[Intestinal protozoal infestation in persons with occupational sewage contact].

Intestinal protozoal infestation and antibodies against Entamoeba histolytica were determined in 614 employees of the Hamburg sewage works and compared with control groups without occupational sewage contact. The protozoal infection rate was significantly higher in sewer-men than in control persons. There was a positive correlation between the duration of exposure to sewage and the frequency of intestinal protozoal infestation. The risk of infection by sewage was higher than by a short journey to tropical or subtropical countries. Immunodiagnostic results showed no significant differences of invasive activity of E. histolytica between both investigated groups. On the basis of parasitological results classification of amoebiasis and giardiasis as occupational diseases in exposed persons is recommended.

[Value of computed tomography for tumors of the epi- and oropharynx].

The results of computed tomography in 89 patients with tumours of the epipharynx and oropharynx involving the mouth are compared with the findings of other forms of clinical examination. In 26 patients, CT provided additional relevant information, in eight patients CT provided less information than clinical examination. The value of CT depended largely on the localisation of the tumour; the poorest results were found in tumours of the tonsils and the soft palate. CT provided additional information most commonly with large tumours, since these are frequently associated with bone destruction, local infiltration and occlusion of large vessels.

Carcinoma, Squamous Cell↗

[Value of computer tomography in gynecological tumors].

Abdominal CT examination is the method of choice when examining ovarial tumours. Abdominal CT is redundant in the early stages of carcinoma of the collum and if the carcinoma has penetrated into the vagina. In advanced stages of carcinoma of the collum, CT enables accurate staging. CT is at least equal in relevance to gynaecological examination in respect of assessment of parametraneous infiltration. Distant metastases, as well as complications of the efferent urinary tract can be visualized. In view of this, the following procedure appears meaningful in advanced carcinoma of the collum: First of all, gynaecological examination is performed without anaesthesia to confirm the findings in the regions of the portio and vagina; as far as possible, the existence of the central parametraneous infiltrations is confirmed. For the purpose of further staging of the parametraneous infiltration, as well as of the paraaortal lymphomas and possible metastases, as well as for the purpose of excluding hydronephrosis, computed tomography is performed. This avoids burdening the patient with an examination under anaesthesia, and it also avoids invasive examination methods, such as urography and lymphography.

CT of malignant choroidal melanoma - morphology and perfusion characteristics.

The computed tomographic morphology of malignant choroidal melanoma and its perfusion characteristics are described. Thirty-three static and serial CT examinations made on 29 patients with choroidal melanoma, three with pseudotumors of the macula and one with choroidal metastasis revealed the choroidal melanoma to be usually a hyperdense, markedly perfused tumor, while the non-contrast diagnostically undifferentiable pseudotumors and the choroidal metastasis, revealed no significant change in density after the administration of contrast material. Density values of perfusion characteristics of choroidal melanoma that are outside of the normal range are a result of secondary changes within the immediate surroundings of the tumor, such as detachment of the retina, tumor-induced glaucoma, or tumor necrosis.

In vivo diagnosis of massive hepatic infarction by computed tomography.

A patient is presented with a postpartum hepatic artery thrombosis in association with presumed fibromuscular hyperplasia. Massive hepatic infarction developed characterized clinically by fever, coma, ascites, ileus, jaundice, and renal failure; and biochemically by markedly elevated SGOT and SGPT, alkaline phosphatase, total bilirubin levels, and decreased thromboplastin time. The diagnosis was made in vivo by computed tomography (CT). Angiography revealed thrombotic occlusion of the hepatic artery in association with presumed fibromuscular hyperplasia. Laparoscopy and biopsy confirmed the diagnosis.

[Possibilities of locating foreign bodies by computed tomography].

Clinical and experimental examinations are described for the localization of intra-ocular and intra-orbital foreign bodies by CT scan. The minimal detectable diameter size was 0.2 X 0.3 mm for metal, and 0.5 mm for glass. We had difficulty in distinguishing wood from air bubbles, mainly in the retrobulbar space. First clinical results have shown CT-localization to be useful in cases where conventional X-ray techniques led to doubtful results.

[Computed tomography of the mediastinum in myasthenia gravis. Normal findings, thymic hyperplasia, thymomas (author's transl)].

Computed tomography of the mediastinum was performed in 45 patients with myasthenia gravis. Surgery was carried out in fourteen. Amongst these, there were four thymomas, one thymolipoma, eight thymic hyperplasias and one normal thymus gland. A further patient, who did not have surgery, probably also had a thymic tumour. The normal thymus and thymic hyperplasia cannot be distinguished on computed tomography. Differentiation of small thymomas from normal thymus is not always possible. Invasion by thymomas can only be appreciated with large tumours.

[Computer angiography in renal transplants (author's transl)].

The results of computed angiography in 12 patients with renal transplants are described. In five patients with normal function of the transplant and normal appearances on a conventional angiogram, under-perfusion averaging 36% was demonstrated. In two out of three patients with a stenosis at the anastomosis, the effect of the stenosis on function could be shown. In two patients with acute rejection of the graft and hypoxia of the tubules, the differential diagnosis could be clarified by means of computer angiography. Computer angiography can therefore be regarded as a valuable addition to conventional angiography for the differentiation between tubular hypoxia and acute rejection and for underperfusion of a renal transplant.

[Computer tomography of the normal larynx (author's transl)].

The normal computer tomographic anatomy of the larynx and neighbouring structures is illustrated. Special attention is paid to the preepiglottic and para-glottic spaces, which cannot be adequately demonstrated by any other method. The technique is discussed with examinations done on a cadaver and on patients.

[First experiences with serial computed tomography in choroidal melanomas (author's transl)].

22 patients with extending intraocular lesions are examined by serial computed tomography after a bolus injection of contrast medium. The changes of X-ray density of the tumor areas are compared with those of the vitreum and of brain tissue. Choroidal melanomas exceeding 2 mm in prominence are identified as highly vascularized areas in which contrast enhancement according to certain brain tumors takes place. A differentiation from large pseudotumors seems to be possible. The value of this method for the differential diagnosis of choroidal melanomas is discussed.

[Computer tomographic findings in sacral destruction].

The diagnostic significance of computerized tomography in the diagnosis of destructive and deforming processes of the sacrum is demonstrated by means of 69 examinations of 55 patients. The results are compared with those obtained by means of conventional roentgenography, including tomography and bone scintigraphy. It is confirmed that recent CT scanners are the most sensitive instruments in existence for the visualization of affections of the sacrum in respect to identification and extension.
