[Long lasting intracranial artery stenosis of embolic origin].

INTRODUCTION: The term evanescent, or vanishing, has been used to describe those intracranial stenoses that are consecutive to the partial recanalization of an embolic occlusion. In general terms it has been admitted that those stenoses quickly disappear in parallel to the complete recanalization of the artery. This behaviour allows their differentiation from the atherosclerotic stenoses. CASE REPORT: We present here a 61-year-old male who suffered a middle cerebral artery territory stroke secondary to an atrial fibrillation. In the acute phase, a stenotic flow was found in the proximal portion of his left middle cerebral artery. Although it was an embolic occlusion, one year passed before the stenosis disappeared. DISCUSSION: Findings like this contradict the protocols in use (which consider a stenosis as atherosclerotic if it remains for more than three months), and stress the need for new studies which clarify the natural history of the intracranial arterial occlusions.

[Protrusio acetabuli. An update on the primary and secondary acetabular protrusion].

Acetabular protrusion refers to intrapelvic displacement of the medial wall of the acetabulum and is defined as inward movement of the acetabular line so that the distance between this line and the laterally located ilioischial line is 3 mm or more in adult men and 6 mm or more in adult women. As discussed in this article, acetabular protrusion may be found in many bone disorders such as degenerative joint disease, Paget's disease, rheumatoid arthritis, ankylosing spondylitis, osteomalacia, Marfan's disease and as an effect of irradiation. Protrusio acetabuli appearing in absence of any recognizable cause is termed primary acetabular protrusion or Otto pelvis. Primary acetabular protrusion usually affects both hips in young to middle aged women with a history of diminished abduction, rotation and hip pain since puberty. Radiographically one notes a bilateral axial migration of the femoral head without joint space loss and with moderate degenerative changes.

Recombinant human erythropoietin correction of anemia. Dialysis efficiency, waste retention, and chronic dose variables.

A clinical review of patients who were treated with recombinant human erythropoietin was undertaken to evaluate the effect of partial correction of anemia upon dialysis function and waste retention. Assessment of possible variables that might influence the dosage response was also undertaken, both in a retrospective and prospective manner. Finally, a chart review of patients' symptoms while on dialysis was done to evaluate the effect of correction of anemia. Hematologic improvement was noted in all patients placed on hormone therapy with the correction of hemoglobin and hematocrit. There was no effect on white cell count, although a slight change in the differential was noted at target and one year; platelets rose to a new steady state. Dialysis efficiency was decreased slightly for urea and markedly for creatinine, probable reflecting the distribution characteristics of the latter. Retention of urate, phosphorus, and potassium was noted at target, with a drop toward usual serum levels with minimal dialysis prescription change. Variables such as age, gender, race, and dialysis prescription were compared in an effort to establish dosage variation, but no statistically significant changes were seen. There was an inverse trend in dosage, however, with increasing dialysis efficiency. Patients also seemed to be more tolerant of their dialysis treatments when nonanemic, compared to when anemic. While there is retention of some substances with improvement of anemia, the levels were easily corrected with adjustment of the dialysis prescription. No definite relationship could be seen in any of the variables studied and hormone dose requirements. Attention must be paid to adequate dialysis prescription in the erythropoietin treated patient.

[Synovial cysts of the hip. Diagnosis by x-ray computed tomography].

Report of eleven synovial cysts of the hip, associated with osteoarthritis or rheumatoid arthritis, whose the diagnostic problems have been resolved by computed tomography. This technique is proving so efficient in the diagnosis of nature and extension that the other radiological examinations become useless.

The use of pure FSH alone or in combination for ovulation stimulation in in vitro fertilization.

The experience of the Norfolk Program, from the beginning of 1981 until mid 1984, is reviewed. Two groups of patients are compared. The initial experience goes from the very beginning until early 1984 in which knowledge has accumulated; the patients treated afterwards up to mid 1984 where the previous experience was used to monitor them in a more accurate way is reviewed as a separate category. Different protocols, in each group, are compared: concomitant combination of hFSH/hMG (protocol # 1), sequential combination of the two gonadotropins (protocol # 2), and pure FSH administration (protocol # 3). Several parameters were used in the evaluation of the results: 1) day of laparoscopy, 2) volume of the follicles obtained, 3) number of preovulatory oocytes obtained, 4) number of immature oocytes obtained, 5) number of atretic oocytes induced, 6) number of oocytes with abnormal zona pellucida, 7) fertilization rate of preovulatory and immature oocytes, 8) abnormal fertilization rate of preovulatory and immature oocytes, 9) cleavage rate of the normally fertilized preovulatory immature oocytes, 10) transfer rate, single and multiple, 11) follicular and luteal phases as evaluated by estradiol levels in the preovulatory phase and by E2 and progesterone in the post ovulatory phase, 12) pregnancy rate. Pure FSH seems to prolong somewhat the follicular phase. Preovulatory oocytes derived from small follicles may originate pregnancies that may end in abortion. The concomitant combination rendered a larger number of preovulatory oocytes. The number of immature oocytes tend to be higher in the pure FSH stimulated group, but this seems to depend on the way FSH is used. The normal fertilization rate for preovulatory oocytes tends to be similar as the criteria used in the stimulation protocol improved. The abnormal fertilization rate for both types of oocytes decreased in the latest series. The number of embryos transferred was higher in the concomitant combination stimulation. Shortening the hMG/hCG interval in the pure FSH category seems to induce a deficient follicular phase except in cases with rapid high response. When using the hFSH/hMG combination the values of estradiol in the follicular phase tend to be much higher than when the original pure hMG protocol is used. This does not seem to be true when pure FSH stimulation is used. The number of multiple transfers increased substantially in both types of stimulation as experience increased.(ABSTRACT TRUNCATED AT 400 WORDS)

[Role of x-ray computed tomography in infections of soft tissue].

The authors relate their own experience in 64 cases of soft tissue infection investigated by computed tomography: 25 lesions only located in soft tissue of the extremities and 39 associated with bone and/or joint lesions, including the spine. The describe CT findings of abscesses, "solid" infections and cellulitis. Advantages and bounds of the CT in this field are defined.

Early hospital discharge of preterm very low birth weight infants.

OBJECTIVE: The objective of this study was to investigate whether early discharge from the hospital was feasible for selected very low birth weight (VLBW) infants. STUDY DESIGN: A randomized clinical trial of discharge of VLBW infants from the neonatal intensive care unit at 1300 gm versus 1800 gm was done comparing weight gain and incidence of infection. Forty-three VLBW infants treated in the neonatal intensive care unit and follow-up clinics of the Hospital Universitario del Valle, Cali, Colombia, were entered into the study at 1300 to 1350 gm when they met behavioral criteria for discharge and the family home was approved. RESULTS: There were no differences in weight gain or incidence of infection in the home group compared with the hospital group. A significant saving in hospital days and hospital costs was realized for the home group. Family cooperation was heightened in the home group. CONCLUSIONS: Early discharge from the hospital at weights as low as 1300 to 1350 gm is safe for the VLBW infant when properly selected on the basis of behavioral criteria and environmental approval. The potential savings in hospital costs should be considered when resources are allocated for continued support for these infants.
