A cooperative study on the detection of colorectal cancer and polyps in France.

Systematic investigations testing for the presence of polyps and cancers of the large bowel were conducted in a population of 1,369 inpatients and outpatients, aged 45-70 years, in eight university departments of gastroenterology or abdominal surgery (Toulouse, Dijon, Paris, Marseille, and Strasbourg). Double-contrast barium enema and proctosigmoidoscopy examinations were carried out in all cases, whereas total coloscopy was performed only in the case of detected tumors. A questionnaire including 233 parameters (age, sex, family, and personal history and symptoms) was completed for each patient. A total of 414 lesions were detected in 252 patients, including 245 adenomatous or villous polyps, 8 transformed polyps, and 30 carcinomas; 3 of 4 lesions were located in the rectum or sigmoid. Cancer or adenomatous or villous polyps were found in 13% of the patients. The prevalence of these lesions in the population studied was increased in patients with rectorrhagia (19%) or with a personal history of surgery for colorectal cancer or polyp (23%). In the patients without rectorrhagia or a history of intestinal tumor, the incidence was 9.7%. It was significantly increased in males and patients more than 50 year old. The efficacy of proctosigmoidoscopy and double-contrast barium enema was compared in 909 patients. Sensitivity and specificity were, respectively, 35% and 99% for endoscopy, 96% and 94% for radiology.

[Ciliated epithelium of the Fallopian tube in women. An ultrastructural study (author's transl)].

The steps in the ultrastructural changes that occur in new cilia formation in tubal epithelium have been well demonstrated in animals. In women, it is difficult to show cilia growing again and some authors deny that new growth occurs. Transmission electron microscopy studies on 32 specimens have shown that human new growth of cilia does occur. It arises in the midst of a cellular population which has as its characteristic a cytoplasm that is more clear than those of the neighbouring cells. Different ultrastructural aspects can be described. They resemble those that have been demonstrated in different animal species. On the other hand no mitoses have been shown. We will study the correlations between plasma hormone levels, the numbers of tissue receptors and the appearance of these clear cells in another article.

[Study on the effects of bromopride on intestinal peristalsis (author's transl)].

The effect of 4-amino-5-bromo-N-[2-(diethylamino)-ethyl]-2-methoxy benzamide (bromopride, Viaben) on the intestinal motility was studied in dogs. Highly sensitive transducers were implanted on the intestinal serosa, allowing to measure both electrical and mechanical impulses at the same point from the small intestine. These studies were performed in 5 dogs to which bromopride was administered in i.v. or i.m. doses of 10, 20 and 30 mg. The results of the studies were as follows: 1. Bromopride causes an increase in both the number and amplitude of the spikes; at the same time there is a significant increase in the number and amplitude of the intestinal contractions. 2. The average duration of the cycle of the myoelectric complex does not change. This indicates that bromopride has no effect on the normal cycle of motoric activity in the intestine. 3. Bromopride seems more effective in the fasted state than in the post-prandial phase. 4. The effect of i.v. infection is greater than that of i.m. injection and begins earlier.

[Comparison of the variations in the levels of beta2-microglobulin and carcino-embryonic antigen in the breast cancer and malignant melanoma (author's transl)].

A variety of markers have been used in the surveillance of carcinoma of the breast and malignant melanoma, including carcino-embryonic antigen (CEA) beta2-microglobulin (beta2m) and prolactin. The dual purpose of the surveillance was the detection of early recurrence or metastasis and the monitoring of the treatment. In cancer of the breast 92% of patients having bone metastases have elevated levels of CEA or beta2m. In malignant melanoma 2/3 of the patients in relapse have elevated beta2m levels.

[Klinefelter's syndrome in 19 year old adolescents. (100 cases detected during selection for National Service)].

An analysis of 100 cases detected at the age of 19 years during selection for National Service. A somatic, genetic, psychological and hormonal profile emerges from this homogeneous sample. A study of olfactory function and insulin secretion was made. Testosterone deficiency was moderate, DHT deficiency being much more marked and insensitive to stimulation, suggestive of a 5 alpha-reductase defect. Impairment of oestrogenic function of the testis was demonstrated.

[Plasma estorgens and testosterone in the normal young girl before and during puberty].

Base levels of estrone (E1), estradiol-17-beta (E2) and testosterone (T) were measured by radio-immunoassay. Results were classified according to the stages of puberty (Tanner scale) in 89 normal girls ranging from 1 to 22 years of age. Fourty-three pathological cases were studied. In 22 cases of ovarian agenesis E2 and T were most often low. In 7 cases of isolated breast prococity, 4 cases of isolated pilous precocity, one case of isolated genital hemorrhage and 5 cases of hypopituitarism, E1, E2 and T levels were similar to those of impuberal girls. Levels of oestrogens were increased in 4 cases of true precocious puberty.
