A sixth case in the world literature is reported of a 59-year-old male with diabetes insipidus (DI) associated with chronic myelogenous leukemia (CML). The patient is unique in that his CML was diagnosed 10 years before he presented with vasopressin-responsive DI. Radiation to the central nervous system failed to reduce the daily requirement of his need for vasopressin.
Following Needleman et al.'s (1979) report of a correlation between tooth lead estimates in children and reaction time as measured by Rodnick and Shakow's (1940) delayed reaction time paradigm, a version of the procedure with two delay periods of 3 s and 12 s was developed for automated presentation and scoring on a VIC-20 microcomputer. Data are presented from a study of 300 children aged 6-14 years. Mean reaction time over six trials for each delay period related in a curvilinear fashion with age, but no relationships were found with sex or intelligence. Age-adjusted reaction time related significantly with blood-lead levels, but accounted for only about 1 per cent of the variance. The effect was mainly observed in younger (6-10 years) children in whom higher lead was associated with slower reaction time.
Peptides derived from both proenkephalin and prodynorphin have been identified in guinea pig adrenal medulla. In extracts of whole adrenal glands radioimmunoassays directed to the prodynorphin-derived peptides alpha-neoendorphin, dynorphin A, and dynorphin B detected high concentrations of immunoreactive material ranging from 113 to 216 pmol/gm. The concentrations measured by radioimmunoassays directed to the proenkephalin products met-enkephalin-Arg-Gly-Leu and met-enkephalin-Arg-Phe were 878 and 484 pmol/gm, respectively. No metorphamide or dynorphin(1-8) could be detected in the adrenals. Leucine-enkephalin immunoreactivity which can be generated from either prodynorphin or proenkephalin could also be measured in the extracts. Gel filtration showed the immunoreactive material, with the exception of that measured by the alpha-neoendorphin radioimmunoassay, to be predominantly of high molecular weight ranging from Mr = 3,000 to 12,000. Immunocytochemistry, using well characterized antisera to alpha-neoendorphin and met-enkephalin-Arg-Gly-Leu, demonstrated that the prodynorphin and proenkephalin products were present in the same cells in the medulla region of the gland. The results show that two opioid peptide precursors can be localized in the same cells and exhibit some common features in their processing. As a relatively homogeneous, localized system, the guinea pig adrenal gland should prove a valuable, in vivo model for the study of co-localized opioid precursors.

The effect of 2,450-MHz pulsed microwave radiation on the enzyme activity of membrane-free acetylcholinesterase was studied while the enzyme was in the microwave field. We found no significant effect of microwave radiation on enzyme activity using a wide variety of power densities, pulse widths, repetition rates, and duty cycles. This suggests that simple, direct modification by microwave energy of acetylcholinesterase structure and enzymic activity is not related to microwave alteration of acetylcholinesterase central nervous system levels.
Low energy helium neon laser has been suggested as an effective adjuvant in the healing of open wounds. To date, supportive studies have been performed in loose-skinned animals. For such data to be clinically meaningful, it was felt necessary to study this effect in an animal with a dermal structure more closely resembling that of man, the pig. After creating 62 partial thickness wounds on the dorsum of domestic swine, one-half of these were randomized to receive laser treatment. The remainder served as controls. Laser treated wounds received a daily treatment of 15 sec/cm2 at an irradiance of 64 mW/cm2 (energy fluence = .96 J/cm2/day). With these treatment parameters, we could not demonstrate any clinically significant laser induced acceleration of open wound healing.
Antigen-antibody complexes formed in the presence of serum do not precipitate. This complement-dependent function is impaired in approximately half of all patients with seropositive rheumatoid arthritis (RA) but not in patients with other chronic inflammatory arthropathies. As patients with seropositive RA have normal or elevated serum complement levels, this findings suggests that an inhibitor is present in the serum of these patients. Although degree of impairment of solubilization is correlated with rheumatoid factor (RF) titre, decreases in RF titre in patients receiving gold therapy were not always accompanied by improvement of the solubilization process. Thus we can conclude that impaired solubilization is related to, but may be distinct form, RF. Impaired solubilization was associated with the presence of subcutaneous nodules, but not with other systemic features of RA. Thus this phenomenon may be of pathogenetic importance.
The rehabilitation of 8 cold injury lower limb amputees is described, 7 of whom were alcoholic and had significant personality disorders. Delayed wound healing was the only common physical problem but the psychosocial difficulties were substantial and were the principal determinant of outcome following rehabilitation.
A phage typing set composed of 32 phages is described for differentiating Escherichia coli. Eight hundred sixty-six isolates from cases of bovine mastitis were used in this effort. Of these cultures 829, or 96%, were characterized successfully, and 178 phage types were observed. Thirty-seven isolates were not typable.
The function of sensory and motor neurons below the level of a spinal cord injury (SCI) was assessed in 23 patients by recording the nerve conduction velocities and the amplitudes of evoked nerve and muscle compound action potentials for sural and posterior tibial nerves. Sensory and motor conduction velocities were generally normal when obtainable. Two patients had a unilateral reduction in the sural nerve and abductor hallucis muscle compound action potentials which may have been due to peripheral nerve compression. In 11 patients the abductor hallucis muscle compound action potential was below the normal range, while sural nerve action potentials remained within the normal range. Motor unit counts showed a reduction in the number of functioning motor units in these muscles.

53 diabetic patients took part in a study to assess the safety of reusing plastic disposable syringes for insulin injection. Each patient used the same plastic syringe for 7 days. The syringe needles were changed when blunt. No patient developed signs of infection at the injection site. Bacteriological assessment of the used syringes and needles showed no contamination in any of the syringes, although one needle was contaminated. The effectiveness of bacteriostatic additives to insulin preparations was demonstrated. The presence of these additives renders safe the practice of a diabetic patient reusing a plastic disposable syringe over a 7-day period.
The effect of inhaling sodium cromoglycate on the development of chest pain in 8 patients with exercise-induced angina pectoris was studied in a double-blind cross-over placebo controlled trial. Blood levels of sodium cromoglycate of 24-136 ng/ml did not affect either the time to develop pain on exercise or the rate-pressure product at the time of pain. In view of the original observation that Khellin reduces angina, it is suggested that further studies should be performed to assess the effect of SCG at higher concentrations in the plasma.
This review of sleep disorders in children uses a background of sleep physiology and normal development to propose an approach to the common disorders of wakefulness. A group of disorders associated with emergent sleep is presented. The less common disorders of hypersomnia are not discussed.
