Brush cells in the human fetal trachea.

We performed a morphologic examination of human fetal lung tissue, using scanning and transmission electron microscopy, in order to establish the presence of brush cells in extrapulmonary and intrapulmonary airways, and developing acinar epithelium. Brush cells, characterized by a border of regular straight microvilli containing a filamentous core, were observed within the tracheal epithelium of a 19-20 week gestational age fetus. These cells constituted 0.5% of the total epithelial cell population. Brush cells were not seen within the bronchial, bronchiolar or developing acinar epithelium. Our study shows that brush cells occur infrequently but normally in the developing tracheal epithelium of the second trimester fetus.

Interactive morphometry of normal and hyperplastic peritoneal mesothelial cells and dysplastic and malignant ovarian cells.

This study used computerized interactive morphometry to evaluate the differential characteristics of mesothelial normal cells, mesothelial hyperplastic cells, and carcinomatous cells, and also compared hyperplastic mesothelial cells with ovarian dysplastic cells from a previous study. The procedure included extraction of multiple descriptors of the nuclear profile: perimeter length, area, longest chord, circularity factors, standard deviations of these characteristics, and a 10-bin size distribution table of the nuclear area. The final classification is achieved by stepwise discriminant analysis of these variables. The analysis classified all cases correctly with high posterior probabilities.

Resistance to apramycin in two enterobacterial clinical isolates: detection of a 3-N-acetyltransferase IV.

Considering the possible role of farm animals in the contamination of human consumers by plasmid-mediated apramycin-resistant enterobacteria strains, this type of resistance should be tested more systematically in human isolates. Very recently we isolated in Zaragoza one apramycin-resistant Escheria coli strain obtained from the blood of a hospitalized patient; this clinical isolate produced a plasmid-mediated 3-N-aminoglycoside acetyltransferase IV. We describe also the isolation in Madrid of one multiresistant Klebsiella pneumoniae clinical strain. This isolate harbored a single plasmid and carried determinants for apramycin, gentamicin, tobramycin, hygromycin B, streptomycin, and ampicillin, which could be transferred en bloc to E. coli K-12 J62. Extracts from donor and transconjugant strains carrying pUZ6776 plasmid produce acetyltransferase activity AAC(3)-IV and double phosphotransferase activity (HPH and APH(3'')).

Microbiological characteristics of Weeksella virosa (formerly CDC group IIf) isolated from the human genitourinary tract.

Weeksella virosa (formerly CDC group IIf) is a nonsaccharolytic, oxidase- and catalase-positive, gram-negative rod which is unable to grow on MacConkey agar. At 48 h of incubation on blood or chocolate agar, the colonies present a characteristic appearance: intensely mucoid, adherent, and cream colored as a result of the production of a nondiffusible yellow pigment. This microorganism has been isolated predominantly from the female genitourinary tract, which indicates the opportunity for sexual transmission. We present the microbiological study of three strains of W. virosa identified in a study of 707 female genital samples, representing an incidence of 0.42%. At the same time, we analyzed the principal biochemical tests used in the identification of this microorganism and the susceptibilities of the organism to the different antimicrobial agents assayed.

Mitogenic signalling through the bombesin receptor: role of a guanine nucleotide regulatory protein.

Bombesin and structurally related peptides including gastrin releasing peptide (GRP) are potent mitogens for Swiss 3T3 cells. The early cellular and molecular responses elicited by bombesin and structurally related peptides have been elucidated in detail. Further understanding of the molecular basis of the potent mitogenic response initiated by bombesin is required in order to elucidate the mechanism by which the occupied receptor communicates with effector molecules in the cell. Transmembrane signalling mechanisms involving either a tyrosine kinase or a guanine nucleotide-binding regulatory protein (G protein) have been proposed. Here we summarize our experimental evidence indicating that a G protein(s) is involved in the coupling of the bombesin receptor to the generation of intracellular signals related to mitogenesis. Evidence for the role of G proteins in bombesin signal transduction pathways has been obtained by assessing the effects of guanine nucleotide analogues on both receptor-mediated responses in permeabilized cells and ligand binding in membrane preparations. We found that [125I]GRP-receptor complexes were solubilized from Swiss 3T3 cell membranes by using the detergents taurodeoxycholate or deoxycholate. Addition of guanosine 5-[gamma-thio]triphosphate (GTP gamma S) to ligand-receptor complexes isolated by gel filtration enhanced the rate of ligand dissociation in a concentration-dependent and nucleotide-specific manner. These results demonstrate the successful solubilization of [125I]GRP-receptor complexes from Swiss 3T3 cell membranes and provide evidence for the physical association between the ligand-receptor complex and a guanine nucleotide binding protein(s).

[Sedation and analgesia with nitrous oxide].

Analgesic-sedation with nitrous-oxide has been used in 609 patients with excellent clinical results. The average concentrations used have been 40% N2O and 60% O2 and a flow of eight liters per minute. Patients have behaved very cooperatively when treated previously with analgesic-sedation allowing us to perform treatment such as scaling and root planning and other clinical procedures without applying local anesthetics. Analgesic-sedation is a very technique-sensitive procedure and a thorough training of clinical personal is completely mandatory.

Internal bleaching of severe tetracycline discolorations: four-year clinical evaluation.

Two hundred fifty-eight teeth severely discolored by tetracycline were treated with an internal bleaching technique. Endodontic treatment was required in all teeth before the walking bleaching procedure could be undertaken. Four years of clinical and radiographic evaluation revealed good esthetic results for all patients. Endodontic procedures were considered to be successful, and not one patient showed external cervical resorption after 4 years. This type of treatment is a valid alternative to porcelain laminate veneers for those teeth that exhibit severe tetracycline discoloration and in which the dental enamel and anatomic crown are intact.

[Analysis of the sensitivity of mesophilic Aeromonas isolated from feces to antibiotics of everyday use].

The sensitivity to antibiotics of 56 mesophilic strains of Aeromonas (20 A. caviae, 18 A. sobria and 18 A. hydrophila) isolated from the feces of patients with enteritis, 89.2% of strains had some type of resistance, such as 83.9% to ampicillin, 32.1% to carbenicillin, 28.8% to cefalotin, 5.3% to co-trimoxazole and 1.7% to chloramphenicol. 48.2% of strains were simultaneously resistant to two of the tested antibiotics, 3.5% to three and 5.3% to four. 95.7% of ampicillin resistant strains were beta-lactamase producing. A. sobria was more sensitive to cefalotin (MIC less than 8 micrograms/ml) than the remaining species. This finding supports the use of that marker for identification.

Morphometric analysis of glomerular basement membranes (GBM) in thin basement membrane disease (TBMD).

We measured the thickness of glomerular basement membrane in 46 patients with thin basement membrane disease (TBMD), (age range 15-50 years, almost equal M:F ratio), and compared with that in a control group of 5 patients (age range 5-38 years) with normal glomerular morphology. The measurements of glomerular basement membrane taken from electron micrographs (magnification x 12,500) were analyzed using an interactive image analysis system assembled around an INTEL 10 microcomputer, with a high resolution touch sensitive screen as the interactive peripheral. Calculation was done by printing on an electron micrograph a grating replica (21,600 lines/cm), with the same magnification as the electron micrographs of the glomeruli and calibrating the arithmetic (AM) and harmonic (HM) mean for each case. Comparing the results of TBMD cases (AM 129-202 nm; HM 128-213 nm) with those of the control group consisting of 5 cases of "minimal change nephrotic syndrome" (AM 287-317 nm; HM 300-333 nm) it was found that GBM in TBMD is remarkably thin. The thinning was caused mainly by the decreased width of the lamina densa (TBMD group: 71.4-147.0 nm; HM 72.4-154.4 nm in comparison with the control group: AM 174.4-235.5 nm; HM 184.2-249.6 nm). This finding allows us to differentiate thin basement membrane disease from other glomerulopathies presenting primarily with isolated or recurrent hematuria.

Morphometric analysis of germinal centers in nonthymomatous patients with myasthenia gravis.

A morphometric analysis of germinal centers (GCs) in the thymus of nonthymomatous patients with myasthenia gravis was performed in 26 patients, including 14 women and 12 men. The GCs were evaluated for number, diameter, perimeter, and cross-sectional area using a computerized interactive morphometric analysis. The GCs ranged from 2 to 19 per case, with an average diameter of 0.02 to 0.43 mm, a perimeter of 0.38 to 1.35 mm, and a cross-sectional area of 0.01 to 0.14 mm2. In thymic sections with fewer than five GCs, the GCs had a mean area of 0.05 mm2, a perimeter of 0.84 mm, and a diameter of 0.24 mm. In thymic sections containing five or more GCs, the GCs had a mean area of 0.04 mm2, a perimeter of 0.62 mm, and a diameter of 0.19 mm. The clinical improvement in these patients inversely correlated with the above parameters, with improvement being observed in patients whose GCs had a mean area of 0.02 mm2, a perimeter of 0.58 mm, and a diameter of 0.17 mm. No correlation was observed between the number of GCs and clinical improvement. The results of our study appear to indicate that computerized interactive morphometric analysis may represent a potentially useful method for the evaluation of the morphometric features of thymic GCs and their correlation with clinical outcome in patients with myasthenia gravis.

Development of splenic natural suppressor (NS) cells in Ehrlich tumor-bearing mice.

Spleen cells from C57BL/6J mice bearing Ehrlich carcinoma growing as a solid tumor show progressive unresponsiveness to concanavalin A (Con A) and lipopolysaccharide (LPS) mitogens. This is accompanied by striking spleen enlargement with marked hematopoietic activity. Lymphoproliferative assays of normal spleen cells in co-culture with tumor-bearing spleen cells (TBSC) show that: (a) TBSC contain non-specific suppressor cells able to abrogate both Con A and LPS responses, or mixed lymphocyte reaction, of normal spleen cells and (b) suppression by TBSC is MHC-unrestricted, non-prostaglandin-mediated and greatly enhanced by Con A supernatants. Suppressor cells associated with TBSC are large, low-density cells without markers of mature B or T lymphocytes or of the mononuclear phagocyte system. Most appear to be asialo-GM1-negative, as suppression was only partially inhibited by treatment with anti-asialo-GM1 and complement. Since NK activity is lacking in TBSC, our data strongly suggest that these "null" suppressor cells are related to the natural suppressor (NS) cells found described in normal bone-marrow and neonatal spleens, or induced in adult spleens by total lymphoid irradiation, graft-vs.-host disease, or cyclophosphamide treatment.

[Chronic fatigue syndrome associated with Epstein-Barr virus infection].

Epstein-Barr virus (EBV) infection is ubiquitous and may result in multiple and widely different clinical features; the most common of these is infectious mononucleosis (IM). Recently, a group of patients has been included in the chronic EBV infection syndrome (EBVIS), with a sustained nonspecific syndrome consisting of asthenia, anorexia, low grade fever and changes in mood, associated with a viral infection not necessarily caused by EBV; this has been called chronic fatigue syndrome (CFS). We report a patient who fulfilled the criteria for CFS associated with EBV after an acute, well documented EBV infection. We discuss its etiological and pathophysiological implications, emphasizing the need for extreme caution in the diagnosis of CFS. A merely clinical diagnosis may hide severe mistakes.

Characterization of ovarian dysplasia by interactive morphometry.

This study defines, by morphometric analysis, criteria for the diagnosis of ovarian dysplasia. Areas of surface epithelium, adjacent to Stage I ovarian serous papillary adenocarcinoma, were diagnosed as dysplastic and evaluated for architectural and cytologic changes by means of two specially designed interactive morphometric procedures. Statistical evaluation was based on stepwise discriminant analysis of multiple quantitative descriptors. A data base was generated by analyzing histologic slides from eight normal (control), and 13 malignant ovarian tissues. Confirmation of the diagnosis was achieved in each case. The histologic criteria for ovarian dysplasia differ from those used for ovarian borderline malignancy. Interactive morphometric image analysis offers an objective method for delineating cancer precursors that could be used in ovarian biopsies taken by laparoscopy or during incidental abdominal surgery. It could be applied to cancer precursors in other locations.

Vanadate counteracts glucagon effects in isolated rat hepatocytes.

The incubation of isolated rat hepatocytes with vanadate increased the concentration of fructose 2,6-bisphosphate without modifying 6-phosphofructo-2-kinase activity. Vanadate also reverted and prevented the decrease of fructose 2,6-bisphosphate levels, of the "active" form of the 6-phosphofructo 2-kinase and of the pyruvate kinase activity ratio produced by glucagon, by probably counteracting the increase in cyclic AMP concentration.

Interactive morphometric procedures and statistical analysis in the diagnosis of ovarian dysplasia and carcinoma.

We report on our continued experience with an interactive morphometric method recently introduced by us for the definition and diagnosis of ovarian dysplasia vs. normal or malignant epithelium. The main quantitative differences between these three diagnostic categories are based on 1) cytology of the nuclei (nuclear area, circularity factor, maximum chord) and 2) on stratification (distances of nuclear centers to the basement membrane and number of cells per unit length of basement membrane). We implemented our approach on live video images viewed on a monitor overlaid with a touch sensitive screen by one of two interactive procedures: 1) by tracing nuclear profiles (procedure DRAW) or 2) by tracing the basement membrane and touching the center of all nuclei (procedure NU-MEAS). In all cases statistical analysis was performed on a string of multiple variables by stepwise discriminant analysis. Now we have straightened our data basis and are able to obtain diagnosis of unknown samples with very high posterior probabilities. Both procedures are effective but NU-MEAS requires the least effort and seems to give the best statistics.
