Ectopic pregnancy subsequent to laparoscopic sterilization.

Ectopic pregnancy after laparoscopic sterilization is analyzed and compared with the occurrence of ectopic pregnancy in a fertile female population of Denmark. The incidence of ectopic pregnancy in laparoscopic-sterilized women is significantly decreased compared with the nonsterilized fertile female population. When a poststerilization pregnancy occurred, ectopic pregnancy occurred 76% of the time, and the incidence of ruptured ectopic pregnancy is significantly increased after previous laparoscopic sterilization.

The arterial blood supply of the parametrium.

The possible influence of tubal sterilization on the ovarian function is still debated. In this study, we have investigated the course of the main arteries in the parametrium. This study concludes that it is unlikely, from an anatomical point of view, that the ovarian function can be disturbed after tubal sterilization with bipolar cautery, silicone-rings or Filshieclips due to changes in the arterial blood supply.

Grand multipara.

Grand multiparity is often considered a clinical entity, as certain complications during pregnancy, delivery and puerperium are thought to occur with increased incidence in these women. In this prospective investigation no difference in complications was found between women with up to 6 deliveries and women with 7 or more deliveries. However the incidence of preterm deliveries was significantly higher in the group with up to 6 deliveries.

Maternal condylomata and juvenile laryngeal papillomas in their children.

In the literature a connection is postulated between maternal condylomata and juvenile laryngeal papillomatosis in their children. We have attempted to shed light on the connection partly by seeking information about the children of 77 mothers with condylomata during delivery, and partly by seeking information about the mothers of nine children with juvenile laryngeal papillomatosis. Our research has not shown a clinical connection between the two diseases.

Laparoscopic sterilization with Filshie Clips.

A study comprising 221 women sterilized laparoscopically with Filshie Clips is presented. The pregnancy rate was 1.36%, which is considered acceptable as the method was new in the Department and performed by 10 surgeons with different degrees of skill in laparoscopy.

Laparoscopy after previous abdominal surgery.

A total of 87 women underwent sterilization by primary laparotomy because of previous abdominal surgery. In no case were adhesions seen between the bowel and the anterior abdominal wall and in only 5 women were the adhesions between the omentum and anterior abdominal wall or uterus/adnexae so significant that the view at laparoscopy would have been insufficient. It is concluded that sterilization of women with a history of previous uncomplicated laparotomy can be done laparoscopically if the history and physical examination arouses no suspicion of severe adhesions. The procedure should be done by experienced senior staff in cases with previous abdominal surgery.

Is low temperature of the follicular fluid prior to ovulation necessary for normal oocyte development?

Because low temperature in the testis is essential to normal spermatogenesis, we speculated whether a low follicular temperature could also be of importance for the oocytes as they resume meiosis at ovulation. We measured the temperature of the follicular fluid and of the ovarian stroma in 26 follicles in 14 women. In 25 follicles (13 women) the follicular fluid was colder than the ovarian stroma. This temperature difference increases toward ovulation (maximum, 2.3 degrees C). In one case, the measurements of the follicular/stromal temperature difference showed an inverse pattern, the follicular fluid being 0.2 degree C warmer than the stroma. This woman was infertile, and oocyte evaluation for in vitro fertilization had five times shown the oocytes unable to divide in culture. The present study supports the idea that low temperature of the follicular fluid prior to ovulation may be essential for normal oocyte development.

Occipitofrontal circumference in newborns of betamethasone treated mothers.

Celestona (betamethasone-disodiumphosphate) and Celeston (betamethasone-acetat + betamethasone-disodiumphosphate) given intramuscularly to pregnant women in order to reduce the frequency of respiratory distress syndrome does not reduce the occipitofrontal circumference of the newborn infants. Before the results from the actual work were available an impeding effect of the evolution of the brain was found in the light of animal experiments. The material is comprised of 52 newborn infants whose mothers were treated with Celestona 8 mg daily for 3 days and 46 infants treated with Celeston 12 mg daily for 3 days. The untreated population comprises 1012 newborn infants. Excluded were non-caucasian infants, twins, stillborns and cases of uncertain gestational age. The occipitofrontal circumference in normal infants and those infants treated with betamethasone were compared in Figs. 2 and 3. Focusing only upon cases in which corticoids were given more than 7 days before delivery and in which no placental dysfunction was diagnosed, no differences from the normal were found (Tabs. II and III, and Figs. 3 A and B).

Serum estriol level during treatment with pivmecillinam.

Asymptomatic bacteriuria in pregnant women shall always be treated, because of the risk of development of acute pyelonephritis. Some antibiotics reduce the level of serum estriol and therefore are less suitable in the treatment of cystitis in the last trimester of pregnancy. Mecillinam is found effective against the most frequent urinary pathogen in cystitis in pregnancy (escherichia coli), and as it is found not to reduce the level of serum estriol it is concluded that pivmecillinam/mecillinam is an alternative in the treatment of symptomatic and asymptomatic urinary tract infections in the last trimester of pregnancy.

Serum estriol level during treatment with betamethasone in the last trimester of human pregnancy.

A comparison was made between betamethasone disodiumphosphate/betamethasone acetate (Celeston) and betamethasone disodiumphosphate (Celestona) in antenatally prophylactic treatment of respiratory distress syndrome syndrome (RDS). Altogether 65 pregnant women in the last trimester of pregnancy were treated with the first-mentioned drug and 62 with the second. The RDS-preventive effect was the same in the two groups. Celeston has a much longer depressive effect on the level of plasma estriol than has Celestona. Use of this drug, betamethasone disodiumphosphate, is therefore recommended in the prophylaxis against RDS.
