[History of spermatozoon and changing views].

With Ham, Leeuwenhoek discovered animalcules in human semen in 1677, without theoretical interpretations. Discussions focused on the respective role of ovum and animalcules during two centuries with erroneous doctrines. Modern ideas on status and origin of animalcules are associated with the development of the cell theory. Animalcules became spermatozoa. In 1875, Hertwig observed that the head of spermatozoon becomes a pronucleus and combines with the female pronucleus, thus establishing the concept that fertilization is the conjugation of two cells. During the first half of 20th century, endocrinology and genetics influenced management of infertile couples. Cryopreservation, analyses of ultrastructural morphology improved knowledges of normal and abnormal male gametes. In Vitro Fertilization and more IntraCytoplasmic Sperm Injection opened new views on the role of spermatozoa in human generation. Genetics and procreation were more and more so linked that each perspective of further advances shocked ethical considerations as transgression of natural biology. The future of spermatozoon in human reproductive programs remains a mystery according to, for example, experimental development using stem cell cultures.

[History of the birth and development of the Ecole de médecine de Hanoï].

Between 1898 and 1900, France sent a mission to Indochina conducted by Jeanselme in order to inquire about the possible actions to protect people from infectious diseases especially plague. In the conclusions of the final report appeared the necessity to create a medical school for and with local people. Paul Doumer, governor general of Indochina, received the order from the French government to found at Hanoi such a structure with an hospital including managing staff purposing to cure all kinds of pathology principally tropical diseases. Yersin, Pasteur's disciple, was asked as its director. Several civil and military physicians formed with him a very efficient teaching group and first courses began in 1902 with thirteen french-speaking students. At the end of this year successful results were obtained. This medical school structured as a french pattern of a medical teaching hospital obtained a full legal status after many years. Galliard and Huard played a major role in its development by their rare qualities of mind. Scientific works were impressive in parasitology, anatomy and surgery. Ton-That Tung (1912-1982) was the most famous among Huard's several students. He became a great surgeon of his time, specialized in hepatology. The medical school of Hanoi illustrated a model of a well balanced development between traditional medicine strictly preserved and the resource of european medicine. It also constituted an exceptional example of possible cooperation between two peoples so different in culture.

Protein composition of follicular fluid and oocyte cleavage occurrence in in vitro fertilization (IVF).

The present study was carried out from in vitro fertilization (IVF) attempts to analyze further the total and specific protein contents of 47 follicular fluids yielding one oocyte. The aim was to find correlations between the follicular concentrations of such proteins and the occurrence of coupled oocyte cleavage. These findings would be used as markers of IVF outcome. Two groups of follicular samples were distinguished: one group with cleavage occurrence (25 cases) and another group without cleavage or even fertilization (22 cases). In the group with cleavage a significantly higher level was observed for six proteins: C3 complement fraction and ceruleoplasmin (P less than 0.02), alpha-antitrypsin and transferrin (P less than 0.01), and alpha 2-macroglobulin and beta 2-microglobulin (P less than 0.001). The data are discussed with respect to changes in follicle permeability with advancing maturity.

[Celioscopy with peridural anesthesia. Techniques, indications, results in 220 cases].

The authors used epidural anaesthesia to carry out laparoscopy in 220 patients. The chief indications for the laparoscopies were GIFT (intratubal transfer of gametes) and tubal sterilization. The technique used was slightly different according to the indications for the use but they had to be sure of anaesthetising up to T4. In 90% of cases the patients tolerated the procedure well. No change in ventilation or in metabolic measurements. As far as fertilization was concerned studies carried out in various parameters failed to show any untoward side effects due to the use of local anaesthetics. Finally so long as the anaesthesia is only used for a short length of time this technique seems to be suitable for day cases.

Influence of the quality of the cervical mucus on sperm penetration: comparison of the morphologic features of spermatozoa in 101 postcoital tests with those in the semen of the husband.

The purpose of this study was to determine the frequency of abnormal spermatozoa in mucus by a postcoital test and to compare the findings with the teratospermia in a control analysis of the husband's semen. These data obtained from 101 hypofertile couples were correlated with the quality of mucus, which was classified into three categories according to rigorous scoring procedures. The morphologic features of spermatozoa were evaluated by the same investigator and classified on the basis of a system that allowed for multiple entries. This method provided an exact description when several anomalies were identified on the same spermatozoon. By this approach, spermofiltration of the mucus was determined both qualitatively and quantitatively. The proportion of abnormal spermatozoa decreased in the cervical mucus, compared with semen, when the mucus was abundant and of normal quality (32.3% +/- 8.7% in mucus, versus 55.7% +/- 13.9% in semen). The elimination of tail anomalies depended upon the quality of the cervical secretion. A normal mucus eliminated spermatozoa with abnormal heads only if they were associated with a tail anomaly.

[Anatomical measurements during fetal growth of the kidney. Its value for the ultrasonographer and the anatomo-pathologist (author's transl)].

The rate of growth of the kidneys was analysed by examining 390 anatomical specimens that had been taken from premature babies or stillbirths at term, or had been carefully selected. The gestational ages ranged from 26 to 41 weeks of amenorrhoea. Statistical analysis of the results has led to the ability to establish significant correlations between the length of the fetus and its weight and relationship to the growth in weight, and throughout the three dimensions of fetal kidneys. Some measurements have been made on 8 fetuses between 14 and 24 weeks. The comparison of the numerical figures that have been obtained with the histological appearances has made it possible to work out the steps of the development of the kidney in utero. This organ, even in the case of a small-for-dates fetus, is good evidence of the gestational age of the fetus. By collecting all these observations together they could contribute to a better appreciation of the growth of the kidney viewed by ultrasound and to quantifying better the pathological abnormalities that can occur during its development.

Human seminal fibrinolytic activity: specific determinations of tissue plasminogen activator and urokinase.

The determination of total fibrinolytic activity of ejaculates was realized by fibrin plate method. For the same seminal samples, tissue plasminogen activator (t-PA), urinary plasminogen activator (uPA) antigens and uPA activity were specifically quantified. The seminal values were fifty times higher than in the blood for t-PA and fifteen times for uPA. There was no correlation between the both levels but from split ejaculates measurements a higher concentration was observed in all first fractions. By zymography assays, it was shown that seminal plasminogen activators are under active forms. The lack of proUrokinase in semen was also demonstrated.
