Osteochondroma post osteomyelitis.

We report a case of radial metaphyseal osteochondroma secondary to osteomyelitis in a premature baby of 28 weeks. This is the second case with these characteristics that has been described in the literature. A possible pathogenesis is related to trauma during drainage of the abscess or following the associated inflammatory process.

International participation in major intensive care journals. "The smaller the better".

OBJECTIVE: This study seeks to assess the scientific contributions in the field of intensive care medicine from each of the major countries (per million inhabitants) and discusses the possible factors which may contribute to any differences. DATA SOURCE: Review of scientific journals. STUDY SELECTION: We examined the total number of published papers (between 1989 and 1993) in five major respiratory and intensive care journals: Intensive Care Medicine, Critical Care Medicine, Chest, The American Review of Respiratory Disease, and Circulatory Shock. DATA SYNTHESIS: Amongst the American journals, USA and Canada were the predominant contributors followed by the other countries in the following order: Switzerland, Sweden, Belgium, the Netherlands, Finland, Austria, Denmark, UK, France, Spain, Italy, Norway, Japan and Germany. CONCLUSIONS: The present study reveals that the smaller European nations have a greater participation than the larger European nations in the major intensive care journals. This phenomenon could be due to different submission practices in these countries compared with the larger European countries, where a considerable number of papers are submitted to local renowned journals. There was no absolute correlation between a nation's gross national product and the total number of publications.

The effect of antibiotics on nitrification processes. Batch assays.

The effect of different antibiotics at several concentrations of ampicillin (0-250 mg/L), benzylpenicillin (0-250 mg/L), novobiocine (0-150 mg/L), oxytetracycline (0-250 mg/L), and chloramphenicol (0-50 mg/L) on a stabilized nitrifying sludge was evaluated under aerated and lithoautotrophic conditions. No effect resulting from the presence of antibiotics on the biomass and nitrate production was noticed. The specific growth rate and volumetric nitrification rate average values for the controls were 8.28 x 10-3/h-1 and 2.74 x 10-3 g/L.h, respectively. Similar rate values were found when different kinds of antibiotic and concentrations were tested. These results may be explained by the nature of the floc or the instability of the antibiotics.

Plasticity of astrocytes derived from aged mouse cerebral hemispheres: changes with cell passage and immortalization.

This study was targeted at the beginning to understand the functional status of glial cells derived from aged brain. We have previously characterized passaged cell cultures derived from aged mouse cerebral hemispheres (MACH) and found them to contain large populations of astrocytes, type 1, as well as limited numbers of astrocytes, type 2, oligodendrocytes, and progenitor cells. Using the activity of the astrocyte marker, glutamine synthetase (GS), as an index, we found that MACH astrocytes continue to respond to several microenvironmental signals, including the cAMP-enhancing agents dibutyryl cAMP and R020-1724 (an inhibitor of phosphodiesterase). In addition, whereas the basal activity of GS increased with cell passage, their response to these agents was cell-passage dependent, increasing at early (21-22) passages and decreasing at later (46-51) passages. Because neurotrophins (i.e., NGF and EGF) also provide microenvironmental signals essential to normal glial function, MACH cultures were assessed for their response to these factors. MACH cultures at passage 35 responded to treatment with NGF and EGF with a dose-dependent increase in GS activity by both neurotrophins. With the intention of arresting these cultures at a specific stage of differentiation, these cells were immortalized at passage 19 by transfection with the gene encoding SV40 Large T antigen. These immortalized MACH responded to exposure to dBcAMP and RO20-1724 with a marked decrease in GS activity, mimicking the response of normal MACH glia at late passage. Finally, because it has been shown that glia from both immature and adult brain contain neurotrophins and respond to neurotrophins via a receptor-mediated pathway, we examined expression of NGF protein as well as NGF (p75) and EGF receptor protein in various passages and colonies of normal and immortalized MACH cultures. We found a consistent expression of all three proteins in the various cell populations. Results of this study suggest that astrocytes from aging brain continue to function normally with respect to several parameters (i.e., response to neurotrophins and differentiating agents). Thus, they retain their plasticity to a great degree through early cell passages. However, with advancing cell passage this plasticity declines and cell homeostasis is impaired. We propose, therefore, that astrocytes undergo several critical periods in their functional lifespan, one of which is represented by the functional transition demonstrated in this study.

Nosocomial outbreak of Burkholderia pickettii infection due to a manufactured intravenous product used in three hospitals.

Forty-six cases of nosocomial infection caused by Burkholderia pickettii were reported between June and November 1993 in three metropolitan hospitals in Madrid. A case-control study of the outbreak was conducted to identify its cause. Seventy-four percent of the patients were males; the mean age +/- SD of the patients was 54 +/- 20 years. Sixty-five percent of the patients presented with some gastrointestinal disorder, and 80% had a peripheral catheter; 98% were treated with intravenous fluids, and 96% were treated with intravenous ranitidine. On the basis of results of a descriptive study and knowledge of the epidemiologic features of B. pickettii, a provisional causal hypothesis was formulated: intravenous ranitidine was the source of the outbreak. As a control measure, it was advised to stop treatment with this drug. On the basis of results of logistic regression and the microbiological isolation of B. pickettii in an ampule of the drug, we concluded that intravenous ranitidine was the cause of the outbreak.

Effect of neoadjuvant endocrine therapy (combined androgen blockade) on normal prostate and prostatic carcinoma. A randomized study.

The morphologic changes induced by neoadjuvant combination endocrine therapy were evaluated in prostatectomy specimens from patients diagnosed with localized prostate cancer. These patients participated in a prospective, randomized clinical trial investigating the effect of 3 months of combination therapy with flutamide and an LHRH agonist prior to radical prostatectomy versus radical prostatectomy alone. Ninety-six radical prostatectomy specimens processed according to the same protocol were evaluated without knowledge of prior treatment. Forty-seven patients were randomly assigned to the neoadjuvant combination therapy group and 49 to the control arm. Compared with the control group, several changes were strongly and significantly associated with exposure to neoadjuvant combination therapy. The nonmalignant prostatic tissue showed strong prominence and hyperplasia of the basal cell layer, accompanied by epithelial cell vacuolization and markedly reduced occurrence of prostatic intraepithelial neoplasia (p < 0.001) after combination therapy. Prostate cancer tissue, on the other hand, showed smaller nucleoli (p < 0.001), cell vacuolization (p < 0.001), rare intraluminal crystalloids (p < 0.001), higher Gleason grade (p < 0.001), lower prevalence of capsular penetration (p < 0.001), and less frequent invasion of the perineural spaces (p < 0.001) and surgical margins (p = 0.002). Tumor volume, was also reduced by more than 40% in the treated group (p = 0.007). The present findings show that preoperative endocrine combination therapy induces highly characteristic changes in both nonmalignant and cancerous prostatic tissue. Furthermore, following endocrine treatment, the surgical margins are less likely to be involved by cancer and capsular penetration is reduced.

Comparison of five plating media for isolation of Salmonella species from human stools.

A comparative study was carried out to evaluate the performances of different culture media for the recovery of Salmonella spp. from 1,000 routine samples of human stools. By direct plating we tested Salmonella-Shigella agar (SS), Hektoen enteric agar (HE), bismuth sulfite agar (BS), novobiocin-brilliant green-glycerol-lactose agar (NBGL) and SM-ID medium (SM), and after selenite enrichment, we tested all of the media except HE. C8-esterase and oxidase tests were used for the screening of Salmonella spp. on SS and HE. The total number of Salmonella isolates from direct culture was 74, with respective sensitivities and positive predictive values (PPVs) of 78.4 and 61%, 64.9 and 18.7%, 36.5 and 34.2%, 55.4 and 20.7%, and 39.2 and 43.9% for NBGL, SS, HE, BS, and SMID, respectively. After enrichment, the total number of Salmonella isolates was 88. The respective sensitivities and PPVs obtained were 90.9 and 62.5%, 92 and 17%, 90.9 and 32% and 93.2 and 71.3% for NBGL, SS, BS, and SM, respectively. According to our results, NBGL in direct plating was the medium with the highest sensitivity with respect to the sensitivities of the other media, with significant statistical differences (P < 0.05). Likewise, the PPV for NBGL was also the highest (61%). After enrichment in selenite broth, the sensitivities of the four media tested were similar, with the best PPV obtained with SM (71.3%); this was followed by NBGL (62.5%). When C8-esterase was used on SS or HE, the PPVs improved from less than 40% to about 100%.

Use of alpha-N,N-bis[carboxymethyl]lysine-modified peroxidase in immunoassays.

Horseradish peroxidase was activated by periodate oxidation of the carbohydrate moiety and then modified by the covalent attachment of alpha-N,N-bis[carboxyethyl]lysine (CM-Lys) by reductive alkylation using sodium cyanoborohydride. The resultant CM-Lys peroxidase was charged with nickel ions and then used as a specific labeling reagent for histidine-tagged recombinant proteins. This labeling method was effective for proteins that are soluble or insoluble in the absence of chaotropic agents. The labeled proteins were very effective in direct sandwich enzyme-linked immunosorbent assay for detecting antibodies against the protein in sera as demonstrated by assays for antibodies to such diverse viral proteins as hepatitis B surface and core proteins, hepatitis C core and helicase protein (NS3), and retroviral core proteins.

Liver accessory lobe torsion in the infant.

A case of vomiting and selective epigastric pain secondary to volvulus of the pedicle of a liver accessory lobe, in an infant, is reported. Ultrasonic findings are useful and probably sufficient for a correct diagnosis.

Changes in the prothrombin time, haematology and serum proteins during experimental aflatoxicosis in hens and broiler chickens.

Two groups of 32 laying hens and two groups of 32, 23-day-old broiler chickens were given 2.5 and 5 mg kg-1 of aflatoxin in their diet for four, eight, 16 and 32 days; 16 hens and 32 broiler chicks served as control groups. In the laying hens, aflatoxicosis was characterised mainly by an increase in prothrombin time, and decreases in total plasma proteins and albumin levels; there were increases in red blood cell (RBC) counts and haematocrit which were greater in the group fed 5 mg kg-1. There was a decrease in beta-globulins and an increase in gamma-globulins. In the broiler chickens, there was only a slight increase in prothrombin time and a decrease in RBC counts. This data proves the good sensitivity and utility of prothrombin time values as an indicator of aflatoxicosis in birds.

Contribution of oligosaccharide and polysaccharide digestion, and excreta losses of lactic acid and short chain fatty acids, to dietary metabolisable energy values in broiler chickens and adult cockerels.

1. Two experiments were conducted, using both adult cockerels from a layer strain and 3-week-old broiler chickens. In the first experiment, one of the 2 diets investigated was composed mainly of maize and soyabean meals, the other one containing the latter ingredients diluted with 475 g/kg mature pea seeds. For these 2 diets, the apparent metabolisable energy values corrected to 0 nitrogen retention (AMEn) were derived, together with the apparent digestibilities of nitrogen, amino acids, total lipids, starch, individual oligosaccharides, and non-starch polysaccharides (NSP). Excretions of lactic acid and short chain fatty acids (SCFA) were also determined. 2. In the first experiment, the mean apparent digestibilities of starch, lipids, total amino acids, NSP, sucrose and alpha-galacto-oligosaccharides in adult cockerels were 0.946, 0.785, 0.835, 0.045, 0.99 and 0.99, respectively. In broiler chickens, they were 0.938, 0.675, 0.830, -0.016, 0.988 and 0.867, respectively. The bird type effects were significant (P < 0.05) for the digestibilities of starch, lipids, NSP (for the maize-soyabean meal diet, only) and alpha-galacto-oligosaccharides. Broiler chickens excreted a mean of 11.032 g organic acids/kg diet against 4.190 in adult cockerels (P < 0.001). These digestibility measurements enabled the contribution made by each dietary component to the AMEn value of the diets to be calculated. AMEn values were lower in broiler chickens than in adult cockerels, with on average 0.8 MJ/kg dry matter difference resulting from bird type. This difference was accounted for by differences between bird types in energy supplied by lipids (34.0%), starch (7.5%), alpha-galacto-oligosaccharides (8.7%), NSP (14.2%), and in energy losses from lactic acid excretion (16.4% of the difference in AMEn between bird types). 3. In the second experiment 2 diets were studied, consisting of a basal and the basal diluted with 30 g/kg lactose (a fermentable sugar in chickens) and 12 g/kg of a water-soluble gel-forming component containing 50% polygalacturonic acids. Lactose digestibilities reached 0.928 and 0.712 in adult cockerels and chickens, respectively. The digestibilities of the water-soluble polygalacturonic acids were similar in cockerels and broiler chickens, with a mean value of 0.672. Figures similar to those of the first experiment were found in the comparison between cockerels and broiler chickens, for the AMEn values of diets, the digestibilities of starch and lipids and the excretion of lactic acid. Broiler chickens excreted 4.580 g lactic acid/kg dry food intake, compared with 0.740 g in the adult.(ABSTRACT TRUNCATED AT 400 WORDS)

Chromosomal mapping of the genes GPRK5 and GPRK6 encoding G protein-coupled receptor kinases GRK5 and GRK6.

G protein-coupled receptor kinases (GRKs) play an important role in phosphorylating and regulating the activity of a variety of G protein-coupled receptors. Chromosomal mapping of the human genes for the two most recently identified members of the GRK family, GRK5 (GPRK5) and GRK6 (GPRK6), was accomplished by correlation of the presence of the GPRK5 and GPRK6 loci with specific chromosome regions in a rodent-human hybrid panel. These analyses revealed that GPRK5 maps to chromosome region 10q24-->qter while GPRK6 maps to 5q35. A GPRK6 related locus maps to 13pter-->q21.
