Analgesia in piezoelectric SWL: comparative study of kidney and upper ureter treatments.

BACKGROUND: The development of newer-generation lithotripters has reduced the pain associated with SWL, but many patients still require some form of sedation. We prospectively compared the analgesic requirements for kidney and upper ureteral treatments. Predictive factors for pain during piezoelectric SWL were also studied. PATIENTS AND METHODS: A total of 102 consecutive patients without any previous experience of SWL were treated for renal (N = 70) or upper ureteral (N = 32) calculi using the EDAP LT02 lithotripter. The stones' largest diameter ranged from 4 to 30 mm (mean 9 mm). Patients were given an oral dose of 60 mg of dextropropoxyphene hydrochloride and 800 mg of paracetamol associated with 100 mg of ketoprofene per rectum 30 minutes before treatment. The SWL session was begun at low intensity and increased to the maximal range of energy as rapidly as could be tolerated by the patient. The amount of pain during treatment was recorded according to a visual analogue scale (VAS). Further analgesia using intravenous alfentanil was given as required by the severity of the pain. Visual analog pain scores, additional sedation requirements, and success rates after one session were analyzed. RESULTS: The VAS scores and intravenous sedation requirements were significantly lower for patients with upper ureteral stones than for those with renal calculi (P < 0.01). The stone-free rates after one session were, respectively, 90% and 73% (P < 0.05). On the other hand, SWL tolerance was significantly lower for women presenting with renal stones (P < 0.05). CONCLUSION: Piezoelectric SWL without intravenous sedation is suitable for the treatment of upper ureteral calculi. However, such an approach is less efficient in the management of kidney stones, especially for female patients.

[Anterior urethral valve in a five year old boy (author's transl)].

The authors report the case of a valve of the anterior urethra in a five year old boy, which was carefully explored from a radiological, endoscopic and urodynamic standpoint. The lesion was successfully treated by transurethral resection. The features of this type of obstructive problem are discussed in the light of the case, together with the diagnostic value of urodynamic studies and endoscopic photography and, finally, the possibilities of treatment, emphasising the simplicity and effectiveness of transurethral resection.

[Y-V plastic repair of the neck of the bladder. A study of 30 cases (author's transl)].

The authors report on their own series of 30 Y-V plastic repairs of the neck of the bladder done according to the technique of B. W. Young on male and female patients ranging in age from 8 months to 74 years. They have treated using this technique 8 primary diseases of the neck of the bladder, 11 cord bladders, 5 myocardium bladders and 6 idiopathic retentions. A post-surgical observation period of 1 to 8 years showed a 43 per cent success rate, 30 per cent of improvement and 27 per cent of failure. The best results are obtained in diseases of the neck of the bladder. Protection of the genital functions make the Y-V plastic repair a preferred technique in the young male.

[Long-term results of Burch's cervico-cystopexy. Apropos of 40 cases].

Forty patients with stress urinary incontinence, treated between April 1982 and June 1988 according to Burch's technique, were reviewed. 24 (60%) successes, 8 (20%) major improvements and 8 (20%) failures were recorded with a mean follow-up of 85 months. Compared to the early postoperative findings, a marked deterioration in urinary continence was observed in 4 cases (10%). These results are in contrast with earlier papers reporting remarkable stability of the therapeutic performance of this type of cervicocystopexy. Although too brief to allow reliable statistical analysis, this series simply draws attention to the hypothetical long-term efficacy of Burch's operation.
