[APC mutation analysis in sporadic colorectal cancer].

APC gene, responsible for familial adenomatous polyposiscoli, is one of the tumor suppressor genes involved in tumorigenesis of sporadic colorectal cancer. Because the majority of APC mutations are located in the 5' end of exon 15 and 95% of them generate stop codon, a coupled in vitro transcription and translation system (IVSP) was used to detect somatic APC mutation of exon 15 in sporadic colorectal cancer. Forty-nine mutated sporadic cases containing 59 truncated bands (9 cases with 2 bands) were identified. The results of the study showed: 1. APC frameshift mutations were more frequent than point mutations (60% vs 40%). Most of the frameshift mutations (32/35) were deletion or insertion of 1-2 bp, whereas 16 of all 23 point mutations presented C-to-T transitions. Mutations (57/59) were mostly but unevenly distributed in segment 2 and 3 of exon 15, while in segment 3, 69% (45/59) of all the mutations were predominantly located in mutation cluster region (codon 1286-1531). In seven of the 9 cases with 2 mutations their close proximity with each other is suggestive of the possibility of mutations at 2 independent alleles. The similarities between APC somatic and germline mutations suggest that APC gene might well have some common functions during the tumorigenesis of sporadic and hereditary coloretal cancer.

[Altered cytokeratin expression of the epidermoid formation in the tympanic cavity of human fetuses].

The epidermoid formation (EF) in the developing human fetal middle ear is believed to have relationship with congenital cholesteatoma. Sixteen EFs were included in this study, of which 15 were simple EFs and one was hyperplastic EF. Four cytokeratin antibodies PK, CK13, CK18, and CK19 were used in an ABC immunohistochemical technique to compare the expression of the cytokeratin in the EFs, middle ear mucosa and external acoustic meatus skin. The results showed that the simple EFs had the same staining pattern of middle ear mucosa for all four antibodies and were different from external acoustic meatus skin. It was suggested that the simple EFs do not have the properties of an epidermis. The hyperplastic EF had an altered cytokeratin expression comparing to that of the middle ear mucosa. The expression of CK19 disappeared in the structure while the normal middle ear mucosa nearby had a positive staining for this antibody. PK staining pattern, characterized by the more intensive in the surface layer of the EF, was similar to that of external acoustic meatus skin. It is suggested that hyperplastic EF may acquire certain properties of epidermis, and may develop into congenital cholesteatoma.

[Twenty-years health surveillance of workers in dipterex packaging].

Data of health surveillance from 1970 to 1989 for the workers employed in dipterex packaging were collected and statistically analyzed with a microcomputer by scoring their whole blood choline esterase activities and symptoms and signs. Results showed working environment improved and air pollution and absorption of pollutants via skin decreased with technology innovation and longterm hygienic supervision, monitoring and health surveillance in the factory. Incidence of dipterex poisoning in workers employed in the department of pesticide packaging lowered significantly, from 25.26% in the early 1970's to 5.17% in late 1970's and to 1.85% during 1980's, with periodical physical examinations for them, and timely management of the patients with poisoning and the cases at high risk.

[Exploration of certain factors affecting the airway resistance and its determination with interruption technique for normal old, middle-aged and young persons].

The airway resistance (Raw) was determined with interruption technique for 327 normal old, middle-aged and young persons after exploration of the effect of forced respiration, lung volume and upper airway compliance on it. FVC, V75, V50 and V25 were also performed. The regression equations of expiratory and inspiratory resistances (R. R1) were built with age, height and FVC as independent variables. The results showed that both expiratory and inspiratory resistances had a tendency of increase with aging, and FVC, V75, V50, V25 were declined with aging. This reflected that various organs and tissues including respiratory organs gradually oldened with aging. The decrease of elastic recoil of lung and central airway can lead to an increase of airway resistance in various degrees or no change. The data and regression equations can be recommended for the application in public health and clinical care.

[Pericardial devascularization in the treatment of portal hypertension with Larjet's innervation].

From 1982, 48 cases of portal hypertension were treated by pericardral devascularization with Larjet's innervation. Results were compared with that without preserving the Larjet's nerve in 57 cases. The incidence of rebleeding, mortality, abdominal distension, diarrhea, and gastric retension was 2%, 6.3%, 6.3%, 4.2%, and 0%, respectively in the former; and 12.5%, 12.3%, 24.6%, 15.6%, and 14%, respectively in the later. The authors believe that gastric function remained normal postoperatively when the Larjet's innervation was preserved.

[The postoperative effects of tapered terminal ileal continent urinary reservoir].

We report postoperative effects of 23 cases having tapered terminal ileal continent urinary reservoir is reported. The operative technique was characterized by tapering the terminal ileum with stapler to form a continent efferent tract, segments of intestine were cut open and reformed a pouch, and the ureter implantation was performed by means of inserting the end of ureters into the pouch. Ileocolonic pouch wes constructed for 14 patients and colonic pouch for 9 patients. 22 patients were followed up for 1 to 6 years except one died of cerebral hemorrhage 2 weeks after operation. Urine continence were achieved in all patients and self catheterization could be easily undertaken with 20F-22F catheter in 21. Difficulty in catheterization was happened in one early case. One year after operation, the maximum volume of the ileocolonic pouch was 900 ml-1000 ml. The basal and intermittent pressure of the filling pouch was 0.49 kPa-1.96 kPa respectively. The maximum volume of the colonic pouch was 400 ml-500 ml. The basal and intermittent pressure of the filling pouch of the colonic pouch was 0.98 kPa-1.9 6 kPa and 1.96 kPa-3.93 kPa respectively. The complication of pouch overdistention was found in 6 cases, pouch stone formation in 2 and pouch infection in 12 episodes. There was no evidence of ureteral obstruction or reflux and significant hyperchloric acidosis. It is recognized that tapered terminal ileum is a good way to construct a continent efferent tract, both ileocolonic pouch and colonic pouch can meet the need of low inner pouch pressure so long as the intestines were cut open and reformed, and efforts must be made to prevent the complications of pouch overdistension, stone formation and pouch infection.

[Treatment of irresectable hepatocellular carcinoma with repeated transient dearterialization].

A joint clinical prospective study between SUMS and Lund university was reported. 40 patients with the irresectable hepatocellular carcinoma (HCC) admitted to the department of HPB surgery in the First Affiliated Hospital of SUMS were randomized into two groups: (20 each) from Feb. 1994 to April, 1995. The patients were treated with hepatic artery ligation (HAL) and repeated transient dearterialization (RTD) respectively. Postoperative response to treatment, liver function change (ALT), AFP, imaging examination of the tumor, patient's survival were evaluated. It has been shown that RTD is superior to HAL in terms of the objective response to the therapy, reduction of tumor size, patient's symptom relief, liver function and AFP changes and patient's survival. In the RTD group, the effective rate was 70%, the mean survival time was 8.2 months, and the 6-month survival rate was 79.7%. In HAL group, the effective rate was only 5%, the mean survival time was 5.1 months, 6 months survival rate was 35.8%. It has been postulated that RTD may prevent the rapid development of collateral circulation and increase the production of oxygen-derived free radicals, which may be the responsible factors for the ischemic treatment of hepatic tumours. We consider that RTD would be a promising polliative method for HCC.

[Immunogenicity of recombinant S. typhimurium ex. pressing a hybrid antigen of Plasmodium falciparum].

We have expressed a 74-peptide hybrid Plasmodium falciparum antigen as fusion protein in attenuated Salmonella typhimurium SL3261. Live organisms were orally immunized Rabbits with a dose of 2 x 10(9)cfu. Specific anti-serum were detected by ELISA after immunization. Obvious Delayed type hypersensitivity (DTH) could be induced by PfAg and GZ-C antigen. The recombinant vaccine had no evident side-effects to the hosts. Our studies indicate that attenuated Salmonella typhimurium SL3261 can express synthetic P. falciparum antigen with several epitopes and live organisms can activate special cell-mediated immunity and humoral immunity.

Identification of inhibitory and calmodulin-binding domains of the PDE1A1 and PDE1A2 calmodulin-stimulated cyclic nucleotide phosphodiesterases.

Using a bovine 61-kDa (PDE1A2) calmodulin-stimulated phosphodiesterase (CaM-PDE) cDNA and a bovine lung 59-kDa (PDE1A1) CaM-PDE cDNA reported here, we have identified two new regions within the primary structure of these two related isozymes that are important for regulation by Ca2+/CaM. PDE1A1 is identical to the PDE1A2 isozyme except for the amino-terminal 18 residues. In agreement with earlier studies, the CaM concentration required for half-maximal activation (KCaM) of recombinant PDE1A1 (0.3 nM) was approximately 10-fold less than the KCaM for recombinant PDE1A2 (4 nM). A series of deletion mutations of the PDE1A2 cDNA removing nucleotide sequence encoding the first 46-106 aminoterminal residues were constructed and expressed using the baculovirus system. Deletion of the amino acids encompassing a previously identified, putative CaM-binding domain (residues 4-46) produced a polypeptide that was still activated 3-fold by CaM (KCaM approximately 3 nM). However, complete CaM-independent activation occurred when residues 4-98 were deleted. To determine the location of the additional CaM-binding domain(s), the inhibitory potency of seven overlapping, synthetic peptides spanning amino acids 76-149 of PDE1A2 was tested using the CaM-activated enzyme. One peptide spanning amino acids 114-137 of PDE1A2 appeared to be the most potent inhibitor of CaM-stimulated activity. These results reveal the existence of a CaM-binding domain located approximately 90 residues carboxyl-terminal to the putative CaM-binding domains previously identified within the PDE1A1 and PDE1A2 isozymes. Moreover, a discrete segment important for holding these CaM-PDEs in a less active state at low Ca2+ concentrations is located between the two CaM-binding domains.

Stoichiometry of p22-phox and gp91-phox in phagocyte cytochrome b558.

The phagocyte NADPH oxidase complex is an unusual electron transfer system. Its principal component, cytochrome b558, is a heme-containing integral membrane protein consisting of two subunits, gp91-phox and p22-phox. We used a novel method to measure precisely the gp91-phox:p22-phox stoichiometry. Cytochrome b558 was isolated in high purity from human neutrophil membrane preparations using a novel affinity purification method. We performed direct peptide sequencing of purified cytochrome b558 and detected two amino acid sequences which matched predicted sequences for gp91-phox and p22-phox. We quantitated amounts of both amino acids released from p22-phox and gp91-phox in each sequencing cycle. Averaging over 25 cycles, the mean p22-phox:gp91-phox ratio of released amino acids was 0.93 +/- 0.01. To correct for recovery differences between individual amino acids, we measured individual p22-phox:gp91-phox ratios for the eight different amino acids common to both p22-phox and gp91-phox in the first 25 positions. The mean of individual p22-phox:gp91-phox ratios for the eight common amino acids was 0.96 +/- 0.05. The p22-phox:gp91-phox ratios for each of the eight common amino acids varied from 0.81 to 1.20. Taken together, measured ratios for total and individual amino acids are consistent with a predicted ratio of 1.0 for 1:1 p22-phox:gp91-phox stoichiometry in cytochrome b558.

Identification of two amino acid residues on the extracellular domain of the lutropin/choriogonadotropin receptor important in signaling.

The lutropin/choriogonadotropin receptor (LH/CG-R) is a G protein-coupled receptor with a relatively large extracellular domain. The cDNAs of LH/CG-R wild type and 15 point and double mutants, which encoded residues of opposite charge to that of wild type, were transiently transfected into COS-7 cells. Human choriogonadotropin (hCG) binding was determined, as was hCG-mediated cAMP production. Most of the replacements resulted in no substantive effect on the binding affinity of hCG to LH/CG-R or on hCG-stimulated cAMP production, although the mutants expressed at a lower level than LH/CG-R wild type. The most interesting observation was noted with two point mutants of LH/CG-R, Glu332-->Lys and Asp333-->Lys, which bound hCG but failed to give increased cAMP production. Several of the mutant forms of LH/CG-R that expressed at low levels were further analyzed by soluble binding assays and Western blots. There was no evidence of any significant degree of intracellular trapping of hCG-binding mutant receptors. The expected major (93 kDa) and minor (78 kDa) forms were found for LH/CG-R wild type and several of the mutants. The Lys235-->Asp and Asp333-->Lys mutants exhibited primarily the lower M(r) form, indicating that receptor processing was impaired or that the mutant higher M(r) form was more rapidly degraded than LH/CG-R wild type. These results demonstrate that Glu332 and Asp333, which are located near the first transmembrane helix, are important in receptor activation, while other conserved ionizable residues of LH/CG-R appear important in cell surface expression or stability but not in binding or signaling.

Isocitrate dehydrogenase from bovine heart: primary structure of subunit 3/4.

Bovine NAD(+)-dependent isocitrate dehydrogenase was shown previously to contain four subunits of approx. 40 kDa (subunits 1-4) possessing different peptide maps and electrophoretic properties [Rushbrook and Harvey (1978) Biochemistry 17, 5339-5346]. In this study the heterogeneity is confirmed using enzyme purified by updated methods and from single animals, ruling out allelic variability. Subunits 1 and 2 were differentiated from each other and from subunits 3 and 4 by N-terminal amino acid sequencing. Subunits 3 and 4 (subunits 3/4) were identical in sequence over 30 residues. The N-terminal residues of subunits 1 and 2 were homologous but not identical with the beta- and gamma-subunits respectively of the comparable pig heart enzyme. Subunits 3/4 were identical over 30 residues with the N-terminus of the pig heart alpha-subunit. Full-length sequence, including that for mitochondrial import, is presented for a protein with the processed N-terminus of subunits 3/4, deduced from cloned cDNA obtained utilizing the N-terminal sequence information. The derived amino acid sequence for the mature protein contains 339 amino acids and has a molecular mass of 36,685 Da. Complete identity with N-terminal and Cys-containing peptides totalling 92 residues from the alpha-subunit of the pig heart enzyme [Huang and Colman (1990) Biochemistry 29, 8266-8273] suggests that maintenance of a particular three-dimensional structure in this subunit is crucial to the function of the enzyme. An electrophoretic heterogeneity within the pig heart alpha-subunit, similar to that shown by bovine subunits 3/4, was demonstrated. One reordering of the Cys-containing peptides of the pig heart alpha-subunit is indicated. Sequence comparison with the distantly related NADP(+)-dependent enzyme from Escherichia coli, for which the three-dimensional structure is known [Stoddard, Dean and Koshland (1993) Biochemistry 32, 9310-9316] shows strong conservation of residues binding isocitrate, Mg2+ and the NAD+ moiety of NADP+, consistent with a catalytic function.

Evidence of autocrine modulation of macrophage nitric oxide synthase by alpha-melanocyte-stimulating hormone.

alpha-Melanocyte-stimulating hormone (alpha-MSH) is a potent inhibitory agent in all major forms of inflammation. To identify a potential mechanism of antiinflammatory action of alpha-MSH, we tested its effects on production of nitric oxide (NO), believed to be a mediator common to all forms of inflammation. We measured NO and alpha-MSH production in RAW 264.7 cultured murine macrophages stimulated with bacterial lipopolysaccharide and interferon gamma. alpha-MSH inhibited production of NO, as estimated from nitrite production and nitration of endogenous macrophage proteins. This occurred through inhibition of production of NO synthase II protein; steady-state NO synthase II mRNA abundance was also reduced. alpha-MSH increased cAMP accumulation in RAW cells, characteristic of alpha-MSH receptors in other cell types. RAW cells also expressed mRNA for the primary alpha-MSH receptor (melanocortin 1). mRNA for proopiomelanocortin, the precursor molecular of alpha-MSH, was expressed in RAW cells, and tumor necrosis factor alpha increased production and release of alpha-MSH. These results suggest that the proinflammatory cytokine tumor necrosis factor alpha can induce macrophages to increase production of alpha-MSH, which then becomes available to act upon melanocortin receptors on the same cells. Such stimulation of melanocortin receptors could modulate inflammation by inhibiting the production of NO. The results suggest that alpha-MSH is an autocrine factor in macrophages which modulates inflammation by counteracting the effects of proinflammatory cytokines.
