[The second phase clinical observation of anti-radiation effect by superoxide dismutase].

Multiple center randomized controlled double blind clinical trait was conducted to evaluate the anti-radiotherapy effect by SOD (produced by Hunan Biochemical Work) in 159 patients. Injection of 4000U SOD immediately after receiving radiotherapy significantly reduced the occurrence rate of skin, oral mucosal, pelvic visceral and systematic adverse reaction, only the reduction of leukopenia did not reach the statistical significant level. No adverse effect of SOD injection was observed. The results suggest that SOD is a safe and effective agent to attenuate the radiotherapy reactions.

[The clinic significance of serum hyaluronate and tumor necrosis factor alpha levels in liver diseases].

Serum levels of hyaluronic acid (HA) and tumor necrosis factor alpha (TNF alpha) in 94 patients with various liver diseases and 31 healthy controls were studied by RIA, Serum TNF alpha in hepatitis, liver cirrhosis (LC) and primary hepatic carcinoma (PHC) was higher than that of the control. Serum HA in chronic hepatitis, LC and PHC was higher than that in the control. The serum level of HA showed a positive correlation with the serum level of TNF alpha. It is considered that the increase of serum HA indicates an early fibrogenic tendency in patients with liver diseases. TNF alpha is involved in the formation of hepatic fibrosis.

[Effect of prolonged inhalation of nitric oxide on chronically hypoxic rats].

OBJECTIVE: To explore the effect of prolonged nitric oxide (NO) inhalation on chronically hypoxic rats. METHODS: Male Wistar rats were randomly divided into 3 groups: 1) room-air control (C); 2) hypoxia (H); 3) hypoxia with the inhalation of 20 ppm NO(2NH). RV/LV + S, mPAP and the histopathology of the lung were examined. Plasma levels of ET-1 and cGMP were detected by radioimmunoassay. RESULTS: 1. Hypoxia caused marked elevation in mPAP and RV/LV + s (both P < 0.001 vs Group C). NO significantly reduced mPAP and RV/LV + S; 2. Hypoxia raised the percentage of intra-acinar muscular artery (MA) and lowered that of nonmuscular artery (NAM). MA decreased from 35.2% to 16.8% in Group 2NH; NMA increased from 43.6% to 64.8% in Group 2NH, 3. The plasma level of ET-1 increased and that of cGMP droped significantly in Group H(both P < 0.001 vs Group C). NO resulted in a marked reduction in the plasma ET-1 and an increase in plasma cGMP; 4. Inhalation of NO had no effect on methemoglobin and lung index in hypoxic rats. CONCLUSIONS: Prolonged inhalation of 20 ppm NO can attenuate hypoxic pulmonary hypertension and inhibit the hypertrophy of the pulmonary vascular smooth muscle and the right ventricle NO, when inhalated at a concentration of 20 ppm for a long duration, will cause no toxicity in chronically hypoxic rats.

[The effect of nitric oxide administration on pateints with pulmonary hypertension: a dose-response study].

OBJECTIVE: To explore the dose-response relationship in patients with pulmonary hypertension after inhalation of nitric oxide (NO). METHODS: The effect of various concentrations (20, 40, 60 and 80 ppm) of NO on hemodynamics and cardiac function in 16 patients with pulmonary hypertension was studied with right heart catheterization. RESULTS: Pulmonary artery pressure and pulmonary vascular resistance reduced significantly after inhalation of various concentrations (20, 40, 60 and 80 ppm) of NO. The vascular response to NO was not concentration-dependant with a maximal effect obtained at 20 ppm. At inhalation of low dose of NO (20 ppm), the mean pulmonary artery pressure and pulmonary vascular resistance dropped by 26.5% and 40.3% (P < 0.0001), respectively. With the increase of inhaled NO concentration, pulmonary artery pressure and pulmonary vascular resistance did not decline. CONCLUSION: The best concentration of inhaled NO was not more than 20 ppm in the patients of this study.

[Effect of furosemide on the carbonic anhydrase activity in vestibule].

In order to explore the mechanism that the furosemide dehydrates and improves the vestibular function in the endolymphatic hydrops. The effects of furosemide on the carbonic anhydrase activity in vestibule were studied by using histocytochemistry and image analysis. The results demonstrated that the furosemide can obviously inhibit the carbonic anhydrase activity. There was no significant difference between the normal ear and the ear with endolymphatic hydrops in the location of the carbonic anhydrase in the vestibule. It suggested that one of the dehydrant mechanisms is inhibition of carbonic anhydrase activity.

[Voice measurement with computer and analysis in normal adult].

Acoustic parameters of 90 normal adults (20-50 years old) were measured with computer. The results showed that fundamental frequency (F0), frequency perturbation quotient (FPQ) and amplitude perturbation quotient (APQ) between male and female in same ages were statistically significant difference. F0 and FPQ were bigger but APQ was smaller in female. FPQ and APQ were significantly related to F0. OCT wasn't obviously different among sexes or ages. The article indicated that voice of adults and the vibration of vocal cords had distinctive characteristics in different sexes or mode of phonation, which could provide objective methods and grounds for clinical voice evaluation.

[Clinical study on protective effect of xinmaitong capsule on damage of vascular endothelial cells].

OBJECTIVE: To assess the effect of Xinmaitong (XMT) capsule in treating coronary heart disease (CHD). METHODS: Thirty-eight patients of coronary heart disease with myocardial ischemia were divided randomly into XMT group (20 cases) and control group (18 cases). Conventional western medical therapy was given to both groups and the XMT group received additional XMT treatment. The changes of endothelin (ET) and calcitonin gene-related peptide (CGRP) levels, ST segment of ECG and clinical symptoms after treatment in all the patients were observed. Data of 14 healthy persons were taken as normal control. RESULTS: The ET level of all patients was significantly higher than that of the normal control (P < 0.001), and level of CGRP in patients was not different from normal control significantly (P > 0.05). After treatment, results showed that: (1) The ET levels and the scores of clinical symptoms of both groups decreased significantly (P < 0.01), and ST segment elevated markedly (P < 0.01) as compared with before treatment, and the changes revealed more evident in XMT group in comparison with those of the control group (P < 0.05-P < 0.01). (2) The level of CGRP was significantly increased in XMT group (P < 0.01) while unchanged in the control group (P > 0.05). CONCLUSIONS: There is severe damage of vascular endothelial cells in CHD patients. XMT could not only reduce significantly the plasma ET content, but also enhance markedly the production and release of CGRP, so it has a good anti-ischemic effect, which may be closely related with its action on improving the function of vascular endothelial cells and regulating metabolism of ET and CGRP.

[The effects of phorbol 12-myristate 13-acetate and microcystine-LR on calcium channel in resistance vascular smooth muscle cells in SHRsp].

OBJECTIVE: To investigate the effects of phorbol 12-myristate 13-acetate (PMA) and microcystine-LR (MCYST-LR) on the whole-cell barium currenta of voltage-dependent calcium channels (VDCs) of the resistance vascular smooth muscle cells (VSMC) from the A4-A5 branches of mesenteric artery in stroke-prone spontaneously hypertensive rats (SHRsp) and its normotensive control Wistar rats. METHODS: Using the whole-cell Ba2+ current recording of the patch clamp technique. RESULTS: It was found that PMA, a protein kinase C (PKC) agonist or MCYST-LR, a protein phosphatase IA and IIA inhibitor by which the enhancement of phosphorylation induced could apparently activate the VDCs of the visceral resistance VSMC. CONCLUSIONS: This activation would be significantly potentiated during hypertension, in which the increase of L-type calcium channel openings induced by PMA was obviously manifest, as the T-type calcium channel activity weakly potentiated. In normotensive Wistar rats, the increase of T-type calcium channel openings was greater than that of SHRsp. However, the effect of MCYST-LR appeared that only the increase of L-type calcium channel current in SHRsp was significant. It is suggested that the action of PKC and/or phosphorylation on the different components of calcium currents should be emphasized in the investigation of the mechanism in the increase in peripheral resistance during hypertension.

[Effects of GTP gamma S and GDP beta S on calcium channels of resistance vascular smooth muscle cells in SHRsp].

OBJECTIVE: To investigate the effects of GTP gamma S and GDP beta S on the whole-cell barium currents of voltage-dependent calcium channels (VDC) of the vascular smooth muscle cells (VSMC) from the A4-A5 branches in SHRSP mesenteric artery. METHODS: Using the whole-cell Ba2+ current recording of the patch clamp technique. RESULTS: (1) GTP gamma S, a G-protein agonist, increased whole-cell Ba2+ currents both of SHRSP and Wistar, but SHRSP with a higher ratio of amplitude change than Wistar. In addition, GTP gamma S shifted D infinity to right both in SHRSP and Wistar, but D infinity shifted more in SHRSP than that in Wistar. (2) GDP beta S, a G-protein antagonist, decreased whole-cell Ba2+ currents both of SHRSP and Wistar, but SHRSP with a higher inhibition ratio of amplitude change than Wistar. In addition, GDP beta S shifted F infinity to right and in SHRSP shifted more than control under HP = -80 mV. CONCLUSIONS: The G-protein mechanism may promote Ca2+ influx and contribute to the increase of peripheral resistance during hypertension.

[Thoracoscopic lung reduction surgery for emphysema].

OBJECTIVE: To study the surgical treatment of emphysema. METHOD: Five patients with severe diffused pulmonary emphysema underwent surgical bilateral lung volume reduction by video-assisted thoracoscopy. All patients were male and their age ranged from 60 to 71 years (mean 64.3 years). Before surgery, 5 patients required supplement oxygen with exertion and one oxygen supplement at rest. Preoperative and postoperative assessments included standard pulmonary function studies, chest CT scan and quantitative nuclear lung perfusion and ventilation scan. Wedge resection was performed were in 3 patients, simple plication in 1, and combined wedge resection and plication in 1. There were no early or late mortality and no requirement for immediate postoperative ventilatory assistance. RESULT: Follow-up ranged from 10 to 12 months. After surgery the mean forced expiratory volume in 1 second (FEV(1)) was significantly increased from 0.46 L to 1.13 L (P < 0.001) and the mean forced vital capacity (FVC) was markedly improved from 1.74 L to 2.93 L (P < 0.001). Pulmonary compliance was significantly improved and resistance of airway decreased (P < 0.001). The mean distance during 6-min walk test increased from 10 m to 400 m in 4 patients. CONCLUSION: Thoracoscopic bilateral lung volume reduction is an effective and safe technique for patients with severe emphysema. It can relieve dyspnea and improve in excise tolerance and quality of life.

[Clinical application of 2 mm micro-thoracoscopic surgery].

OBJECTIVE: To determine the clinical safety and efficacy of micro-thoracoscopic surgery. METHOD: From July 1996 to July 1997, video-assisted thoracoscopic surgery was performed in 47 patients by using 2 mm microthoracoscope. There were 36 males and 11 females. Their age ranged from 40 to 75 years with a mean age of 56.7 years. Surgical procedures included bullectomy (16 patients), wedge resection (3), lung cancer staging (4), lobectomy (2), exploration (15), and resection of mediastinal tumor (2), esophagectomy (1) and repair of pneumothorax (4). Local anesthesia was employed in 18 patients and general anesthesia with double lumen endotracheal tube in 29. RESULT: There were no operative mortality and postoperative complications. CONCLUSION: 2 mm micro-thoracoscopic surgery could be used in the management of a variety of thoracic conditions. It may be of significant value for diagnostic and therapeutic purposes, but the further study for this new technique is required to determine its safety and efficacy.

[The surgical treatment of tetralogy of fallot in 136 adults].

OBJECTIVE: To summarize the experience of surgical treatment of tetralogy of Fallot (TOF) in adults. METHOD: From April 1975 to December 1997, 136 adult patients underwent total corrective repair of tetralogy of Fallot. The patient age ranged from 14 to 52 years old. The ventricular septal defects were closed with Dacron patch in 112 patients, with pericardial patch in 24 patients. A right ventricular outflow patch was required to eliminate right ventricular outflow tract obstruction in 126 patients. RESULT: Five patients died during hospitalization, with a hospital mortality of 3.68%. Four early postoperative deaths were due to severe low cardiac output syndrome. CONCLUSION: In view of current improved and standardized techniques, we conclude that the total corrective repair of TOF may have an excellent result in adult patients.

[Relationship between changes of phospholipid and lipid peroxide of erythrocyte membrane and diabetic retinopathy].

OBJECTIVE: To study the relation between the changes of phospholipid and lipid peroxide (LPD) in erythrocyte membrane in diabetes mellitus with the occurrence of diabetic retinopathy (DR). METHODS: The levels of triglyceride (TG), cholesterol (TC), glycosylated hemoglobin (HbA(1)c), phosphatidylethanolamine (PE), phosphatidylinostiol (PI), phosphatidylserine (PS), phosphatidylcholine (PC), sphingomyelin (SM), lysophosphatidylcholine (LPC), lipid peroxide (LPO) and the activity of superoxide dismutase (SOD) and glutathione peroxidase (GSH-PX) were investigated in 44 diabetic patients with DR, 20 diabetic patients with no DR (NDR) and 30 sex and age matched normal control subjects. RESULTS: A significant decrease in PE, PI, PS, PC, SM, SOD and GSH-PX and a significant increase in LPC, LPO, TG, TC and HbA(1)c were detected in DR and NDR group when compared with the normal controls. These changes were more obvious in DR group. CONCLUSION: The lipid peroxidation and the alteration of the erythrocyte membrane phospholipid compositions may contribute to the occurrence and development of DR.

[A study of Na(+)-K(+)-ATP ase activity in erythrocyte membrane from diabetic retinopathy patients].

OBJECTIVE: To study the changes of Na(+)-K(+)-ATPase activity in erythrocyte membrane and its significance. METHODS: Na(+)-K(+)-ATPase activity in erythrocyte membrane was investigated with malachite green colorimetric analysis in 40 patients with diabetic retinopathy (DR), 20 diabetic patients with non DR (NDR), and 20 sex and age metched normal control subjects. The relations of Na(+)-K(+)-ATPase activity with content of Na(+) and K(+) erythrocyte deformability index (EDI), levels of fasting blood glucose (FBG), glycosylated hemoglobin (HbA(1)c), and the course of disease were also studied. RESULTS: Na(+)-K(+)-ATPase activity in erythrocyte membrane was significantly decreased in diabetic patients (P < 0.01). The decrease was most significantly in patients with DR (P < 0.01). The decreased Na(+)-K(+)-ATPase activity was related to the content of Na(+) and K(+) in erythrocyte, the changes of EDI, FBG, HbA(1)c, and the course of disease (P < 0.01). CONCLUSIONS: The decreased Na(+)-K(+)-ATPase activity in erythrocyte membrane affects the deformability of erythrocyte. It may contribute to the occurrence and development of DR.

[Inhibitory effect of Howiinol A(GHM-10) on the synthesis of biological macromolecules in L1210 cells].

Howiinol(GHM-10), a new compound isolated from Goniothalamus howii, had been shown to have marked anticancer activity both in vitro and in vivo. In the present study, the effect of GHM-10 on the biosynthesis of macromolecules in L1210 cells was investigated. By using [3H] labeled precursor incorporation assays, we found that the biosyntheses, of DNA, RNA and protein in L1210 cells were inhibited markedly after the cells were pretreated with GHM-10 in 4-12 micrograms.ml-1 for 6 h and the inhibition of DNA synthesis was the most obvious one. We also used the methods of [3H]TdR incorporation curve, shift of UV-absorption spectrum and shift of fluorescence spectrum to investigate the mechanism of inhibitory action of GHM-10 on DNA biosynthesis. The results showed that the inhibition of DNA synthesis already became irreversible after the L1210 cells were treated with GHM-10 in 6-8 micrograms.ml-1 for 1 h, indicating that GHM-10 may induce damage on DNA molecular structure. However, when calf thymus DNA was treated with GHM-10, the UV-absorption spectrum and the fluorescence spectrum of the DNA were not changed significantly. It is suggested that GHM-10 was not able to intercalate into DNA molecules or to damage the structure of DNA directly.

[Studies on the anticancer effect of howiinol A, a new compound isolated from Goniothalamus howii].

The effect of howiinol A (GHM-10) on the growth of several cancer and normal cells were studied, using the methods of cell growth curve determination, MTT test and soft-agar colony formation assay. The results showed that GHM-10 exhibited potent inhibitory effect on cancer cells with an IC50 of 2 micrograms.ml-1 approximately. Normal cells, especially bone marrow progenitor cells, were shown to be less sensitive to GHM-10. A drug-resistant cell line, KB/VCR200, was found to be as sensitive as its parental KB cells. GHM-10 did not induce HL-60 cell differentiation. In murine transplantable tumors, GHM-10 was found to have significant therapeutic effect on mice bearing solid tumor hepatoma H22, Lewis lung carcinoma and S180 ascites.
