Pemphigus.

Pemphigus is an autoimmune blistering disorder. Paradoxically intact blisters are rarely seen as the superficial blister is so fragile. The disease is frequently fatal if untreated. Pemphigus is an ideal model for autoimmunity as the antigen has been characterized and the antibody demonstrated to be pathogenic.

Generalized pustular psoriasis precipitated by trazodone in the treatment of depression.

Severe psoriasis is commonly associated with depression and may be its cause. The hazards of treating such patients with lithium are well known (Lazarus & Gilgor, 1979), but other drugs may also destabilize psoriasis. We report a patient in whom the anti-depressant trazodone hydrochloride (Molipaxin, Roussel), a serotonin antagonist, provoked generalized pustular psoriasis (GPP).

Squaric acid dibutyl ester in the treatment of alopecia areata.

A controlled trial of contact allergen therapy of alopecia areata (AA) using squaric acid dibutyl ester (SADBE) was performed on 17 patients. The right side of the patient's scalp was treated and the left side acted as an untreated control. Minimal signs of terminal hair growth were present in only 3 patients after 8-12 weeks of therapy. A majority of patients experienced adverse effects which led to the early withdrawal of 2 patients and the premature termination of the trial. We do not recommend the use of SADBE in the treatment of AA.

Does UV-A potentiate 'short contact' dithranol therapy?

The effect of ultraviolet-A (UV-A) irradiation in combination with 'short contact' dithranol treatment was studied in ten patients with chronic plaque psoriasis. Increasing concentrations of dithranol were applied daily to the psoriatic plaques on both sides of the body. One side was irradiated with UV-A daily. There was no difference in the rate of clearance between the two sides. The effectiveness of 'short contact' dithranol was confirmed.

An assessment of the novel antihistamine BW 825C in the treatment of chronic idiopathic urticaria. A placebo-controlled study.

20 patients with a diagnosis of chronic idiopathic urticaria were entered into a double-blind placebo-controlled cross-over study. All patients completed the trial and during the assessment period they were treated with placebo, BW 825C (4 mg) and BW 825C (8 mg) according to a fully randomised and balanced treatment plan. Both doses of BW 825C were found to be highly effective and significantly better than placebo in controlling signs and symptoms of urticaria. Few adverse reactions were reported and in this small group of patients there was no significant difference from placebo in reports of drowsiness or any other side-effects.
