Simple anthropometric measurements--relation to body fat mass, visceral adipose tissue and risk factors of atherogenesis.

Abdominal type of obesity is linked to risk factors of atherosclerosis and to metabolic diseases. 216 women, aged 17 to 67 years, mean age 43.1 +/- 0.76, mean weight 108.7 +/- 1.6 kg and mean BMI 39.57 kg/m2, were examined. Body composition was assessed by hydrodensitometry, fat distribution was determined by simple anthropometric measurements (waist circumference, sagittal abdominal diameter at the level L4,5, waist to hip ratio-WHR and waist to thigh ratio-WTR), by computed tomography estimating of visceral fat area and by the ultrasound assessment of visceral fat. Close correlation of visceral fat area determined by CT with the ultrasound assessment of visceral fat was found (r = 0.71). In contrast to WHR, a strong correlation of the waist circumference and sagittal abdominal diameter to total body fat content (r = 0.89, r = 0.84, respectively). Close relation of the waist circumference, sagittal abdominal diameter and WTR to visceral fat examined by ultrasound was discovered (r = 0.61, r = 0.66 and r = 0.58, respectively). The sample was divided into quintiles according to waist circumference, abdominal sagittal diameter and WHR. After adjustment for weight and age, waist circumference and sagittal abdominal diameter tend to be more closely related to the metabolic variables (total cholesterol, HDL-cholesterol, triglycerides, fasting blood glucose, fasting insulin, uric acid) and to the arterial blood pressure than WHR. In the group followed only weak relation was found between anthropometric measures of fat distribution and occurrence of diseases complicating obesity, probably as a result of a high overall fatness connected with a high content of visceral fat.

[Treatment of macroscopic hematuria complicating renal biopsy by embolization of a branch of the renal artery].

The authors describe a case of recurrent macroscopic haematuria which developed after renal biopsy in a young patient with IgA dominant focal and segmental mesangioproliferative glomerulonephritis. As conservative treatment was not successful, renal angiography was performed which revealed a false aneurysm of the segmental branch of the renal artery at the site of the assumed puncture. After selective embolization with gelaspon granules macroscopic haematuria receded quickly.

[The effect of corticoids on bone tissue in myasthenia gravis].

Myasthenia gravis is an autoimmune disease where corticoids are the basis of therapy. They are taken for short periods in large amounts, as well as for prolonged periods in medium or small doses. The authors investigated in the described groups the effect of corticoids on bone tissue. They provided evidence of a significant effect on the diffraction pattern, geometrical arrangement of the apatite grid and ion changes in relation to the period of corticoid administration without significant clinical manifestations of osteoporosis, when respecting therapeutic principles.

[Mechanical irrigation and selective decontamination in critically ill patients].

The authors debugged and launched into routine operation an automated monitoring system using computer techniques. Working off-line, they monitored microbiological, immunologic and biochemical parameters of mechanical irrigation of the mouth and selective decontamination of the gastro-intestinal tract. They ran and classified the data concerned into two index-sequence subsets defined on a set of patients treated with artificial pulmonary ventilation and patients with reduced defenses, and/or corticoid-dependent patients with myasthenia gravis. At present, they study basic sets, representative enough to permit the statistically significant examination of all the parameters under study as well as their exact interpretation with regard to the efficiency of the therapeutical strategy chosen.

[Desmoid fibromatosis in absorption infrared spectroscopy, emission spectral analysis and roentgen diffraction recording].

The authors present results of serial quality and quantity microanalyses of bone patterns and dental tissue patterns in patient with desmoid fibromatosis. Methods of absorption spectroscopy, emission spectral analysis and X-ray diffraction analysis with follow-up to x-ray examination are tested. The above mentioned methods function in a on-line system by means of specially adjusted monitor unit which is controlled centrally by the computer processor system. The whole process of measurement is fully automated and the data obtained are recorded processed in the unit data structure classified into index sequence blocks of data. Serial microanalyses offer exact data for the study of structural changes of dental and bone tissues which manifest themselves in order of crystal grid shifts. They prove the fact that microanalyses give new possibilities in detection and interpretation of chemical and structural changes of apatite cell.

[Personal experience with the use of sonography in the diagnosis of acute pancreatitis].

The authors tested in a group of 52 patients the possibility to detect pathological changes in the pancreatic area in acute pancreatitis. The conclusive evidence of changes in 77% the patients can be increased by repeated examinations after intervals of several days. Further sonographic controls may signalize the development of complications of the disease.

[Cervical vertebrae replacements].

The authors analyze caused and indications for prostheses of bodies of cervical vertebrae. As caused they mention bone tumours, metastases, benign bone tumours and injuries. For operation they indicate patients with neurological symptoms and instability of the spine. For prostheses they use bone grafts from the blade of the ilium. They insert the graft during traction, as described by Crutchfield and do not routine fix it. After operation they immobilize by "Minerva" for three weeks. The method is in case of metastases and bone tumours only palliative, but improves the patients' mobility and prevents the development of early neurological complications.

HLA--Dw3 in Sjögren's syndrome.

The increased frequency of HLA-B8 in Sjögren's syndrome was recently reported independently by Gershwin et al. (1975) and Iványi et al. (1976). The association of HLA-B8 antigen with other diseases, characterized generally by impaired immunologic reactivity, has been shown (see Svejgaard et al. 1975, Dausset & Hors 1975), and in some of these diseases MLC typing has revealed the strong association also with the Dw3 determinant. The degree of association with each of the two, B8 and Dw3, varied between different diseases, in some of them having been reported to be higher for antigen B8 (Möller et al. 1976) and in the others for the determinant Dw3 (Thomsen et al. 1975, 1976). In some of the diseases, the association was found to be equally strong for both determinants (Solheim et al. 1976, Thorsby et al. 1975). In this communication, we describe the typing of HLA-Dw3 in 29 patients with Sjögren's syndrome (Ss). They represent part of the experimental group reported in a previous study (Iványi et al. 1976).

HL-A in Sjögren's syndrome.

HL-A phenotype frequencies were studied in 36 patients with Sjögren's syndrome and compared with those of 350 healthy individuals from the same geographical area. Patients suffering from Sjögren's syndrome had a significantly higher frequency of HL-A8 (P corrected less than 0.01). The relative risk of developing Sjögren's syndrome is 3.96 for HL-A8 positive individuals.
