Cervical microglandular hyperplasia is an unusual lesion that may confused with malignancy, by both macroscopic and colposcopic examinations. Even the cytologic and histologic characteristics may mimic an adenocarcinoma. When, in addition, the simultaneous occurrence of an area of cervical intraepithelial neoplasia is found, it could complicate the correct diagnosis. The clinical implications are discussed on the basis of a single case and the available literature survey. We suggest an in-depth colposcopic and histopathologic examination prior to making a definite diagnosis.
Macroscopic and histologic findings in 11 patients operated on for isolated tubal torsion suggested a common pathophysiologic process. During laparotomy, tubal torsion appeared to involve the fimbrial end of the fallopian tube, distal to the site of compression by the sterilization procedure, the ovarian ligament, or a dense adhesion. Histologic study of the tube showed signs of vascular disturbances. Similar signs, although to a lesser extent, were present in the fimbrial end of the contralateral tube in sterilized patients. A causative mechanism resulting in tubal torsion is proposed. The present report suggests that tubal torsion can be a late complication caused by some methods of tubal sterilization.
The presented study is a report of epidemiologic data collected between 1978 and 1980 from patients with gestational trophoblastic disease in the Netherlands. Review of curettings from 344 patients with hydatidiform mole and 33 patients with partial mole resulted in review diagnoses of hydatidiform mole in 283 women, of partial mole in 25 women, and of mere hydropic degeneration in 69 women. Upon review of the initial diagnosis of complete and partial hydatidiform mole, 18% of the cases were considered false-positive. After the evacuation of a molar pregnancy, 10% of the patients needed chemotherapy because of the diagnosis of persistent trophoblastic disease. In patients with a review diagnosis of hydropic degeneration, this was never necessary.
The disappearance time of serum human chorionic gonadotropin (hCG) after the evacuation of hydatidiform mole, partial mole, and hydropic degeneration was investigated. A statistically significant difference existed between the disappearance time of serum hCG after the evacuation of hydatidiform mole as compared with partial mole and hydropic degeneration. The average disappearance time of serum hCG after hydatidiform mole was 99.3 days, after partial mole 58.9 days, and after hydropic degeneration 50.7 days. It is not recommended to start chemotherapy for persistent trophoblastic disease before 100 days after the evacuation of hydatidiform mole, provided there is a steady downward course of the serum hCG level. It is advised to submit cases of supposed hydatidiform and partial moles to a tissue committee for a second opinion.

Morphological changes are described in pelvic lymph nodes excised 10 days after C. parvum (CP) treatment of patients with cervical carcinoma. Guinea-pig popliteal and inguinal lymph nodes were investigated from Days 1 to 10 after an injection of 70 micrograms CP into the footpad. Eosinophils were detected from the first few hours after stimulation, initially in the marginal sinus, then in the medullary sinuses and subsequently in the efferent lymphatics. From Day 2 to Day 6, histiocyte accumulations with the appearance of epithelioid cells were found mainly in subcapsular and interfollicular areas, and small granulomas were also seen in the paracortex. The granuloma formation in the lymph node was considered as an indication of the activation of histiocytes. Besides small granulomas in the paracortex, activated interdigitating cells, surrounded by scattered lymphoblasts and eosinophils, were also present. We considered this lymphoblastic response and eosinophilic accumulation as likely to be due to blastogenic factor and eosinophil stimulation promotor. Eight to 10 days after CP stimulation, the macrophage lymphoblast eosinophil response was replaced by a B-cell reaction: germinal-centre activation and medullary plasma cells. Such a B-cell reaction was also found in the human pelvic nodes removed at operation, but this reaction could not be attributed to CP treatment alone, since cervical-carcinoma patients not treated with CP also showed such reactions. In contrast, pelvic lymph nodes removed at necropsy from females killed in traffic accidents showed no predominance of either B- or T-cell stimulation.

The effect of adjuvant immunotherapy with a single neighbourhood injection of 2 mg C. parvum (CP) was investigated in a randomized study involving 43 patients with carcinoma of the cervix uteri, all of whom were treated by radical surgery. All patients had carcinoma confined to the cervix, the upper part of the vagina or the parametrial region. When the malignancy had spread to the parametrial region, additional postoperative radiotherapy was given. 22 patients received immunotherapy 10 days before surgery, whereas the remaining 21 control patients received no immune stimulation. Only minor side effects of CP were encountered. Follow-up shows a relapse rate of 5% in the CP treated group and of 29% in the controls (P less than 0.05). A further 15 patients with more advanced malignancies were added to our studies. In these, CP stimulation had no effect on relapse rates, but the relapse-free intervals were longer after immune stimulation: control 3.5 months (mean) +/- 1.5 (s.d.), CP 13.0 months +/- 7.0 (P less than 0.05). The number of peripheral T cells and the ability to become sensitized to DNCB were increased after CP stimulation. A decrease was found in the number of blood monocytes and the number of monocytes capable of transforming into active macrophages, indicating a possible sequestration of these cells in the tissues.
The cell surface organization of cancer cells is of potentially great significance, as it may not only allow (early) diagnosis, but as it may also harbour markers for refined prognosis (degree of oncogenetic and metastatic potential), and targets for selective cancer (chemo- and immuno) therapy. With these aspects in mind, the present review deals with SEM work done on (pre-) malignant cells, both in vivo and in vitro, and in animal models. Attention, however, is focused on human cancer cells. Cancer cells in vitro may lose many of their original malignant characteristics, and show adaptations to culture conditions. Many other factors have been shown to influence cell surface morphology, such as cell cycle, cell contacts, and preparations technique. Cancer cells differ in their surface morphology from normal cells, and have an extra ordinary amount of surface activity. Human malignant epithelial cells show abundant long. pleomorphic microvilli, especially those present in effusions. In squamous epithelium (bladder, cervix) microridge system present on normal superficial cells are progressively replaced by microvilli which increase in number and degree of pleomorphism during experimental and clinical oncogenesis. The question of whether or not the appearance of long. Pleomorphic microvilli reflects an irreversible alteration of the epithelium, and thus provides an early marker of irreversible neoplastic transformation is considered and assessed on the basis of our work with (pre-) malignant cells of the human uterine cervix. Although SEM has contributed significantly to the description of oncogenesis, up to now it has no early diagnostic, prognostic or therapeutic significance.

A double-blind placebo-controlled multicentre study with ritodrine, a beta-mimetic uterine relaxant, has been performed in 91 patients in premature labour. All patients were treated according to a fixed dosage scheme consisting of an intravenous infusion followed by oral tablets for a total of seven days. Ritodrine arrested premature labour in 80%, the placebo in 48% of the patients (P=0.02). This short treatment, however, was usually not sufficient to prolong gestation till term. Apart from a slight to moderate rise in maternal heart rate and a slight rise in systolic blood pressure, ritodrine did not give rise to any maternal or fetal side effects. The problems of patient selection and of evaluation of the results are discussed.

It is generally accepted to consider vulvar intraepithelial neoplasia grade III (VIN III) a premalignant condition. The lesion is more frequently diagnosed in younger patients. Conventional surgical treatment is often mutilating and recurrence rates have been reported of approximately 30%. Laser vaporization is a promising alternative therapy. Ten patients with VIN III were treated with CO2 laser. Two patients were retreated with laser for residual disease, and two patients for recurrent disease. One failure was observed, and one patient was off-study. In all patients excellent cosmetic results were obtained. Laser vaporization appears to be an effective and nonmutilating therapy, and preferable for young VIN patients.
Androgen receptors (AR) predominated (98%) in the cytosols of human ovarian carcinomas (n = 40), compared to estrogen (ER)-45%, and progestin (PgR)-53% receptors. The average age of the patients was 58 years. Serum levels of estradiol-17 beta (E2), testosterone (T), follicle stimulating hormone (FSH) and luteinizing hormone (LH) were within the normal range. One third of the cases showed a measurable aromatase activity, while in the majority of the cases this activity was undetectable. The distribution of PgR in the 40 tumors studied appeared to correlate with aromatase activity. The distribution of ER and AR did not differ significantly in aromatase positive and aromatase negative tumors. Interestingly the majority of aromatase positive cases were the endometrioid type of carcinomas. Patients with this type of ovarian carcinoma may probably benefit from anti-estrogen, progestin or aromatase inhibitor treatment. All tumors which were negative for aromatase activity contained AR. In these aromatase negative tumors androgens may not be further metabolized into estrogens and the tumor cells may be influenced by the androgens through the AR present in them. Anti androgen treatment could possibly benefit the patients with such tumors.
All human ovarian tumors (n = 32) analysed were positive for cytokeratin except for a single case. OV-TL 3 stained 96% (26/27) and 1 out of 4 controls, while OC-125 stained 86% (24/27) and 3 out of 4 non-ovarian carcinomas. Androgen receptors predominated (AR-96%) compared to estrogen (ER-32%) or progestin (PgR-50%) receptors in the ovarian carcinomas. Aromatase activity was detected in 6 ovarian carcinomas and in 3 non-ovarian carcinomas. No definite relationship was observed between the parameters measured. Ovarian carcinomas stained brightly with OV-TL 3 and OC-125 had high AR levels, while focal or absent reaction was related to low AR levels. A role for androgens in ovarian cancer is suggested.
