[Primary cardiac tumours: diagnosis and surgical treatment (author's transl)].

A teratoma of the cardiac skeleton, a rhabdomyoma of the right ventricle and an intramural fibroma in the left ventricle were observed in three children, aged eight weeks, eight months and eight years, respectively. Atrial myxoma was found in a 42-year-old and a 49-year-old patient. History and symptoms showed no particular common features in the various patients, merely signs of flow obstruction in four of the five patients. Amongst specific cardiological diagnostic methods only angiocardiography was reliable in the demonstration of intra- or paracardiac space-occupying lesions. All five patients were successfully operated on under cardiopulmonary bypass. The surgical technique varied according to localisation, extent of the tumour and its site in relation to essential cardiac structures. Follow-up examination indicated that the patients were free of symptoms and recurrence.

[Analysis of the statistical distribution of aminolevulinic acid dehydratase in a population not exposed to occupational chemical factors].

The subject of the paper has been to check the aminolevulinic dehydratase (ALAD) activity in those with no occupational exposure to inhibitors of this enzyme. Altogether 293 persons have been examined, 132 women and 161 men, aged 20-69. The resultant ALAD activity distribution showed no differences between women and men, though statistically significant differences have been found between the mean values. The distribution did not deviate from normal in both groups. The analysis of ALAD activity distribution in particular age groups, whether female or male, showed statistically significant differences. No clear dependence between ALAD activity and haemoglobin concentration in blood has been found. On the other hand, clear differences in ALAD activity between groups of high and low haemoglobin concentrations have been found. In addition to the mentioned examinations, 32 persons (12 women and 20 men) presently exposed to lead had their lead concentration determined in blood. No correlations have been found between lead concentrations in blood and ALAD activity.

[Mechanisms of the protective effect of methohexital on cerebral energy metabolism].

The purpose of the present investigation was to study the effect of alpha-(+/-)-5-allyl-1-methyl-5-(1-methyl-2-pentinyl) barbituric acid (methohexital, Brevimytal sodium) on brain energy metabolism. The model of the isolated perfused rat brain was used. The high-energy phosphates, some substrates of glycolysis and the intracellular distribution of hexokinase activity were measured in the cortical tissue of the isolated rat brain after 30 min of perfusion, after ischemia or anoxia and after various recovery periods. The EEG was recorded as a parameter of the neuronal activity. The following results were obtained: 1. Ischemia and anoxia accelerated the glycolysis rate which was inhibited by methohexital. 2. The energy metabolism was more rapidly normalized after ischemia or anoxia when methohexital was added to the perfusion medium (0.2 mmol/l). 3. Compared to the corresponding preparation, the spontaneous electrical activity of the isolated brain was maintained in ischemia or anoxia for a longer period and appeared again more rapidly in the recovery periods when methohexital was present. 4. Phosphofructokinase activity was inhibited by methohexital in the recovery period after ischemia or anoxia, respectively. 5. Inhibition of hexokinase activity predominated in the surgical stage of anesthesia when glycolysis rate was not accelerated. Methohexital seemed to inhibit hexokinase activity by solubilizing the mitochondrial bound form.

Induction and cross-species activity of rat fibroblast-derived interferon.

Interferon derived from rat fibroblasts (RFA-1) was pH 2 stable and heat labile. Denaturants such as SDS and urea inactivated this interferon but beta-mercaptoethanol alone did not. IFN yield after Newcastle disease virus was multiplicity of infection (moi) dependent. Optimal induction was obtained with an moi of 10. RFA-1 cells produced small amounts of IFN (less than or equal to 10(1.5) U/ml) when exposed to varying amounts of poly(rI): poly(rC). Addition of DEAE-dextran and/or pretreatment with IFN did not change this response. Pretreatment resulted in increased interferon production, priming, but on the average it was only about a 2-fold increase. However, the kinetics of interferon production were altered by pretreatment with IFN whether there was an increase in production or not. Both IFN synthesis and production peaks were detected 2-4 h earlier in IFN-pretreated RFA-1 cells. Rat IFN exhibited a great deal of cross-species activity. It is 100-300% cross-reactive on mouse (L929B and 3T3) cells, and 5-10% active on guinea pig fibroblasts, human (GM 2767) and bovine (MDBK) cells. These cells were more sensitive to rat IFN than Jensen sarcoma cells (a rat tumour cell line). Neither chicken embryo fibroblasts nor monkey (Vero) cells developed an antiviral state when exposed to rat IFN.

[Iron metabolism and serum lysozyme as exponents of nonspecific resistance of workers exposed to CS-2].

The paper was aimed at an evaluation of nonspecific humoral resistance in CS2 chronically exposed workers. The studies covered 68 men of medium, 16 years, duration of employment in exposure and 20 controls. In all subjects the concentration of lysozyme and some ferrum economy parameters were determined. In the group of chronically CS2 exposed workers the serum lysozyme index was found to be statistically significantly decreased. In addition, in 14.7% of subjects, increased ferrum concentration and increased index of plasmatic transferin saturation with ferrum were found as well as statistically significant decrease in lysozyme concentration and index. The observed changes in the parameters concerned, contributing to the system of humoral nonspecific resistance may result in an increased infectious diseases incidence among the CS2 exposed workers.

Conduit-integration in correction of right ventricular outflow obstruction.

A new method for correction of right ventricular flow tract obstruction is described. The method combines the advantages of patch-plasty-procedure and external valved conduit by conduit integration into the right ventricular and pulmonary arterial wall. Good results had been achieved in 3 cases (2 cases of Fallot's tetralogy and one case of double outlet right ventricle).

Studies on the linkage of energy metabolism and neuronal activity in the isolated perfused rat brain.

An isolated rat brain preparation was perfused using glucose-free (= aglycemic) media. The high-energy phosphates, substrates of the glycolytic pathway, free amino acids, acetylcholine as well as the intracellular distribution of hexokinase activity were determined in brain tissues. The EEG was evaluated visually. The levels of glycolytic substrates, glutamate, and glutamine in cortical tissue decreased after aglycemic perfusion whereas the asparte level increased and the GABA level remained unchanged. The high-energy phosphate content seemed to be unaffected for about 15 min of aglycemic perfusion and fell significantly after 20 min. The EEG of the isolated brain changed rapidly after starting aglycemic perfusion and became isoelectric after 12--15 min. Hyperglycemic perfusion (35 mmol glucose per liter perfusion medium) did not alter the energy metabolism of the isolated brain. The breakdown of cerebral energy metabolism and of EEG activity was postponed when thiopental was added to the perfusion medium. The soluble hexokinase activity measured in cortical tissue was reduced after aglycemic perfusion and was enhanced after thiopental. Hyperglycemic perfusion did not influence the intracellular hexokinase distribution. The acetylcholine level in the striatum of the isolated rat brain was significantly decreased by aglycemia and was increased in hypothalamus by thiopental. It was suggested that hexokinase bound to the mitochondrial membrane may play an important role in the relationship of energy metabolism and neuronal activity.

[Biochemical polymorphism in a population exposed to environmental pollutants. Importance of studying the problem].

Genetically conditioned interpersonal differences in reactions to identical doses of medicines make us suppose that some of those exposed to environmental pollutants are at a special risk. This is revealed by detection of some biochemical differences in blood, conditioning a special susceptibility to the effect of some toxic substances. Another method is the so called loading test, aimed at observing the personal reaction to innoxious doses of medicines or toxicants. Recently, biochemical tests have been regarded more and more valid; these tests allow to foresee what medicine metabolism will be like in organism and consequently, how extensive the potential risk of the given person is.
