A computer program for calculation and interpretation of pulmonary function studies.

A computer program to calculate and interpret the standard pulmonary function tests has been developed on a programmable calculator system. The program computes both predicted and measured values for static and dynamic lung volumes, airways resistance and diffusing capacity. It then checks for differences between predicted and obtained values and interprets them according to a specific, clinically useful set of diagnostic criteria. By acquisition of a reliable and easy-to-use data processing system, a pulmonary function laboratory can significantly increase the efficiency and accuracy of its day-to-day work.

Transbronchial lung biopsy via the fiberoptic bronchoscope. Experience with 107 consecutive cases and comparison with bronchial brushing.

Transbronchial lung biopsy (TBB) was performed during fiberoptic bronchoscopy under fluoroscopic guidance in 107 patients. TBB was diagnostic in 17 of 21 (81 percent) cases with localized malignant lesions greater than 4 cm in diameter and bronchial brushing was positive in 12 (57 percent). TBB was diagnostic in 14 of 24 (58 percent) cases with localized malignant lesions equal to or less than 4 cm in diameter, whereas brush biopsy was positive in 7 (29 percent). In 13 cases with suspected Pneumocystis carinii pneumonia, the combination of TBB and brush biopsy was diagnostic of P carinii pneumonia in all 11 patients; TBB was diagnostic in 10 and brush biopsy diagnostic in 5. Cytomegalovirus pneumonitis was diagnosed in the other two cases. In 20 patients with localized infiltrates or nodules, a TBB diagnosis of acute or chronic inflammation excluded malignancy in 15 of 18 cases (follow-up 3 to 24 months) and tuberculosis was diagnosed in 2. Malignancy was found in one patient with acute inflammation on TBB. TBB accurately diagnosed 23 of 29 (79 percent) cases of diffuse lung disease. Following TBB, one patient had pneumothorax and nine patients had hemoptysis not requiring treatment. TBB and bronchial brushing via the flexible fiberoptic bronchoscope offer good diagnostic accuracy and a low complication rate in both diffuse and localized lung diseases.

Antibody-mediated modification of the binding properties of a protein related to galactose transport.

A galactose-binding protein related to the mglP transport system of Escherichia coli increases its affinity and binding capacity for the substrate when exposed to both d-galactose and specific antibodies. For this increase to occur, the binding protein has to be in contact with d-galactose for at least 2 min prior to the addition of the antibodies. This reaction was used to show that other substrates of the mglP transport system compete with galactose for a site(s) of the binding protein and that the degree of competition is comparable to that observed in vivo. A model for substrate translocation is presented postulating a cellular component that can induce conformational changes in the galactose-binding protein similar to those caused by antibodies.

Feasibility of MR diffusion studies in the kidney.

Renal apparent diffusion coefficients (ADCs) were anisotropic within and significantly different between cortex and medulla using a relatively motion-insensitive one-dimensional technique in 20 volunteers. ADC values ranged from 1.79 +/- .39 to 2.95 +/- .58 (x 10(-3)mm2/sec), relatively high but similar to other reports. Further investigation may help clarify this data, and determine whether the findings result from diffusion properties (and/or radially oriented parenchymal architecture), or artifacts due to factors such as bulk motion.

Unknown ESUR cases 2004.

The authors present 14 cases from the film interpretation session of the 11th annual meeting of the European Society of Urogenital Radiology presented in September, 2004. The cases demonstrate the imaging findings, differential diagnoses, and clinical relevance of a wide variety of genitourinary tract diseases. The cases include examples of benign and malignant urinary tract neoplasms, inflammatory processes, vascular diseases, traumatic injuries, and congenital anomalies.

CT diagnosis of fracture of the coracoid process of the scapula.

A case of fracture of the coracoid process of the scapula is presented in which the findings were not apparent on plain films. CT demonstrated the fracture, and the absence of soft tissue mass suggested a remote traumatic etiology rather than metastatic disease. The findings were confirmed surgically and pathologically.

Magnetic resonance imaging of systemic venous anomalies.

Seven cases of anomalous development of the systemic great veins were found in the first 18 months of adult body imaging with a 0.15 T resistive magnetic resonance unit. Comparison was made with CT. In most cases, CT and MRI were equivalent in demonstrating the abnormality. In one case, MRI was superior to a drip-infusion CT. MRI was less successful when the low signal abnormal vein was adjacent to normal structures of low signal. Awareness of the MRI appearance of venous anomalies will aid their recognition as incidental findings.
