[Effects of perindopril on left ventricular hypertrophy, coronary reserve and mechanical properties of the papillary muscle of the rat with renovascular arterial hypertension].

We investigate the effect of a new angiotensin-converting enzyme inhibitor: Perindopril (IRIS) on regression of left ventricular hypertrophy (LVH), coronary blood flow and mechanical performance of isolated papillary muscle in renovascular hypertensive (Goldblatt 2 kidneys-1 clip) Sprague-Dawley male rats. Sham operated rats (G1) and half of hypertensive rats (G2) were studied after 8 weeks. The other half of 8 weeks long hypertensive rats (G3) were treated during 8 weeks with Perindopril in drinking water at a dosage adjusted to maintain blood pressure (BP) measured with tail cuff method under 140 mmHg. The study of each rat included 1) coronary blood flow and resistance measurements under resting conditions and after coronary dilation by carbochrome infusion (9 mg/kg) using left atrial injection of radioactive microspheres (method of Wicker and Tarazi) 2) the study of mechanical performance of the isolated papillary muscle 3) weight of left ventricle after separation of septum and free wall whose subendocardial and subepicardial layers were counted separately. Results (mean +/- SD): (table; see text) MAP: mean pressure. LV/BW: left ventricular mass (mg) per gram of body weight; CR (C): minimal coronary resistance after carbochrome (mmHg/ml/min/100 mg); DL/Dt: peak velocity of shortening at L max preload; Vrelax: peak velocity of relaxation; THR: time of half relaxation; p less than 0.05; p less than 0.01 compared to SHAM. In this model, hypertension induced a 50 p. 100 LVH whose regression was nearly complete after 8 weeks of treatment with Perindopril. Minimal coronary resistance after carbochrome were higher in hypertensive rats compared to sham and return to normal after regression of LVH.(ABSTRACT TRUNCATED AT 250 WORDS)

[Double-contrast colonic radiography. Comparative, randomized study of 4 colon preparations].

Since diagnostic value of double contrast colon imaging is dependent on the colon preparation used, tolerance and efficacy of 4 colon preparations were evaluated by a simple-blind, comparative, randomized study in 185 outpatients. Colon preparation involves variable combinations of 3 means: residue-free diet, laxatives and wash-out enemas. Principal criteria were evaluated under blind conditions by the same investigator during all examinations, with quantitative rating by segment (left, transverse and right colon) of adherence and colon cleanliness, and global assessment of quality of preparation. The 4 groups were comparable: mean age 44.7 years, sex ratio (male/female) 51.3%. Tolerance and compliance were elevated without significant inter-group differences. The colon preparations studied did not allow a homogeneous preparation to be obtained throughout the colon, efficacy in left colon being superior to right colon, whatever the method used. Analysis of cleanliness and adherence by segment failed to show significant differences between procedures, probably related to weak differences in their effectiveness and the strict nature of the study. The use of a global criterion tended to show value of wash-out enemas as a routine procedure, whether in outpatients or in the Radiology department.

[X-ray computed tomographic aspect of gastric leiomyoma. Apropos of a case].

This report studies the diagnostic problem raised by benign pedunculated gastric leiomyoma identified by CT scan imaging and associated with hepatic metastases. Evaluation of the angle of attachment to the wall, the conventional feature enabling distinction between malignant and benign submucous tumours is not always a reliable indicator.

[Post-stress nonlithiasic acute cholecystitis. Contribution of ultrasonics to the diagnosis and treatment in 50 cases].

Fifty cases of post-stress acute acalculous cholecystitis were observed during the past 9 years, mainly after major surgery or trauma. The apparently increasing incidence over the last 4 years (42 cases) could probably be explained by a better diagnostic approach of this condition by routine use of ultrasonography. No specific etiological factor could be found; however total parenteral nutrition and/or sepsis and/or use of narcotics could possibly play a role in the appearance of this complication. Although diagnosis can occasionally be suspected in the basis of abdominal and infectious signs, diagnosis was made primarily on the following ultrasonographic signs: enlarged gallbladder with thickened wall, sludge, and occasionally a double-wall aspect and a pericholecystic collection. In this series, most of the patients were treated by cholecystectomy, but a new therapeutic approach was used in 10 cases: percutaneous transhepatic drainage under sonographic control. Outcome is still poor, with a 50 p. 100 mortality rate.

[Small intestine transit anomalies in a patient with chronic abdominal pain and treated for Hansen's disease].

This new case of enteropathy due to Clofazimine provided demonstration of the diagnostic value of small bowel follow-through examination in this complication. Various small intestine anomalies have been reported: ileal or partial jejunal lesions; variations in caliber; loss of physiologic folds; irregularly outlined surface and border lacunae and persistence of suppleness of pathologic loops. Knowledge of these anomalies should avoid exploratory laparotomy, Clofazimine-induced enteropathy regressing after discontinuation of treatment. Repeat small bowel follow up examinations can be performed to review course of this regression.

[Carcinosarcoma of the gallbladder. Echographic and anatomopathological diagnosis. Apropos of a case with review of the literature].

Precise diagnosis of carcinosarcoma of gallbladder, a rare tumor, is based on histopathology since clinical and epidemiologic elements fail to differentiate it from other gallbladder cancers. Ultrasound imaging is the most effective radiodiagnostic technique since it allows visualization of the tumor and suggests the presence of a carcinosarcoma when a polypoid or vegetating mass is detected within the gallbladder.

[Leiomyosarcomas of the inferior vena cava].

Leiomyosarcoma of the inferior vena cava in a rare tumor which is predominantly seen in women. The symptomatology is non specific and depends mostly of the segment of the vessel which is involved. Radiologic approach for correct preoperative diagnosis is based on ultrasonography, computed tomography and inferior vena cavography. The best treatment is en bloc resection of the mass associated with pre and post-operative chemotherapy. Although the tumor is slow-growing, the prognosis is poor and the average survival is less than two years, due to local recurrence and metastases.

Contribution of modern imaging in rectosigmoid neoplastic disease (comparison with fiberoptic endoscopy).

Double-contrast enema (DCE) remains the primary diagnostic exploration. Its specific method and interpretation require skillful handling. Unlike colonoscopy, DCE results in false-positives, the frequency of which varies widely in function of the skill of the interpretation. False negatives reach a significant level from a threshold of size which can be fixed at 5 mm. Despite these drawbacks, double-contrast enema, which is an exploration involving a two level of invasiveness and which provides a good panoramic image of the colon should remain the primary exploration in routine check-ups. DCE is an essential part of loco-regional evaluation. It is important in post-operative check-ups to screen for relapses. Post-operative follow-up should be systematic, involving an initial reference examination after 4 months, a check-up every 6 months for two years and then an annual check-up.

Sulmazole effects on mechanical performance of hypoxic and reoxygenated isolated mammalian myocardium.

The present study intended to investigate the effects of 2-[2-methoxy-4-(methylsulfinyl)phenyl]-3H-imidazo[4,5-b]pyridine (sulmazole, AR-L 115 BS) on mechanical performance of rat myocardial fibers under hypoxia, and subsequent reoxygenation. Because of a possible caffeine-like mechanism of action of sulmazole similar experiments were performed with theophylline and the results compared. 1. Under normoxic conditions, sulmazole and theophylline displayed a positive inotropic action with acceleration of relaxation at small concentrations (sulmazole: 6.4 mumol l-1; theophylline 7.35 mumol l-1). Increasing doses led to a negative inotropic effect with slackened relaxation and loss of its load sensitivity (up to 390 mumol l-1 for sulmazole; up to 350 mumol-1 for theophylline). In agreement with other reports, the dose-effect curve was shifted towards low concentrations. 2. During ischemic crisis, therapeutic doses of sulmazole (6 mumol l-1) and theophylline (7 mumol l-1) induced no additional depression of contractility and protected relaxation. During subsequent reoxygenation, better recovery of contractility was observed without any very significant improvement of relaxation. 3. During reoxygenation beyond 120 mumol l-1, increasing doses of sulmazole depressed contractility. An additional depressive effect on mechanical performance was observed during hypoxia only for the highest dose (390 mumol-1).

[Is it possible to predict the cleanliness of the colon before double-contrast enema? Study of the image of the abdomen without preparation, and the number of liquid stools].

Correlations between evaluations by two observers of cleanliness of colon on straight abdominal images (ASP) and on barium enema (LDC) results were studied, as well as predictions of cleanliness of LDC by ASP. Correlation between observers was practically nul for ASP and was poor for LDC. There is lack of correlation between cleanliness of colon on ASP and cleanliness of LDC. It is concluded that no purpose is served in performing ASP before LDC in order to predict cleanliness and that the definition of cleanliness itself requires updating. In contrast, however, the presence of less than three liquid stools appears to be related to diminished cleanliness (p less than 0.05).

[Hepatic metastases. Value of chemotherapy by the intra-arterial route].

Forty-three patients with metastatic liver cancer were treated with multiple chemotherapy by the intra-arterial route. Metastases originated from primary cancer of the breast (16) or colon (10), melanoma (7) and miscellaneous tumours (10). In 35 patients chemotherapy was administered by selective catheterization of the hepatic artery via the axillary artery; it usually (31 cases) consisted of doxorubicin (60 mg/m2) and mitomycin (10 mg/m2) injections, and continuous infusion of 5-fluorouracil (1 g/24 or 72 hours). In 14 patients (10 responders to the above method and 4 new cases), a catheter with subcutaneous chamber was implanted surgically so that chemotherapy could be continued through the chamber. Blood toxicity was usually moderate. The main complication of injections through the catheter (114) or through the chamber (60) was thrombosis. However, except for 1 lethal cerebral thrombosis, the others (axillary artery 3, hepatic artery 5) were unattended by functional symptoms. Transient biochemical signs of hepatic cytolysis were frequent after each course. Hepatic insufficiency was severely aggravated in 2 cases. Painful digestive disorders were relieved by symptomatic treatments. Four complete responses, 16 partial responses, 8 stabilizations and 10 failures were observed; 5 patients died soon after one single course of intra-arterial therapy. The high response rates (greater than 50%) in metastases from cancer of the breast and colon, and chiefly the 3 complete responses obtained in patients with mammary carcinoma were most encouraging, bearing in mind that one of these 3 patients had not responded to intravenous chemotherapy.

[Ischemic accidents of the lower limbs with preservation of the peripheral pulse after arteriography (3 cases )].

Marked ischemia of the lower limbs and abdomen, with conservation of peripheral pulses, was noted several hours after arteriography in 3 patients. The origin of the lesion was due to migration of microemboli (cholesterol crystals) in one patient, but was conjectural in the other two cases. Possible etiologies include a chemotoxic effect of the contrast medium used, as well as the action of other factors (sterilization of materials), and the arteriographic technique applied (reflux technique in two cases).

[The effect of the injection of water-soluble triiodized monomer contrast media for intravenous urography on glomerular filtration, uricemia and urinary elimination of uric acid in normal subjects].

One mechanism suggested as being the cause of nephrotoxicity following the administration of triiodized contrast media is intratubular urate precipitation. Thirty healthy volunteers were investigated by IVU using three different media. The iodized injections (0,56 g/kg) failed to produce significant modifications of glomerular filtration and were followed by only very moderate variations in blood and urine uric acid levels.

[Malabsorption and complex superior mesenteric vascular anomalies. Apropos of a case].

Radiological, and particularly arteriographic examinations in a 28-year-old man investigated for a malabsorption syndrome with exsudative enteropathy demonstrated the presence of a complex anomaly of mesenteric artery and vein vascularization. The diagnosis, based on these findings, was chronic or recurrent valvulus of the mesentery on an intestinal malposition.

[Value of superior vena cava angiography in Schwartz-Bartter syndrome (author's transl)].

Schwartz-Bartter syndrome often results from lung cancer. With reference to two observations, attention is drawn to the value of opacifying the superior vena cava for demonstrating the tumor. This investigation also allows guided biopsy sampling as exemplified in the second observation. Vascular investigations seem all the more useful as small-cell carcinoma often develops outside the bronchi and are often inaccessible by endoscopy.

[External retromalleolar lymphography of the superficial and deep lymphatics of the lower limb (author's transl)].

External retromalleolar lymphography was employed to explore lower limb lymphatics in 39 patients. Though this type of approach enables deep lymph vessels of the thigh to be studied--at the price of a more difficult catheterization than when the classical approach is used--it is ineffective for demonstrating deep lower leg lymph vessels, filling occurring only in the superficial network. Its indications are therefore limited to cases following unsuccessful attempts with the classical approach.
