[Thallium poisoning a clinical analysis of 5 cases].

OBJECTIVE: To sumarize the clinical features, diagnosis and treatment of thallium poisoning by analysis of clinical cases. METHODS: Five cases of thallium poisoning were reported. The clinical manifestations, quantitative-analysed thallium level, methods and effect of treatment, and prognosis were analysed. RESULTS: Three of the 5 cases were acute, and the other two chronic. Four of them had a history of poison contact. The clinical features included alopicia (4 cases), transverse white stripes in the nails (Mee's stripes, 2) polyneuropathy, cranial nerve and central nervous system impairments (5), and abdominal pain and other digestion disturbances (3). All of the 5 cases were treated with antidotes and other methods to increase thallium excretion. One case died of lung infection 3 days after diagnosis. The serum and urine thallium levels of the 4 survivors decreased to normal after treatment (3 recovered, one had neurologic sequelae). CONCLUSION: It is not difficult to diagnose thallium poisoning by definite poison contact history, classical manifestation, and serum or urine thallium quantitative analysis. Attention should be paid to those patients with indefinite exposure to thallium. For those suspected cases, it is necessary to measure the thallium level in blood and urine in order to verify the diagnosis.

[A model of nasopharyngeal carcinoma with spontaneous highly lymphatic metastasis in nude mice and its biological characteristics].

OBJECTIVES: To establish a model of nasopharyngeal carcinoma with spontaneous highly lymphatic metastasis in nude mice and to study its biological characteristics. METHODS: The clone cells of nasopharyngeal carcinoma(NPC), CNE-2Z-H5, were inoculated into nude mice. The cancer cells of lymph node metastatic foci were transplanted into nude mice again when the metastasis of nude mice were observed. After repetition of this procedure for 9 cycles, the metastatic rate and the metastatic paths were observed in nude mice of every passage; The changes were also observed in proliferation ability, expression of growth factor and its receptor in vitro, proliferation index(PI), and tumor weight and doubling time of the transplantable tumor in every passage in vivo. RESULTS: The lymphatic metastatic rates were higher than 85%. The metastatic paths were single, the proliferation ability was increased in cancer cells of every passage in vivo and vitro after passaging cancer cells of lymphatic metastatic foci for 9 cycles in nude mice. CONCLUSIONS: A model of nasopharyngeal carcinoma with spontaneous highly lymphatic metastasis has been established in nude mice, and the possibilities of metastasis and proliferation are increased after progression of NPC cells in nude mice. There is a close relationship between the metastasis and proliferation in NPC.

[Experimental investigations on cartilage regeneration from perichondrium].

Cartilage regeneration was histologically evaluated in the pedicle thyroid perichondrial flaps bridging between the thyroid and the cricoid of 13 dogs. The result showed that the new cartilage presented mainly at the part of the pedicle end of the flap but disappeared gradually at middle part of the flap, and was replaced gradually by fibrosis from the pedicle end to the free end of the flap. It seemed that the perichondrial flap processed potential but limited capability of cartilage regeneration.

Serological diagnosis of nasopharyngeal carcinoma by enzyme linked immunosorbent assay: optimization, standardization and diagnostic criteria.

OBJECTIVES: To produce an enzyme linked immunosorbent assay (ELISA) to detect antibodies against Epstein Barr virus (EBV) specified nuclear antigen 1 (EBNA 1) and the 18 kD EBV matrix protein, and to determine and optimize its sensitivity and specificity for the diagnosis of nasopharyngeal carcinoma (NPC). METHODS: We used a combination of highly purified glutathione transferase fusion proteins of the 40 kD carboxy domain of EBNA1 and the 18 kD EBV matrix protein for coating ELISA plates. In three separate studies, we tested for IgA antibodies in serum specimens from 28 EBV seronegative donors, 284 EBV seropositive donors and 160 newly diagnosed NPC patients. By comparing the sensitivity and specificity of diagnosis obtained for different cutoff values, we derived several quantitative parameters to evaluate assay performance, establish objective diagnostic criteria which optimize the intrinsic diagnostic capability of the assay and assess the significance of individual test results, respectively. Optimum cutoff optical density (OD) is defined as the cutoff OD where sensitivity of the assay equals its specificity, and resolution of the assay is indicated by the value of sensitivity (or specificity) determined at the optimum cutoff OD. Diagnosis of NPC was achieved by setting a cutoff zone at +/- 20% of this value. RESULTS: All the EBV seronegative donors tested were not reactive, and most of the EBV seropositive donors were weakly reactive, while the majority of NPC patients were moderately or strongly reactive. While the assay was thus shown to be specific for EBV, there was an overlap in the level of these serum antibodies between few individuals of the two latter groups. It was shown that the assay performed equally well in two separate studies conducted under different testing conditions and using different collections of sera in that assay resolution determined on these occasions were 86% and 87% respectively. Diagnosis of NPC can be achieved at the same expected sensitivity of 89% and 83% determined at the lower and upper limits of the cutoff zones, with the corresponding values of specificity being 78% and 91%. It was further shown in the third study that resolution of the assay can be increased to 90% using an assay produced with a higher concentration of the same antigens, and that diagnosis of NPC can be achieved at a higher sensitivity ranging between 86% and 95% at a corresponding specificity of 93% and 86%. CONCLUSIONS: After optimization and standardization, the ELISA can achieve a sensitivity ranging from 86% to 95%, with corresponding specificities of 93% and 86% respectively for the diagnosis of NPC.

Experimental study on liver microcirculation disturbance following transplantation and the protective effect of prostaglandin E1 in the rat.

OBJECTIVE: To determine the effect of PGE1 on liver microcirculation disturbance following orthotopic liver transplantation in rats. METHODS: Forty male adult Wistar rats were divided randomly into 3 groups. Eight transplantations were established in both the experimental and control group, while in the sham group, the liver was dissected like in the experimental group, but no resection was performed. In the experimental group, PGE1(0.5 microgram/kg.min-1) was injected intravenously into the donor before the operation, and added (1 mg/L) to the flush and preservation fluid, while PGE1 was replaced by normal saline in the control group. Confocal laser scan microscopy, biochemical test, and optical and electronic microscopy were used. RESULTS: In the control group the reperfusion state was poor, leukocyte infiltration appeared in the center of lobule, and transaminase rose after transplantation. In the experimental group distinctive improvement was seen as compared with the control group (P < 0.05). Histological findings showed progressive degeneration and necrosis following transplantation in the control group, while in the experimental group the histological changes were improved to some degree by the use of PGE1. CONCLUSIONS: In liver transplantation, ischemic reperfusion damage may lead to hepatic microcirculation disturbance, which is the major cause of graft failure. Infusing PGE1 into the donor intravenously before ischemia and adding PGE1 to the cold storage fluid could improve hepatic microcirculation, and thus reducing ischemic reperfusion damage in liver transplantation.

Pericardial devascularization combined with Latarjet's innervation in the treatment of patients with portal hypertension.

OBJECTIVE: To study the effect of pericardial devascularization combined with Latarjet's innervation on portal hypertension. METHODS: Forty-eight patients undergoing pericardial devascularization combined with Latarjet's innervation were compared with 57 patients with devascularization. Clinical results were evaluated by postoperative portal vein pressure, postoperative rebleeding, operative mortality, abdominal distension, sudden diarrhea, and gastric retention. RESULTS: The incidence of rebleeding, mortality, abdominal distension, sudden diarrhea, and gastric retention was 2%, 6.3%, 6.3%, 4.2% and 0% respectively in pericardial devascularization combined with Latarjet's innervation, and 12.5%, 12.3%, 24.6%, 15.6% and 14% respectively in devascularization. CONCLUSIONS: Pericardial devascularization combined with Latarjet's innervation preserves the normal function of gastric emptying. This method plays an important role in maintaining nutritional supply and digestive function.

[Hepatectomy for hepatolithiasis treatment of 354 cases].

OBJECTIVE: To evaluate the effect of hepatectomy for hepatolithiasis. METHOD: 354 patients with hepatolithiasis underwent hepatectomy in the past 10 years. The results were analysed retrospectively, including clinical findings, distribution of stones, patterns of operation, postoperative complications, and residual stones. The clinical data of the patients before 1990 were compared with those thereafter. RESULT: Left hepatolithiasis was the most common form (323 patients). Left lateral lobectomy and left hepatectomy were most commonly employed (91.2%). 166 of the patients underwent concurrent cholangiojejunostomy. 13.8% patients had residual stones, and postoperative complications occurred in 60 patients, including 4 deaths. Few patients were reoperated on and the incidence of residual stones was lower after 1990. 88% the patients showed excellent or good result. CONCLUSION: Hepatectomy is a procedure for the management of hepatolithiasis, but still requires combined plasty of stenotic intrahepatic bile ducts and cholangiojejunostomy to reduce the incidence of residual stones and recurrence.

[Expression of eNOS and IL-10 gene in signal transduction for liver regeneration].

OBJECTIVE: To detect the effects of eNOS and IL-10 on liver regeneration under physiological and pathological condition. METHOD: The rat model of liver regeneration was made by partial (70%) hepatectomy. Some rats were injected with CCl(4) and performed splenectomy. Hepatocytes DNA replication was tested by flow cytometry method (FCM) and the gene expression of eNOS in liver cells were analysed using RT-PCR. The IL-10 gene expression in liver cells was shown by Dot Blot hybridization method. The synthesis and distribution of IL-10 protein in liver tissue were examined by immune histochemical and morphometric method. RESULT: The expression of eNOS gene promoted liver regeneration while the expression of IL-10 gene inhibited it. CONCLUSION: eNOS and IL-10 play an important immunomodulatory role in liver regeneration under physiological or pathological conditions. The effects of eNOS and IL-10 on liver regeneration differ and they show an opposite modulatory effects on liver regeneration. Such effects are affected by spleen and general immune system.

[Dynamic change of androgen-receptor in tumor and surrounding liver tissue of nude mice model with transplanted human HCC].

OBJECTIVE: To research into the law of androgen-receptor (AR) dynamic change in developing course of human HCC. METHOD: Dynamic measurement of AR in tumor and peritumorous tissue was performed in nude mice model with orthotopic transplanted human HCC. RESULT: Tumor nodules were present in 90% nude mice 4 weeks after transplantation, tumor necrosis was found in the 10th week, and remarkable necrosis occurred in the 12th week. The quantity of AR in tumor and peritumorous tissue was highest in the early stage of tumor (102.32 +/- 21.42 fmol/mg protein and 72.45 +/- 10.11 fmol/mg protein respectively), and then decreased progressively with time to the 10th week (40.98 +/- 21.11 fmol/mg protein and 53.39 +/- 7.01 fmol/mg protein respectively). The difference in AR between each two weeks was significant. CONCLUSION: Androgen promotes the development of tumor because of existence of AR, and its action may decline after establishment of tumor. Therefore, antiandrogen treatment may be ineffective after establishment of tumor.

[A preliminary study on cytokines in proliferative vitreoretinopathy].

OBJECTIVE: To investigate the roles of cytokines like interleukin-6 (IL-6), tumor necrosis factor (TNF) and local immune reaction in proliferative vitreoretinopathy (PVR). METHODS: Immunohistochemical procedures were applied to identify IgG and complement C(3) deposits in the specimens of epiretinal membranes surgically obtained from patients with PVR. Vitreous aspirates from 33 eyes with idiopathic and traumatic PVR and 8 normal eyes were examined for IgG and complement C(3) by using ELISA; the total vitreous protein (TVP) was determined by Coomassie Brilliant Blue Test; the relative activity of IL-6 in vitreous was tested by means of a proliferative response of IL-6 dependent cells to vitreous samples; and the biological activity of TNF in vitreous was measured with its killing effect on L(929) tumor cells. RESULTS: Diffuse deposits of IgG and C(3), TVP in epiretinal membranes of PVR eyes were significantly higher than those in the control group; the activities of IL-6 and TNF (0/8) were not detected in any of the normal control, IL-6 was detected in 13 of 17 vitreous samples and TNF was detected in 3 of 11 vitreous samples from PVR eyes; and vitreous IL-6 activity was found to be positively correlated with the severity of PVR. CONCLUSION: The deposits of IgG and C(3), their elevation in amount and the detection of IL-6 and TNF activity in the vitreous samples from patients with PVR suggest that the cytokines and local immune reaction play a role in the pathogenesis of PVR.

[The A1- and non A1-effects of N6-(5-hydroxy-2-pyridyl)-methyl-adenosine on rat vas deferens].

N6-(5-hydroxy-2-pyridyl)-methyl-adenosine (HPMA) is a novel N6-substituted adenosine analogue recently obtained from Armillaria mellea (an edible fungus on which depends the growth of the famous Chinese traditional drug Gastrodia elata). It has been shown to have some characters of A1 receptor agonists of purinergic nerve. In this study, we compared the effects of HPMA with that of N6-Cyclohexyladenosine(CHA), an A1 selective agonist, on rat vas deferens in vitro, and found remarkable differences between them. In our study, HPMA dose-dependently decreased the contraction responses to exogenous Phenylephrine (PE), Norepinephrine (NE) and Acetylcholine(ACh) on rat vas deferens, while CHA showed no effect on these responses. 8-Cyclopentyl-1,3-dipropylxanthine(DPCPX), an A1 selective antagonist, did not show any influence on these effects of HPMA, indicating that the depression effect of HPMA may be through a non-A1 mechanism. Using HPMA to decrease about 50% of the twitch responses evoked by field-stimulation on rat vas deferens, the responsiveness to exogenous ACh seemed to be similar to that without HPMA pretreatment. These indicate that HPMA at this dosage (IC50 dosage) preferentially acted on presynapse (may be the A1 receptor) to attenuate the release of neurotransmitters. At a high dosage (10(-5) mol.L-1), HPMA abolished the neurogenic twitch responses evoked by electrical field-stimulation, while the responsiveness of rat vas deferens to exogenous ACh was decreased showing both pre-synapse and post-synapse depression.

[Determination of 7 bio-active alkaloids in Stephania plants by RP-HPLC].

Seven bio-active alkaloids (stepholidine, sinoacutine, isocorydine, l-tetrahydropalmatine, crebanine, fanchinoline and tetrandrine) in Stephania plants were determined by RP-HPLC, using UV detection (282 nm) and gradient elution. The reversed phase system consisted of ODS column and methanol-water-triethylamine as mobile phase. The flow rate was 1.0 ml.min-1. Good linearity between peak heights and concentrations of the alkaloids was obtained in the concentration range. The HPLC method proved accurate, precise and sensitive. The results showed that there were some differences in the occurrence and content of the alkaloids between various species and between the same species from different habitats and collecting time. Based on the results, some species with high content of the 7 bio-active alkaloids were selected. The study provided some useful information for the utilization of medicinal plant resources in the genus Stephania.

[Preparation and characterization of monoclonal antibodies against Clonorchis sinensis].

AIM: To develop and identify monoclonal antibody (McAb) against adult Clonorchis sinensis. METHODS: BALB/c mice were immunized with soluble antigen of adult Clonorchis sinensis, the spleen cells of immunized mice were fused with myeloma cell line SP2/0. Then the hybridoma cell lines secreting high titer of McAbs were screened. The Ig subtypes and titer of the McAbs were assayed with ELISA. The antigens recognized by the McAbs were examined with IFAT and identified by immunoblot analysis. The cross reactions between the McAbs and antigens of Schistosoma japonicum, Paragonimus westermani and Cysticercus cellulosae were detected. RESULTS: 5 hybridoma cell lines secreting high titer of McAbs against adult Clonorchis sinensis were established. All five clones of McAbs belonged to IgM subtype. The antigens recognized by the McAbs were located at the intestinal tract wall of adult Clonorchis sinensis. The McAbs had no cross reactions with antigens of Schistosoma japonicum, Paragonimus and Cysticercus cellulosae. CONCLUSION: 5 hybridoma cell lines against adult Clonorchis sinensis secreting high titer of McAbs with high specificity were established.
