A deletion in the gene for glycoprotein IIb associated with Glanzmann's thrombasthenia.

The platelet fibrinogen receptor is composed of a complex of glycoproteins (GP) IIb and IIIa on the surface of platelets. Deficient function of this receptor prevents normal platelet aggregation, resulting in Glanzmann's thrombasthenia (GT). In this paper, we describe a black thrombasthenic patient who is either homozygous or hemizygous for a deletion within the GPIIb gene. Initial Western blot analysis of platelet proteins from this patient did not detect any GPIIb, but did detect small amounts of GPIIIa of normal mobility. Quantitation of vitronectin receptor (VNR) demonstrated that this thrombasthenic patient had approximately 1.5-2 times the number of these receptors per platelet compared with controls, a finding that has previously been noted in other thrombasthenic patients with defects in GPIIb. Genomic Southern blot studies demonstrated a deletion in the GPIIb gene of approximately 4.5 kilobasepairs (kb). Analysis of the isolated GPIIb gene demonstrated that the deletion begins between two Alu repeats within intron 1 and ends in intron 9. Polymerase chain reaction (PCR) studies using platelet RNA and oligonucleotides directed to both the 5' and 3' ends of the GPIIb cDNA sequence easily detected GPIIb transcript, suggesting that the genomic deletion of exons 2-9 does not significantly decrease the level of the GPIIb mRNA. Sequence analysis of PCR-generated GPIIb cDNA showed that a cryptic AG splice acceptor sequence was being utilized, resulting in a transcript that contained a portion of introns 1 and 9, as well as having a deletion of exons 2-9. Unlike the GPIIb gene, the GPIIIa gene appears to be intact by Southern blot analysis. PCR studies using platelet RNA and oligonucleotides directed to the GPIIIa cDNA sequence demonstrated the presence of GPIIIa mRNA. In summary, the thrombasthenic state in this patient appears to be due to a GPIIb gene deletion resulting in an abnormal transcript and no detectable platelet GPIIb. Platelet GPIIIa levels were secondarily low presumably due to the known instability of GPIIIa in the absence of GPIIb.

The pathophysiology and epidemiology of myocardial infarction. A review.

Myocardial infarction continues to represent a major cause of death in the Western world, and although there have been significant reductions in its incidence in recent years, some countries such as Scotland and Finland still have high mortality rates. Thrombotic occlusion, in association with varying degrees of plaque disruption and coronary artery spasm, represents the major cause of acute myocardial infarction (AMI). At the cellular level, this results in a shift towards anaerobic metabolism, depletion of energy stores, disrupted membrane integrity, alterations in ionic gradients, myocyte oedema, inhibition of contraction and a proarrhythmic potential. Reperfusion can exacerbate the damage, producing calcium ion accumulation and free radical generation. Infarct expansion and ventricular remodelling can often follow AMI as can additional necrosis, in the form of infarct extension/reinfarction. Rational and optimal treatment of AMI should be based on an understanding of the epidemiological influences and the pathophysiological processes involved. This review considers some of the important features in the pre-, peri- and postinfarction periods.

Indomethacin and ibuprofen inhibit the uptake of 45Ca2+ by washed human platelets through a thromboxane A2-independent mechanism.

Indomethacin and ibuprofen inhibited adrenaline- and calcium ionophore A23187-stimulated 45Ca2+ uptake by isolated human platelets in a concentration-dependent manner. Mediation of these effects by thromboxane A2 (TXA2) inhibition was discounted since under the same experimental conditions, adrenaline did not stimulate TXA2 synthesis and A23187-stimulated TXA2 synthesis was only marginally inhibited by concentrations of ibuprofen and indomethacin that inhibited 45Ca2+ uptake by 50%. These data indicate that the inhibitory action of non-steroidal anti-inflammatory drugs (NSAIDs) on platelet activity may be due, at least in part, to effects on calcium mobilisation at the plasma membrane level. The present results may also be of relevance to the anti-inflammatory action of NSAIDs.

Diurnal changes in the haemoglobin level, red blood cell number and mean corpuscular haemoglobin in foals during the first 13 weeks of life and in their lactating mothers.

1. In six foals and their mothers (of Standard breed) diurnal changes in the haemoglobin level, red blood cell number and mean corpuscular haemoglobin during the first 13 weeks of foal life were studied. 2. Studies begun when a foal reached 7 days of age and were repeated every two weeks till 13 weeks of foal life. Blood was taken every 6 hr in foals and every 4 hr in mares. 3. No diurnal rhythmicity in parameters studied either in foals or in mares was found. 4. Decrease of haemoglobin level in lactating mares (14.0-12.5 g/100 ml) without changes in red blood cell number were observed. Mean corpuscular haemoglobin changed distinctly from one study day to another. 5. In foals were stated: significantly higher haemoglobin level and erythrocyte number and lower mean corpuscular haemoglobin than in mares.

Seasonal cyclicity in carbohydrate metabolism parameters in the European bison, Bison bonasus L.

1. In 197 European bison divided into four groups (Group 1, 0-3-year-old males; Group 2, 0-3-year-old females; Group 3, mature bulls, over 3 years old; Group 4, mature cows, over 3 years old) seasonal changes in the level of lactic and pyruvic acids, glucose and alkaline reserve were studied. 2. Seasonal cyclicity was found only in lactic acid levels in all groups. 3. In the pyruvic acid level cyclicity was found only in males. 4. In the glucose level cyclicity was found only in mature cows. 5. In alkaline reserve cyclicity was found only in mature bison. 6. Six out of 9 acrophases of cyclic indices occurred in the period from the second half of August until mid-December, i.e. before the winter time.

Development of diurnal rhythm in some metabolic parameters in foals.

1. The development of diurnal rhythm activity of FDPA, AspAT and A1AT and in levels of cortisol, T3 and T4 was observed in the blood serum of six foals. 2. The studies began when a foal was 7 days old and were repeated every month until foals reached 1 year of age. Blood samples were taken every 4 hr for one day each month. 3. As a control group four barren mares were used, kept and examined in the same conditions. 4. In mature mares, diurnal rhythms in activity of A1AT (acrophase at 2200 hr), AspAt (2400 hr) and cortisol (0630 hr) but in T3 only in summer months (acrophase at 0100 hr) were observed. 5. During the first 6 months of foal life, significantly higher mean levels of FDPA, A1AT, T3 and T4 than in control mares were found. 6. The cortisol level in foals was half as much as that of mature mares throughout the year. 7. In foals the diurnal rhythm in A1AT activity occurred in the 5th month and in AspAt--in the 12th month (acrophase at 2400 hr), but in cortisol levels it was developed already in the second month of foal life (acrophase at 0830 hr).

Suitability of cryopreserved samples for pharmacological studies of ciliary activity.

The effect of terbutaline sulfate on the ciliary activity of fresh and cryopreserved human nasal epithelium was evaluated. Cryopreservation had no effect on baseline ciliary beat frequency. Both fresh and cryopreserved samples exposed to 10(-4) M terbutaline showed a statistically significant increase in ciliary beat frequency (27 and 25%, respectively). When the percentage changes after drug challenge for fresh and cryopreserved samples were compared no statistical difference emerged. It is concluded that cryopreservation in liquid nitrogen at -196 degrees C does not affect membrane receptors, at least beta-adrenergic receptors, and therefore cryopreserved samples are suitable for pharmacological studies of ciliary activity.

Efficacy and safety of vapor-heated anti-inhibitor coagulant complex in hemophilia patients. FEIBA Study Group.

The study reported here was designed to measure the efficacy and safety of a vapor-heated anti-inhibitor coagulant complex (FEIBA-VH) for the treatment of bleeding episodes in patients with hemophilia A who have inhibitors to factor VIII (FVIII). FEIBA-VH, a second-generation complex, is vapor-heated for 10 hours at a temperature of 60 degrees C and a pressure of 1190 millibar (mbar) and for 1 additional hour at 80 degrees C and 1375 mbar. The current study was performed because of concern that this vapor-heating process would reduce the efficacy of FEIBA-VH as compared with non-heat-treated FEIBA (FEIBA). Forty-one patients received FEIBA-VH for 106 evaluable bleeding episodes. Ninety-three (88%) episodes were controlled, and 13 (12%) were not. Eighty-three (79%) episodes were controlled within 36 hours of the first infusion. No significant toxicity was seen. These results were compared with those of an earlier study with FEIBA. FEIBA-VH was at least as effective as FEIBA in controlling bleeding episodes and can be compared favorably to any reported treatment of bleeding episodes in hemophiliacs with inhibitors to FVIII.

The role of the high dependency unit in postoperative care: an update.

The current experience of a high dependency unit established 5 years ago for the postoperative care of high-risk patients undergoing surgery is reported. The resource implications and contributions to the safety and quality of post-operative care, particularly pain relief, are described.

The effect of cryopreservation on ciliary beat frequency of human respiratory epithelium.

The effect of cryopreservation on ciliary activity of human nasal respiratory epithelium was evaluated. Samples were cryopreserved in a solution containing nutrient medium, 10% fetal calf serum, and two different concentrations (10 or 20%) of dimethyl sulfoxide and stored in liquid nitrogen at -196 degrees C for 2 weeks. Ciliary beat frequencies (CBF) of the samples before and after cryopreservation were compared. Mean CBF values did not differ significantly with both concentrations of dimethyl sulfoxide. The mean intrasample coefficient of variation of the CBF decreased significantly after cryopreservation. After thawing, CBF remained unchanged for at least 4 hr. It is concluded that normal ciliated epithelial cells can be frozen and stored in liquid nitrogen at 196 degrees C while maintaining their CBF.
