The authors compared the family interaction patterns of Japanese-American and Caucasian families in Hawaii by rating videotapes of structured family interactions. There were significant differences between the two groups in many aspects of family interaction, such as power, coalition, closeness, negotiation, clarity of self-disclosure, responsibility, invasiveness, affect, and empathy. The differences in family interaction could be explained by the cultural differences between the two groups and indicate that profiles of healthy families differ between distinct cultures, and there is a need to establish a culturally relevant family interaction profile; otherwise normal interactions in families outside the mainstream could be misinterpreted as pathological.

As part of a larger study on family functioning, the authors administered a questionnaire on individual attitudes toward family values to 158 Japanese-American and Caucasian families. Differences between the generations on questions of authority and responsibility were predictable; few differences were found between ethnic groups. However, differences were striking between adolescent boys and girls, regardless of ethnicity: Girls valued family affiliation, closeness, and emotional expression significantly more highly than did boys. The authors emphasize the need for families to value girls' needs for closeness and emotional expression as highly as boys' needs for independence and self-differentiation. They suggest that the concept of separation-individuation as the major goal of adolescence be reexamined.
Functional recovery in 133 patients with brachial palsy is documented over a five-year period. Clinical examination is more reliable than electromyogram or myelogram for obtaining prognostic information. Erb's C V, VI palsies have improved function with cuff transfers and elbow flexor plasties. Posterior cord palsies often require extension plasties of the hand and glenohumeral fusions. C V, VI, VII palsies may require all of these procedures. Klumpke C VIII-T I palsies develop arm-trunk holding power. Adults with total palsies often complain of continued pain, which may be relieved by orthoses.
In a 256-bed general hospital, psychiatric patients are cared for on a single unit with medical patients. The unit, developed in collaboration with a state university's medical school, has been employed successfully in the teaching of psychiatric residents and medical students and has provided benefits to both the psychiatric and medical patients. A normalizing effect on the disturbed behavior of psychiatric patients has been observed and has been attributed to the presence of nonpsychiatric patients, and the treatment of medical patients on the unit has been humanized through greater nurse-patient interaction and the provision of activity programs and recreational facilities. The experience indicates that a wide range of psychiatric patients can be cared for in a mixed setting and that such a setting fosters continuity of care. The limitations of the unit and the policies and conditions necessary for its operation are discussed.
The effect of tris (hydroxymethyl) aminomethane concentrations on the rate of calf intestinal alkaline phosphatase-catalyzed hydrolysis of p-nitrophenyl phosphate was studied, in the pH range 8-10, where no transphosphorylation reaction could be detected. Kinetic analysis of the results permitted description of the effect of Tris concentrations T on the rate of enzyme catalyzed hydrolysis (V) by the following equation: (see article). The rate-accelerating effect of Tris concentrations can be ascribed to two different mechanisms: At moderate Tris concentrations (0.01-0.20 M) the enzyme forms a reversible addition complex with a Tris molecule. This complex has an enhanced catalytic activity. We suggest that the binding of Tris to the enzyme could potentiate a second active site of the enzyme, due to its ionization effect upon an acidic group of the enzyme of pK = 8.9. The modest linear rate accelerating effect of Tris at high concentrations (0.20-0.60 M) could be ascribed to the change of the dielectric constant of the medium, the degree of solvation of the protein, or change in the tertiary structure of the enzyme.
