Chemotherapy of disseminated gastric cancer. A joint effort of the Northern California Oncology Group and the Japanese Gastric Cancer Chemotherapy Group.

A consortium of Northern California and Japanese investigators studied 142 patients with advanced gastric adenocarcinoma. Patients were randomized to one of two combination chemotherapy programs in each nation (one common therapy shared and one therapy unique to each nation). Median duration of survival ranged between 5 and 45 weeks, depending on performance status, extent of disease, and chemotherapy program. More importantly, there was considerable comparability with respect to toxicity and outcome between patients in the US and Japan. Overall median survival was 26 weeks for Japanese and 27 weeks for US patients. This study helps provide the basis for future comparative multinational trials.

Effective participation in cooperative clinical trials by an independent community organization. An NCOG Model.

The NCCP has established a model for community involvement in study group clinical trials, based on the use of independent community groups as participating entities. The structure and operation of the Sacramento, California demonstration project are presented. Evaluation of the NCOG small cell lung cancer study 2061 reveals that the data submitted by the community group equalled or exceeded university-generated and group-wide data for evaluability, response rate, survival, and quality control.

Sequential combination of bleomycin and mitomycin in advanced cervical cancer--an American experience: a Northern California Oncology Group study.

Eighteen evaluable patients with advanced squamous cell carcinoma of the cervix were treated with a sequential combination of bleomycin and mitomycin. We observed no complete responses and only four partial responses (22% response frequency). The median duration of response was only 3.4 months, and the median survival time for the partial responders was 8.5 months versus 4.0 months for the nonresponders. Three cases of bleomycin pulmonary toxicity were reported, two of a severe grade. It appears that this combination is of limited effectiveness and is inferior to other combinations.

Effect of norepinephrine and isoproterenol on in vitro detrusor muscle contractility and cyclic AMP content.

Norepinephrine, isoproterenol, and the phosphodiesterase inhibitor, theophylline, caused relaxation of in vitro dog detrusor strips associated with significant increases in detrusor cyclic AMP content. Bethanechol-induced detrusor contractions and alpha-adrenergic detrusor contractile responses to norepinephrine after propranolol were not associated with changes in cyclic AMP. Cyclic nucleotide metabolic pathways exist in detrusor muscle and offer another investigative approach to bladder dysfunction.

Antimicrobial selection by a computer. A blinded evaluation by infectious diseases experts.

An evaluation of a computer-based consultation system called MYCIN was made. Eight independent evaluators with special expertise in the management of meningitis compared MYCIN's choice of antimicrobials with the choices of nine human prescribers for ten test cases of meningitis. MYCIN received an acceptability rating of 65% by the evaluators; the corresponding ratings for acceptability of the regimen prescribed by the five faculty specialists ranged from 42.5% to 62.5%. The system never failed to cover a treatable pathogen while demonstrating efficiency in minimizing the number of antimicrobials prescribed. The study design may be useful in assessing the performance of other computer-based clinical decision-making systems.

Anesthetic management of conray toxicity.

Conray (meglumine iothalamate), the contrast media frequently used in shuntograms for diagnosing malfunctioning ventriculo-peritoneal shunts, will occasionally cause severe muscular spasms and seizures. In this article, the authors describe anesthetic and critical care management of a case with this complication.

Fast hemoglobins and red blood cell metabolites in citrate phosphate dextrose adenine stored blood.

Blood was drawn from 10 fasted, healthy volunteers and stored under standard blood bank conditions in citrate-phosphate-dextrose-adenine (CPDA-1). Blood was sampled before storage (Day 0) and on Days 5, 12, 19, 26, and 35. Laboratory testing for glucose, HbAla + b, HbAlc, pyruvic acid, lactic acid, adenosine triphosphate (ATP), 2, 3-diphosphoglycerate (2,3DPG), plasma free hemoglobin (Hb) and pH (blood gases) were performed. In addition, P50 was also serially measured in two of the individuals and in their stored blood. Significant elevations of HbAla + b and HbAlc (fast hemoglobins) were found on Days 12 and 19 of storage (p less than 0.05). These elevations of fast hemoglobins are due to hypoxia, acidosis, and hyperglycemia. Following the initial elevation of the fast hemoglobins (Hbs), there was a decline in their concentration, from Day 12, which could partly be explained by cell death.

Phase II study of sequential methotrexate and 5-FU plus mitomycin and leucovorin in patients with disseminated large bowel cancer: a Northern California Oncology Group study.

Sequential methotrexate (MTX) and 5-FU plus leucovorin and mitomycin (MMC) was given to 52 patients with disseminated, measurable colorectal cancer. Complete and partial responses were seen in 19 of 49 (39%) evaluable patients. Nine additional patients achieved a minimal response. Median overall survival was 8.8 months, while that for patients objectively responding was 13.6 months. This study utilized a MTX exposure period of 12 hours prior to 5-FU. In comparison to other trials utilizing sequential MTX and 5-FU, the addition of MMC to the regimen may improve the response rate but increases the incidence of adverse effects, particularly myelosuppression and renal toxicity. This phase II trial suggests that sequential MTX and 5-FU plus leucovorin and MMC is an active combination therapy and warrants further comparative trial in patients with large bowel cancer.
