[Luteal insufficiency and benign breast diseases. Study in the light of data from the combined LH-RH + TRH test together with the study of ovarian steroids].

Luteal deficiency has been attributed a patho-physiologic role in benign fibro-cystic breast disease, whereas Prolactin would not be involved in this disease. To tes these hypotheses, patients with fibro-cystic breast disease have been investigated through the combined LH-RH + TRH test, coupled with ovarian steroids estimations. 63 menstruating patients with ovulatory cycles, as evidenced by the temperature curve and proven fibro-cystic breast disease, as demonstrated by senography and thermogrphy and/or operation, have been selected for this study. The findings do not favour the role of luteal dificiency but rather the role of PRL hyperactivity in the pathol-physiology of fibro-cySTIC BREAST DISEASE.

Therapeutic hypogonadism induced by a delayed-release preparation of microcapsules of D-Trp-6-luteinizing hormone-releasing hormone: a preliminary study in eight women with endometriosis.

Eight menstrually cycling women with proven endometriosis were treated with a delayed-release preparation of the superactive agonist D-Trp-6-LH-RH in biodegradable microcapsules, administered intramuscularly at intervals of 16 to 36 days for a period of 3 to 5 months. After the first injection which caused a transient stimulatory response, a sustained hypogonadal state, characterized by an estradiol (E2) level less than 50 pg/mL, was induced in all patients by day +14.7 (range 7.19). Subsequent injections caused no more stimulation, but a continuous decrease in E2 levels was observed. In all patients, E2 levels fell to zero after the second or third injection. Gonadotropins remained in the normal range throughout the course of treatment. Hot flashes and severe dyspareunia (related to vaginal dryness) were the main side effects. Resumption of normal pituitary-ovarian activity was documented in all patients after the last injection; the end of the hypogonadal state, ovulation and menses, occurred on days 41 (range 32-59), 58 (range 51-64) and 70 (range 65-76), respectively. All patients showed a clinical improvement. Our results demonstrate that a long-lasting, reversible hypogonadism can be induced in cyclic women by intramuscular injections of D-Trp-6-LH-RH microcapsules at monthly intervals. This simple and convenient treatment should be useful in treating endometriosis and other conditions.

[Plasma concentrations of clomipramine and its desmethyl metabolite in man. Methodologic limits].

The purpose of the present work was to study the correlations between clinical response and plasma concentrations of clomipramine or desmethylclomipramine, using two different methods of dosage: radio-immunoassay and high pressure liquid chromatography, compared with mass spectrometry method. The results vary according to the method used, suggesting that methodological problems might explain, at least partly, the discrepancies observed in the literature.

Unexpected normal serum beta-microglobulin (B2M) levels in multiple myeloma.

Measuring serum beta-2-microglobulin (B2M) in 90 patients with multiple myeloma (MM) from diagnosis to death (83 cases), or to the terminal phase of their disease (7 cases), we have obtained evidence that 9% of them retained normal serum B2M levels during the whole of their follow-up. This was observed in 6 IgA MM, in one light chain MM only and in one non-secretory MM. These data suggest a lack of B2M production and/or secretion by some myeloma tumors. This could limit the use of B2M as clinical marker in MM.

[Measurement of basal plasma levels of 3 anterior pituitary hormones during acute or chronic treatment with tricyclic antidepressants].

Prolactin, growth hormone and thyrotropin plasma levels have been evaluated in depressive in-patients, either during the first day of clomipramine or amitriptyline treatment, or after their chronic administration. Prolactin levels temporary rise during the first day of clomipramine or amitriptyline treatment in 6 patients out of 11, with a lag in relation to the drug plasma peak. A significant increase is observed after a 28 days treatment with clomipramine and a non significant decrease, after a 28 days treatment with amitriptyline. As for human growth hormone, a rise is found in 5 out of 8 clomipramine treated subjects but neither any variation with amitriptyline nor any significant variation with chronic administration of both drugs occur. Finally, thyrotropin plasma levels display no variation after acute or prolonged treatment with clomipramine or amitriptyline. These results are compared with those of literature, then discussed in the light of present theories on pituitary hormones secretion aminergic control and of tricyclic antidepressants effect on these hormones.

[A histological and hormonal study of 399 women near the menopause or with confirmed menopause (author's transl)].

This work is concerned with 399 women who were either near the menopause or had had the menopause and on whom 476 hormone levels and 169 examinations of the endometrium had been carried out. It is possible to put the women into three groups. 13 p. 100 showed a diphasic cycle, with more or less normal ovarian function. Among the group of women whose cycle was not diphasic an appreciable number had normal oestrogenic activity, or were indeed hyperoestrogenic. The others were hypo-oestrogenic. Several facts can be derived from this study. Raised levels of E.2 can be found even when there is no cyclical activity and even in women who have raised levels of FSH. Normal ovarian function may be resumed several months after the menopause, although when the levels of FSH are raised the occurrence of the postmenopausal state would seem to be confirmed. The presence of hot flushes does not seem to correspond to a particular hormonal state. Marked differences seem to exist between the levels of the plasma hormones and the endometrial biopsy results. It is possible to have an atrophic endometrium with a raised level of oestradiol-17 beta, or on the other hand a polypoidal endometrium in women who are hypo-oestrogenic. The presence of levels of progesterone such as are found in a normal luteal phase does not indicate that the endometrium will necessarily be secretory. These contradictions cannot be resolved by studying the intra-cellular concentrations of the hormone receptors. The approach of the menopause is not always characterized by a progressive and continuous state of oestrogen deficiency or by hypophysial overactivity. This classical picture can be heavily shaded. All kinds of hormone profiles and responses by the receptor organs may be met and the clinician should be aware of these facts when he chooses therapy.

[Comprehension of food label claims by university undergraduate students].

The purpose of this study was to determine whether claims made on food labels are well or poorly understood and whether the sex and the experience of consumers in buying food products influence their understanding of the various terms used in the food industry. A questionnaire was randomly distributed to 285 students registered in various undergraduate programs at the University of Ottawa. The average grade of all the respondents to the questionnaire was 53.6% which indicates a lack of understanding of the meanings of the various terms used in the food industry. The results also indicate that more frequent exposure to advertising by the food industry is apt to confuse the consumer. Finally, the respondents without experience in buying food products answered significantly better on three questions; in general, men without experience had significantly better results than women on some questions.
