[Chronic inflammation 30 years after tibial intramedullary nailing--foreign body reaction or bacterial infection?].

30 years after two-fold marrow nailing of the tibia due to the formation of a pseudarthrosis and timely removal of the implants in a miner who was 17 years of age at that time, we found a chronic irritation that required surgical therapy. After excision of a two inch long sequester and escape of a small and cheesy core without identification of germs the patient is symptom-free since one and a half year. A foreign-body reaction to corrosion products of the implant or a late infection with low-virulent germs of the bradytrophic medullary callus appears to be the underlying cause.

[The value of radioimmunoscintigraphy compared to computed tomography in the diagnosis and relapse diagnosis of colorectal tumors].

Radioimmunoscintigraphy (= RIS, scintigraphic "specific" imaging of benign and malignant diseases by means of radioactively marked monoclonal antibodies) has been performed in Germany in clinical studies since 1985 in patients suffering from colorectal cancer. After having been successfully proven in primary studies, RIS is now being used in the early diagnosis of recurrences and metastases. In the prospective study presented here the clinical usefulness of RIS was assessed in comparison against well-tried diagnostic methods including computed tomography in patients suffering from colorectal cancer. It was shown that RIS in SPECT technique (= single photon emission computed tomography) with 99mTc-labelled monoclonal CEA antibodies can visualise local recurrences if diagnostic findings are doubtful, with a sensitivity of 78% versus 50% for CT findings.

[Border-area cerebral ischemia--3D-SPECT for the imaging of the regional cerebral blood flow].

15 patients with suspected cerebral ischaemia were examined using 99mTc-HM-PAO-SPECT to assess cerebral blood flow. Results obtained from SPECT were correlated with CT and Doppler ultrasound findings. A 3D-display algorithm is proposed to enhance the lesion to non-lesion contrast compared to the standard 2D-image interpretation. In all cases contrast was superior on 3D-display to 2D-display (-0.38 versus -0.09). 3D-display of SPECT-data enhances lesion contrast due to postreconstructive data processing and may improve exact interpretation of cerebral blood flow studies.

[The regional cerebral circulation in infarct patients. Rapid dynamic T2*-weighted MRT after a bolus injection of gadolinium-DTPA].

The aim was to validate the MRI assessment of regional cerebral blood flow. Measurements were performed on a 1.5 T imaging system using a fast T2*-weighted gradient-echo sequence. After intravenous injection of gadolinium-DTPA 30 images were acquired in the same slice position during 84 seconds. In 12 volunteers we observed a symmetrical cortical decrease of signal intensity during the passage of the contrast medium. In 9/23 patients with impairment of cerebral blood flow a circumscribed area of reduced signal intensity decrease (hypoperfusion) was found. In 4/23 patients the decrease of signal intensity was more pronounced than in normals (hyperfusion). In 9/23 patients signal intensity changes were normal. HMPAO-SPECT confirmed successful MRI assessment of cerebral blood flow in 22/23 patients.

[The regional blood flow in intracranial tumors: a comparison of HMPAO-SPECT with a newer magnetic resonance tomographic procedure].

We compared the value of gadolinium-enhanced first-pass MRI perfusion studies and HMPAO-SPECT for the assessment of regional cerebral blood flow in a prospective study of 23 intracranial tumour patients. In five tumours with homogeneous hypoperfusion and eight tumours with homogeneous hyperperfusion, tumour blood flow patterns in MRI and HMPAO-SPECT were similar. By contrast, in ten patients with inhomogeneous tumour blood flow pattern only MRI was able to differentiate between tumour areas with no or low flow, tumour tissue with high flow, and perifocal oedema with reduced flow. In HMPAO-SPECT, these inhomogeneous tumours were represented as areas of homogeneously reduced tracer retention corresponding to different tumour constituents and perifocal oedema. In conclusion, the high spatial resolution of MRI enables a detailed analysis of tumour blood flow.

[The determination of cerebral dopamine (D2) receptor density by using 123I-IBZM-SPECT in Parkinson disease patients].

An alteration of the dopaminergic nigrostriatal system is believed to be the main pathogenetic factor of Parkinson's disease (PD). We report on our initial results on the determination of the post-synaptic dopamine-D2-receptor binding of 123I-IBZM in patients with PD. Drug-native patients showed a significantly higher IBZM binding in the basal ganglia as compared to patients on specific dopaminergic medication. Age, duration of the disease and the severity of the disease do not seem to influence the IBZM-receptor binding. We conclude that 123I-IBZM-SPECT is an extremely useful tool for the evaluation of the functional state of cerebral dopamine-D2-receptors.

Acetazolamide stimulation test in patients with unilateral internal carotid artery obstructions using transcranial Doppler and 99mTc-HM-PAO-Spect.

Fifteen patients with symptoms of cerebral ischaemia and angiographically confirmed unilateral stenoses or occlusions of the extracranial internal carotid artery (ICA) and 20 controls were studied by a 2 MHz transcranial Doppler (TCD) at rest and after stimulation with 1 g acetazolamide i.v., a cerebral vasodilator. In addition, the patients underwent 99mTc-HM-PAO-Spect measurement of regional cerebral blood flow (rCBF) at rest and after stimulation with 1 g acetazolamide. In 10 patients with ICA stenoses greater than 80% or occlusions, time-mean velocity (Vmean) increase and pulsatility index (PI) decrease in the postobstructive middle cerebral artery (MCA) as well as the increase of the ipsilateral rCBF were reduced in comparison with the contralateral side. The remaining 5 patients showed a normal Vmean increase and PI decrease in TCD.

[MRT and 99mTc-HM-PAO SPECT of the brain in myasthenia gravis].

Fourteen patients suffering from myasthenia gravis underwent cranial MRI and 99mTc-HM-PAO SPECT examinations. T2-WI revealed solitary white matter and/or basal ganglia lesions in four, multiple lesions in eight patients. The evaluation of the rCBF demonstrated a circumscript decrease of cortical perfusion in one patient whereas eight patients showed multiple cortical areas of hypoperfusion. In the context of an increased incidence of intrathecal immune reaction as well as of a constantly impaired cognitive performance of these patients, imaging findings might be a correlate of a primary cerebral manifestation of myasthenia.

Vascular graft infection: detection by 123I-labeled antigranulocyte antibody (anti-NCA95) scintigraphy.

A total of 40 scintigraphic examinations were performed after vascular reconstructive surgery in 27 patients in whom there was a clinical suspicion of vascular graft infection. Whole-body gamma camera images were obtained at 4 and 24 h after i.v. administration of 111 MBq 123I-labeled antigranulocyte antibody Anti-NCA95. Scan results were interpreted without clinical information and were subsequently correlated with computed tomography. Prosthetic vascular graft infection was confirmed in 9 patients and excluded in 18 by surgical findings, bacteriology and/or clinical course. Scintigraphy revealed true-positive results in 16 of 40 and false-negative results in 1 of 40 examinations. True-negative results were found in 19 and 16, false-positive results in 4 and 7 examinations at 4 and 24 h p.i., resp. The sensitivity was calculated to be 94% for both early (4 h) and late (24 h) images whereas the specificity was 83% and 70%, resp. In all cases the application of the murine antibody was safe and no side effects or complications were noted. Limitations of this diagnostic procedure are accumulations of granulocytes in hematomas which may be observed in the non-complicated early course following reconstructive surgery.

[Radionuclide cisternography: SPECT- and 3D-technique].

Radionuclide cisternography is indicated in the clinical work-up for hydrocephalus, when searching for CSF leaks, and when testing whether or not intracranial cystic lesions are communicating with the adjacent subarachnoid space. This paper demonstrates the feasibility and diagnostic value of SPECT and subsequent 3D surface rendering in addition to conventional rectilinear CSF imaging in eight patients. Planar images allowed the evaluation of CSF circulation and the detection of CSF fistula. They were advantageous in examinations 48 h after application of 111In-DTPA. SPECT scans, generated 4-24 h after tracer application, were superior in the delineation of basal cisterns, especially in early scans; this was helpful in patients with pooling due to CSF fistula and in cystic lesions near the skull base. A major drawback was the limited image quality of delayed scans, when the SPECT data were degraded by a low count rate. 3D surface rendering was easily feasible from SPECT data and yielded high quality images. The presentation of the spatial distribution of nuclide-contaminated CSF proved especially helpful in the area of the basal cisterns.

Respiratory viruses and mycoplasma as cofactors for epidemic group A meningococcal meningitis.

To investigate the role of coincident respiratory viral and mycoplasmal agents in the pathogenesis of meningococcal meningitis, we performed a matched case-control study of 62 patients with group A meningococcal meningitis during an epidemic in Chad. Case patients were more likely than controls to have nasal colonization or infection with respiratory viruses and Mycoplasma species (matched odds ratio, 23; 95% confidence interval, 3.1 to 170). Respiratory pathogens were found more commonly in older patients with meningitis (odds ratios were 2.9 for children under age 5 years and 46.5 in those over age 15 years), consistent with the increasing risk of meningitis with age during epidemics. In controls, the presence of respiratory pathogens increased the risk of upper-respiratory-tract symptoms but did not significantly increase meningococcal carriage.

A large outbreak of epidemic keratoconjunctivitis: problems in controlling nosocomial spread.

Between July 1985 and January 1986, 401 patients with adenovirus epidemic keratoconjunctivitis (EKC) were seen at the Illinois Eye and Ear Infirmary. Of the cases, 110 (27%) were nosocomial; the other 291 patients had community acquired infection. The highest attack rates of EKC occurred in patients attending specialty clinics; the overall attack rate among clinic patients was 4.7/1,000 clinic visits. All nosocomial cases were caused by adenovirus type 8; community acquired cases were a mixture of adenovirus types 8 and 37. Adenoviruses were isolated from conjunctival cultures up to 14 d after the onset of clinical illness. Initial efforts to prevent nosocomial transmission were unsuccessful. However, when a plan to triage all patients on entry to the infirmary and to sort patients and personnel caring for infected patients into cohorts was implemented, nosocomial transmission of EKC was promptly and effectively halted, despite the continuation of the community epidemic for another 4 mo. This outbreak clearly demonstrates the efficacy of rigorous infection control in preventing nosocomial transmission of adenovirus EKC.

Drug smuggling by ingested cocaine-filled packages: conventional x-ray and ultrasound.

PURPOSE: To evaluate the imaging characteristics and the diagnostic value of both abdominal ultrasound and conventional abdominal x-ray in identifying ingested drug packages in "body packers." MATERIALS AND METHODS: Twelve individuals were studied prospectively by abdominal ultrasound and by abdominal x-ray to rule out incorporated drug containers. RESULTS: Both abdominal ultrasound and abdominal x-ray correctly identified the 7 of 12 individuals who had ingested multiple drug packages. In 5 of the 12 individuals, ultrasound as well as abdominal x-ray were correctly unremarkable with regard to abdominal foreign bodies. CONCLUSIONS: Abdominal ultrasound and abdominal x-ray both represent valuable diagnostic tools in the assessment of ingested drug packages. Abdominal x-ray is superior to ultrasound in differentiating ingested drug-filled condoms from "pseudocondoms."

Initial experience with SPECT examinations using [123I]IBZM as a D2-dopamine receptor antagonist in Parkinson's disease.

[123I]IBZM is a new radioactive labelled ligand which has a high affinity and specificity to D2-dopamine receptors. The in vivo kinetics of [123I]IBZM were studied in patients with unilateral and bilateral accentuated idiopathic Parkinson's disease. The uptake in the basal ganglias and the imaging properties of this D2 receptor antagonist as a radiopharmaceutical for SPECT examinations had to be investigated. 5 patients, aged 42-66 years, (2 m/3 f) were examined. Each patient received 185 MBq [123I]IBZM intravenously. Blood samples were taken 0-120 min post injection (p.i.) and time activity curves were plotted. Three SPECT examinations were performed (I: 30-50 min; II: 50-70 min; and III: 70-90 min p.i.). The count rates (counts/pixel) in the basal ganglias and the cerebellum were measured for each SPECT series on transverse slices using the region-of-interest technique. The time-activity curve of [123I]IBZM shows a rapid decline in plasma during the first 10 min followed by a plateau until 120 min after injection. The SPECT examinations demonstrate the highest count rate in the basal ganglia during SPECT series III (i.e., 70-90 min p.i.). The side-to-side difference of the count rates were in the range of 3% in four patients, and 10% in one patient. The biokinetic data of [123I]IBZM make this substance capable as a radiopharmaceutical for SPECT examinations. The basal ganglia are best visualized 70-90 min p.i., thus [123I]IBZM seems to be a promising imaging agent for diseases of the D2-dopaminergic receptor system.
