The post-auricular myogenic response: a new instrument which simplifies its detection by machine scoring.

A portable apparatus is described which can be used for routine audiometric screening in a children's clinic. The apparatus measures the postauricular myogenic response to click stimuli and the design of the machine makes for great simplicity of operation. A simple form of machine scoring is employed so that a completely objective result is obtained. Evaluating the equipment on 102 subjects produced no false positive results, but a small percentage of normally hearing adults and children failed to produce a response. The equipment was successfully evaluated in a routine audiometric screening situation on 46 babies aged 6 to 9 months.

The medical treatment of secretory otitis media. A clinical trial of three commonly used regimes.

A clinical trial was undertaken to evaluate three medical treatments commonly used for chronic secretory otitis media. The treatments compared were Ephedrine nose drops, an oral antihistamine and decongestant (Dimotapp) and autoinflation of the middle ear. Changes in middle ear compliance and pressure were used as objective criteria of the efficacy of treatment in addition to changes in pure-tone threshold and to clinical assessment. Symptoms and abnormal signs tended to remit during the trial but there was no evidence from pure-tone audiometry and tympanometry that any of the treatments was beneficial. The period of observation enabled 28% of the children to avoid surgical treatment. Good and bad prognostic features are described which should help in deciding whether to manage a case conservatively or whether to proceed directly to surgery.

Chelating agents: their effect on the permeability of root canal dentin.

An in vitro assessment was made of the ability of three chelating agents used in endodontics to alter the dye penetration of dentinal tubules from the root canal surface. The experiment and control for this study were within the same root. It was found that all three agents significantly reduced the dye penetration into dentin, but there was no difference among the agents in the degree of reduction of dye penetration.

Labyrinthine window rupture.

Some cases of sensorineural deafness are due to labyrinthine window rupture. Three cases have been presented to illustrate different aspects of diagnosis and managment of this condition. The indications for surgical intervention have been discussed. The importance of making the diagnosis is that operation can relieve vertigo and restore the hearing.

A preplanned treatment for malignant hyperpyrexia.

Although a rare complication of general anesthesia, unanticipated malignant hyperpyrexia requires a rapid, organized treatment plan. With the aid of a treatment protocol sheet and a compact drug and cooling package, treatment can be implemented quickly in each operating room.
