorectomy, long-acting Gn-RH agonist or antiandrogen cyproterone acetate did not improve the associated insulin resistance. In opposition to insulin resistance in the tissues responsible for metabolism of carbohydrate, the ovary remains sensitive to the effects of pancreatic hormone. Presumably this mechanism involved the interaction with IGF-I receptors to stimulate thecal and stromal androgen production. Insulin may sensitize the stroma to the stimulatory effect of LH. In the mechanism of follicular arrest take part increased level of binding proteins for IGF-I, mainly IGFBP 2, -4 and 5 inhibit FSH and IGF-I action.

[Transfer RNA (tRNA) in primary and metastatic endometrial cancer tissue].

Two dimensional polyacrylamide gel electrophoresis of 3' end labeled total tRNA from primary and secondary (metastatic) endometrial cancer was used to obtain mapping of isoacceptors. In tRNA from primary tumor 87 isoacceptors were found whereas in metastatic cancer only 52 spots corresponding to isoacceptors were detected. Normal myometrial tRNA used as a control contained 51 isoacceptors. The isoacceptors specific for neoplasia were observed in both cancer tRNAs.

Female urinary stress incontinence in terms of connective tissue biochemistry.

The role of connective tissue in the aetiology of female stress incontinence has been investigated. Collagen content and extractability as well as estrogen receptor concentration in vesico-vaginal fascia were measured after small tissue biopsies had been obtained during vaginal repair surgery in cases of urinary incontinence. The mean concentration of estrogen receptor in vesico-vaginal fascia among incontinent women was 49.4 +/- 14.8 fmol/mg of protein as compared to 29.6 +/- 13.1 in continent control group (P < 0.03; t-test). The mean hydroxyproline concentration in vesico-vaginal fascia of incontinent women was 13.8 +/- 2.6 micrograms/mg wet weight, whereas in the control group it was significantly higher 20.6 +/- 2.4 (P < 0.001). The role of connective tissue components in the aetiology of female stress incontinence is discussed.

[Fertility after adnexal surgery].

Among 64 women undergoing tubal-ovarian surgery 40.6% became pregnant spontaneously. Because of 47% of the remainder patients demonstrated anovulation the induction of ovulation was performed except the cases with hypergonadotrophic hypogonadism (9.4%). After treatment the total population with ovulatory cycles increased to 73.4% and pregnancy ratio upwards 48%. The pregnancy was noted in the half of the patients with endometriosis and all multiparas and after ectopic pregnancy.

[Evaluation of ovulation induction in infertile women who are underweight].

The induction of ovulation in 74 underweight normally menstruated sterile women were analyzed with regard to the body mass index (BMI). The underweight patients presented 10.5% of the treated anovulatory women among them 4.8% presented serious weight loss. The concentration of serum gonadotropins and estradiol just before treatment did not allow to anticipate the results of therapy. The interesting observation concerned the cases with low concentration of serum FSH and LH which was more frequently found in nonresponding patients. The ovulatory ratio was not high and was comparable in all groups but pregnancy ratio in the responders was 1.5 times higher in slightly underweight patients. Among the nonresponders more than half of them demonstrated follicular growth confirmed by USC, serum estradiol and mucus viscosity.

[Analysis of cause and test treatment of amenorrhea in teenagers].

The population of 49 young, secondary amenorrheic women aged 17 to 20 was divided according to reaction on i.m. injection of progesterone in oil. The positive category was subdivided into I--polycystic ovarian disease with elevated serum LH and II--hypothalamic-pituitary dysfunction (HPD) with normal (or decreased) level of LH. The negative category consists of subgroup, III--hypothalamic-pituitary failure (HPF) with normal or low serum FSH and IV--ovarian failure with elevated level of FSH (two patients were noted). The bleeding response was obtain in two third of the patients, and also two third of them ovulated after clomiphene citrate treatment, including all six women with POC. The ovulatory response in patients with hypothalamic pituitary failure was noted in more than 40%. The low concentration of serum FSH (below 5 mU/ml) was more frequent in HPF (57%) in comparison with HPD (48%). The resistance to clomiphene citrate treatment was correspondingly 62.5% and 70%. When the ovulation occurred no difference in the regular function of corpus luteum in POC, HPD and HPF patients was noted.

[Evaluation of the treatment of ovarian infertility in women with moderate hyperprolactinemia].

The authors evaluated the therapy by means of clomiphene++ and hCG in a group of 34 menstruating women aged 18-41 who suffered from ovarian infertility and hyperprolactinaemia not exceeding 100 ng/ml. Previous treatment of the women by means of bromoergocryptine decreasing the prolactin concentration did not provoked ovulation. It was found that moderate hyperprolactinaemia still blocked ovulation in one third of the women undergoing therapy. In a group of patients with a concentration of PRL from 50 to 100 ng/ml of blood serum there was a lower percentage of ovulation (62% v 67%) and subsequent pregnancies (23% v 33%). The authors did not find hypofunction of corpus luteum in women who reacted to the therapy. The average progesterone concentration amounted to 11.2 +/- 2.6 ng/ml of blood serum. The hormone concentration was significantly lower in a group of women with prolactinaemia exceeding 50 ng/ml (10.4 +/- 2.5 ng/ml vs 12.6 +/- 3.1 t = 1.86, p less than 0.1). The course of subsequent pregnancies in all cases, also in the group of women with prolactinaemia exceeding 50 ng/ml, was favourable.
