[Bursitis iliopectinea--a rare differential diagnosis of painful inguinal swelling].

The syndrome of iliopectineal bursitis is an important differential diagnosis of a painful swelling in the inguinal region. Symptoms are local swelling and pain, radiation of pain along the femoral nerve, troubles of arterial or venous circulation and even dysuria or dysmenorrhoea in case the bursa penetrates into the pelvis. The diagnosis is established by conventional radiology (signs of osteoarthritis as one of the pathogenetic reasons), ultrasound (liquid mass lateral to the femoral vessels), punction (clear fluid, eventually synovial cells), contrast injection (dimension of the bursa and communication to the hip joint) and computed tomography (dimension, relation to hip and vessels). The treatment is the excision of the bursa, if conservative therapy is not successful. The excision requires tedious, cautious dissection because of the important structures adjacent and a possible communication to the hip joint must be eradicated.

[Wound management].

The following article is a check-list for wound care giving some practical hints. Special interest has been given to the themes of local anesthesia and prevention of infections. The indications and limits of the ambulant wound care are also discussed. Finally, a short explanation is given for the treatment of wounds situated at delicate regions of the body.

Ambulatory Surgical Procedures

[Angiosarcoma following teratocarcinoma: "mutation", successive malignancy or irradiation].

Angiosarcoma is a rare malignant soft-tissue tumor and affects preferably skin and adjacent subcutaneous tissue. Very rarely the tumor is radiation-induced. We report the case of a 28-year-old Swiss who developed an angiosarcoma of the gut 5 years after the successful therapy of metastasising teratocarcinoma of his right testis by surgery and radiation therapy. We discuss three possible etiologies for the genesis of the angiosarcoma, which appeared rapidly after the primary tumor at an unusual site in an unusual patient.

[Malformations (duplications) of the terminal ileum in adults. Report of 3 unusual cases].

Three cases of rare malformations of the ileo-cecal region (duplications) are discussed and compared to 20 others of the same kind already published. The main characteristics are an inflammatory symptomatology, especially in the right hypogastrium, with periodic recurrences. Since the clinical, radiological, endoscopic and biopsy findings often simulate Crohn's disease, patients must frequently undergo years of unsuccessful medical treatment and innumerable follow-up examinations. Nevertheless, when certain findings are taken into consideration they can lead to correct diagnosis of this rare disease, which is established through pathological anatomy. The special features of the disease are described in detail. It can be definitively cured through resection of the ileo-cecal segment.

[Somatostatin-producing endocrine pancreatic tumor in Recklinghausen's neurofibromatosis. Case report and literature review].

Somatostatin-producing tumors of the pancreas were first described in 1977. In 1983 a syndrome involving multiple endocrine neoplasias (MEN) was named type III A. This syndrome consists of carcinoid of the duodenum, often producing somatostatin, and von Recklinghausen's disease (neurofibromatosis) or pheochromocytoma. The case is reported of a 62-year-old man with familial neurofibromatosis and a tumor of the head of the pancreas spreading into pars II of the duodenum. After Whipple's duodenopancreatectomy the patient exhibited no further symptoms. Immunohistochemistry served to prove the production of somatostatin and small amounts of calcitonin in the tumor.

[Surgery in the regional and district hospitals of German Switzerland].

The Regional Hospital is the hub in the surgical service for the broad population. According to its importance, it has furthermore an eminent role in instruction and postgraduate teaching. The requirements and profile for such competent medical chiefs are delineated. The broad surgical requirements, the executive duties in hospital administration, which are shared with the nursing and administration services, and the permanent personal education make it mandatory, that the position of the head surgeon be partitioned by two equally competent and polyvalent general surgeons. The ultimate responsibility for such adequate positioning rests with the political authority.

General Surgery

Gastric emptying of organic acids in the dog.

Test meals of 300 ml. of six different organic acids were instilled into the stomach of six healthy mongrel dogs. Citric, acetic, propionic, lactic, tartaric and succinic acid were given in 50, 100, 150, and 200 mN concentrations. 2. During the emptying process, the gastric contents were aspirated and immediately re-instilled at 10 min intervals, and the following parameters were recorded: volume, concentration of the organic anion, pH, hydrogen ion concentration and osmolarity. 3. By multiple stepwise regression analysis, the combination of parameters which most effectively determines gastric emptying rate was found to be: concentration of the organic anion, followed by intragastric volume and number of previous test meals given on the same day. These three parameters appear in the equation for gastric emptying rate in which the individual characteristic of each acid is expressed by a constant. 4. Among the various acids, inhibition of emptying rate increases with rising number of carboxylic groups of the acid and its molecular weight. 5. After proximal gastric vagotomy, emptying rate of organic acids is independent of volume, and emptying approaches an exponential pattern. 6. A model for gastric emptying of organic acids with at least three different receptors is proposed: one for the structure of the organic acid, one for concentration and one for intragastric volume.

[Regulation factors of stomach emptying in dogs].

Gastric emptying of organic acids was studied in 6 healthy mongrel dogs. After chronic esophagostomies were performed according to the method of Komarov, a total of 340 test meals were instilled. Each test meal consisted of 300 ml of 6 different organic acids with decreasing molecular weight and different concentrations. After the experiments were achieved, each dog underwent a proximal gastric vagotomy according to the method of Amdrup, and experiments with citric acid were repeated. The results may be summarized as follows: multiple stepwise regression analysis of the data is consistent with a model in which gastric emptying of organic acids is regulated by 3 receptors. The receptors respond to concentration of the organic acid, the actual volume, and the type of acid. The volume receptor is located in the corpus of the stomach because the effect of volume accelerates the emptying rate after proximal gastric vagotomy while the effects of concentration and type of acid remain unchanged.

[Surgical preparation planning of hemodialysis candidates].

An early beginning of chronic hemodialysis is postulated to prevent dangerous uremic complications. Subcutaneous arteriovenous fistulas or autologous saphenous vein grafts in cases where multiple attempts to create a sufficiently functioning arteriovenous fistula have failed turned out to be the best procedures to obtain a suitable access to the blood vessels. The problems arising on a total of 57 patients to get an adequate blood flow by the time of first hemodialysis are discussed.
