[A case of malignant Sertoli cell tumor of the testis].

A 74-year-old man consulted our hospital about a painless swelling of the right scrotal contents. Laboratory findings showed no abnormalities. A right high orchiectomy was performed. The tumor measured 4.2 X 6.0 cm in size and was yellow-white in color. Histologically, the tumor was diagnosed as a malignant Sertoli cell tumor on the basis of findings of invasion and a high mitosis rate. Combined chemotherapy was performed after the operation and no recurrence was found. The Sertoli cell tumor of the testis is a rare neoplasm, and its malignant type is extremely rare. In Japan, 7 cases have been reported, and only 2 of them have been malignant. This case is the 3rd instance malignant Sertoli cell tumor in this country.

Seasonal variations in vanillylmandelic acid and homovanillic acid in infants' urine.

Seasonal variations in urinary levels of vanillylmandelic acid (VMA) and homovanillic acid (HVA) have been investigated over a two-year period, using 31,466 filter-paper specimens of urine from infants of 6 months of age who underwent Sapporo City's screening for neuroblastoma. Levels of both VMA and HVA (micrograms/mg creatinine) were significantly higher in summer (11.48 +/- 7.40, n = 7,569, and 18.70 +/- 8.18, n = 7,582, respectively) than in winter (10.72 +/- 5.76, n = 8,143, and 16.46 +/- 7.17, n = 8,148, respectively) throughout the two years (P less than 0.01, t-test). Although the cause of this difference could not be found, its actual clinical relevance was thought to be small since there was no significant relation between monthly rate of re-screened infants and monthly mean levels of VMA or HVA (P greater than 0.05, Spearman-Rank correlation).

[Two cases of paratesticular rhabdomyosarcoma].

Two cases of paratesticular rhabdomyosarcoma are reported. A 15-year-old boy was admitted in July, 1983, with a 3-week history of gradually increasing painless scrotal swelling on the right side. With the suspicion of testicular cancer, right radical orchiectomy was performed and pathological examination revealed an embryonal rhabdomyosarcoma of the paratesticular region. Chemotherapy and retroperitoneal lymphadenectomy were performed, and the latter revealed lymph node metastasis at the bifurcation of the aorta. Radiotherapy was not done because of adherent ileus. About 8 months after the orchiectomy, he died of recurrence at pelvic cavity and brain metastasis. A 6-year-old boy presented in March, 1985, with a complaint of right scrotal swelling that was painless and gradually increasing for about a month. Transinguinal exploration revealed a paratesticular neoplastic lesion and right radical orchiectomy was performed. Pathological diagnosis was paratesticular embryonal rhabdomyosarcoma. No evidence of metastasis was found by chest X-ray, excretory urogram, abdominal CT-scan or lymphangiography. As post-operative treatment, only chemotherapy was performed, and 9 months after the orchiectomy, the patient was asymptomatic. Ninety four cases of paratesticular rhabdomyosarcoma found in the Japanese literature are reviewed and mainly the policy of treatment is discussed.

Establishment of cut off values for vanillylmandelic acid and homovanillic acid in 24-hr urine accumulation during mass screening for neuroblastoma.

The establishment of cut off values in vanillylmandelic acid (VMA) and homovanillic acid (HVA) in 24-hr urine specimens for the purpose of separating true positive cases from normal infants has been discussed. The specimens were collected at the time when thorough examinations under a mass screening programme for neuroblastoma were taking place, and data from 17 infants with neuroblastoma (patients) and 50 healthy infants (controls) have been employed. The function square root VMA2 + HVA2 = 34 (micrograms/mg of creatinine) separated the two groups with no overlapping cases. This function represents the distance from the origin of the VMA-HVA co-ordinates, and is thought to be one of the most reasonable discriminant methods when considering the distribution pattern of the data for normal cases and patients.

Simple liquid-chromatographic measurement of vanillylmandelic acid and homovanillic acid in urine on filter paper for mass screening of neuroblastoma in infants.

We describe a simple and rapid method for measuring catecholamine metabolites in urine collected on filter paper. By this method, "high-performance" liquid chromatography, urinary vanillylmandelic acid and homovanillic acid can be measured within 15 min after being eluted from filter paper treated with tartrate buffer. One hundred urine samples can be pretreated within 2 h and 100-120 samples analyzed at 18-min intervals overnight in two chromatographic systems. Moreover, urinary creatinine also can be measured rapidly because of the use of microtiter plates. We applied this method in a mass screening program for early detection of neuroblastoma in infants. Between April 1986 and March 1987, two patients with neuroblastoma were detected. In all, neuroblastoma has been detected in 17 of 88,887 infants so examined since April 1981.

Hyperamylasemia associated with endometrioid carcinoma of the ovary: case report and immunohistochemical study.

Hyperamylasemia was noted in a patient with a stage I well-differentiated endometrioid adenocarcinoma of the ovary. Serum levels of amylase decreased rapidly after removal of the ovarian tumor. Immunohistochemical study revealed intracytoplasmic localization of amylase in many tumor cells. Although it was strongly suspected that the tumor cells had produced amylase in the present patient, further studies are required to be sure that amylase can be considered an important tumor marker for the endometrioid type of ovarian tumors.

Determination of urinary vanillylmandelic acid and homovanillic acid by high performance liquid chromatography for mass screening of neuroblastoma.

In order to develop a mass screening method for the early detection of neuroblastoma, we measured urinary vanillylmandelic acid (VMA) and homovanillic acid (HVA) by a high performance liquid chromatography (HPLC) equipped with an electrochemical detector. Urinary samples were collected on a piece of filter paper and then pre-treated by a simple method. Measurement of VMA and HVA was performed within a short period of time, and by about 80 samples per a day could be analyzed by automatic system. The method described herein proved to be reliable, and we thus recommend it as a useful method for measurement of both VMA and HVA, leading to more accurate detection of neuroblastoma in mass screening test.

[Clinical study of testicular tumors (59 cases in Sakai Municipal Hospital)].

Fifty-nine cases of testicular cancers were admitted to our hospital between 1960 and 1984. Thirty cases were seminoma (patients aged 12-64 years) and 29 cases were non-seminoma (6 patients were 1-7 years old and 23 were 17-44 years old). One of each of the seminoma and non-seminoma cases was not included in our analysis due to death before sufficient treatment. Of the remaining 29 cases of seminoma, 24 were in stage I, 4 in stage II, and 1 was in stage III. Of the remaining 22 adult cases of non-seminoma, 15 were in stage I, 2 in stage II, and 5 in stage III. Most of the cases with seminoma were treated with radiation after orchiectomy. Two of the cases of advanced seminoma and all of the cases of non-seminoma without mature teratoma were treated with retroperitoneal lymphadenectomy and/or chemotherapy after orchiectomy. Of the seminoma cases, only one patient (stage II cancer) died. On the other hand, only one of the non-seminoma patients in stages II and III remained alive. Since 1979, 5 patients with stage III or bulky stage II disease have been treated with the PVB protocol and 4 were evaluable. Only one patient achieved complete remission (CR) but he suffered a relapse within a short period (4 months). Three patients were partial responders and two of them achieved CR following resection of residual disease and additional VAB-6 induction. Currently, none of the patients who were treated with the initial PVB protocol alone remain in CR.

Vanillylmandelic acid, homovanillic acid, and catecholamines in urine of infants with neuroblastoma 6- to 11-month-old.

The amounts of vanillylmandelic acid (VMA), homovanillic acid (HVA), and catecholamines (adrenaline, noradrenaline, and dopamine) in 24-hr urine specimens of 10 infants with neuroblastoma detected through mass screening in Sapporo City (patient group) were compared with those of 24 healthy infants (control group). The ages of the infants of both groups ranged from 6 to 11 months, which agrees with the age when mass screening is performed. For VMA and HVA, the mean value plus two or three times the standard deviation of the control group clearly separated the two groups in the units of mg/day, microgram/mg of creatinine, mg/kg/day, and mg/m2/day. The cut off values of 24 micrograms/mg of creatinine for VMA and 25 micrograms/mg of creatinine for HVA are now being used by us for assaying 24-hr urine specimens. No significant difference was found between the groups in levels of catecholamines, but their data were thought to be useful as normal values, since few published data regarding their normal values are available.

Primary gastrointestinal lymphoma. A clinicopathologic study of 102 patients.

Clinicopathologic findings in 102 patients with primary gastrointestinal lymphomas were reviewed. Abdominal pain was the common presenting symptom. The primary sites of the tumors were: 67 in the stomach, 24 in the small intestine, 7 in the ileocecal region, 3 in the large intestine, and 1 in the esophagus. The disease more frequently affected males than females and showed peak incidence in the 5th decade of life. Gastric lymphomas usually presented with a single lesion, but multiple lesions were frequent in the small intestine. The body and/or antrum of the stomach were the commonest sites of the lymphomas. Gastric lymphomas were diagnosed at an earlier stage than intestinal lymphomas. There was 1 case with Hodgkin's disease. The remaining 101 patients with non-Hodgkin's lymphomas were classified according to the Rappaport and the Kiel classifications. The proportion of nodular lymphomas in the present series was 7%. The frequencies of diffuse histiocytic type ard germinal center cell tumors were 62 and 74%, respectively. Cox's multivariate analysis for prognostic factors revealed that the stage of the tumor, sex, and age were prognostically significant.

[Transverse ectopia of the right testicle and tumor of the left testicle with immature uterus and fallopian tubes: report of a case].

A case of transverse ectopia of the right testicle and the left testicular tumor with the immature uterus and the fallopian tubes is reported. The patient, a 27-year-old married man, was admitted to our hospital with the suspicion of left testicular tumor and right undescended testicle. In his left scrotum, a double hen-egg-sized tumor was palpable and the right scrotum was empty. Exploration of the left inguinal canal disclosed double testicles with hernia uteri inguinalis. The right testicle showed transverse ectopia, and the histological finding of the left testicle was choriocarcinoma. In our review of the Japanese literature, we discovered 67 reported cases of transverse testicular ectopia. 33 (49.3%) out of 67 cases had a uterus and 9 cases (13.4%) had testicular tumor.

[A case of non-specific inflammatory granuloma of the bladder with marked eosinophilic infiltration].

A case of non-specific inflammatory granuloma of the bladder with marked eosinophilic infiltration is reported. A 54-year-old woman was admitted to our hospital on March 9, 1983, with the suspicion of bladder cancer. Her primary complaints were miction pain, frequency and gross hematuria. Cystoscopic examination revealed a hemi-sphere-shaped tumor at the dome of the bladder. Biopsy revealed no malignancy and marked eosinophilic and macrophagocytic infiltration of the submucosa. The patient was treated with antiallergic drugs and antibiotics with excellent relief of symptoms and objective remission of bladder findings on repeated cystoscopy and computed tomography.

Uterine teratoma with lymphoid hyperplasia.

A mature teratoma of the uterine cervix, which was found nine months after delivery, was presented. This case was the ninth in the world, the first in Japan. Differential diagnosis and histogenesis of the uterine teratoma were discussed on the basis of histologic findings of this tumor and a review of literatures. Among the microscopic findings obtained, the conspicuous lymphoid tissue in the tumor was worthy of note, because it could be speculated to be an immunological reaction to the cells composing the tumor. It was also suggested from the post-partum occurrence that a fetal-cell origin would be possible in the histogenesis, although further positive studies on this opinion should be necessary in coming cases.

Hemodialysis-associated asthma in a renal failure patient.

In a renal failure patient, asthma developed after 1 year hemodialytic treatment. The attack was exclusively associated with each hemodialysis. Skin test to the usual allergen such as house dusts or pollens was negative. The provocation test by inhaling acetate solution or oral intake of acetic acid was positive resulting in an increase in the pulmonary resistance, or asthmatic attack. This asthma disappeared completely after substitution of acetate in the hemodialysis by the use of a bicarbonate dialysate. Acetate might therefore precipitate the attack of asthma.
