In-vivo validation of on-line and off-line geometric coronary measurements using insertion of stenosis phantoms in porcine coronary arteries.

Geometric coronary artery measurements with the Phillips Digital Cardiac Imaging System (DCI) and the Cardiovascular Angiography Analysis System (CAAS) were validated using percutaneous insertion of radiolucent stenosis phantoms in swine coronary arteries. Angiographic visualization of the stenosis lumens (phi 0.5, 0.7, 1.0, 1.4, 1.9 mm) was simultaneously recorded on DCI and cinefilm. The acquisition systems were calibrated by either the diameter of the guiding catheter (catheter CAL) or the isocenter method (isocenter CAL). Minimal luminal diameters (MLD) obtained with CAAS and DCI on 20 corresponding cineframes were compared with the true phantom diameters (PD). The accuracy of MLD measurements with the CAAS using isocenter CAL was -0.07mm, the precision 0.21 mm (r = 0.91; y = 0.30 + 0.79x; SEE = 0.19), with catheter CAL the accuracy was 0.09 mm, the precision 0.23 mm (r = 0.89; y = 0.19 + 0.74x; SEE = 0.19). The accuracy of MLD measurements using the DCI with isocenter CAL was 0.08 mm, the precision 0.15 min (r = 0.96; y = 0.08 + 0.86x; SEE = 0.14), with catheter CAL the accuracy was 0.18 mm, the precision 0.21 mm (r = 0.92; y = 0.09 + 0.76x; SEE = 0.17). DCI underestimated PD with isocenter CAL (p less than 0.05) and with catheter CAL (p less than 0.001). MLD can be measured with high accuracy, both applying on-line digital as well as off-line cineangiographic analysis. The results of digital measurements demonstrate high reliability of the new digital software package.

Social-economic impact of head injury.

The socio-economic costs of traffic road accidents were analysed data epidemiological studies and compared with reported data. The costs are calculated as a function of accident type and vehicle involved, severity of head trauma, patients individual characteristics, type of care--intensive and emergency services, other hospital bed costs, including recovering and rehabilitation such as in-and out-patient services. Finally the costs of repairing materials (cars, walls, roads, etc.) are also estimated. The author concludes that the medical doctor must take part in compiling the statistics so as to be able to discuss the economics of injury and the social priorities.

Arachnoid cysts in children: a European co-operative study.

The data on arachnoid cysts in children (0-15 years) operated upon between 1980 and 1988 were analysed in a retrospective, co-operative study. The results from 285 patients indicate a predominance of these lesions in boys (64%) more than girls (36%) and a mean age of 6 years at onset of symptoms. Focal EEG patterns corresponding to the cyst's location were encountered in 32%. About 40% of all cysts were located along the midline, the sylvian fissure representing the predominant location. Open surgery, i.e. total excision or marsurpialization (together 43.3%), emerged as the first-choice surgical procedure. The type of surgery switched somewhat to shunting procedures in cases of lesions in deeper locations (22.8%). Morphological results on follow-up revealed a reduction of the size of the cyst in a significant majority (61%); in 18% the cyst had disappeared completely on CT scans. There was an obvious correlation between postoperative morphological findings and clinical outcome.

Edge detection versus densitometry in the quantitative assessment of stenosis phantoms: an in vivo comparison in porcine coronary arteries.

The aim of this study was the in vivo validation and comparison of the geometric and densitometric technique of a computer-assisted automatic quantitative angiographic system (CAAS system). In six Landrace Yorkshire pigs (45 to 55 kg), precision-drilled phantoms with a circular lumen of 0.5, 0.7, 1.0, 1.4, and 1.9 mm were percutaneously introduced into the left anterior descending or left circumflex coronary artery. Twenty-eight coronary angiograms obtained with the phantom in a wedged intracoronary position could be quantitatively analyzed. Minimal lumen diameter, minimal cross-sectional area, percent diameter stenosis, and cross-sectional area stenosis were automatically measured with both the geometric and densitometric technique and were compared with the known phantom dimensions. When minimal lumen diameter was measured using the geometric approach, a nonsignificant underestimation of the phantom size was observed, with a mean difference of -0.06 +/- 0.14 mm. The larger mean difference observed with videodensitometry (-0.11 +/- 0.20 mm) was the result of the failure of the technique to differentiate the low lumen videodensities of two phantoms of smaller size (0.5 and 0.7 mm) from a dense background. Percent cross-sectional area stenosis measured with the two techniques showed a good correlation with the corresponding phantom measurements (mean difference between percent cross-sectional area stenosis calculated from the quantitative angiographic measurements and the corresponding phantom dimensions was equal to 2 +/- 6% for both techniques, correlation coefficient = 0.93 with both techniques, SEE = 5% with the geometric technique and 6% with the densitometric approach).(ABSTRACT TRUNCATED AT 250 WORDS)

Reasons related to adherence in community-based field studies.

This study identified participants' reasons for good, marginal or poor adherence, or withdrawing from community-based clinical studies using a dietary and/or drug intervention. Adults aged 48-75 years participated in one of three studies related to decreasing colon polyp recurrence. Qualitative data from progress notes (N = 675) and end-of-study evaluations (N = 87) were coded using constant comparative analysis with 100% content validity panel agreement. Most common reasons for non-adherence were barriers such as side-effects, interference with vacation plans, unrelated illness, forgetting and competing outside stressors. Participation motivators were benefits such as altruism, medical benefits, free service and staff rapport. Findings supported the Health Behavior in Cancer Prevention model-based approach to adherence interventions and provided directions for adherence promotion in future community-based clinical studies.

Relation between decreased mental efficiency in mice and the presence of cerebral lesions after experimental encephalitis caused by yellow fever virus.

The behavior of 1,072 mice that had recovered from encephalitic infection with intracerebrally injected yellow fever virus 17D and of 216 normal mice was tested in a maze and on a horizontal rod rotating around its axle. Infected animals needed more time (average, 8.90 min) to find their food in a maze than did normal animals (average, 4.37 min). Infected mice were able to stay on the rotating rod for a shorter time (average, 6.4 seconds) than were normal animals (average, 9.0 seconds). The correlation between the concentration of virus injected and the performance of the mice was confirmed by the extent of lesions found by histologic study: animals that had anatomic lesions after surviving encephalitic infection showed abnormal behavior.

Operative treatment of cerebral arteriovenous aneurysm of vein of Galen complicated by congestive heart failure.

A rare cause of congestive heart failure in the neonatal period is an intracranial arteriovenous malformation, but this condition should be borne in mind when there is unexplained right-sided congestive heart failure. A case is reported of an aneurysm of the great vein of Galen, complicated by congestive heart failure. Successful surgical treatment was carried out using a two-stage procedure.

Value of tumor markers in the treatment of endodermal sinus tumors and choriocarcinomas in the pineal region.

A case of embryonal carcinoma in the pineal region of a 17-year-old boy is presented. The tumor included elements of choriocarcinoma and endodermal sinus tumor, and the use of human chorionic gonadotropin and alpha-fetoprotein as tumor markers is discussed. The markers were demonstrated both within the tumor and in the cerebrospinal fluid (CSF) and blood. The patient was treated with a postoperative program of irradiation and cancer chemotherapy, and at follow-up examination 20 months after operation no signs of residual tumor were present. It is suggested that human chorionic gonadotropin and alpha-fetoprotein should be measured in the blood and CSF before the treatment of midline tumors.

[Levels of immunoglobulins and numbers of lymphocytes in the venous blood from the neighbourhood of oral carcinomas Relations to short-time-prognosis (author's transl)].

Lower levels of immunoglobulins A and G than in a cubital vein were found in the venous blood from the neighbourhood of non-metastatic oral carcinomas. The authors these results interprete as an increased connection of antibodies to the receptors of tumor-cells. It is evident that the receptors are saturated in metastatic carcinomas. Favourable shorttime-prognosis is often connected with an increased number of lymphocytes in blood of tumour- and cubital-vein.

Quantitative determination of the disintegration of nerve cells in the cortex caused by viral encephalitis (17 D-yellow fever) (Preliminary communication).

The counting of nerve cells give an answer to the question whether there is a significant loss of cells in the cortex in the course of viral encephalitis. For normal animals, average cell numbers of 275.42 were counted; for infected animals divided into two groups depending on the severity of illness, 144.95 and 192.87 cells, respectively (p = 0.0001).

[Non-secreting plasmacytomas].

In non-secernent plasmocytomas there are no characteristic changes of serum protein. Diagnostic difficulties may be overcome by cytomorphological examinations, immunofluorescence and electron microscopy. By referring to 4 own observations the value of the electron microscopic characteristics of plasma cells is demonstrated. Subtile investigations in protein diagnostics are required for avoiding incomplete monoclonal immunoglobulins which may greatly enter the kidneys to be overlooked. Generally the prognosis of non-secernent plasmocytomas is not worse than that of other forms.
