[Coordinative abilities of arthroplasty patients].

AIM: The aim of the study was to examine the coordinative abilities of arthroplasty patients in comparison to age-matched healthy individuals with a special test profile. Furthermore, we wanted to evaluate the effect of a coordinative training program performed in the late period of rehabilitation on the patients' coordinative status. METHODS: We measured the coordinative abilities of 75 arthroplasty patients and age-matched healthy individuals with a special test profile for arthroplasty patients. In a prospective study, the effect of a six-week comprehensive coordinative training program 3-6 months after implantation was evaluated for 56 patients. A test profile developed for this investigation served to evaluate the effect of the training on the coordinative abilities. RESULTS: There are significant differences in the coordinative abilities between arthroplasty patients 3-6 months after implantation and age-matched healthy probands. The coordinative abilities can be improved significantly by special coordinative training. CONCLUSION: By means of our test profile, rehabilitation outcome with regard to motor coordination becomes measurable and rateable. The values of healthy individuals serve as a baseline. A classification of the motor coordination before and after rehabilitation is possible. In the late period of rehabilitation clear deficits remain. A special training can improve the motor coordination of patients with arthroplasties. The training program is regarded as an important and fundamental part of the rehabilitation.

[The influence of ultrasonography on the diagnosis and treatment of so-called congenital hip dislocation].

By means of a retrospective analysis of the hospital reports of 107 children with 131 dislocated hips we investigated the influence of ultrasound examination on the diagnosis and therapy of congenital dislocation of the hip. We could show, that although the ultrasound examination had increased since 1985 the number of dislocated hips receded only little. However the diagnosis was made earlier in children who had been examined by ultrasound. In these children operative reposition of the dislocated hip had to be performed less frequently compared to the unexamined children. Several case reports are given to demonstrate typical faults in ultrasound diagnostics and treatment of congenital hip dislocation. Our results support the call for a general ultrasound screening of the newborn hip.

[Comments on the use of the "life-table method" in orthopedics].

In the description of long term results, e.g. of joint replacements, survivorship analysis is used increasingly in orthopaedic surgery. The survivorship analysis is more useful to describe the frequency of failure rather than global statements in percentage. The relative probability of failure for fixed intervals is drawn from the number of controlled patients and the frequency of failure. The complementary probabilities of success are linked in their temporal sequence thus representing the probability of survival at a fixed endpoint. Necessary condition for the use of this procedure is the exact definition of moment and manner of failure. It is described how to establish survivorship tables.

[Is the implantation of hinged knee joint prostheses still justifiable today? 15 years' experience using the Blauth knee joint prosthesis].

In objection to knee hinge prostheses there is often mentioned a higher complication rate. An increased load impact on the bone-cement interface results from the close connection of tibial and femoral components. This is assumed to cause bad results, as reported from early artificial knee joint replacements, characterised by weight bearing axes and direct contact metal to metal. These are contrasted to long term results of the Blauth hinge prosthesis. The Blauth prosthesis is constructed according to the low friction principle without a weightbearing axis. A prospective multicentric long term follow-up study reports on 556 prostheses. 463 (83%) were controlled between 1 and 15 years after operation (average: 43 months). Aseptic loosenings had to be confirmed in 1.3% of the patients, deep infections in 2.6%. The survival analysis did not show an erratic deterioration in dependence of the observation period. After 10 years there is still a probability of 89% that a prosthesis does not show a deep infection or loosening. The efficiency of artificial knee joint replacement by hinge joints should therefore not be judged on the results of the first generation of these models.

[Patellar changes during long-term observations on the Blauth knee joint endoprosthesis].

Changes in the position and structure of the patella and their importance with regard to pain patterns are described on the basis of long-term observation of the Blauth prosthesis. Examinations were performed on 206 knee prostheses on average 37.6 months (min. 12, max. 120 months) after implantation. Axial roentgenograms showed the patella to be laterally subluxated in 12% of the cases, and luxated in one. Also in 12% of the cases, osteolytic destruction of the posterior surface of the patella was found; there was a statistically significant correlation between the two groups, but they were not identical. In cases with changes in the patellar structure, initial pain on walking was significantly more frequent, though this was not so in cases where the kneecap had shifted from its normal position. The authors discuss possible connections with curvature radii and with the distances between the center of rotation and the sliding support on the femoral part of the prosthesis as causes of increased complaints in low kneecaps and for the increased incidence of structural changes in high kneecaps. The term "retropatellar pain syndrome" is applied to patients who experience pain when climbing stairs, rising from chairs and when starting to walk, in whom at least 2 of these complaints are positive: A total of 10.2% of the patients complained of considerable retropatellar pain. A patellectomy was performed in 8 of these patients. The radiological and histological analyses of 3 of the patellae removed showed evidence of arthrosis deformans at different stages, with concomitant synovitis.

[Follow-up and ultrasound-controlled early treatment of hip dysplasia].

PROBLEM: The reliability of ultrasound of the hip is examined by the courses of mature and physiologically immature hips. The results of sonographically controlled therapy of the congenital dysplasia of the hip are investigated. METHOD: 470 children out of a hip-screening program could be controlled at least one more time clinically and by ultrasound. X-rays were taken at the end of treatment in children who were treated by an abduction device. The sonographical and radiological results were compared and grafically presented. RESULTS: None of 570 matured hips deteriorated. 88% of the physiologically immature hips matured spontaneously while 12% got a cast because of unsatisfactory development. 68 pathological hips improved soon under therapy in an abduction device applied a few days after birth; they reached the borderline of 60 degrees at the maximum age of 80 days. X-ray examinations at the end of therapy showed normal acetabular indices in all cases. Six of 22 girls showed deterioration of the acetabular indices in further x-ray controls at the age of one year. CONCLUSION: Even in successful therapy of the congenital dysplasia in the newborn hip x-ray examinations are highly recommended to recognize deteriorations and to initiate another therapy if neccessary.

[Spatial orientation of the facies patellaris femoris].

The present article reports on the geometrical conditions of the physiological movement of the patella. The geometrical shape of 18 femoral condyles and patella sliding areas was investigated in order to describe basic data for the design of endoprostheses. Surface and direction of the facies patellaris femoris were determined by means of radiographic, mechanical and optical measurements. The curvature of the deepest patella sliding groove proves a constant correlation with the dorsal condylar curvature. In the frontal plane the lowest points of the sliding area run with a dispersion of +/- 4 degrees to the vertical line related to the transverse tangent on the dorsal condylar surface. Considering deviations of leg alignment the measures come close to an angle of about 0 degrees. So the direction of the patella sliding groove differs from the normal valgus position of the distal femur. Therefore in artificial knee replacement a lateral tilt of the patella sliding groove should not be propagated as 'physiological'.

["Recurrent" epiphysiolysis capitis femoris--need for simultaneous stabilization of both hip joints].

In the operative therapy of slipped capital femoral epiphysis, a controversial discussion exists about the sense of a simultaneous stabilization of both epiphyseal plates and about the correct time of removing the implants. We show the epikrisis of two patients to demonstrate our therapy regimen: in the first case the implants were removed too early. The consequence was a further slipping of the femoral epiphysis consecutive on both sides and the necessity of further surgery. In the other case we observed an outgrowing of the implant out of the epiphyseal plate by use of unthreaded wires. The growth plate was not closed. The femoral epiphysis was stabilized again, because an outgrowing compares to an implant removal at a too early stage. These two cases show our point of view. We recommend to stabilize both sides simultaneously with threaded wires and not to remove the implants before closure of the epiphyseal plate. Furthermore, we hold it advisable to perform a restabilization, when the implants grow out of the unclosed epiphyseal plate during adolescence.

[Long-term results of tibial head osteotomy].

QUESTION: Long term outcome after high tibial osteotomies is essential too for the actual indication of these procedures. Complications and success rate were analysed in our patients in order to find out predictive determinants for the outcome. METHOD: 200 high tibial osteotomies were performed at the Orthopaedic clinic of Kiel University between 1974 and 1983. 148 knees (74%) could be reviewed with an average follow-up period of 10 years. 103 knees were personally examined, 45 cases were investigated by questionnaire. RESULTS: 74% were content with the result of the operation. Deterioration of complaints could be stopped in many cases, although the radiographic changes were not parallel to subjective relief of pain. The survival rate without implantation of knee prostheses was 96% 12 years postoperatively. In 10 cases which had continuous complaints knee arthroplasties were performed one to two years after high tibial osteotomy. 6 knees required additional surgery because of loss of correction. Serious complications did not occur apart from one peroneal nerve palsy and two deep though completely healed infections. CONCLUSION: High tibial osteotomy in a modified procedure according to Coventry thus guarantees long term success with low risk of failure only.

[Results of knee joint arthrolysis surgery (author's transl)].

This work reports the results of operative treatment of knee-stiffness from 1973-1981 in the Department of Orthopaedic Surgery, University Kiel. 42 out of 46 occurrences of age, were post-operatively controlled. In most cases the stiffness was of traumatic origin. In more than one half of the cases the pre-operative controlled. In most cases the stiffness was of traumatic origin. In more than half of the cases the pre-operative degree of motion was less than 60 degrees. Intraarticular revision alone or extraarticular revision alone or a combination of both and other bone surgery was performed in approximately 1/4 of the patients respectively. The absolute range of motion, an average of 58 degrees pre-operatively, improved to 114 degrees post-operatively. The mean relative improvement of motion, in regard to a norm of 140-0-0 degrees, was 68% at discharge. Among our patients complication were seldom. In our opinion operative arthrolysis in indicated cases is a tried and proven method for treatment of stiff knees.

[Arthrography in extension stiffness of the knee joint].

In the present paper the authors point out in the importance of arthrography in the preoperative diagnosis of stiffness of the knee joint; their findings are based on a study of 29 cases. The majority of the roentgenograms show the knees of patients who subsequently underwent surgical arthrolysis. The technique and special features of the procedure are described. The evaluated contrast films may be classified in three groups, the first including joints with a residual mobility of up to 50 degrees, the second including those with between 50 and 90 degrees and the third those with more than 90 degrees. Typical for the first group is an obliteration of the upper recessus and an omega-shaped form in AP films. Films from Group 2 only show constrictions of the recessus suprapatellaris. In the third group only slight changes are found, which in some cases could only be assessed more accurately by comparison with the healthy side. In cases of knee-joint stiffness due to extra-articular causes there is no correlation between the arthrogram and the range of mobility. The authors conclude that X-rays of the interior of the joint using contrast medium can be a valuable help in planning surgical treatment for fibrotically stiffened knee joints.

[Patellofemoral contact zones in knee endoprostheses].

Failures after total knee arthroplasty often are caused by problems of the patellofemoral articulation. We investigated 10 different femoral components and 12 different patellar implants. The size of the patellofemoral contact areas was evaluated by pressure sensitive films every 10 degrees up to 90 degrees of flexion. The forces of the quadriceps muscle and the patella ligament were measured by force transducers. The compressive forces acting between patella and gliding grove were calculated. The geometry of the components and forces was documented. The retropatellar contact areas are specific for each prosthesis. The patterns of displacement are similar for some groups of prostheses. The absolute size of the contact areas and their size relative total surface of the patellar implant is very small. This induces high local pressure of the polyethylene.
