Parapneumonic effusions secondary to community-acquired bacterial pneumonia in human immunodeficiency virus-infected patients.

The purpose of this study was to determine whether the clinical and microbiological characteristics of parapneumonic effusions in patients with community-acquired pneumonia (CAP) infected with the human immunodeficiency virus (HIV) were different from those observed in patients without HIV infection. One hundred and thirty seven patients with parapneumonic effusions were included and divided into two groups depending on whether they had HIV infection or not. The parapneumonic effusion rate was significantly higher in HIV-positive than in noninfected patients (21 vs 13%). Their clinical course was more severe, presenting a higher rate of bacteraemias (58 vs 18%). Pleural fluid in patients infected with HIV had significantly lower glucose levels than that of patients without HIV infection. Chest tube drainage was more frequent in parapneumonic effusions of patients infected with HIV than in those without HIV infection (71 vs 44%). Staphylococcus aureus was the most common microorganism found in the bacteriological samples of patients with CAP infected with HIV (53 vs 12%). We conclude that patients with community-acquired pneumonia and HIV infection have a higher rate of parapneumonic effusions and a more severe clinical course than non-HIV patients, and that Staphylococcus aureus predominates in their bacteriological samples.

Seroprevalence of HDV and HIV in two populations at risk for infection with HBV in Spain.

We have studied 1765 patients attending our clinic during 1991, with the aim of determining the prevalence of HDV and HIV infection in our country in two populations at risk for parenteral and sexual HBV transmission. A second objective was to determine whether or not HBV-HIV co-infection was associated with a greater risk for HDV. Our findings indicated that sexual transmission of VHD is not common, unlike parenteral transmission, and that there is a significantly greater prevalence of infection with HDV in Chi-square who have the HBsAg complex and are HIV positive.

[Possible cardiotoxicity induced by orally administered fluoropyrimidines].

4 more cases of cardiotoxicity associated to 5-FU or oral fluoropirimidine (Ptorafur, UFT) in patients without previous heart disease (except in one case) are presented. The toxicity found was myocardial ischemia and EKG changes (in 3 patients) in patients whose enzymes were measured. The etiopathogenesis and features of this toxicity are discussed.

[Leptomeningeal carcinomatosis as the primary manifestation of a carcinoma of probable renal origin].

Leptomeningeal carcinomatosis (LC) is commonly observed in malignant lymphoproliferative diseases (leukemias and lymphomas), and it can rarely be the presenting feature of a solid tumour. The types of tumour most commonly associated with that condition are breast, lung and gastrointestinal carcinomas and malignant melanoma. We report a patient with a carcinoma of probable renal origin who presented with meningeal carcinomatosis, without any symptom attributable to the initial tumour. We review and discuss the diagnostic criteria, the pathophysiology and the therapeutic outlook of this relatively uncommon type of metastatic disease.

[Epidermoid carcinoma of the esophagus secondary to achalasia].

We present the case of a 78-year-old patient who developed epidermoid carcinoma of the esophagus 5 years after Haller myotomy for achalasia, which produced an improvement in the clinical symptoms, as well as in the radiological, endoscopic and manometric manifestations. The appearance of carcinoma after a moderate period of time underlines the need for continued periodic follow-up of these patients.

[Is it possible to relate reflux esophagitis to epidermoid carcinoma of the esophagus?].

The authors present a retrospective study of 120 cases of epidermoid carcinoma of the esophagus, in which they proposed to investigate the etiopathogenic relation between gastroesophageal reflux and noplastic development. A statistically significant relation was found between esophagitis and cancer of the distal esophageal third, indicating the need for vigilance of patients with reflux and for randomized prospective studies to clarify the problem definitively.

[Superficial gastric carcinoma. Clinical analysis].

While in Japan the incidence of superficial gastric cancer (CGS) is very high (35%), with a mean 5-year survival rate of 95%, in our environment it is infrequent. Although the global incidence of gastric cancer is declining, recently, since endoscopy became routine, an increase has been seen in the diagnosis of superficial gastric cancer. In the period between 1976-1982 two cases were diagnosed in our service, but between 1982-1986 the total number of cases found was 23, representing 0.37% of all the endoscopies performed (approx. 6,800) and 8.6% of those diagnostic of gastric cancer (266 cases). Of the lesions, 60.8% were located in the antral region and the most common accompanying symptomatology was ulcerative syndrome (47.3%). The predominant tumoral type was ulcerative (73.9%) (classification of the Japanese Endoscopy Society). The benefits of radiological study were nil, while endoscopic biopsy was diagnostic in 100% after an average of 1.8 (1-4) endoscopies per patient with 6-12 biopsy samples obtained in each endoscopy. Of these 23 patients, 8 underwent total gastrectomy, 14 partial gastrectomy and one refused surgical treatment. The survival was 91.3%, with a follow-up of 6 to 60 months, and two deaths were due to nontumoral causes.

[Individualized prognosis in non-small-cell carcinoma. A multivariate approach].

With the aim of providing a system to give individualized initial prognosis for patients with non-small cell lung cancer (NSLC), we performed a multivariate discriminant analysis with combinations of various prognostic factors, studying 93 patients with diagnoses of stage II NSLC and complete followup information. All had undergone surgery at our hospital. Survival longer than or less than one year was defined as the dependent variable: independent variables more clinical, analytical, lung function, histological, anatomical and surgical data. Among the 31.2% of patients who died within one year, tumor size was noticeably greater (6.0 +/- 2.1 cm versus 4.8 +/- 2.0 cm; p = 0.11) and serum albumin was lower (3.7 +/- 0.7 g/dl versus 3.9 +/- 0.6 g/dl; p = 0.039); we found no significant differences among the remaining independent variables in the preliminary univariate analysis. The linear function obtained with discriminant analysis allowed us to classify the patients correctly in 87.1% of cases, with a diagnostic sensitivity of 79.3%, specificity of 90.6%, positive predictive value of 79.3% and negative predictive value of 90.6% for patients who died during the first year. The independent variables that were associated with poor prognosis were large tumor size, long duration of symptoms, low albumin level, high alkaline phosphate level, presence of 2 or more N1 affected nodes, pneumonectomy and presence of perioperative complications. Our findings allow us to conclude that the simultaneous analysis of a variety of prognostic factors can help to give an accurate prognosis for individual patients with the same anatomical stage classification.

[Adenoid cystic carcinoma of the trachea and bronchi. Presentation of 2 cases and review of the literature].

Adenoid cystic carcinoma is a rare but insidious tumor of the trachea and bronchi. It is infiltrative and tends to recur at the same site. Diagnosis is often late, generally when a large part of the bronchial opening is occluded and with extensive invasion of submucosal tissue, making elective surgery difficult and complementary radiotherapy necessary in most cases. The prognosis, however, is fairly good, with survival after 5 years in 75% of patients. We present cases diagnosed recently at our hospital, one with a large longitudinal tracheal tumor and the other with a located in the carina. The literature is reviewed.

[Chondrosarcoma of the sinuses: an infrequent tumor].

The case of 69 year old male patient who developed sinusal mesenchymal chondrosarcoma is presented. At the time of diagnosis the tumor had spread into the intracranial cavity, which explains why no surgical treatment was carried out. A combined palliative treatment of chemotherapy and radiotherapy was used which did not improve the course of the tumor. The rare location of this form of tumor is discussed. Also commented on are the therapeutic--both surgical and other modalities (chemotherapy and radiotherapy)--difficulties encountered with this condition, given the little experience accumulated with it and its habitual resistance to the therapeutical regimens employed.
