[Induction in the scarred uterus].

Labor was induced in 41 women with uterine scarring and a fetus in the cephalic presentation and normal pelvis size. Due to various problems, these patients would have had to undergo cesarean section if this alternative had not been attempted. The use of prostaglandins E2 to increase maturation of the cervix, of Syntocinon and peridural analgesia made it possible to obtain delivery through the genital tract in 24 cases with the same level of fetal morbidity as for the babies delivered by cesarean.

[Use of etilefrin as local vasoconstrictor in lower gynecologic surgery ].

In this randomized prospective study the authors have compared the effectiveness and side-effects of two local vasoconstrictor agents, etilefrine (Effortil) and ornithine 8 vasopressin (Por 8) in vaginal gynaecological surgery. Thirty-three patients entered the trial and were divided into two groups: G1 (15 patients) received Effortil, and G2 (18 patients) received Por 8. The products, administered at random, were diluted in saline 40 ml and injected into the cervix through 6 points. A 3-minute interval was allowed between injection and incision. The results were assessed on the basis of trans- and postoperative haemorrhage and haemodynamic variations. Palor of the cervix was achieved after 3 minutes in both groups; moderate bleeding was observed in only one of the G1 patients. Postoperative renewal of packing was necessary in 2 patients in G1 and 4 patients in G2. No electrocardiographic anomaly was recorded in any of the two groups. Diastolic BP was significantly higher in G2 than in G1 (P less than 0.002, Fisher test). Systolic BP was also elevated in that group (P less than 0.03, chi 2 test). Moderate reduction in heart rate was observed in both groups (P less than 0.3, Fischer test), but severe (48 beats/min) bradycardia was noted in one G2 patient.

Detection of Mycoplasma hominis antibodies by three different techniques, application to women with pelvic inflammatory disease.

Mycoplasma hominis, a mycoplasma potentially isolated from the genital tract of healthy women, seems to have some role in pelvic inflammatory diseases (PID). Three serologic techniques were developed to detect antibodies to M. hominis: ELISA (IgG, IgM), microimmunofluorescence (MIF) and Western blot. Rabbit and human control sera were used to establish the standard assay conditions. ELISA proved to be more sensitive but less specific than MIF. Cross-reactivity with antibodies to M. pneumoniae existed with ELISA but not with MIF. Twenty six and 60 kDa proteins were responsible for that cross-reactivity, as assessed by Western blot analysis. Among 33 patients with PID, 18 to 35 year old, 67% had antibodies to M. hominis by ELISA and 33% by MIF. Among 52 age comparable healthy women, 44 and 13% were positive for M. hominis by ELISA and MIF respectively. The Western blot analysis showed heterogeneity in the antibody response to M. hominis, but a 76 kDa protein reacted with half of the tested sera. Our study agrees with some role for M. hominis in PID. M. hominis was certainly the primary pathogen in one case, played a role that was probable in seven cases, and possible in three patients.

[Fetus in fetu and acardiac monster: can the similar patterns of these 2 malformations be explained by a common morphogenic mechanism?].

Fetus in fetu and acardiac monster are two unusual malformations (estimated incidences: 1 in 500,000 and 1 in 34,600 deliveries respectively) which present very similar morphological patterns. The authors report two cases of acardiac monster and a case of fetus in fetu which emphasize this fact. These findings suggest that a single morphogenic mechanism leads to the defects observed in these two groups of malformations.

[Thrombophlebitis of the ovarian veins. A case report. Review of the literature].

The authors report on one case of thrombophlebitis of the two ovarian veins complicated by thrombosis of the vena cava inferior after a septic abortion. The diagnosis suggested by the persistence of symptoms of a severe pelvic infection was confirmed using computerized axial tomography. Faced with a clot floating in the vena cava inferior, a Greenfield filter was placed subrenally and combined with antibiotic therapy and IV heparin therapy. Clinical symptoms resolved but the scanographic image of thrombosis of the vena cava persisted after 7 months of treatment. This is a rare disease which is currently easy to diagnose, thanks to computerized axial tomography and nuclear magnetic resonance.

[A randomized study of the treatment of threatened premature labor. Nifedipine versus ritodrine].

The authors compare the efficiency of two drugs in the treatment of threatened premature labour, the one being the calcium inhibitor (nifedipine) and the other a beta-mimetic drug (ritodrine). 62 patients after random selection were divided into two groups: 32 treated with ritodrine and 30 treated with nifedipine. The treatment was carried out over 7 days. The success rate was similar in both groups -72% for the ritodrine group and 63.33% for the nifedipine group. Women receiving nifedipine had slightly greater gain in weeks, six as against five for ritodrine. The side effects which were often found with nifedipine were; hot flushes (in 10 cases) and headaches (4 cases). These symptoms appeared 15-30 minutes after the first dose and were transitory. No neonatal complications were found. The ease with which a calcium inhibitor can be given suggests that it should be used more frequently in the treatment of threatened premature labour and particularly when there are contra-indications to the use of beta-mimetic drugs.

[Vulvo-vaginal mycoses].

Vulvo-vaginal mycoses are extremely frequent and relatively easy to treat. Nevertheless, recurrent forms are extremely debilitating and there is no miracle treatment for them.

[Isolated necrotizing angiitis of the cervix uteri. Apropos of a case].

A necrotizing arteritis isolated to the uterine cervix without multisystem disease was fortuitously discovered in a 52-year old woman after hysterectomy for uterine leiomyomas. This rare lesion discovered by routine histopathological examination was cured by surgery. The aetiology and pathogenesis of this arteritis are unknown.

[Hyperthermia during labor. Role of peridural analgesia].

Over a period of 4 months, the authors have observed 414 deliveries, including 30 which presented a hyperthermia exceeding or equal to 37.8 degrees C (7.2%). 27 hyperthermias occurred under peridural analgesia. The thermogenic effect of this loco-regional anesthesia seems debatable since it occurs at an early stage, is transient and has a peripheral effect. The fever noted during labor indicates, most of the time, an infection. Peridural analgesia is advocated in most primiparous patients as well as in imperative labor inducements. Sometimes, this permits accepting a longer-lasting labor, exposing the patient to the risk of amniotic infection.

[Prevention of infectious complications after cesarean section by the use of cefotetan].

A prospective, randomised study was carried out on 110 patients to find out the effect of giving Cefotetan prophylactically as against no treatment in the prevention of infections after cesarean sections. The cases were selected before or at the beginning of a trial of labour in 72 cases and after a failure of trial of labour in 38 cases. A single dose of 2 gr of Cefotetan was given during the operation, after the cord had been clamped. It caused a significant drop in infection and in particular the number of cases of endometritis and urinary tract infections, which were twice as common in the non-treated as compared with the treated group.

[Transplacental passage of doxorubicin].

The transplacental passage of doxorubicin, an anthracycline used for lymphoproliferative disorders and breast cancer, was studied by in vitro perfusion of term human placenta. Placentas from women with uncomplicated pregnancy were collected immediately after vaginal delivery and put into a 37 degree C thermostatically-controlled hood. A cotyledon was chosen and placed into the perfusion chamber; fetal and maternal compartments of the isolated lobe were thus perfused with separated "open" circuits. Perfusion of fetal surface of the placenta was started immediately at a flow rate of 6 ml/mn and then so was the perfusion of the intervillous space at a rate of 12 ml/mn. Perfusion medium used was Earle's solution. Antipyrine, to validate experience, and doxorubicin were added to the maternal perfusate. Samples were collected from arterial inflow and venous outflow respective of the maternal and fetal compartment and timed measurements of the fetal venous return were used to calculate flow rates. After a stability study of doxorubicin solutions under experimental conditions, the transplacental transfer of doxorubicin was then investigated for three doses: 3 mg/l, 30 mg/l and 150 mg/l. The global transfer value is low (2.96% +/- 0.75%) and doesn't seem dose-dependent. Adriamycinol, a plasma doxorubicin metabolite, has not been found even for the greatest concentration. The low transfer value can explain the rarity of fetal accidents in clinical reports.

[Prevention using cefotetan of post-partum and post-abortion infectious complications in intra-uterine procedures].

A prospective, randomized study was conducted in 113 women to evaluate the effect of antibiotic prophylaxis with cefotetan versus no prophylaxis in the prevention of post-partum and post-abortion sepsis. The administration of a single 2 g dose of cefotetan at the time of surgery significantly reduced the number of infectious complications, removal of the placenta or an internal inspection were carried out.

[Malaria and pregnancy. Epidemiological situation in Kinshasa (Zaïre)].

By regular blood smears to 730 women (430 pregnant women and 250 non pregnant) authors state precisely epidemiologic situation of malaria to women at Kinshasa. The prevalence of malaria of pregnant women is 22 per cent against 6.1 per cent for non pregnant adult women. Malarial infestation in gravido-puerperal period is : mother : 23.7 per cent ; umbilical cord : 3.1 per cent ; newborn : 5.4 per cent ; placenta : 10.1 per cent. Plasmodium falciparum is the principal agent of malaria at Kinshasa. Neither age, nor parity constitute risk factors of malaria. Many cases of malaria without fever exist at Kinshasa. Newborn with malaria and from pregnancies with infected placentas present at the birth a small weight. Placentas of pregnancies with malaria and infected have invariably the same weight.

[Placentocentesis, a new technic in the prenatal diagnosis of chromosome aberrations in the 3d trimester. A preliminary study apropos of 5 cases].

The authors report 5 cases in which placentocentesis was used for late antenatal diagnosis between the 25th and 34th week of amenorrhoea. As the placenta was anteriorly placed, samples were obtained by aspiration through the abdominal route under ultrasound control. The fetal karyotype could be determined in 2 days using a direct method. It showed normal chromosome pictures in all 5 cases. There was no complication in any of these cases following the procedure.

[Prenatal echographic diagnosis of iniencephaly. Apropos of a case].

Iniencephaly is a rare lethal malformation. It is associated with a defect in the occiput, alterations in the spinal column that are variable and retroflexion of the fetus. Antenatal diagnosis by ultrasound is possible because of the typical appearance: the fetal head is hyperextended, with distortion and shortening of the cervical spine. The occiput can be fused with lower vertebrae. Many associated malformations may be found: polyhydramnios, neurologic, facial and visceral abnormalities.

[In vitro study of the transplacental passage of chloroquine sulfate].

The authors, by performing in vitro perfusion on six human placentas, have studied the passage of chloroquine sulphate into the placenta. The drug levels were recorded by high performance liquid chromatography (HPLC). The placental perfusions lasted one hour and drug passage into placental tissue was low (14.21%) which seems to be due to the high degree of fixation of chloroquine on placental tissue.

[Role of Ureaplasma urealyticum in premature rupture of the membranes].

In 79 hospitalized patients in delivery, the search for U. urealyticum was carried out at the level of the vagina, the rim of the placenta, the edge of the PRM orifice and in the throats of newborns, 3 groups were formed: group I: 38 term patients delivering after ARM, group II: 36 patients delivering after 28 weeks, with PRM group III: 5 patients with premature delivery without PRM. The results demonstrate: a highly significant correlation between the women with PRM and those who have a positive vaginal sample; a highly significant correlation between women with PRM and those who have a positive sample on the membranes; a significant correlation between women with PRM and those who have a positive sample on the placenta. The search for ureaplasma would be carried out in case of PRM.
