Renin production in congenital mesoblastic nephroma in comparison with that in Wilms' tumor.

The case of an infant with congenital mesoblastic nephroma and associated reninism is presented. The patient was a 39-day-old boy who presented with a left abdominal mass and mild hypertension (112/70 mm Hg). Both plasma renin activity (PRA) and plasma total renin concentration (PTRC) were elevated; the PTRC value of 1458 pg/mL of this case was the highest we had seen in patients with childhood renal tumors. The tumor was successfully removed and histologically confirmed as a congenital mesoblastic nephroma (cellular variant). The patient's PRA and PTRC returned to normal after nephrectomy. Indirect immunoperoxidase staining showed renin localized predominantly in the juxtaglomerular apparatuses adjacent to the glomeruli entrapped by the tumor. No positive staining was seen in the tumor cells. These findings indicate that the entrapped glomeruli may play a significant role in producing renin. More important, PTRC was extremely high compared to the moderate increase of PRA and to the mild elevation of blood pressure. The mechanism of renin production by childhood renal tumors is discussed, and the importance of studying inactive renin and total renin is emphasized.

Bcl-2 related proteins are dramatically induced at the early stage of differentiation in human liver cancer cells by a histone deacetylase inhibitor projecting an anti-apoptotic role during this period.

Expression of the Bcl-2 family members in a human hepatocellular carcinoma cell line (HCC-T) after sodium butyrate-treatment was investigated. Sodium butyrate, a histone deacetylase inhibitor, induced differentiation of the cell line into its normal counterpart without inducing apoptosis at the concentration of 2 mmol/l. Since sodium butyrate has effects on both differentiation and apoptosis, we investigated the expression profile of bcl-2 related genes in HCC-T. The expression of Bcl-2 and Mcl-1/EAT was up-regulated 4-12 h after the treatment while Bcl-XL was up-regulated 2-3 days after the stimulation. On the other hand, the expression levels of Bax protein remained unchanged during differentiation. The HCC-T cells entered a cell cycle arrest at G1 and showed neither cellular fragmentation nor apoptosis during this period, which was concomitantly associated with up-regulated expression of a cell cycle regulator, p21WAF-1. These results demonstrate that induction of anti-apoptotic bcl-2 related proteins at an early stage of differentiation is important for the maintenance of HCC-T cell differentiation by antagonizing pro-apoptotic molecules such as Bax.

Expression of interleukin-6 in cerebral neurons and ovarian cancer tissue in Trousseau syndrome.

Interleukin-6 (IL-6) is reportedly increased in serum and CSF from acute stroke patients. However, the cellular origin and possible role of IL-6 in CNS after stroke are unclear. We describe a woman with recurrent stroke, disseminated intravascular coagulation (DIC) and non-bacterial thrombotic endocarditis (NBTE) caused by ovarian cancer (Trousseau syndrome). The patient died 50 days after the final episode of cerebral embolism. The immunohistochemical study revealed IL-6 protein to have been expressed both in cerebral neurons spared from ischemic insult and in epithelial cells of the ovarian tumor. We speculate that IL-6 produced in ovarian cancer may be associated with the hypercoagulable state and the development of NBTE in this patient. In contrast, IL-6 induction in cerebral neurons may contribute to the survival of these neurons after a stroke.
