Dimensions of retention: a national study of the locational histories of physician assistants.

This study describes the locational histories of a representative national sample of physician assistants and considers the implications of observed locational behavior for recruitment and retention of physician assistants in rural practice. Through a survey, physician assistants listed all the places they had practiced since completing their physician assistant training, making it possible to classify the career histories of physician assistants as "all rural," "all urban," "urban to rural" or "rural to urban." The study examined the retention of physician assistants in rural practice at several levels: in the first practice, in rural practice overall and in states. Physician assistants who started their careers in rural locations were more likely to leave them during the first four years of practice than urban physician assistants, and female rural physician assistants were slightly more likely to leave than men. Those starting in rural practice had high attrition to urban areas (41 percent); however, a significant proportion of the physician assistants who started in urban practice settings left for rural settings (10 percent). This kept the total proportion of physician assistants in rural practice at a steady 20 percent. While 21 percent of the earliest graduates of physician assistant training programs have had exclusively rural careers, only 9 percent of physician assistants with four to seven years of experience have worked exclusively in rural settings. At the state level, generalist physician assistants were significantly more likely to leave states with practice environments unfavorable to physician assistant practice in terms of prescriptive authority, reimbursement and insurance.

Modeling the UMLS using an OODB.

The Unified Medical Language System combines many well established authoritative medical informatics terminologies in one system. Such a resource is very valuable to the healthcare industry. However, the UMLS is very large and complex and poses serious comprehension problems for users and maintenance personnel. Furthermore, the sets of concepts of semantic types are not semantically uniform and thus are difficult to study. We describe a method to represent two components of the UMLS, the Metathesaurus (META) and the Semantic Network, as an OODB. The resulting UMLS OODB schema is deeper and more refined than the Semantic Network. It offers semantically uniform classes, which improves support for comprehension and navigation of META. The UMLS OODB also exposes problems in the semantic type classifications.

Utility of obtaining radiographs in patients with trigger finger.

We performed a combined retrospective and prospective study on the utility of obtaining radiographs in patients with trigger finger. Ninety-three patients with a total of 110 involved fingers were reviewed. The average age of the patients was 52 years, and 61% had involvement of their dominant hand. There were 54 women and 39 men. Eighty-four percent of the radiographs were reviewed retrospectively, and 16% of the radiographs were reviewed prospectively. There were no abnormal findings in 62% of the radiographs [corrected]. Thirty-one percent had radiographic abnormalities that were not currently clinically significant. Four percent had radiographic findings that correlated with other clinical problems. No radiographic finding changed our management. Patients with stenosing flexor tenosynovitis without a history of injury or inflammatory arthritis do not need routine radiographs.

Loneliness as a predictor of hospital emergency department use.

BACKGROUND: Recent studies indicate that loneliness is a significant risk factor for many ailments from colds to heart disease. If lonely patients are at greater risk for illness, then we might expect that they would use the emergency department (ED) more often and incur greater medical costs than those who are not lonely. Our goal was to determine the prevalence of loneliness in patients in an ED and to evaluate it as a predictor of ED use, hospital admission, and chronic illness. METHODS: We evaluated a convenience sample of 164 ED patients with the University of California-Los Angeles Loneliness Scale, Version 3 and a survey of patient characteristics. Using medical record review and patient self-report, we determined total ED visits, the presence of chronic illness, and discharge diagnoses during a 1-year retrospective period. We evaluated data with least mean square regression and a 2-tailed t test. RESULTS: We found a statistically significant correlation between loneliness score and total hospital ED visits (P <.001). The mean loneliness score (39) was equal to that of normal populations. Patients scoring higher than the mean used the ED 60% more per year than patients who scored lower (P = .008). There was no association between a patient's loneliness score and baseline chronic illness or severity of current illness (P = .56). Spanish-speaking patients had higher loneliness scores than English-speaking patients (P = .001). CONCLUSION: Loneliness is a predictor of hospital ED use independent of chronic illness and is potentially very expensive to society. We recommend further studies be done to examine if allocating resources for preventing, diagnosing, and treating loneliness would be cost effective.

New sensitive discovery histoculture model for growth-inhibition studies in prostate cancer and BPH.

BACKGROUND: A new, total-immersion three-dimensional histoculture (TIH) method was developed to evaluate growth of tissue containing a mixture of benign prostate hyperplasia (BPH) and prostate cancer in vitro. METHODS: Efficacy of inhibitors, such as genistein, was determined by measuring 3H-thymidine incorporation per microgram protein. Inhibitory effects obtained in TIH were compared to those in sponge-gel supported histoculture (SSH). RESULTS: 3H-thymidine incorporation was 2-5-fold higher in tissue cultured in TIH than in SSH. The average inhibition by genistein at a concentration of 18 JIM was 73% in TIH, vs. 31% in SSH. TIH also appeared to be more sensitive, since the lowest concentration of genistein that significantly inhibited growth of BPH mixed with prostate cancer tissue was 2.3 IJM, while in SSH the lowest concentration was 9.2 F,M. Although the within-assay coefficient of variation (CV) was similar for both TIH and SSH, the between-assay CV was better in TIH. CONCLUSIONS: These data suggest that TIH can be used as a discovery model for screening and evaluating inhibitors of prostate tissue growth in vitro.

Surgical orthotopic implantation allows high lung and lymph node metastatic expression of human prostate carcinoma cell line PC-3 in nude mice.

BACKGROUND: Prostate cancer is the second leading cause of male death in the United States. When diagnosed, nearly half the cases have metastatic lesions. An animal model of human prostate cancer demonstrating spontaneous metastasis from the orthotopic site after tumor implantation should be of great help for us to understand the disease and to formulate treatment strategy. We report here a high metastatic model of human prostate cancer PC-3. METHODS: We developed microsurgical techniques, termed surgical orthotopic implantation (SOI), to implant histologically intact tumor tissues orthotopically in immunodeficient mice. In this study intact tissue of the human prostate cancer cell line PC-3, harvested from a subcutaneous tumor in a nude mouse, was implanted to the ventral lateral lobes of the prostate gland in a series of nude mice. Mice were sacrificed when found moribund, and autopsy and histology were performed subsequently. RESULTS: A high frequency of lymph node and lung metastasis was noted upon histological examination. The extensive and widespread lung metastasis following orthotopic implantation of PC-3 is, to the best of our knowledge, the first report in the literature. CONCLUSIONS: In contrast to orthotopic injection of cell suspensions, no multiple metastatic cell selection was necessary after SOI for significant expression of the metastatic potential of PC-3. We conclude that the stromal tissue architecture maintained in the implanted tumor played a critical role in tumor growth and progression.

Genistein inhibits the growth of human-patient BPH and prostate cancer in histoculture.

BACKGROUND: There is strong epidemiological evidence that prostate disease is significantly less prevalent in the Orient, where the intake of soy products is very high, than in the United States. We therefore undertook a study of the effects of genistein, a major component of soy, on growth of human-patient benign prostatic hypertrophy (BPH) and prostate cancer tissue in three-dimensional collagen gel-supported histoculture. METHODS: Surgical specimens of human BPH and cancer were histocultured for 5 days to study the effects of genistein on growth, as measured by inhibition of 3H-thymidine incorporation per microgram protein on day 5. RESULTS: Genistein in doses of 1.25-10 micrograms/ml decreased the growth of BPH tissue in histoculture in a dose-dependent manner, with little additional effect at higher doses. Prostate cancer tissue in histoculture was similarly inhibited by these doses of genistein. CONCLUSIONS: Genistein decreases the growth of both BPH and prostate cancer tissue in histoculture. The data suggest that genistein has potential as a therapeutic agent for BPH and prostate cancer.

Shape- and weight-based self-esteem and the eating disorders.

OBJECTIVES: To determine the psychometric properties of the Shape- and Weight-Based Self-Esteem (SAWBS) Inventory in women with eating disorders, and to compare SAWBS scores in women who have eating disorders with women from psychiatric and normal control groups. METHOD: Women with eating disorders (n = 48), women with other psychiatric disorders (n = 44), and undergraduate control women (n = 82) completed the SAWBS Inventory and measures of depression, self-esteem, and eating disorder symptomatology. Twenty women from the eating disorder group completed the SAWBS Inventory a second time 1 week later. RESULTS: Similar to previous work in undergraduate samples, SAWBS scores were stable over 1 week, and demonstrated concurrent and discriminant validity in women with eating disorders. In between-group comparisons, SAWBS scores were higher among women with eating disorders than in either control group, even after controlling for age, socioeconomic status, body mass index, and self-esteem. A differing relationship between depression and SAWBS emerged as a function of group; SAWBS scores differed significantly among depressed, but not nondepressed women from the three groups. CONCLUSION: The psychometric properties of the SAWBS Inventory were established in women with eating disorders. As expected, SAWBS scores were higher in women with eating disorders than in the control groups. Clinical implications of these findings are discussed.

The efficacy of involuntary outpatient treatment in Massachusetts.

One means to address some of the unintended consequences of the shift of treatment for individuals with serious mental illness from hospitals to communities has been involuntary outpatient treatment (IOT). Using Massachusetts data, 19 patients with court orders for IOT were matched to all and to best fits on demographic and clinical variables, and then to individuals with the closest fit on utilization before the IOT date. Outcomes indicated the IOT group had significantly fewer admissions and hospital days after the court order. The full impact of IOT requires more study, particularly directed toward IOT's effects on insight and quality of life.

Neurologic and vascular structures at risk during anterior-posterior locking of retrograde femoral nails.

OBJECTIVE: To evaluate the neurovascular structures at risk with the placement of anterior-posterior locking screws in the proximal femur. DESIGN: Cadaveric study. MAIN OUTCOME MEASURE: Anatomic relationships. RESULTS: The femoral artery lies medial to the femur, and its branches cross the anterior femur more than four centimeters distal to the lesser trochanter. Branches of the femoral nerve cross the anterior femur proximal to the lesser trochanter and start four centimeters distal to the piriformis fossa. CONCLUSIONS: Risks to the neurovascular structures during anterior-posterior locking in the proximal femur are diminished if locking is performed above the level of the lesser trochanter.

Assessing parent attributions for child behavior using open-ended questions.

Assessed parent attributions for child behavior using open-ended questions. Sixty-one parents of children with attention deficit hyperactivity disorder (ADHD) and 49 parents of nonproblem children participated. An open-ended, thought-listing question asked following child behavior indicated that parents spontaneously offer causal attributions for their children's behavior. Responses to a second open-ended question asking specifically for attributions for the child behavior indicated that both groups of parents attributed prosocial (PRO) child behaviors more than problem behaviors to internal, controllable, and stable causes or to the external situation and attributed problem behaviors most often to uncontrollable and unstable causes within the child or to factors within the parent. Compared with parents of nonproblem children, parents of children with ADHD were less likely to see themselves as the cause of child behavior and were more likely to mention medication. Analyses indicated that, although attributions elicited by rating scales were related to attributions provided in an open-ended format, each method provided unique information.

Converting an integrated hospital formulary into an object-oriented database representation.

Controlled Medical Vocabularies (CMVs) have proven to be extremely useful in their support of the tasks of information sharing and integration, communication among various software applications, and decision support. Modeling a CMV as an Object-Oriented Database (OODB) provides additional benefits such as increased support for vocabulary comprehension and flexible access. In this paper, we describe the process of modeling and converting an existing integrated hospital formulary (i.e., set of pharmacological concepts) into an equivalent OODB representation, which, in general, we refer to as an Object-Oriented Healthcare Vocabulary Repository (OOHVR). The source for our example OOHVR is a formulary provided by the Connecticut Healthcare Research and Education Foundation (CHREF). Utilizing this source formulary together with the semantic hierarchy composed of major and minor drug classes defined as part of the National Drug Code (NDC) directory, we constructed a CMV that was eventually converted into its OOHVR form (the CHREF-OOHVR). The actual conversion step was carried out automatically by a program, called the OOHVR Generator, that we have developed. At present, the CHREF-OOHVR is running on top of ONTOS, a commercial OODB management system, and is accessible on the Web.

Comparison of androgen-independent growth and androgen-dependent growth in BPH and cancer tissue from the same radical prostatectomies in sponge-gel matrix histoculture.

BACKGROUND: In order to determine androgen sensitivities of prostate cancer and benign prostatic hypertrophy (BPH) tissues from the same patient in vitro, we used a histoculture technique to measure androgen-independent and androgen-dependent growth and compared them in paired specimens of BPH and prostate cancer from 23 radical prostatectomies. Both androgen-independent growth and androgen-dependent growth are measures of important biological characteristics of benign and malignant prostate tissue. METHODS: The effect of hydroxyflutamide and antiandrogens on dihydrotestosterone (DHT)-stimulated incorporation of 3H-thymidine into both paired specimens of BPH and cancer was utilized to measure androgen-independent and androgen-dependent growth. The percentage decrease in 3H-thymidine incorporation/microgram protein in the flutamide-treated specimen compared to the DHT-treated specimen represented androgen-dependent growth. Residual 3H-thymidine incorporation/microgram protein during hydroxyflutamide administration represented androgen-independent growth. RESULTS: Androgen-independent growth was significantly greater (P = 0.015) in the BPH compared to the cancer paired tissue. Androgen-dependent growth was significantly higher in 23 paired specimens of cancer compared to BPH (P < 0.03). CONCLUSIONS: In paired specimens of BPH and prostate cancer from the same radical prostatectomy specimen, androgen-independent growth appeared greater in BPH compared to cancer specimens; androgen-dependent growth, however, was greater in prostate cancer than in BPH. There was no correlation of either growth parameter with Gleason tumor grade. Future clinical correlations will indicate whether either growth parameter represents an important prognostic factor for prostate cancer aggressiveness stimulated 3H-thymidine incorporation into DNA.

Partitioning a vocabulary's IS-A hierarchy into trees.

Controlled medical vocabularies are useful in application areas such as medical information-systems and decision-support. However, such vocabularies are large and complex, and working with them can be daunting. It is important to provide a means for orienting users to the vocabulary's contents. This paper introduces a methodology for partitioning a vocabulary into small, meaningful pieces. The partitioning is done with respect to the vocabulary's IS-A hierarchy. The methodology, based on a set of rules for refining the IS-A hierarchy, is a process carried out by a user in conjunction with the computer. The methodology is demonstrated on a complex portion of a vocabulary.

Issues and concerns associated with different risk models for eating disorders.

OBJECTIVE: The present paper examines issues and concerns associated with different risk models in identifying individuals who may be vulnerable for eating disorders. METHOD: Studies were located by computerized search and the authors' knowledge of the literature. For the purposes of this paper, different risk models are grouped according to three types of sample selection criteria: exposure to environmental pressures toward thinness, parental psychopathology, and intraindividual characteristics. Intraindividual characteristics were subdivided into biological and behavior vulnerability markers, and within the behavior risk approach, symptom and nonsymptom risk models were further identified. RESULTS: Our literature review indicates that risk research on eating disorders is still in its formative years. More well-planned prospective risk studies are needed. CONCLUSIONS: Among these risk models, the nonsymptom risk approach, which defines risk on the basis of nonsymptom vulnerability markers, represents one of the more promising avenues for future risk research and deserves further exploration.

Utilizing OODB schema modeling for vocabulary management.

Comprehension of complex controlled vocabularies is often difficult. We present a method, facilitated by an object-oriented database, for depicting such a vocabulary (the Medical Entities Dictionary (MED) from the Columbia-Presbyterian Medical Center) in a schematic way which uses a sparse inheritance network of area classes. The resulting Object Oriented Health Vocabulary repository (OOHVR) allows visualization of the 43,000 MED concepts as 90 area classes. This view has provided valuable information to those responsible with maintaining the MED. As a result, the MED organization has been improved and some previously-unrecognized errors and inconsistencies have been removed. We believe that this schematic approach allows improved comprehension of the gestalt of large controlled medical vocabulary.
