Antiphospholipid antibodies (APAB) were first detected as a result of the disturbances they cause in routine biological tests. These effects include lupus anticoagulant (LA) in APTT test and false serological reactions for syphilis in VDRL test. With more sensitive techniques using purified phospholipid antibodies (ELISA, RIA) antibodies directed specifically against cardiolipidin or other phospholipids can be assayed. Beside being responsible for positivity of the VDRL test in the context of syphilis, APAB (false serological reactions for syphilis, LA, anticardiolipin antibodies) have also been detected in systemic lupus erythematosus (SLE) and lupus-like syndromes, after intake of certain drugs, and, more rarely, in a number of diseases (table I). The specificity of APAB detected in syphilis (antiphosphatidylcholine) is different from that of APAB detected in SLE (antiphosphatidylserine and anticardiolipin). A given APAB can react with several phospholipids, but such crossreactions do not occur haphazardly with all phospholipids. Crossreactions are observed between APAB and polynucleotides on the one hand, and between antinuclear antibodies and phospholipids on the other. This pattern has led to speculate that APAB are directly involved in the pathogenesis of SLE; however since these crossreactions are not extensive, such speculation is controversial. The presence of APAB in the form of LA, false serological reactions for syphilis or anticardiolipin antibodies is associated with a number of problems including recurrent thrombosis, repeated abortions, thrombocytopenia and positive Coombs' tests or, more rarely, pulmonary hypertension, migraine, epilepsy, chorea, and transverse myelitis.(ABSTRACT TRUNCATED AT 250 WORDS)

The present randomized, placebo-controlled double-blind multicenter study included a population of 36 subjects with frequent recurrences (at least once a month) of herpes labialis. Most of the patients had failed to respond adequately to previous treatment with other therapeutic tools, including acyclovir. Either 50 mg of thymopentin or of placebo was administered 3 times a week, by the subcutaneous route, for 6 weeks. Subsequently, the patients were observed for nearly 6 months on the average. The results achieved with thymopentin for the individual parameters were significantly superior to those obtained with placebo; thus significant improvement was seen in patients on thymopentin in the duration of the longest symptomfree period (prolonged from 2.1 weeks to 20.9 weeks, p = 0.000), in the number of relapses (reduced from 1.6 to 0.4 episodes/month, p = 0.001), and in the total duration of herpes symptoms per month (shortened from 2.0 to 0.3 weeks, p = 0.000). Placebo treatment also resulted in considerable improvement (p less than 0.05 or 0.01), but was significantly inferior to the improvement obtained with thymopentin. The longest symptomfree period in the placebo group was prolonged from 2.4 to 11.2 weeks. The number of relapses per month was reduced from 1.4 to 0.8, and the total duration of herpes symptoms per month from 2 to 0.9 weeks. The results of intergroup analyses, in which the observed parameters and the improvement achieved in either group were compared, significantly favored thymopentin treatment. The effect of thymopentin was in all but one parameters superior to that of placebo and highly significant (p less than 0.01).
A retrospective study of 40 melanocytic conjunctival tumors (22 naevi and 18 malignant melanoma) is reported. Cases of non-degenerated conjunctival melanosis were not observed. The particular aspects of diagnosis and the prognostic behaviour of these proliferations are discussed and compared with their cutaneous homologues. Epithelial inclusions within tumor cell proliferation are considered as the best criterion of benignity. Because conjunctival melanomas are rather uncommon and technical difficulties in their study are very frequent (poor quality of tissue samples, small size of specimens) many reports are contradictory. Some diagnostic signs are specific for this kind of tumor in the conjunctiva: nevertheless, the particular conjunctival anatomy makes it impossible to classify according to criteria defined for cutaneous malignant melanoma. No criteria would have a predictive value. The severe prognosis of conjunctival melanoma (44% deaths in the present series over 10 years after diagnosis) has been underestimated. Most often carcinologic therapies have not been used in previously reported cases.

Although IgE antibody is able to provoke a late cutaneous allergic response (LCAR), the mechanism of its development is not clear. It seems to involve vasoactive and clotting mediators released from mast cells and basophils. Substances biologically similar to plasma kallikrein (KK), a Hageman factor activator, have been shown to be released by basophils in the course of an IgE-dependent reaction. Because compound 48/80 induces LCAR-like responses in the skin, we compared the ability of tissue KK and compound 48/80 to induce late cutaneous reactions (LCRs). All 40 test subjects showed an immediate wheal and flare reaction (WF) after both KK and 48/80. The WF was followed by an LCR characterized by diffuse edema, pain, and erythema in 36 of 40 subjects with 48/80 and 26 of 40 with KK. The LCRs evoked by the two agents did not differ in their appearance or in their time of development. These reactions increased until the 5 hr mark, began to decrease at the 10 hr mark, and were gone after 24 hr. Histologic study showed edema and a mixed cell infiltration with fibrin deposition. Challenge of the original injection site and a new site after 1 or 2 wk showed a local refractory state lasting 2 wk. Prednisone almost totally suppressed the LCRs induced by both KK and 48/80.
The skin of a patient with giant axonal neuropathy was studied by light and electron microscopy. Enlarged melanocytes protruded into the papillary dermis. An increased number of microfilaments (100 A), which accumulated in rounded or irregular clusters was the main pathological feature. Melanosomes and melanisation were normal. A similar increase of microfilaments was seen in axons and Schwann cells of peripheral nerves, fibroblasts and endothelial cells. This suggests that this metabolic, probably inborn and genetic disorder, does not only affect the nervous system but also the whole microfilament system (cytoskeleton).
This article deals with two new cases of perforating generalized granuloma annulare characterized, from a clinical and biological standpoint, by a long course of evolution for one case and the presence of diabetes and dyslipidemia for the other case. Histologic study confirms the possible coexistence of typical granuloma annulare lesions with epidermic corbelar aspects pinching necrotic material which could lead to complete epidermic perforation. Based on the study of previous cases in the literature, the process of transepidermic elimination and the actual pathophysiologic concepts of granuloma annulare are reviewed. From the therapeutic standpoint, if the first case did not appear to profit from the administration of an alkylating agent, the second case seems to be presently improved through gluco-lipid re-equilibrium associated with lipocaic heparin.
Dermatitis herpetiformis and bullous pemphigoid are similar enough clinically to have been considered for some time as two forms of the same disease. Their histologic and ultrastructural characteristics are a more accurate mean of differenciation: employed essentially with respect to early lesions and the mode of bullous formation. But immunologic analysis is especially helpful in distinguishing the two. This study involves the analysis of acquired data concerning these two entities: 1) The nature, form, and topography of immunoglobulin and complement deposits examined under the light microscope and by immuno-electron microscopy methods. 2) Other modifications of he immunologic status of affected subjects. 3) The supposed nature of causative antigens. 4) The suggested pathogenic mechanisms. There exist discrepancies in the information at hand concerning these domains: the most important number of latest data relate to DH and notably: - The relationship between DH and gluten sensitive enteropathy for which are considered the clinical, histologic, immunologic characteristics and the relationship of this enteropathy with celiac sprue disease, the role of a gluten free diet on skin lesions and immune deposits. - The relationship between DH and specific histocompatible antigens. The data has revealed some important perspectives on the pathogenesis of these bullous skin lesions.
Cutaneous form of periarteritis nodosa. Four personal cases are added to 70 cases of the previous literature. The clinical and biological picture of the cutaneous form of P.A.N. are different from the systemic one by the lack of visceral involvement and a noteworthy good long-term prognosis. Venous (superficial thrombophlebitis) and distal arterial involvement in some cases, raise doubts about diagnosis and classification. The solution is in an accurate definition of the diagnostic clues and specially of the histopathological ones.
Comparative study of achromic and normally pigmented skin of three piebald patients from two families is reported here. DOPA-positive cells were not seen in the achromic areas, but the number of ATPase sites appeared to be normal; therefore the assumption of the replacement of the melanocytes by Langerhans cells cannot be supported. Through electron microscope, Langerhans cells looked strictly normal. Melanosomes appeared reduced in size and some alterations were noticed in their shape and contents. Ultrastructurally, a more prominent change consisted in increased undefined clear cells in the achromic skin. These dendritic suprabasal cells were devoid of melanosomes or Langerhans granules and might represent undifferentiated melanocytes. These features were discussed according to previous literature.
Electron microscopic observations in three cases of granular cell myoblastoma revealed that this tumor is made up of two types of cells. 1) Granular cells grouped into nests, surrounded by a basement membrane, and displaying characteristic inclusions. Numerous processes were observed close to these cells, showing filamentous and microtubular components lying parallel to their axes. 2) Satellite fibroblasts which included angulated bodies. The relationship between these two types of cells is discussed.

About one personal case, authors consider clinical, histological and ultrastructural aspects of the infantile digital fibromatosis. They insist on the caracteristic features of the large granular and fibrillar cytoplasmic inclusions observed in the proliferative fibroblasts, and on the different signs of the metabolic hyperactivity of these cells. Elastogenesis was very scarce within the swelling but showed no evidence of qualitative abnormality. The discussion provides no definitive clue as to the nature or origin of the inclusions.

An ultrastructural study of Kaposi's disease showed often immature vascular proliferation bordered by large globular or fusiform endothelial cells. Other cell types participated in this proliferation: inflammatory cells, pericytes and fibroblasts. These various cell types are the site of active erythrophagocytosis. It is reasonable to accept that the disease is the result of reticulo-endothelial proliferation with multiple morphological types and predominant vascular differentiation.
