Successful enucleation of a necrotizing fibroid causing oligohydramnios and fetal postural deformity in the 25th week of gestation. A case report.

BACKGROUND: In approximately 2% of pregnancies, uterine fibroids are detected. In 10% of diagnosed cases some complications are to be expected during pregnancy and/or delivery. CASE: A 32-year-old woman presented in the 25th week of gestation with a fibroid causing oligohydramnios and fetal postural deformity. A laparotomy and conservative myomectomy were performed with the hope of preserving the pregnancy. Ultrasound examinations performed postoperatively showed that the fetal postural deformity had ended and that the pace of fetal development had returned to normal. A healthy, male newborn weighing 3,600 g was born in the 40th week of gestation via cesarean section. CONCLUSION: The medical literature generally agrees upon conservative therapy; however, there are indications for laparotomy performed during the course of pregnancy complicated by a fibroid. In this case the fetal postural deformity, retardation in development and oligohydramnios, recognized preoperatively, provided indications for laparotomy. The success of the operation was established by postoperative sonography, which showed cessation of the fetal postural deformity and registered a normal volume of amniotic fluid.

Spontaneous bilateral cornual uterine dehiscence early in the second trimester after bilateral laparoscopic salpingectomy and in-vitro fertilization: case report.

A bilateral cornual uterine dehiscence is reported, which occurred 14 weeks after in-vitro fertilization (IVF) in a patient having a medical history of previous bilateral salpingectomy via laparoscopy. Uterine rupture is a rare obstetric complication usually occurring during the third trimester of pregnancy within a uterus which has previously undergone an operation. Ectopic pregnancy is a well known complication of IVF. Post-salpingectomy cornual localization with rupture has also been published. Possible causes are discussed and the attention of the counselling physician is directed to the necessary awareness of such a complication in this high risk population. The reported case is an extreme rarity: a similar case has not been previously published in the literature.

[The place of pelvic adhesions in gynecology. Possibilities of prevention].

The authors review the recent international literature dealing with the role of pelvic adhesions in gynaecologic surgery. They point out that pelvic adhesions can be detected via laparatomy or laparoscopy in every third patient suffering from chronic pelvic pain, whereas abdominal adhesions revealed rarely correlate with anamnestic chronic pelvic pain. Correlation of pelvic pain and adhesions is neither consequent nor mutual. Lacking more detailed information on the pathomechanism of pain, the role of the vegetative innervation of the visceral and parietal peritoneum and of the excitation of the nerve fibers in the neoformed-and vascularized adhesions can only be postulated. According to the attractive but not well established theory extensive adhesions can directly provoke pain through fixing the pelvic organs hampering their motility and mobility. It is generally accepted, that lysis of adhesions in symptomatic patients can resolve or reduce the complaints. However the duration of the beneficial effect and its transitoric or definitive nature cannot be predicted in advance. Subsequently the authors review the adjuvant methods complementary to microsurgical techniques possibly preventing the formation and neoformation of pelvic adhesions. Despite of general acceptance and widespread usage, the effectiveness of crystalloids, macromolecular solutions, intraperitoneal heparin and steroids in the prevention of adhesions cannot be scientifically supported. The encouraging results already achieved by mechanical barriers (intercede, Gore-Tex) can hopefully be surpassed by biodegradable barriers actually studied in animal models.

Successful pregnancy after bilateral internal iliac artery ligation monitored by color Doppler imaging.

Ligation of the internal iliac (hypogastric) arteries can be a life-saving procedure in cases of severe hemorrhage from pelvic viscera. In young women it is the only way to preserve fertility when conservative methods have failed to control the bleeding. We present the case of a 21-year-old woman whose first pregnancy was terminated, after which profuse vaginal bleeding occurred and could not be controlled by conservative methods. An urgent laparotomy was performed and both internal iliac arteries were ligated. Two years after surgery, the patient conceived and after an uneventful pregnancy delivered a healthy infant. Uterine and fetal circulations were monitored with color Doppler imaging and were normal throughout pregnancy. A normal 3550-g male infant was delivered vaginally at 39 weeks' gestation.

[Laparoscopic management of extrauterine pregnancy by salpingotomy].

The classical therapy of tubal pregnancy has been the salpingectomy for decades. Because of infertility problems caused by previous ectopic pregnancies and the organ preserving philosophy the microsurgical salpingotomy became more popular in the seventies. The technical development in the field of gynaecological endoscopy made it possible to perform the salpingotomy even through the laparoscope. The authors report in detail on their technique based on bipolar coagulation, cold knife linear salpingotomy and leaving the tubal wound open for secondary healing. They summarize their preliminary results after the management of nine patients.

[Importance of the styloid process syndrome in the differential diagnosis of facial pain].

The reported 2 cases of processus styloideus syndrome deserve attention because of their rarity. In the case of complains of uncertain origin and radiating chiefly into the tongue also the processus styloideus syndrome is to be taken into consideration. With the surgical treatment, after having the diagnosis, a well treated illness can be separated from the group of those illnesses in which the main problem is facial pain.

Several years of observation with Mydeton in the treatment of lockjaw.

Several years' observations with the muscle relaxant Mydeton (150 mg coated tablet) in the treatment of lockjaw of different etiology have been discussed. The trismus relieving action of the drug was proved by objective examinations. It has been concluded that Mydeton is of high therapeutic value in relieving muscular spasms of the maxillofocal region of different etiology and it may be used without risk in both hospitalized cases and outpatients.

Effect of nickel ions on spontaneous, electrically and norepinephrine stimulated isometric contractions in the isolated portal vein of the rat.

The effect of low concentrations (1-10 microM) of nickel (Ni2+) was studied on the spontaneous contractile activity of isolated rat portal vein preparations. To distinguish between presynaptic and postsynaptic actions, Ni2+-induced changes of isometric force development evoked either by electrical field stimulation (80V/5 cm; 1 msec; 1-32 Hz) or by exogenous norepinephrine (NE) were compared. Ni2+ inhibited spontaneous isometric force development, decreased basal tone but significantly increased the frequency of contractions. The experimental findings that inhibition of the effect of selective stimulation of adrenergic nerves was significantly more pronounced than the depression of contractions evoked by exogenous norepinephrine indicate that in addition to postsynaptic actions, Ni2+ influences presynaptic mechanisms (i.e. NE-release) in this vessel preparation.
