Pharyngoconjunctival fever.

Sixteen infants and toddlers from four kibbutz day nurseries contracted an illness comprising fever, conjunctivitis, respiratory infection, lymphadenopathy, otitis media, and a new sign, migratory palpebral erythema. Adenovirus type 3 was isolated from the pharynx of the first three children taken ill.

Prostaglandin E2 depresses natural cytotoxicity by inhibiting interleukin-1 production by large granular lymphocytes.

Enriched large granular lymphocytes treated with varying concentrations of prostaglandin E2 (PGE2) for varying time periods showed considerably reduced natural cytotoxicity against K-562 target cells. The same cells when activated by lipopolysaccharide, produced substantially less interleukin-1 (IL-1) as compared to cells not treated with PGE2. It is concluded that the inhibition of natural killer (NK) cell activity produced by PGE2 is due to inhibition of IL-1 production by these cells.

Supernatant derived from a human hepatocellular carcinoma cell line (PLC/PRF/5) depresses natural killer (NK) cell activity.

Harvest fluid derived from a human primary hepatocellular carcinoma cell line (PLC/PRF/5) inhibited normal peripheral blood NK-cell activity when either PLC/PRF/5 or K562 cells were employed as targets. MN cells incubated with the PLC/PRF/5 supernatant for only 6 h demonstrated depressed NK-cell activity, which was partially reversible if the cells were washed and incubated in fresh medium for 24 h. Enriched NK cells obtained on discontinuous Percoll gradients employing human tonsillar cells also showed depressed cytotoxicity when treated directly with the tumour harvest fluid. Furthermore, the binding of these cells to the tumour targets could be inhibited by prior treatment of the NK cells with the tumour supernatant. It is suggested that materials produced by tumours may inhibit NK-cell activity by blocking a receptor site on the effector cell, thereby allowing the tumour to proliferate.

Pseudorheumatoid subcutaneous nodules and the poststreptococcal state. A case report.

A 10-year-old girl with an acute illness fulfilling the Jones criteria for rheumatic fever is described. She had subcutaneous nodules, arthritis, an elevated ESR, and serologic evidence of a recent streptococcal infection. Her clinical course was benign and at ten weeks, the nodules disappeared, the joint pains had subsided, and the ESR was normal. Thus, there was little to support a diagnosis of rheumatic fever. We hypothesize that the patient had pseudorheumatoid subcutaneous nodules coincidental with poststreptococcal arthritis. A revision of the Jones criteria disallowing subcutaneous nodules as an independent major sign of rheumatic fever is proposed.

Pregnancy following cytomegalovirus mononucleosis.

A case of cytomegalovirus heterophile-negative mononucleosis seen in a primary care setting is reported. The patient was a young woman with fever that had persisted for 24 days, and with relative bradycardia, hepatosplenomegaly, suboccipital lymphadenopathy and a rash. Diagnosis was based on clinical and hematological findings which included relative and absolute lymphocytosis with atypical lymphocytes, elevation of serum transaminases and the presence of IgM-specific cytomegalovirus antibodies with a significant rise of IgG-type antibodies as tested by indirect immunofluorescence. The patient's last menstrual period occurred 16 days after defervescence and she gave birth to an apparently normal female term infant with an Apgar score of 10 who has not shed virus in her urine during the first six months of life.

Oxygen transmissibility and hydration of new and used hydrogel contact lenses.

The oxygen transmissibility and hydration of new and used CSI, Hydrocurve II, and Hydromarc hydrogel contact lenses was determined at 37 degrees C. coatings that accumulated on used lenses were observed against a bright light and were graded in four classes from mild to severe. Some coated lenses had a statistically significant increase, and some a decrease in oxygen transmissibility compared with uncoated lenses. These differences are clinically unimportant, however. Although there seems to be a trend toward increased hydration and oxygen transmissibility with increased lens coating, statistically these relationships were found not to be linear. Failure of contact lenses after long-term wear must be due to causes other than a decrease in their oxygen transmissibility.

Cancer incidence in kibbutzim in Israel.

The incidence of cancer among the Jewish members of 43 collective settlements (kibbutzim) in the Beisan and Jezreel Valleys is reviewed. These settlements, apart from their geographical distribution, may be taken as a representative sample of the kibbutzim throughout Israel. This review covers the period 1960-72, during which 415 malignant tumors were diagnosed in a population experiencing 253,232 person-years of risk. The incidence was compared with that of the Jewish population in Israel as a whole, and the result was used as an approximation of the country's urban/kibbutz ratio. Cancers of the lip, malignant melanomas and cancer of the female breast were more common in the kibbutz population. Malignancies of the respiratory tract were less common among males living on kibbutzim than in the total Jewish male population of Israel. These differences are probably related to social and environmental factors.

Families at risk for father-daughter incest.

Forty women who had had incestuous relationships with their fathers during childhood were compared with 20 women whose fathers had been seductive but not overtly incestuous. More of the women who had experienced overt incest reported that their fathers had been violent and that their mothers had been chronically ill, disabled, or battered. Untreated depression, alcoholism, or psychosis or repeated involuntary childbearing were most commonly cited as causes of impaired maternal functioning. Women who had experienced overt incest had a higher rate of running away, suicide attempts, and pregnancy during adolescence than the comparison group. The authors suggest that clinicians be alert to the possibility of incest in families which contain a violent father, a disabled mother, or an "acting-out" adolescent girl.

Thrombotic thrombocytopenic purpura with a subacute course: remission after steroids and high-dose plasma exchange.

A case of thrombotic thrombocytopenic purpura with an unusual subacute course is described. Treatment was initiated after the patient had been ill for four weeks. Low-dose plasma exchange (10 ml/kg body weight per day) was associated with a sharp drop in the reticulocyte count and a transient reduction of lactate dehydrogenase activity to within the normal range. The administration of corticosteroids was followed immediately by the disappearance of neurological symptoms, and after high-dose plasma exchange (25 ml/kg body weight per day) all the symptoms of disease remitted. The remission has lasted for 10 months; we suggest that each of the therapeutic measures employed played a role.

Essential thrombocythemia in a child: platelet ultrastructure and function.

A nine-year-old black girl with essential thrombocythemia developed hemoptysis. Only two other cases in the English literature have been described. Ultrastructure and functional characteristics of this patient's platelets were studied. Twenty-six percent of the patient's platelets were very large (megathrombocytes). Spontaneous aggregated from the patient's platelets were not compact, and the pseudopods did not interdigitate. Both qualitative and quantitative defects in platelet organelles were detected. The microtubular system was faulty in organization. Furthermore, the number of granules (especially alpha granules) was reduced. Platelet aggregation studies demonstrated subnormal aggregation in response to ADP, epinephrine, and collagen, but aggregation with ristocetin was normal. It is postulated that a platelet membrane abnormality may be the cause of their defective platelet aggregation.
