Echocardiography in lone atrial fibrillation.

Twenty-seven consecutive patients with suspected lone atrial fibrillation were studied by M-mode echocardiography. Echocardiography disclosed structural or functional cardiac abnormalities in only two (12%) of 17 patients without any sign of underlying heart disease by history or physical examination, whereas an accompanying or underlying cardiac disorder was found in 6 (60%) of 10 patients with ambiguous clinical findings. It is concluded that echocardiography is of minor value in patients with lone atrial fibrillation if symptoms and signs of other cardiac disorders are totally absent, but the presence of even minor or ambiguous clinical abnormalities seems to be a clear indication for echocardiography in these patients. It is, however, emphasized that echocardiography appears to guide the clinical management in less than 20% of patients with clinically suspected lone atrial fibrillation.

Spontaneous and/or training-induced haemodynamic changes after myocardial infarction.

Maximal oxygen uptake (VO2max) determinations and haemodynamic studies at rest and during two submaximal exercise levels (60 and 85% of the maximal) were performed in 30 male patients under 60 yr of age, 2, 5 and 8 mth after an uncomplicated myocardial infarction. Fourteen patients participated in a physical training programme during the first 3 mth of the study, while 16 patients attended the training during the second 3-mth period. An increase in VO2max occurred at 5 mth in both groups, 16 and 11%, respectively. Corresponding to this improvement cardiac index (CI) at the same relative submaximal work load increased similarly in the two groups. During the second period of the study only slight increments in VO2max and no salient alterations of the haemodynamics were recorded within or between the two groups. The linear relationship between oxygen uptake and CI was not affected throughout the study. As the work load was augmented from 60 to 85% of the maximal during the exercise test, the stroke volume index did not tend to fall. Our data suggest that allowing all patients to resume work in the very early rehabilitation period after an uncomplicated myocardial infarction, the improvement in physical work capacity and cardiac function is not significantly affected by a training programme of low intensity and short weekly duration.

Left ventricular function after myocardial infarction: relation between systolic time intervals and quantitative ischaemic ECG changes.

Twentyfour male patients with sustained myocardial infarction (MI) were studied with 12-lead ECG and systolic time intervals (STI) 5 months after the acute episode. From the ECGs were calculated the summed voltages of the R wave (sigma R), the Q wave (sigma Q), and the ST segment deviation (sigma ST). These ischaemic ECG variables were correlated with the STI parameters of left ventricular function: LVETI, PEP and PEP/LVET. Statistically significant regression equations relating the ECG changes to the STI variables were found in anterior MI, for sigma ST in the entire series, but not in inferior MI. Thus a simple and rapid inspection of the resting 12-lead ECG gives an indirect but reliable quantitative estimate of left ventricular function in patients with a sustained myocardial infarction.

Pulmonary hypertension secondary to minor pulmonary embolism.

The response of pulmonary arterial pressure to minor degrees of pulmonary embolism was examined in 18 patients with embolic occlusion of less than 25% of the pulmonary vascular bed. Patients with pulmonary embolism were compared to normal controls matched for age and sex and to patients with a variety of acute pulmonary disorders without pulmonary embolism. Patients with pulmonary embolism and patients with other acute pulmonary diseases had significantly higher pulmonary arterial pressures and significantly lower values for arterial oxygen tension (PaO2) than did normal subjects. The degree of pulmonary hypertension correlated with the PaO2. Pulmonary hypertension occurring after minor degrees of pulmonary embolism may be a response to mild arterial hypoxemia.

Natural history of vascular ring with clinical manifestations. A follow-up study of eleven unoperated cases.

A clinical follow-up study was performed in 11 unoperated patients with symptoms due to vascular ring. The age at diagnosis was 2-48 months (mean 13 months); 7 of the patients were less than 12 months old. The follow-up time varied between 1 and 22 years (median 7 years). Nine patients were entirely free of symptoms, which they had outgrown in the course of 1-2 years after the diagnosis was made; none of these 9 had symptoms after the age of 4 years. Two patients had symptoms due to either oesophageal compression or to severe associated neuromuscular lesions; the follow-up time was 3 and 1 years, respectively. Our findings lead us to support a conservative attitude regarding surgery, at least if only mild symptoms are present, If the patients can be satisfactorily treated medically, they seem to outgrow their symptoms in early childhood.

Embolic complications in paroxysmal atrial fibrillation.

The incidence of embolic complications among 426 patients with initial paroxysmal atrial fibrillation (PAF) was analysed. A distinct clustering of emboli was seen at the time of onset of PAF. After transition to chronic atrial fibrillation (CAF), which developed in 141 patients (33.1%), the incidence of emboli was seen to rise to a new level several times higher than the incidence level for patients with PAF. Also in this group a distinct clustering of emboli was seen during the first year after transition to CAF. On this background it is suggested that patients with PAF may benefit from treatment with anti-arrhythmic agents in order to prevent the development of CAF and that anticoagulants for stroke prevention seems especially desirable in atrial fibrillation (AF) of recent onset.
