Impact of RT-PCR monitoring on the long-term survival in acute promyelocytic leukemia.

OBJECTIVE: To evaluate the impact of kinetics of molecular remission via retro-transcriptase polymerase chain reaction (RT-PCR) assay on the long-term survival in patients with acute promyelocytic leukemia (APL). METHODS: Seventy patients with newly-diagnosed APL in remission were involved in this study. Monitoring of minimal residual disease (MRD) was performed regularly by RT-PCR assay for PML-RAR alpha during consolidation. RESULTS: A 5-year relapse-free survival (RFS) and overall-survival (OS) were estimated as 46.8% +/- 8.4% and 69.9% +/- 9.4% for the whole group. Fifty-two (74.3%) patients got negative RT-PCR result at least once. Serial monitoring of RT-PCR was available in 38 cases and 24 (63.2%) patients presented with persistent negative PCR results. The achievement and continuous negative RT-PCR result was significantly related to the RFS. CONCLUSIONS: Achievement of negative RT-PCR in remission is associated with favorable RFS and OS. Continuous negative RT-PCR results are associated with long-term relapse-free survival and may be considered as potentially curative. RT-PCR assay for detection of MRD should be performed regularly during post-remission period as an important prognostic factor.

[Effects of bushen yizhi recipe on somatostatin-like immunopositive and somatostatin messenger ribonucleic acid expressed-positive neurons in Alzheimer's disease model rats].

OBJECTIVE: To observe the effect of Bushen Yizhi Recipe (BYD) on somatostatin-like (SS-like) immuno-reactivity and SS mRNA expressed-positive neurons in Alzheimer disease (AD) model rats induced by D-galactose intraperitoneal and ibotenic acid (IBO) intracerebral injection. METHODS: Fifteen months Wistar rats were randomly divided into 4 groups: normal control; AD model control induced by D-galactose intraperitoneal injection and lesion of basal nucleus of Meynert with IBO; Huperzine A (Hup-A) treated group and BYD treated group fed with Hup-A or BYD respectively. The SS-like immunoreactivity and SS mRNA expressed-positive neurons in parietal lobe, hippocampal CA1 field and hilus of the dentate gyrus in the brains of rats, which were detected with immunohistochemistry and DIG-labelling cRNA-probe in situ hybridization. RESULTS: Counts and integrations of optic density of SS-like immuno-reactivity and SS-mRNA expressed neurons in parietal lobe, hippocampal CA1 field and hilus of the dentate gyrus in BYD treated group increased compared with AD model rats. CONCLUSION: BYD might protect the SS-ergic system against impairment of D-galactose and IBO in the brains of the AD model rats.

[The relationship between vac A genotype of Helicobacter pylori clinical isolate and gastric cancer and precancer].

OBJECTIVE: To analyze the relationship between vac A genotype of Helicobacter pylori (Hp) and gastric cancer and precancer in Xi'an area. METHODS: To establish the stock of Hp clinical isolates and perform vac A gene typing of Hp isolates by PCR. RESULTS: 192 Hp clinical strains are isolated from the antrum of 259 patients. In them, type s1 is 174 strains (90.6%); s2 is 18 stains (9.4%). Type m1 and m2 is 99 (51.6%) and 93 (48.4%) respectively. All strains are s1 or s2 and m1 or m2. In gastric cancer group, the expression of type s1 is 94.5% and s1a is more than s1b (P < 0.05). There is no difference between type m1 and m2. In gastritis group, type s1 is 89.8%, s1a is almost equal to s1b and no difference between m1 and m2. CONCLUSION: Type s1 is the main expression in both gastric cancer group and gastritis group. Expression of type s1a is higher than s1b. We can't determine it is a malignant index of gastrointestinal diseases. We need to research it extensively.

[Dural arterovenous fistula involving cavernous sinus].

OBJECTIVE: To study the pathogenesis and treatment of dural arteriovenous fistulas (DAVFS) involving the cavernous sinus. METHODS: 32 cases were embolized with particle by microcatheterization via endovascular approach and ervation by digital substraction angiography. Of the 32 cases, 14 were embolized by microsoils through superior petrosal sinus approach into the cavernous sinus. RESULTS: In 28 of the 32 cases the fistulas were completely embolized angiographically. The fistulas were partly embolized in 4 of the 32 cases. The carotid artery was compressed for 6 months. One week later, the fistula disappeared angiographically. No fistulas were found during the follow-up for 6 months to 8 years. CONCLUSION: Endovascular treatment of DAVFS involving the cavernous sinus is effective.

[Control genes of chondrocyte apoptosis in osteoarthritic articular cartilage].

OBJECTIVE: To investigate the expression of bax and bcl-2 in normal human articular chondrocytes and in osteoarthritic articular cartilage. METHODS: The samples of articular cartilage were obtained from 9 patients and 6 normal subjects. Bax and bcl-2 mRNA were detected by reverse transcriptase/polymerase chain reaction (RT-PCR) and their expression proteins were analyzed immunohistochemically. TUNEL technique was used to study apoptosis in situ. RESULTS: Bax and bcl-2 mRNA was detectable in chondrocytes of both osteoarthritic and normal cartilage. Bax mRNA of chondrocytes from patients with osteoarthritis (OA) was overexpressed compared with the normal controls (P < 0.01), and OA cartilage chondrocyte also expressed more bcl-2 mRNA than the controls (P < 0.05). There was no statistically significant difference of bax/bcl-2 between the two groups. Immunohistochemical staining demonstrated the same level of bax and bcl-2 proteins as their mRNA. A greater proportion of apoptotic chondrocytes were found in the OA cartilage than that in normal controls (4% - 14% versus 0 - 2%). CONCLUSIONS: Our results suggested that chondrocyte apoptosis was co-regulated by both bax and bcl-2. The ratio of bax to bcl-2 may contribute not only to the lower percentage of apoptotic chondrocytes, but also to the chronic pathologic process in OA.

[Microsurgical treatment of large or giant aneurysms in anterior circle of Willis].

OBJECTIVE: To improve the operative results in large or giant aneurysms of the anterior circle Willis. METHODS: Retrospective analysis was performed on 28 patients with large or giant aneurysms of the anterior circle Willis after operation. Three-D CT angiography was used to detect and delineate aneurysm preoperatively. Improved operative technique included cranial base approach, temporary artery occlusion, intra-aneurysm decompression, obliteration of aneurysm with multiple clips, stepwise coagulation clipping and resection of aneurysms. RESULTS: Good results were noted in 24 patients (85.7%), fair in 2, and poor in 1. One patient died, but no patient with Hunt and Hess 0-III grade died. CONCLUSIONS: Three-D CT angiography is useful in delineating the shape, size, neck of aneurysms, and their relation to near arteries and to cranial base structures. These are helpful to improve operative results of large or giant aneurysms of the anterior circle of Willis.

[Study on the genetic difference of SEO type Hantaviruses].

OBJECTIVE: To understand the genetic type of Hantaviruses and the difference between them caused by rodents in Beijing and to furhter explore the source of the infectious factors. METHODS: Hantavirus RNA, isolated from lungs of rodents captured in Beijing and positive with Hantavirus antigens with frozen sectioning and Immunofluorescent assay, were reverse-transcribed and amplified with PCR with Hantavirus-specific primers. RESULTS: Five of the PCR amplifications were discovered and sequenced with 300 bp sequence data of M segments (from 2003 - 2302nt according cDNA of seoul 8039 strain). Nucleotide sequence homology showed that they were sequences of SEO-type Hantavirus. Compared with SEO type Hantavirus, the nucleotide sequence homology of these samples was more than 94% while the homology of amonia acid sequence was more than 98%. When compared with HNT type Hantavirus, the homology of nucleotide sequence became less than 72% with the homology of amonia acid sequence less than 81%. Similar to other Hantavirus of SEO type, their nucleotide sequences and deduced amino acid sequences were highly preserved. Phylogenetic tree analysis showed that the five viruses could be divided into at least 4 branches. CONCLUSION: It was quite likely that there were at least two sub-type SEO viruses with 4 branches that were circulating in Beijing.

[Hemostatic variations associated with acute leukemia: clinical and prognostic significance].

OBJECTIVE: To study the hemostatic abnormalities and their association with bleeding manifestation and prognosis in acute leukemia (AL) patients. METHODS: A series of hemostatic parameters were determined in 93 cases of AL using ELISA or chromogenic assay. RESULTS: At diagnosis, plasma levels of P-selectin, soluble fibrinomonomer complex (SFMC), thrombomodulin (TM), tissue plasminogen activator, D-dimer (D-D) were significantly elevated, but protein C antigen (PC:Ag), plasminogen (PLG), alpha(2)-antiplasmin (alpha(2)-PI) and plasminogen activator inhibitor (PAI) were decreased, and fibrinogen (Fg), PC activity (PC:A) and protein S (PS) were in normal range. All the above abnormalities returned to normal after complete remission (CR) except for PC:A and PC:Ag. Platelet counts, Fg, SFMC, PLG, alpha(2)-PI and D-D were significantly associated with hemorrhagic manifestations. TM, PS and PAI were related to prognosis. CONCLUSION: There existed the consumption of platelets as well as the activation of coagulant, anticoagulant and fibrinolytic systems in AL, which was greatly improved after CR.

[The relations of cellular DNA content to lymph node metastasis in gastric carcinoma].

The objectives of this study were to evaluate the relations of cellular DNA content, S-phase fraction (SPF) and G2/M-phase(G2/MPF) to lymph node metastasis in gastric carcinoma by using flow cytometry. The results showed that the incidence of lymph node metastasis and SPF were significantly higher in aneuploid carcinoma than in diploid carcinoma(P < 0.05). But the G2/MPF values did not correlate with lymph node metastasis. These findings suggest that correlations between cellular DNA content, SPF and the extent of lymph node metastasis are present.

[Treatment of skeletal Class II malocclusion by orthodontic and surgical means].

OBJECTIVE: Skeletal class II malocclusion is often occurred in Oriental and Black. It is difficult to gain the ideal treatment objective either using the orthognathic surgery or orthodontic treatment alone. However, the combined orthodontic and surgical treatment can reach it, so we investigate the methods and characteristics of combined orthodontic and surgical treatment in skeletal class II malocclusion. METHODS: We analyzed the recent 22 cases with skeletal class II malocclusion in Hospital of Stomatology, West China University of Medical Sciences with 7 male and 15 female from 20 to 30 years old. In the study, we discussed the time of extraction, the peculiarity of orthodontic treatment and the selection of surgery type. RESULTS: After the combined orthodontic and surgical treatment, all the patients regained satisfactory face appearance and function. The selection of surgery type is decided by the skeletal characteristics of malocclusion (the malformation degree and the position). The upper first premolars were extracted before the orthodontic treatment for the purpose of better curing of surface of the skeletal wound. And the pre and post-surgical orthodontic treatment were very important and necessary to regain the ideal face appearance and function. CONCLUSION: The combined orthodontic and surgical treatment is an efficient way to cure skeletal class II malocclusion. Suitable orthognathic surgery and rational orthodontic treatment are important to treat adult skeletal class II malocclusion.

[A actinomycete strain C3905 producing antitumor antibiotic sandramycin from Antarctica].

A nocardioform actinomycete strain C3905 was isolated from a soil sample collected in King Geoge island, Antarctica. Its aerial mycelium is white, substrate mycelium is colorless to cream or pink. The hyphae of mycelium 0.5 micron-0.8 micron in diameter and they fragment into irregular rod to coccoid elements with smooth surface. Cell wall type I. The mycolic acids are lacking. Mk-9(H4) is the predominant menaquinone component. Phospholipid type PI. The G + C content of DNA is 68.3-68.9 mol%. Facultative psychrophile. The optimum temperature for growth is 15 degrees C-20 degrees C. Produces antitumor antibiotic sandramycin. Based on the characteristics above as well as the investigation of the molecular genetics. We propose that strain C3905 should be placed in the Nocardioides albus as a variant, named Nocardioides albus var. antarcticus.

[Culture and identification of human vascular endothelial cells].

PURPOSE: To investigate the culture technigue of human vascular endothelial cells in vitro and improve the success rate of culture. METHODS: Human vascular endothelial cells were isolated from umbilical cords by "Irrigative digestion" technique with 0.2% collagenase III and were digested by 0.0625% tripsin when the cells confluent. The cells were identified by morphologic and immunohistochemical assays. RESULTS: The cultured human umbilical vein endothelial cells attached completely after 24 hours in vitro, reached confluence in 3-5 days with the typical appearance of "stone of pave road" and were passaged on time; Immunohistochemically these cells were stained positively by CD34 monoclonal antibody. CONCLUSION: "Irrigative digestion" technique was a reliable method to obtain human umbilical vascular endothelial cells with high success rate. It can be widely used to the culture and researches of human vascular endothelial cells.

[Partial laryngectomy for T3 glottic carcinoma].

OBJECTIVE: To explore the possibility of partial laryngectomy for T3 glottic carcinoma and selection of reconstruction methods. METHODS: From January 1986 to January 1994, 78 patients with T3 glottic carcinomas were treated by partial laryngectomy. The operative techniques and the reconstruction methods were retrospectively analyzed. RESULTS: The overall 3 and 5 years survival rates were 88.5% and 75.6% respectively. The local recurrence rate was 15.4%. The reconstruction methods for laryngeal defect repariring following partial laryngectomy were the use of pyriform sinus mucosa with thyroid perichondrium, epiglottic cartilage and bi-pedicaled and bi-muscular flap of strap muscle. The function of respiration, phonation and deglutition recovered well. CONCLUSION: The partial laryngectomy is suitable for T3 glottic carcinoma. The selection of reconstruction method is based on the size of laryngeal defect.

[Study on the interaction of copper-zinc superoxide dismutase with cobalt(II)-tryptophane by spectral analysis].

The effect of changing the amount of external added Co(trp)n and pH values on the interaction of Cu2Zn2 SOD with organic metal compound (Cobalt(II)-Tryptophane) have been studied by means of ICP, VIS and measurement of enzyme activity. It has found that in aqueous solution, there exists a direct interaction of the metal ions of the active center in the metalloenzyme (Cu2Zn2 SOD) with external added Co(Trp)n. As a result, part of the metal ions in metalloenzyme were replaced and the corresponding metalloenzyme derivatives (Co(II)-substituted derivatives of SOD) were produced and the catalytic activity of enzyme were affected. It has also studied the intensity of interactions in different molar ratios (Cobalt(II)-Tryptophane) and pH values and got some results under the effect of corresponding factors.

[Study on the interaction between nuclease P1 and CuCl2 (II) by visible absorption spectra].

In this paper, we studied the directly interaction between nuclease Pl and CuCl2 in aqueous solution by visible absorption spectra. As a result, the zinc ion in the active site of nuclease Pl can be replaced by additional Cu(II). At the the same time, Cu(II) entered into the active site of enzyme forming the correspondent derivatives of "Cu(II)-Pl". The variety of pH value influenced the types of derivatives and the enzyme activity.

Angiotensin-converting enzyme genotype is associated with Alzheimer disease in the Japanese population.

We compared the distribution of an insertion (I)/deletion (D) polymorphism of the gene coding for the angiotensin-converting enzyme (ACE) in 133 Japanese sporadic Alzheimer disease (AD) patients with 257 controls. The association between AD and ACE genotypes or alleles was found to be significant. The frequency of II genotypes was 1.4 times higher in AD than controls, while that of DD genotypes was only 0.4 times as high. The altered distribution of ACE alleles in patients with AD appeared to be independent of apolipoprotein E.

Studies on treatment of acute promyelocytic leukemia with arsenic trioxide: remission induction, follow-up, and molecular monitoring in 11 newly diagnosed and 47 relapsed acute promyelocytic leukemia patients.

Fifty-eight acute promyelocytic leukemia (APL) patients (11 newly diagnosed and 47 relapsed) were studied for arsenic trioxide (As2O3) treatment. Clinical complete remission (CR) was obtained in 8 of 11 (72.7%) newly diagnosed cases. However, As2O3 treatment resulted in hepatic toxicity in 7 cases including 2 deaths, in contrast to the mild liver dysfunction in one third of the relapsed patients. Forty of forty-seven (85.1%) relapsed patients achieved CR. Two of three nonresponders showed clonal evolution at relapse, with disappearance of t(15;17) and PML-RARalpha fusion gene in 1 and shift to a dominant AML-1-ETO population in another, suggesting a correlation between PML-RARalpha expression and therapeutic response. In a follow-up of 33 relapsed cases over 7 to 48 months, the estimated disease-free survival (DFS) rates for 1 and 2 years were 63.6% and 41.6%, respectively, and the actual median DFS was 17 months. Patients with white blood cell (WBC) count below 10 x 10(9)/L at relapse had better survival than those with WBC count over 10 x 10(9)/L (P =.038). The duration of As2O3-induced CR was related to postremission therapy, because there was only 2 of 11 relapses in patients treated with As2O3 combined with chemotherapy, compared with 12 of 18 relapses with As2O3 alone (P =.01). Reverse transcription polymerase chain reaction (RT-PCR) analysis in both newly diagnosed and relapsed groups showed long-term use of As2O3 could lead to a molecular remission in some patients. We thus recommend that ATRA be used as first choice for remission induction in newly diagnosed APL cases, whereas As2O3 can be either used as a rescue for relapsed cases or included into multidrug consolidation/maintenance clinical trials.

Risk factors for meningioma in adults: a case-control study in northeast China.

A hospital case-control study of meningioma was conducted in Heilongjiang Province in northeast China between September 1989 and December 1996. It included 183 cases of newly diagnosed primary meningioma and 366 individually matched hospital controls with non-neoplastic and non-neurological disease selected from six major hospitals. Cases and controls were matched by sex, age and area of residence and interviewed in the hospital wards to obtain information on medical history, occupation and lifestyle. No association with liquor or beer consumption was apparent. Cigarette smoking was positively associated with meningioma risk in women but not in men. In women, compared with non-smokers, the adjusted OR for pack-years of smoking above the median (124) was 6.2 (CI 2.04-18.87). Both of these observations contrast with the results of a study of glioma in the same population, using similar methods. The risk of meningioma was positively associated with reported occupational exposure to lead, tin, cadmium and ionising radiation in both genders.
