Flow cytometry of normal, hyperplastic, and malignant human endometrium. A study of ploidy and proliferative indices including comparison with in vitro S-phase labeling.

The validity of 48 endometrial proliferative indices derived from flow cytometric deoxyribonucleic acid histograms was tested by comparison with S-phase labeling indices. There was a positive but weak correlation. Despite this correlation, there was a good relationship between the flow cytometric proliferative indices and both the menstrual phase and degree of tissue differentiation in a larger series of 162 normal, hyperplastic, and malignant endometrial specimens examined by flow cytometry alone. Flow cytometric proliferative indices may be useful for prediction of tumor behavior in non-DNA aneuploid endometrial carcinomas that are assessed as being a good prognostic type by other tumor parameters.

Prophylactic short course rectal metronidazole for cesarean section. A double-blind controlled trial of a simple low cost regimen.

One hundred and eighty two patients undergoing elective and emergency cesarean section were entered in a randomized double-blind placebo controlled trial of short course metronidazole rectal suppositories. There was a significant reduction in wound infection, febrile morbidity and postoperative hospitalization in the treated group. The reduction was greatest for serious wound infection. The simplicity and low cost of the regimen make it suitable for hospitals in developing countries.

Symphysis-fundus measurements in Asian and Caucasian women in Bradford.

A study was undertaken to assess the need for a separate chart of symphysis-fundus measurements for the large Asian population of Bradford. 129 measurements from 47 pregnant Asian patients were compared with 193 measurements from 87 pregnant Caucasian patients. There was insufficient evidence to support the production of a separate chart for the Asian population.

Morphometric assessment of ovarian stromal proliferation--a clinicopathological study.

Ovarian stromal hyperplasia is commonly observed in peri- and post-menopausal ovaries. Assessment has, hitherto, been largely subjective or dependent on arbitrary criteria, which has made comparison of studies difficult. In this study, the stroma of a large number of ovaries was assessed morphometrically. Data were related to clinicopathological and cytomorphological information. A significant association was noted between the degree of stromal proliferation and post-menopausal endometrial adenocarcinoma (P = 0.004). There were no significant correlations between the degree of stromal proliferation and average cytomorphological dimensions.

Serum CA 125 antigen levels and disease severity in patients with endometriosis.

We wished to test whether there was a relationship between CA 125 levels and the severity of endometriosis that would allow CA 125 to be used to monitor disease progression. CA 125 levels were measured in 52 women with laparoscopically staged endometriosis and in eight women with normal pelvic findings at laparoscopy. There was a positive correlation (r = 0.63; P less than .0001) between disease severity and CA 125 levels. CA 125 was also elevated, compared with women with a normal pelvis, in patients with mild and moderate disease. There was no relationship between CA 125 levels and the day of the menstrual cycle. These results suggest that after malignancy has been excluded, CA 125 levels may offer a useful method of monitoring disease progress.

Flash labelling of S-phase cells in short-term organ culture of normal and pathological human endometrium using bromodeoxyuridine and tritiated thymidine.

Normal hyperplastic and malignant endometrial specimens were labelled in vitro with bromodeoxyuridine (BrdU). S-phase cells were stained after DNA denaturation, using a monoclonal antibody to BrdU and an indirect immunoperoxidase method. Various conditions for BrdU uptake, DNA denaturation, and staining were tested. BrdU labelling was compared with autoradiography using tritiated thymidine, with good correlation. Glandular labelling indices of proliferative endometrium were significantly higher than both secretory and hyperplastic endometrium but were similar to carcinoma. Stromal labelling showed the same trend but the differences were not statistically significant.

Is routine antenatal booking vaginal examination necessary for reasons other than cervical cytology if ultrasound examination is planned?

Sixteen ovarian cysts, including one borderline ovarian malignancy, and ten trichomonal vaginal discharges but no cases of cervical incompetence or ectopic pregnancy had been detected at booking vaginal examination in 11,622 consecutive antenatal case records abstracted retrospectively. In an attitude survey of pregnant women 77% believed that vaginal examination was reassuring, 55% found it unpleasant, and 18% thought it could cause miscarriage. This, and a review of the literature, suggests that routine vaginal examination if ultrasound is planned has few advantages beyond the taking of a cervical smear.

Complications of pregnancy and benign familial joint hyperlaxity.

A family is described in which four generations were affected by benign familial joint hyperlaxity and, in the two generations for which obstetric data were available, pregnancies were complicated by unexplained mid-trimester vaginal bleeding. This prompted a study to determine whether unexplained antepartum haemorrhage (APH) and premature rupture of the membranes (PROM) both of which might reflect the structure of fetal collagen, were associated with joint hyperlaxity in offspring. The joint hyperlaxity of children born from such pregnancies was slightly greater than that found in age matched children born from uncomplicated pregnancies.

False positive results of tests for syphilis and outcome of pregnancy: a retrospective case-control study.

In a retrospective case-control study 64 women yielding a false positive result to a test for syphilis in pregnancy were compared with 128 controls individually matched for age, parity, hospital of delivery, and year of delivery. There were significantly more unsuccessful pregnancies, mainly spontaneous abortions during the first and second trimesters, among women with persistent false positive results. There was no significant difference between groups in the mean birth weights of liveborn infants. The antibodies responsible for the false positive result may indicate the presence of an immunological disturbance. Women who give a false positive result should be carefully managed throughout their pregnancy.

Oestrogen receptor in glands and stroma of normal and neoplastic human endometrium: a combined biochemical, immunohistochemical, and morphometric study.

Cryostat sections of unfixed human endometrial tissue from normal, hyperplastic, and carcinomatous endometrium were assayed for oestrogen receptor content using a radiolabelled oestradiol binding assay with isoelectric focusing to separate receptor proteins. The proportions of myometrium, stroma, and glandular tissue were estimated in adjacent sections by point counting. Further cryostat sections were stained for oestrogen receptor immunohistochemically with a monoclonal antibody and the relative staining intensity of the various tissue components assessed. The area proportions and relative immunohistochemical staining intensity of each tissue component in the cryostat sections were then used to adjust the biochemical results and derive separate values of oestrogen receptor content in myometrium, glands, and stroma. The greatest difference in oestrogen receptor content through the menstrual cycle and between normal, hyperplastic, and malignant endometrium occurred in the glandular element.

Ovarian cysts in pregnancy: does ultrasound make traditional management inappropriate?

In a retrospective series of 131 ovarian tumors associated with pregnancy, 81 (including one carcinoma and six tumors of borderline malignancy) were removed. Eighty-three cysts were examined by ultrasound. All 15 of less than 5 cm in diameter resolved spontaneously. Of 68 larger than 5 cm in diameter, 39 had a simple internal echo pattern and a smooth wall; 19 of these disappeared, and the histology of seven others suggested they might have resolved if treated conservatively. Three of the tumors of borderline malignancy also showed a simple ultrasound pattern, but higher-resolution machines may have differentiated two of these. We suggest a policy of selective conservative management of ovarian cysts during pregnancy on the basis of the ultrasound appearance.
