Orthotopic heart replacement by nonpulsatile axial blood pumps.

In the last decade, there have been many experiments with nonpulsatile blood pumps to perform functional heart replacement by biventricular assist devices. To determine if life is sustainable with nonpulsatile blood flow, two nonpulsatile blood pumps were implanted in the orthotopic position as an artificial heart after removal of the natural heart in extracorporeal circulation. In three preliminary experiments, the feasibility of total heart replacement with nonpulsatile implantable blood pumps was shown.

Hydraulic driving unit and control system for artificial hearts.

A driving unit for artificial ventricles was constructed in which the power transmission is hydraulic. This avoids the danger of an air embolism, which is characteristic of pneumatic drives in the case of membrane rupture. An electromagnet as a power source drives a rolling membrane pump to move the transmission fluid to the artificial ventricle. This incompressible connection allows direct control of membrane motion and pumped blood volume simply by measuring the armature stroke. The volume-controlled mode is characterized by automatic self-regulation according to Starling's law and self-synchronization when the drive is used as a left (or right) ventricular assist device. Several measures were taken to increase the operational safety. The hemodynamic efficiency of the drive was tested in a number of in vivo experiments. The long-term stability was proven in a 12-month durability test.

[Acute abdomen in leukemias in childhood].

During the last years the treatment of acute leukemia became more successful thanks to aggressive chemotherapeutic regimen. There are nowadays about 70% long term survivors in the literature. On the other hand, complications and infection rate following such an aggressive treatment increased. We found in our patients an accumulation of acute abdominal complaints, mostly due to an acute appendicitis. Eight patients had to undergo appendectomy. We tried to answer the question whether there is a correlation between complications and the intensity of chemotherapy. The indication for operation was always proven to be correct, and no patient was lost in the postoperative course.

The spindle pump--a new concept for a nonpulsatile bloodpump.

The spindle pump--a new nonpulsatile bloodpump--has been developed as paracorporeal ventricular assist pump. The experiments with calves clearly demonstrated, that this pump as LVAD empties the left ventricle completely, the aortic pressure curve showing the typical nonpulsatile profil. A problem is the traumatic hemolysis, the free hemoglobin was in all experiments slightly but always above our standard. To solve this problem we will optimize shapes of the spindle and of the housing and coate all parts of the pump with polyurethane.

[Possibilities of ascites draining in childhood].

Children with congenital or acquired cirrhosis of the liver may lead a relatively normal life for a certain time due to better therapeutical measures, but the prognosis is usually unfavourable. Transplantation of the liver seems to become a possible therapy in the foreseeable future. It may be necessary to drain an ascites resistant to therapy in order to bridge the time between the onset of liver failure and transplantation. The advantages of peritoneo-venous drainage are stressed, the available shunt systems (LeVeen, Agishi , Denver, Cordis Hakim) are presented and discussed.

[Diagnosis of congenital pleuropericardial defects].

Pleuro-pericardial defects have been found incidentally during post mortems or chest surgery but, because of their usual lack of symptoms, are rarely found otherwise. If suspicion of such a malformation arises, it should be clarified because of possible herniation of the atrium or ventricle. The necessary diagnostic steps are discussed in relation to two patients and the value of chest x-rays is stressed. A pleuro-pericardial defect should be diagnosed because of the typical radiological changes of the cardiac contour. Only in exceptional cases should it be necessary to induce a diagnostic pneumothorax or to perform an exploratory thoracotomy.

Giant cell arteritis of the median nerve simulating carpal tunnel syndrome.

We present a distinctly rare case of giant cell arteritis involving the small artery of the median nerve with symptoms that may be confused with carpal tunnel syndrome. The excision of the involved arterial segment cures the patient of the local symptoms, but the administration of corticosteroids as soon as the diagnosis is established is very important for these patients with multiple arterial involvement.

[Staging of Hodgkin's disease in childhood].

Any therapy of the Morbus Hodgkin depends on an exact staging. Until 1969 staging was done after evaluation of different parameters such as histological examination of lymph nodes, scintigraphy of liver and spleen, laboratory tests, and finally the clinical picture. Classification of lymphogranulomatosis was quite ameliorated after introduction of the staging laparotomy with or without splenectomy as a diagnostic routine procedure by Glatstein and Coworkers in 1969. And today any adequate therapy is based on this surgical staging. Before planning the laparotomy some questions are important for the pediatric surgeon and the oncologist: Which complication are to be expected? Should there always be a splenectomy, and does it have dangerous sequelae? Are simultaneous surgical intervention indicated? Which is the right timing for the staging laparotomy?
