Triple lumen catheters for parenteral nutrition.

We retrospectively reviewed the charts of 61 patients in whom a total of 101 triple lumen catheters (TLCs) were used for parenteral nutrition for a total of 1,512 days (mean 15 +/- 11 days). Patients were categorized as those having culture-negative TLC tips either with or without infection elsewhere (groups 1 and 2, respectively) and those with culture-positive TLC tips (group 3). Temperature, WBC, alkaline phosphatase value, and SGOT level were recorded one or two days before TLC removal (period 1) and one or two days or three to five days after TLC removal (periods 2 and 3, respectively). The incidence of catheter sepsis was 4%. Fourteen other tips were contaminated. Patients in group 1 remained afebrile during all three periods, and all tips removed were culture-negative. Removal of the TLC in groups 2 and 3 caused neither defervescence nor decreased WBC. We conclude that TLCs can be used for total parenteral nutrition with a low incidence of infection, that TLC tips need not be cultured in afebrile patients without other sources of infection, and that a TLC can be safely left in place so long as the patient is afebrile. However, the risk of infection or contamination is high for catheters left in place for more than two weeks.

Chorioamnionitis: how useful is the determination of C-reactive protein?

This study was designed to derive the predictive value of C-reactive protein (CRP) in peripheral venous serum of patients admitted to hospital with suspected premature rupture of the membranes (PROM). CRP was assayed by each of 4 separate methods and the results have been compared for accuracy and practical value with respect to clinical outcome and the histopathology of the placenta. Of the 4 techniques used only the latex test had characteristics suitable for a diagnostic screen. While the results were only semiquantitative, when comparisons were made to other techniques no significant change in clinical diagnosis would have been made. The results have confirmed that chorioamnionitis and preterm labour are often associated, but in some instances the extent of inflammatory infiltration was greater than might have been expected from the short time interval between documented membrane rupture and delivery. Thus it may be speculated that some cases of PROM are secondary to, rather than causative of, infection. Finally it is suggested that a controlled therapeutic trial of active intervention in those cases of PROM with elevated CRP in the absence of other clinical parameters suggestive of intrauterine infection should be undertaken.

LH 'dipstick' quantitation by reflectance meter.

The introduction of rapid semiquantitative methods using monoclonal antibodies to measure urinary LH has application to the management of the infertile couple. The interpretation of the colour change end-point has however been subjective and thus liable to misinterpretation with resultant problems in clinical management. This short communication describes the use of a modified reflectance meter to overcome these problems.

Celiprolol in angina pectoris: a controlled study.

This double-blind, placebo-controlled, parallel, multicenter study was designed to evaluate the anti-anginal and anti-ischemic effects of celiprolol in patients with stable, exercise-induced angina pectoris. The study began with a 4-week single-blind, placebo run-in, followed by 4 weeks of double-blind active treatment with celiprolol 200, 400, or 600 mg once daily or placebo, and 1 week of a placebo run-out. Patients studied had a history of chronic, stable angina pectoris and myocardial ischemia (greater than or equal to 1.0 mm ST segment depression) within 3-12 min of treadmill exercise (modified Bruce) after 3 and 4 weeks of placebo. After 2 weeks treatment, treadmill testing results indicated a mean time to myocardial ischemia for patients receiving placebo increased by 0.5 min (8%), whereas the increases for celiprolol 200, 400, and 600 mg were 0.8 min (11%), 2.1 min (33%), and 1.1 min (16%) respectively. Mean time to anginal symptoms for placebo patients was increased by 0.6 min (10%), and for celiprolol patients by 1.8 min (27%), 1.5 min (19%), and 1.0 min (14%) respectively at the 3 dose levels. The maximum exercise double product (heart rate X systolic BP) 24 h after medication, was increased by a mean of 373 (2%) after placebo, but decreased by 3301 (-15%), 3400 (-17%), and 3549 (-16%) for celiprolol 200, 400, and 600 mg respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Post-gastrectomy nutrition: methods of outpatient screening for early osteomalacia.

A cohort of 227 patients treated by partial gastrectomy between 1955-1960 has been followed up regularly for evidence of osteomalacia. Of these, 10 developed clinical or subclinical osteomalacia as proven by bone biopsy. The diagnosis of early asymptomatic osteomalacia in bone biopsies presented many difficulties. As an alternative, patients with a rising serum alkaline phosphatase (SAP) were screened for liver disease, Paget's disease, osteoporotic crush fractures and bony secondaries. In the absence of these a therapeutic trial of eight Calcium and Vitamin D tablets BPC daily was given. In 5 out of 11 patients the SAP fell to normal and these were given a maintenance daily dose of six tablets. These patients were presumed to have had subclinical osteomalacia. This approach, involving regular monitoring of the SAP, is recommended as the most applicable for routine screening of a gastrectomy population, particularly in District Hospitals.

The biochemical determinants of hypoxanthine uptake in Novikoff rat hepatoma cells.

The rate with which Novikoff rat hepatoma cells took up exogenous hypoxanthine increased sharply towards the end of the logarithmic growth phase, remained high for several hours into the stationary phase, and then decreased again. In an effort to account for these phenomena, several biochemical parameters were monitored during culture growth: the activities of the hypoxanthine transporter, of hypoxanthine phosphoribosyltransferase, and of P-Rib-PP synthetase; and the intracellular concentrations of ATP and P-Rib-PP. All of these parameters remained virtually constant during growth of the culture, except for P-Rib-PP, which increased greater than 10-fold in a pattern similar to that for hypoxanthine uptake. The activities of the transporter, synthetase, and phosphoribosyltransferase remained stable over 7 h of treatment with cycloheximide.

Constipation--a simple approach to treatment.

A simple method of treating patients with constipation is outlined and has been used successfully in 40 patients with severe primary constipation or constipation associated with diverticular disease.

Analysis of azanaphthalenes and their enzyme oxidation products by high-performance liquid chromatography, infrared spectroscopy and mass spectrometry.

Adsorption and reversed-phase high-performance liquid chromatography (HPLC) have been successfully used to separate metabolites from the parent heterocycles (isoquinoline, 3-methylisoquinoline, phthalazine, quinazoline, quinoxaline and cinnoline). Retention data are reported. The metabolites, hydroxyazanaphthalenes, which arise as a result of aldehyde oxidase catalysed oxidation, could be extracted in microgram quantities from incubation mixtures by n-butanol. Complete identification of the oxidation products was achieved by collecting fractions corresponding to each compound from the HPLC eluate and subjecting these to infrared and mass-spectral analysis.
