There are two C4 genetic loci and a null allele in the chimpanzee.

Genetic polymorphism in C4 in the chimpanzee was studied by agarose gel electrophoresis of desialated plasma and development of patterns by immunofixation with antiserum to human C4 and by a C4-sensitive hemolytic overlay. In general, immunofixation patterns showed multiple partially overlapping bands of which only the most cathodal had strong hemolytic activity. In analogy to human C4, the latter were designated C4B, whereas those detected by immunofixation which had little hemolytic activity were designated C4A. Chimp C4A and C4B reacted with human and mouse (monoclonal) anti-C4B and human anti-Ch1 but neither reacted with monoclonal anti-C4A or human anti-Ch2, Ch3, Rg1, or Rg2. On sodium dodecyl sulfate polyacrylamide gel electrophoresis, the alpha chain of C4B showed a slightly lower apparent relative mass than that of C4A at around Mr 93,000. There were three C4A variants and two C4B variants inherited in families as autosomal codominant traits, as C4A-C4B cosegregating pairs with no detectable crossing-over. These pairs were inherited with chimpanzee leukocyte antigen types C2 and BF variants without detectable crossing-over. Half-null C4 haplotypes with C4B QO were observed in family studies. Nine BF, C2, C4A, C4B allelic haplotypic combinations (complotypes) were identified among presumably unrelated chimpanzees.

HLA antigens associated to amoebic abscess of the liver in Mexican mestizos.

Worldwide prevalence of amoebiasis is estimated at 4 x 10(8) cases/year, yet only one of about 300 individuals harbouring Entamoeba histolytica suffers tissue invasion and these cases are mostly concentrated in certain areas of Asia, Africa and Latin America. Patients with amoebic abscess of the liver (AAL) represent only a small fraction of that. These contrasting figures have been tentatively explained on the one hand through variations in sex, immunocompetence, nutritional and other socioeconomic features of the host, and on the other hand through differences in parasite virulence. In order to explore a possible association between the major histocompatibility complex (MHC) and AAL susceptibility, we studied the HLA profile in 31 Mexican mestizos with AAL and compared it to race and socioeconomically matched controls. Mexican mestizo patients with AAL revealed a significant increase in HLA-Bw16 and HLA-DR3 which could suggest an HLA-related susceptibility to liver invasion by E. histolytica.

Assessment of Björk-Shiley and porcine mitral valve dysfunction by m-mode echocardiographic left ventricular dimension.

In this study of 46 patients with mitral valve prostheses, we report on the accuracy of the left ventricular maximum filling velocity (LVMFV) index as a diagnostic tool for valve dysfunction. Echocardiographic left ventricular internal dimensions were measured every 0.05 seconds, and every measure was transferred to an axis system. The tangent to this curve was traced at the point of maximum rate of increase of dimension, and the slope in mm/sec represents the LVMFV index. Six patients, each with an obstructed mitral valve prosthesis, had a reduced LVMFV index (p < 0.001). On the other hand, eight patients with a paravalvular leak showed a LVMFV index significantly higher (p < 0.05) than patients with a normally functioning mitral valve prosthesis. The results of this study suggest that analysis of the LVMFV index can be a useful and reliable substitute for other noninvasive diagnostic techniques in evaluating certain patients with suspected prosthetic mitral valve malfunction.

Conservative operation for mitral stenosis with densely fibrosed or partially calcified valves. An eight-year evaluation.

Of 284 patients undergoing open mitral commissurotomy for mitral stenosis from January 1978 to December 1985, 81 patients had a densely scarred or partly calcified valve. In this study, we evaluated the postoperative results in these 81 patients. There were no operative or late deaths. Seventy-seven patients (95%) are in Functional Class I or II. Three patients required reoperation 3, 30, and 50 months after the initial procedure because of moderate or severe residual mitral regurgitation. After valvotomy there were four episodes of embolism. The actuarial rate of freedom from any complication (mortality, reoperation, congestive heart failure, and thromboembolic events) was 89.3% +/- 3.9% (mean +/- standard error of the mean) 8 years after operation. We conclude that for a follow-up period of 8 years, the stenotic mitral valve with anatomical deformities can be salvaged with satisfactory results. Therefore, we believe that this approach, at present, is a better alternative than prosthetic replacement with any type of valve presently available.

Familial occurrence of big-big prolactin as the predominant immunoreactive human prolactin species in blood.

The occurrence of big-big prolactin (PRL) as the major form of circulating immunoactive PRL has been described in ovulatory hyperprolactinemia. In this study, we have analyzed the heterogeneity of circulating human PRL in 17 members of three families in which one member, bearing galactorrhea-hyperprolactinemic syndrome, was known to have serum big-big PRL as the predominant circulating species. Gel filtration patterns of serum samples of all subjects revealed the presence of significant proportions of big-big PRL in more than one member of the same family, thus suggesting the familial occurrence of big-big as the predominant immunoreactive PRL species in blood.

Gene frequencies and admixture estimates in a Mexico City population.

Five hundred and ten students of the Universidad Nacional Autónoma de México were tested to determine the distribution of ABO, MN, Rr-Hr blood groups, and serum haptoglobin, albumin, and Factor Bf types. Based on the results we found that the proportion of Indian and White genes are of 56.16 and 43.84%, respectively in the dihybrid model and 2.93, 56.22, and 40.85% for Blacks, Indians, and Whites in the trihybrid one. The present study reveals a higher proportion of Indian genes in the Mexico City population than estimated in previous publications. Reasons why the present results apply to a much larger group of Mexico City mestizos than the previous ones are given.

Immunogenetic profile of multiple sclerosis in Mexicans.

This is the first study on genetic markers in Mexican Mestizos with multiple sclerosis (MS). Patients were born in Mexico, had no family history of MS are middle-class, and have a high-level education. HLA class I, class II determinants, C2, C4, BF, GLO-1, ABO, and Rb red cell systems were analyzed and compared with results of 295 controls. Measles antibodies, glucose, IgG, total proteins, and cell count were measured in cerebrospinal fluid; anti-neuron, T-cell, and B-cell antibodies were determined in serum. MS in Mexican Mestizos was clinically similar to MS reported in high prevalence countries. DRw6, as previously found in Japanese patients, and its subtype, DRw13, were increased in patients in our study (pc = 0.0007, pc = 0.01, respectively), and the combination A3, B7, DR2, was also elevated (pc = 0.003). The polygenicity of the disease is emphasized by the excess of AB group carriers (pc = 0.01). IgG levels were high in patients with DR2 or DRw6 and 67% of the latter had anti-T cell antibodies. Severity of the disease was also related to the DR markers. It is suggested that at least two HLA-DR linked genes and the industrialized environment are important for the expression of MS in Mestizos.

Progressive cauda equina syndrome and extensive calification/ossification of the lumbosacral meninges.

A patient with longstanding ankylosing spondylitis (AS) developed a cauda equina syndrome. The myelogram showed a block at the L2 level. Vertebral computerised tomography showed calcification in the centre of the spinal canal. The patient also had features suggestive of a diffuse idiopathic skeletal hyperostosis (DISH). Meningeal calcification has never been reported in AS, so we suggest that this is related to an associated DISH. Cauda equina syndrome has not been described in DISH, and calcification of meninges has not been reported in AS, so we suggest that the meningeal calcifications and associated cauda equina syndrome are related to DISH.

Increase of unidentified HLA antigens in pulmonary tuberculosis.

We studied the HLA-A, B,C and DR antigens in 50 patients with advanced pulmonary tuberculosis in an effort to detect some genetic factors which may predispose to the development of this disease. We found a decrease in HLA-A3, A9, B5, B8, B13, B17 and B27. The incidence of "double blank" antigens in the locus A was 18% (2.5% in controls, p less than 0.0005) in the locus B 16% (1.5% in controls, p less than 0.0005) and 28% in the DR locus (7% in controls, p less than 0.0005). Our results suggest that the cells responsible for the immune response in patients with tuberculosis appear to have a diminished number of HLA antigens serologically determined. That phenomenon could be explained by several hypothesis like poor expression of these proteins or masking for another substance. In spite of the mechanism, this abnormal membrane expression of HLA antigens could be important for the development of the disease since the cell-cell interactions critical for the mounting of an adequate immune response might not be appropriate.

Intraoperative contrast two-dimensional echocardiography. Evaluation of the presence and severity of aortic and mitral regurgitation during cardiac operations.

We have used contrast two-dimensional echocardiography in the intraoperative evaluation of aortic and mitral regurgitation in 35 patients undergoing cardiac operations. All of them underwent previous cardiac catheterization in order to document the presence and severity of regurgitation. With the pericardium open, a catheter was introduced into the left ventricle (to document mitral regurgitation) or into the ascending aorta (to document aortic regurgitation). The two-dimensional echocardiographic probe was placed on the anterior surface of the right ventricle to obtain a basal image, equivalent to a conventional parasternal longitudinal view. Dextrose in water (5 ml) was rapidly hand-injected through the catheter, while echocardiograms were recorded on videotape. The observation of contrast medium (microbubbles) flowing in the retrograde direction through the incompetent valve was carefully evaluated with the same scoring system used in the hemodynamic laboratory. In 34 cases there was agreement between angiographic and echocardiographic evaluation of the presence and severity of mitral and aortic regurgitation. Only one case was evaluated as mild aortic regurgitation by angiography and moderate aortic regurgitation by echocardiography. There were no false positives or false negatives in the study. In view of the high degree of correlation between contrast two-dimensional echocardiography and hemodynamic data, we suggest that our method is an important tool for the cardiac surgeon. In addition, the present approach overcame the disadvantages of the conventional intraoperative methods, most of which are performed in a nonbeating or fibrillating heart.

Genetic polymorphism of the sixth component (C6) of rat complement.

The complement protein C6 has been shown to be genetically polymorphic in the rat. Isoelectric focusing of plasma samples from 19 inbred strains demonstrated two electrophoretically distinguishable migration patterns, each consisting of three bands. Breeding studies with the use of the BN and DA strains showed that the C6 patterns were inherited in a manner consistent with the co-dominant autosomal expression of two alleles (C6 A and C6 B). The distribution of the C6 alleles in a backcross mating was compared with eight independently segregating marker genes: RT1.A, RT2, Gdc -1, Igk-1, Hbb, Svp-1, Fh-1, and Es-6. There was no detectable linkage between C6 and any of these eight loci.

Human C4 haplotypes with duplicated C4A or C4B.

In the course of study of families for the sixth chromosome markers HLA-A, C, B, D/DR, BF, and C2, the two loci for C4, C4A, and C4B, and glyoxalase I, we encountered five examples of probable duplication of one or the other of the two loci for C4. In one of these, both parents and one sib expressed two different structural genes for C4B, one sib expressed one, and one sib expressed none, suggesting that two C4B alleles were carried on a single haplotype: HLA-A2, B7, DR3, BFS1, C2C, C4A2, C4B1, C4B2, GLO1. In a second case, two siblings inherited C4B*1 and C4B*2 from one parent and C4B*Q0 from the other. This duplication appeared on the chromosome as HLA-AW33, B14, DR1, BFS, C2C, C4A2, C4B1, C4B2, GLO2. In a third, very large family with 3 generations, a duplication of the C4B locus occurred which was followed in 2 generations. In one individual, there were three C4B alleles and two C4A alleles. One of the C4B alleles had a hemolytically active product with electrophoretic mobility near C4B2 and was designated C4B*22. It segregated with C4B1 in the family studied. The complete haplotype was HLA-A11, CW1, BW56, DR5, BFS, C2C, C4A3, C4B22, C4B1, GLO2. In another family with 12 siblings, one parent and eight children expressed two C4A alleles on the haplotype HLA-AW30, BW38, DR1, BFF, C2C, C4A3, C4A2, C4BQ0, GLO1.(ABSTRACT TRUNCATED AT 250 WORDS)

Extended MHC haplotypes in 21-hydroxylase-deficiency congenital adrenal hyperplasia: shared genotypes in unrelated patients.

HLA, complement, and glyoxalase I alleles were studied in 29 families in which at least one member has classical 21-hydroxylase-deficiency congenital adrenal hyperplasia. A rare complement allele, C4B*31, was found in over 20% of the haplotypes defined in these families and was always part of the complement haplotype BF*F, C2*C, C4A*Q0, C4B*31 (abbreviated FCO,31). The haplotype containing this rare set of complement alleles always carried the rare HLA allele, HLA-Bw47, usually carried HLA-A3, and almost always had the alleles HLA-Cw6, HLA-DR7, and the glyoxalase I (GLO) allele GLO1. Thus over 20% of the haplotypes in the population studied contained all or almost all of the rare extended haplotype HLA-(A3), Bw47, Cw6,DR7, FCO,31, GLO 1. 3 other haplotypes were each found twice in unrelated patients concordant for their disease phenotype and ethnic background. Extended MHC haplotypes may be markers for different genetic mutations causing 21-hydroxylase deficiency.

[Our experience in heart surgery in early childhood (children less than 2 years old)].

We review 109 patients with congenital heart disease under (two year of life, surgically treated between June 1978 and October 1981; 44% were under six months and 71% were under one year. Fifty six patients were operated under cardio-pulmonary by-pass, in 41 we used deep hypothermia with total cardio-respiratory arrest, 21% of them were under three months of life an 53% under one year. We present the morbidity and mortality together with the pulmonary management. Eighty five percent of the cases remained intubated postoperative by for less than 12 hours. The postoperative pulmonary evolution was quite similar for different types of congenital heart disease, such as T. Fallot, A-V canal, transposition, etc., and there was no statistically significant difference between them in terms of respiratory assistance and pulmonary complications. Eleven patients died (19%), all of them under six months of life. In 53 patients operated, without extracorporeal circulation, 38 had total correction and 15 palliative procedures. The mortality in this group was 9% (5 cases), all of the under one month of life. The global mortality was 14%. We discuss our present surgical indications in view of our results.

A prospective randomized controlled trial of cefoxitin versus clindamycin-aminoglycoside in mixed anaerobic-aerobic infections.

Ninety patients infected with presumed penicillin resistant anaerobes were randomized to cefoxitin or clindamycin-aminoglycoside. Cefoxitin was comparable to clindamycin-aminoglycoside in cures of intestinal associated, 16 of 26 versus 11 of 21, and pelvic infections, 20 of 20 versus 22 of 23. Cefoxitin-resistant facultative-aerobic gram-negative rods were found in 16 of 45 patients with intestine associated infection. Probable antibiotic associated nephrotoxicity was less frequent in the patients in the cefoxitin group, zero of 46 versus seven of 44, p less than 0.05, although a false creatinine elevation was noted more frequent, seven of 46 versus one of 44, p less than 0.05. Infections causing failure in patients in the cefoxitin group more frequently contained cefoxitin resistant gram-negative rods at the time of failure than did infections causing failure in those in the clindamycin-aminoglycoside group that contained gentamicin-resistant gram-negative rods, eight of eight versus zero of eight, p less than 0.001. Cefoxitin may be adequate therapy for many patients with mixed anaerobic/aerobic infections; however, the addition of an aminoglycoside may be prudent in those with known, or suspected, cefoxitin resistant gram-negative rods.
