Palilalia: a descriptive study of pathological reiterative utterances.

Palilalia is an acquired speech disorder characterized by reiteration of utterances in a context of increasing rate and decreasing loudness. The condition has been associated with bilateral subcortical neuropathological. The relationship of palilalia to other adult disfluency syndromes, aphasia, and motor speech disorders requires a through understanding of the nature of palilalic speech. To date no detailed description of the nature and severity of palilalic reiteration has appeared in the literature. This case report systematically describes seven distinct types of reiteration, frequency (severity) of reiteration relative to seven types of speech tasks, and consistently and adaptation effects observed in a 29-year-old male.

A pressure profile for elastic stockings.

Special equipment to measure the circumferential compression exerted by an elastic stocking was used to determine the "pressure-girth profiles" of several types of elastic stocking. Once the pressure-girth profile has been determined, the pressure exerted at the ankle, calf, and thigh can be predicted for any size of limb without further pressure measurements. An excellent correlation (r = 0.96) was obtained when this method was compared with another well-established one of measuring the pressures exerted by stockings. The method has several potential applications in quality control during stocking manufacture and, clinically, in determining whether a stocking exerts a graduated pressure on a particular limb.

Value of graduated compression stockings in deep venous insufficiency.

The effect on elastic stockings on ambulatory venous pressure was investigated in 22 limbs with deep venous insufficiency. The failure of some elastic stockings to reduce the ambulatory venous pressure in some limbs is due to the lack of graduated compression, which is caused by ankle-calf disproportion--narrow ankles and wide calves. This can be recognised by using the pressure-girth profile and corrected by specially made stockings with increased tension at the ankle. A pressure-girth profile established for each stocking enabled the exact compression exerted by the stocking along the length of each limb to be determined. Elastic stockings exerting a graduated compression between ankle and calf induced a reduction in the ambulatory venous pressure in all but one limb. The greater the degree of graduated compression between ankle and calf exerted by the stocking, the greater the fall in ambulatory venous pressure. This may explain the beneficial effect of compression in limbs with venous ulceration.

Significance of popliteal reflux in relation to ambulatory venous pressure and ulceration.

Fifty-one patients (55 limbs) who had had deep venous thrombosis (DVT) extending into the femoral or iliofemoral segment three to five years earlier and ten limbs of ten healthy volunteers were studied. The ambulatory venous pressure (AVP) was measured by inserting a needle in a vein on the foot; the presence of reflux in the popliteal vein was determined by a directional Doppler ultrasonic blood velocity detector. All patients had ascending venography. The results suggest that the most important factor in determining the AVP and ulceration in postthrombotic limbs is the condition of the popliteal valves. Ulceration does not occur even in the presence of occlusion if the popliteal valves are competent. The extent of DVT and recanalization or the failure of recanalization is of secondary importance.

Ambulatory calf volume plethysmography in the assessment of venous insufficiency.

Ambulatory calf volume plethysmography has been used to study venous insufficiency in 50 lower limbs. The results demonstrate that it is possible to determine the presence of venous insufficiency and differentiate between normal limbs, limbs with superficial venous insufficiency only, limbs with deep venous insufficiency and limbs with deep venous insufficiency and occlusion by determining the ambulatory volume change and maximum venous outflow or venous volume.
